"ABSTRAK
Praktek kerja profesi (PKP) di Rumah Sakit bertempat di Rumah Sakit Pusat Angkatan Darat (RSPAD) Gatot Soebroto Ditkesad. PKP ini bertujuan agar mahasiswa apoteker memahami peranan, tugas dan tanggung jawab Apoteker, memiliki wawasan, pengetahuan, keterampilan dan pengalaman praktis untuk melakukan praktek kefarmasian, dan memiliki gambaran nyata tentang permasalahan praktek kefarmasian di Rumah Sakit. Kegiatan PKP ini berlangsung selama dua bulan dari tanggal 1 Juli sampai dengan tanggal 31 Agustus 2015 dengan tugas khusus analisis masalah terkait obat pada pasien coronary artery disease (CAD) di unit jantung lantai 2 RSPAD Gatot Soebroto Ditkesad. Berdasarkan kegiatan PKP yang dilakukan, Apoteker di RSPAD Gatot Soebroto Ditkesad secara umum telah melaksanakan tugas dan tanggung jawab dalam menjalankan pekerjaan kefarmasian. Mahasiswa apoteker telah berhasil memperoleh wawasan mengenai kegiatan rutin yang dilakukan di rumah sakit. Kegiatan praktek kefarmasian dalam bidang manajemen yang dilakukan di rumah sakit meliputi pemilihan, perencanaan, pengadaan, penerimaan, penyimpanan, pendistribusian, pengendalian, pemusnahan, pencatatan, dan pelaporan. RSPAD Gatot Soebroto Ditkesad secara umum telah melaksanakan pelayanan farmasi klinis kecuali kegiatan pemantauan kadar obat dalam darah (PKOD).ABSTRACT Profession Internship at hospital was held at Gatot Soebroto Army Central Hospital. Profession Internship was intended to make apothecary student understood the role, duties, and responsibilities of pharmacist, had insights, knowledges, skills, and practical expriences of pharmacy practice, and had a real description about the issues of pharmaceutical practice in hospital. This activity was held for two months from July 1st to August 31st 2015, with special assignment of problem analysis related to drug on coronary artery disease (CAD ) patients in heart unit at the 2nd floor of Gatot Soebroto Army Central Hospital. Based on the activities, pharmacist at Gatot Soebroto Army Central Hospital generally had done the duties and responsibilities of pharmaceutical practices. Apothecary student had succeed obtaining the insight into routine activities in hospital. Pharmaceutical practices in management at hospital included election, planning, procurement, reception, storage, distribution, controling, extermination, recording, and reporting. Gatot Soebroto Army Central Hospital generally had done clinical pharmacy services except the monitoring of drug level in blood"

Rumah sakit adalah institusi pelayanan kesehatan yang menyelenggarakan pelayanan kesehatan perorangan secara paripurna yang menyediakan pelayanan rawat inap, rawat jalan, dan gawat darurat.Apoteker di Rumah Sakit memiliki peran untuk mengelola perbekalan farmasi dan melakukan pelayanan farmasi klinik.Praktek Kerja Profesi Apoteker di RSPAD Gatot Soebroto Ditkesad bertujuan agar calon apoteker mampu memahami peranan, tugas dan tanggung jawabnya di Rumah Sakit, memiliki wawasan, pengetahuan, keterampilan dan pengalaman praktis untuk melakukan praktik kefarmasi di Rumah Sakit, memiliki gambaran nyata tentang permasalahan praktik kefarmasian serta mempelajari strategi dan kegiatan-kegiatan yang dapat dilakukan dalam rangka perkembangan praktik kefarmasian di Rumah Sakit. Kegiatan yang dilakukan diantaranya mempelajari sistem pengelolaan perbekalan kefarmasian di RSPAD Gatot Soebroto Ditkesad , praktek pelayanan farmasi klinik, dan melakukan pengkajian masalah terkait obat pada salah satu pasien dengan penyakit tertentu. Dalam rangka peningkatan kualitas pelayanan kefarmasian di RSPAD Gatot Soebroto Ditkesad baik di rawat inap maupun rawat jalan perlu dilakukan penambahan sumber daya manusia yakni apoteker sehingga pekerjaan kefarmasian dapat dilakukan secara maksimal dan perlu dilakukan peningkatan kerjasama antara tenaga kefarmasian terutama apoteker dengan tenaga medis lainnya ABSTRACT Hospital is a health care institution that organizes personal health services in plenary that provides inpatient, outpatient, and emergency. Pharmacist at Hospital has role to manage pharmaceutical supply and clinical pharmacy service The purposes of Internship Program in RSPAD Gatot Soebroto Ditkesad are for understanding about the roles and responsibilities of pharmacist candidate in pharmaceutical practice and management in hospital, having perceptions, knowledge, skills, real experiences and illustrations about pharmaceutical practice problems which probably happen, and finding strategic ways to develop pharmaceutical practices in hospital. The acitivities were studying about system management of pharmacy in hospital, clinical pharmacy practice, and doing assessment dug related problem in one of patient who has certain disease. In order to improve the quality of pharmacy services at Gatot Subroto Army Hospital Ditkesad both inpatient and outpatient necessary additional human resources which the pharmacist so that work can be done optimally pharmaceutical and necessary to improve cooperation between pharmacy personnel, especially pharmacists with other medical personnel"

Praktek kerja profesi PKP di Rumah Sakit Pusat Angkatan Darat RSPAD Gatot Soebroto Ditkesad bertujuan untuk memahami peranan tugas dan tanggung jawab Apoteker di Rumah Sakit sesuai dengan ketentuan dan etika pelayanan kefarmasian pada khususnya dan pelayanan kesehatan pada umumnya memiliki wawasan pengetahuan keterampilan dan pengalaman praktis untuk melakukan praktek kefarmasian di Rumah Sakit serta memiliki gambaran nyata tentang permasalahan praktek kefarmasian terutama dalam berinteraksi baik dengan pasien maupun dengan tenaga kesehatan lain Berdasarkan kegiatan PKP di RSPAD pengelolaan sediaan farmasi alat kesehatan dan bahan medis habis pakai dilaksanakan di instalasi farmasi rumah sakit dengan sistem satu pintu Pelayanan farmasi klinis masih belum berjalan optimal akibat kurangnya tenaga Apoteker yang berfokus pada pelayanan farmasi klinik Pemantauan kadar obat dalam darah adalah pelayanan farmasi klinis yang belum dilaksanakan Peran Apoteker di RSPAD perlu ditingkatkan melalui kerjasama dan komunikasi dengan tenaga kesehatan lain Kegiatan PKP di RSPAD berlangsung selama dua bulan dari tanggal 1 September sampai dengan tanggal 31 Oktober 2015 dengan tugas khusus yaitu analisa masalah terkait obat terhadap pasien neonatus kurang bulan kurang masa kehamilan dan bayi berat lahir rendah dengan sepsis neonatorum di instalasi rawat inap perinatal resiko tinggi RSPAD Gatot Soebroto Dikesad periode 21 ndash 27 Oktober 2015 ABSTRACT Profession internship at the Gatot Subroto Army Central Hospital was itended to make apotechary students understand the roles and responsibilities of Hospital Pharmacists in accordance with the rules and ethics of pharmaceutical care in particular and healthcare in general have the insight knowledge skills and practical experience of pharmaceutical practice and know the issues of pharmaceutical care in Hospital especially in their interaction with the patients and with other health professionals Based on the activities management of pharmaceutical and medical devices in Gatot Subroto Army Central Hospital is responsibility of hospital pharmacy by one door system Clinical pharmacy services is still not optimal due to the lack of the number of pharmacists that focus on clinical pharmacy services Monitoring of drug levels in the blood is a clinical pharmacy services that have not been implemented here Role of Pharmacists needs to be improved through cooperation and communication with other health professionals Profession internship at the Gatot Subroto Army Central Hospital was held for two months from September 1st until October 31st 2015 in a special assignment that is identification of drug related problems of neonates with less gestation low birth weight and neonatal sepsis in high risk perinatal unit of Gatot Subroto Army Central Hospital on period October 21st until October 27th 2015 "

Rumah sakit merupakan salah satu tempat dilakukannya pelayanan medis atau kesehatan bagi
masyarakat. RSPAD Gatot Soebroto merupakan rumah sakit tipe A Pendidikan yang merupakan
rumah sakit rujukan dari rumah sakit dibawahnya. Salah satu faktor yang sangat mempengaruhi
mutu dari rumah sakit adalah pelayanan kefarmasian. Praktek Kerja Profesi Apoteker yang
dilakukan di Rumah Sakit Umum Pusat Fatmawati ditujukan untuk memahami tugas dan fungsi
apoteker dalam melaksanakan Pelayanan Kefarmasian di Rumah Sakit yang meliputi pengelolaan
Sediaan Farmasi dan Bahan Medis Habis Pakai, serta pelayanan farmasi klinik. Untuk melakukan
pelayanan kefarmasian di Rumah Sakit, khususnya dalam bidang farmasi klinis maka Apoteker
perlu untuk terus mengikuti perkembangan pengetahuan serta peraturan yang berlaku. Strategi
pengembangan praktek kefarmasian di RSPAD Gatot Soebroto adalah dengan melakukannya
kegiatan penyuluhan rutin terkait obat untuk para tenaga kefarmasian maupun tenaga kesehatan
lainnya.

"Praktek kerja profesi di Rumah Sakit Kepresidenan RSPAD Gatot Soebroto Ditkesad dilaksanakan selama dua bulan dimulai dari bulan Juli 2016 hingga Agustus 2016. Tujuan dilaksanakannya praktek kerja profesi ini adalah agar calon Apoteker mampu memahami peranan, tugas dan tanggung jawab apoteker dalam praktek pelayanan kefarmasian di rumah sakit sesuai dengan ketentuan perundangan dan etika farmasi yang berlaku meliputi pengelolaan sediaan farmasi, alat kesehatan dan bahan medis habis pakai serta pelayanan farmasi klinis. Rumah Sakit Kepresidenan RSPAD Gatot Soebroto Ditkesad telah melakukan pengelolaan sediaan farmasi, alat kesehatan dan bahan medis habis pakai sesuai dengan Peraturan Menteri Kesehatan Nomor 58 Tahun 2014 tentang Standar Pelayanan Kefarmasian di Rumah Sakit, sedangkan pelayanan farmasi klinis yang belum dilakukan adalah pemantauan kadar obat dalam darah PKOD dan evaluasi penggunaan obat. Sementara visite dan monitoring efek samping obat masih belum dilakukan secara maksimal oleh apoteker.
Internship at RSPAD Gatot Soebroto Hospital was held two months started at July 2016 until August 2016. This internship was intended to make Apothecary student understand roles and responsilibities of Pharmacist in hospital according to the rule of law and ethical pharmacy include understanding managerial activities of pharmaceutical products, medical devices and single use medical tools and also clinical pharmacy. Moreover, managerial activities pharmaceutical products, medical devices and single use medical tools in RSPAD Gatot Soebroto Hospital are suitable with Regulation of Minister of Health No. 58 Year 2014 about Standarization of Pharmaceutical Care in Hospital. Clinical pharmacy activities in RSPAD Gatot Soebroto Hospital that are not done are drug use evaluation and monitoring of drug levels in blood. While visite and monitoring of drug side effect is still not done optimally by apothecary."
