"ABSTRAK
Apotek adalah sarana pelayanan kefarmasian tempat dilakukan praktek kefarmasian oleh Apoteker. Pelayanan kefarmasian merupakan suatu pelayanan langsung dan bertanggung jawab kepada pasien yang berkaitan dengan sediaan farmasi dengan maksud mencapai hasil yang pasti untuk meningkatkan mutu kehidupan pasien.Apoteker dituntut untuk meningkatkan pengetahuan, keterampilan, dan perilaku agar dapat melaksanakan interaksi langsung dengan pasien. Bentuk interaksi tersebut antara lain adalah pemberian informasi obat dan konseling kepada pasien yang membutuhkan Peran dan fungsi apoteker dalam pelayanan kefarmasian menjadi penentu keberhasilan suatu apotek.

"ABSTRAK
Praktek Kerja Profesi (PKP) di apotek bertempat di Apotek Kimia Farma No. 282 Jakarta Barat.
Kegiatan ini berlangsung selama empat minggu dari tanggal 1 sampai dengan tanggal 30 Oktober
2015. PKP di apotek bertujuan agar mahasiswa apoteker memahami tugas dan tanggung jawab
apoteker dalam pengelolaan apotek serta melakukan praktik pelayanan kefarmasian sesuai dengan
ketentuan peraturan perundang-undangan dan etika yang berlaku, emahami seluruh kegiatan
pelayanan kefarmasian yang dilakukan di apotek, dan memiliki gambaran nyata tentang
permasalahan dalam praktek kefarmasian serta mempelajari strategi dalam mengatasi
permasalahan tersebut. Berdasarkan kegiatan PKP yang dilakukan, APA di Apotek Kimia Farma
No. 282 Jakarta Barat secara umum telah melaksanakan tugas dan tanggung jawab dalam
pengelolaan apotek terkait kegiatan teknis kefarmasian dan kegiatan nonteknis kefarmasian sesuai
dengan peraturan perundang-undangan yang berlaku. Mahasiswa apoteker telah berhasil
memperoleh wawasan mengenai kegiatan rutin yang dilakukan di apotek. Kegiatan pelayanan
kefarmasian di Apotek meliputi 2 (dua) hal, yaitu: kegiatan yang bersifat manajerial berupa
pengelolaan sediaan farmasi, alat kesehatan dan bahan medis habis pakai serta melakukan
pelayanan farmasi klinik. Masalah yang terjadi di Apotek Kimia Farma No. 282 adalah ketiadaan
Apoteker Pendamping yang dapat menggantikan APA ketika sedang tidak ada di tempat. Selain
itu, agar pasien lebih nyaman dan terbuka dalam memberikan dan menerima informasi, maka perlu
disediakan ruang konseling khusus bagi pasien.

"ABSTRAK
Praktik Kerja Profesi (PKP) di apotek bertempat di Apotek Kimia Farma No.96
Jalan Letjen S.Parman Kav.G/12 Jakarta Barat. Kegiatan ini berlangsung selama
empat minggu dari tanggal 3 sampai dengan 30 Agustus 2015. PKP di apotek
bertujuan agar mahasiswa apoteker mengerti peranan, tugas, dan tanggung jawab
Apoteker Pengelola Apotek (APA), memiliki wawasan, pengetahuan, serta
pengalaman praktis tentang pelaksanaan pekerjaan kefarmasian, dan memiliki
gambaran nyata akan permasalahan pekerjaan kefarmasian yang terjadi di apotek.
Berdasarkan kegiatan PKP yang dilakukan, APA di Apotek Kimia Farma No.96
Jalan Letjen S.Parman Kav.G/12 Jakarta Barat secara umum telah menjalankan
fungsi pengawasan dan pengendalian terhadap semua komponen obat di apotek dan
juga melaksanakan fungsinya sebagai apoteker dalam menjamin penggunaan obat
yang rasional. Pengelolaan pelayaan kefarmasian yang dilakukan di apotek meliputi
perencanaan kebutuhan obat, pengadaan obat, pendistribusian obat, penyimpanan
obat serta pelayanan informasi obat untuk obat dengan resep dan obat non resep
atau swamedikasi. Praktek pelayanan kefarmasian di Apotek Kimia Farma No. 96
secara keseluruhan telah berjalan sesuai dengan peraturan perundang-undangan
yang berlaku. Masalah yang terjadi di Apotek Kimia Farma No.96 adalah
kurangnya kedisiplinan petugas farmasi dalam melakukan pencatatan barang di Apotek.

Kegiatan ini berlangsung selama empat minggu dari tanggal 5 sampai dengan tanggal 31Oktober 2015.
PKP di apotek bertujuan agar mahasiswa apoteker mengerti peranan Apoteker Pengelola Apotek
(APA), memiliki wawasan tentang pelaksanaan pekerjaan kefarmasian, dan memiliki gambaran nyata
akan permasalahan pekerjaan kefarmasian yang terjadi di apotek. Berdasarkan kegiatan PKP yang
dilakukan, APA di Apotek Kimia Farma No. 366 Kota Depok secara umum telah melaksanakan tugas
dan tanggung jawab dalam pengelolaan apotek terkait kegiatan teknis kefarmasian dan kegiatan
nonteknis kefarmasian sesuai dengan peraturan perundang-undangan yang berlaku. Mahasiswa
apoteker telah berhasil memperoleh wawasan mengenai kegiatan rutin yang dilakukan di apotek.
Kegiatan teknis kefarmasian yang dilakukan di apotek meliputi pengadaan, penerimaan, penyimpanan,
penjualan, pengelolaan narkotika, pengelolaan psikotropika, dan dokumentasi resep. Adapun kegiatan
nonteknis kefarmasian yang dilakukan di apotek adalah penyusunan resep dan struk pembayaran,
penandaan harga pada resep kredit untuk penagihan, dan pembuatan Laporan Ikhtisar Penjualan Harian
(LIPH). Masalah yang terjadi di Apotek Kimia Farma No. 366 Kota Depok adalah ketiadaan Apoteker

Pharmacy No. 366 Kota Depok is the absence of pharmacist who can substitute Pharmacy Manager
temporarily while Pharmacy Manager was not in place. "

"Praktek Kerja Profesi Apoteker di Apotek Kimia Farma No. 282 bertujuan untuk mengetahui dan memahami peran Apoteker di apotek, khususnya dalam bidang menejemen apotek dan juga pelayanan kefarmasian. Peran Apoteker dalam menejemen apotek meliputi pengadaan, pembelian, penyimpanan, penjualan barang, serta administrasi. Tugas khusus yang diberikan adalah skrining resep yang bertujuan untuk menilai kelengkapan resep, keabsahan serta kerasionalan pengobatan berdasarkan resep tersebut. Berdasarkan hasil skrining terhadap 50 resep yang dilayani pada periode 1 April - 10 Mei 2014 di Apotek Kimia Farma No. 282 tidak satu pun resep yang lengkap secara administrasi.
Apothecary Internship Program at Apotek Kimia Farma No. 282 aims to learn and understand the roles of pharmacists in Pharmacy, especially how to manage of Pharmacy as well as how to deliver pharmaceutical care. The pharmacist at Apotek Kimia Farma had responsible for procurement, purchasing, storage, sale and administration. Specific assignment given was screening of prescription aims to assess and evaluate completeness, validity and then rationality of prescription. Based on the results of the screening of 50 prescription are served during the period April 1st - May 10th 2014 at Apotek Kimia Farma No. 282 then it can be concluded that none of the prescription are complete in the administration."

"Praktek Kerja Profesi di Apotek Kimia Farma No. 50 Bogor bertujuan memahami kegiatan kefarmasian yang dilakukan di apotek, serta memahami tugas, fungsi apoteker dalam pengelolaan sebuah apotek. Tugas khusus yang diberikan berjudul Pelayanan Kefarmasian di Rumah Pada Pasien dengan Obat Indeks Terapi Sempit. Tujuan dari tugas khusus ini adalah untuk meninjau kesesuaikan farmakologis resep yang terdapat obat dengan indeks terapi sempit dan melakukan pelayanan kefarmasian di rumah pada pasien yang mendapat obat dengan indeks terapi sempit. Peran apoteker di apotek ialah menentukan kebijakan pengelolaan apotek, serta melaksanakan fungsi pengawasan dan pengendalian terhadap semua komponen yang ada di apotek, disamping melaksanakan fungsi profesionalnya sebagai seorang apoteker untuk menjamin penggunaan obat yang rasional untuk keselamatan pasien. Adanya pelayanan kefarmasian di rumah diharapkan pasien dengan obat indeks terapi sempit akan mendapatkan efek farmakologis yang maksimal dan memperkecil efek samping atupun efek toksik.
Professional Internship at Apotek Kimia Farma No. 50 Bogor aims to understand the pharmaceutical activity that be done at pharmacies and understand the assignment, function of pharmacists on pharmacies management. The special assignment that is given is entitled Home Care Phamacy on Patients who Take Narrow Therapeutic Index Drugs. The aims of this special assignment is to observe phamacology suitability recipes with narrow therapeutic index drug and give homecare pharmacy to patient with narrow therapeutic index drug. The role pharmacist in pharmacy is determine managemant pharmacies priciple, implement monitoring and control function to all component in pharmacies, be side of implement the proffesional function as a pharmacist to assure utilizing of rational drug to patient safety. The homecare phamacy is hoped the patient with narrow therapeutic index drug will get the maximal phamacology effect and minimize adverse effect or toxicity."

tanggung jawab apoteker dalam pengelolaan apotek serta melakukan praktik pelayanan kefarmasian sesuai dengan
ketentuan peraturan perundang-undangan dan etika yang berlaku, emahami seluruh kegiatan pelayanan kefarmasian
yang dilakukan di apotek, dan memiliki gambaran nyata tentang permasalahan dalam praktek kefarmasian serta
mempelajari strategi dalam mengatasi permasalahan tersebut. Berdasarkan kegiatan PKP yang dilakukan, APA di
Apotek Kimia Farma No. 7Bogor secara umum telah melaksanakan tugas dan tanggung jawab dalam pengelolaan
apotek terkait kegiatan teknis kefarmasian dan kegiatan nonteknis kefarmasian sesuai dengan peraturan perundangundangan
yang berlaku. Mahasiswa apoteker telah berhasil memperoleh wawasan mengenai kegiatan rutin yang
dilakukan di apotek. Kegiatan pelayanan kefarmasian di Apotek meliputi 2 (dua) hal, yaitu: kegiatan yang bersifat
manajerial berupa pengelolaan sediaan farmasi, alat kesehatan dan bahan medis habis pakai serta melakukan
pelayanan farmasi klinik. Tugas khusus yang diberikan berjudul Studi Kelayakan Mengenai Pendirian Apotek di
Apotek Kimia Farma Sukahati Cibinong. Tujuan dari tugas khusus ini adalah untuk mengetahui kelayakan
berdirinya suatu apotek dan mengetahui aspek-aspek apa saja yang menjadi penilaian dalam studi kelayakan
pendirian sebuah apotek. ABSTRACT Professional Internship at Apotek Kimia Farma No. 7 Bogor was held to learn and understand the role of pharmacist
in Pharmacy, especially to manage a pharmacy or to deliver pharmaceutical care. This activity lasted for four weeks

2015. Profession Internship in pharmacy aims to make apothecary students
understand the duties and responsibilities of pharmacists in the management of pharmacy practice and
pharmaceutical services in accordance with the provisions of the legislation and ethics, understand all activities
undertaken pharmaceutical services in pharmacies, and have a real picture of the problems in the practice of
pharmacy as well as learn strategies to overcome these problems. Based on the profession internship activities
undertaken, pharmacist as Pharmacy Manager at Kimia Farma No. 7 Bogor in general have been carrying out the
duties and responsibilities in the management of activities related pharmacy pharmaceutical technical and
nontechnical pharmacy activities in accordance with the legislation in force. Apothecary student have managed to
gain insights into the routine activities performed in pharmacy. Activities of pharmaceutical care includes two (2)
ways, namely: managerial activities such as the management of pharmaceutical, medical devices and medical
consumable materials and conduct clinical pharmacy services. The special assignment is given entitled A
Feasibility Study on The Establishment of Apotek Kimia Farma Sukahati Cibinong. The aims of this special
assignment is to determine the feasibility of the establishment of a pharmacy and to know what aspects of the
