Karya ilmiah akhir KIA merupakan analisis laporan praktik residensi keperawatan medikal bedah yang terdiri dari pengelolaan kasus gangguan sistem perkemihan dengan pendekatan teori Self Care Orem, penerapan evidence based nursing berupa pemijatan pada titik LI-4 untuk mengurangi nyeri kanulasi arteriovenous AV fistula dan melakukan proyek inovasi kelompok berupa tentang seminar sosialisasi konseling dan konselor keperawatan pada pasien gagal ginjal terminal. Pendekatan teori Self Care Orem meyakini bahwa perawatan diri pasien merupakan sesuatu yang dapat dipelajari dengan melibatkan kemampuan yang ada pasien sesuai kondisi penyakitnya. Pemijatan pada titik LI-4 efektif dalam mengurangi nyeri kanulasi AV-fistula pada pasien hemodialisis, mudah diaplikasikan dan tanpa efek samping sehingga dapat diaplikasikan perawat sebagai intervensi manajemen nyeri non farmakologis untuk mengurangi nyeri kanulasi AV-fistula. Seminar sosialisasi tentang konseling dan konselor keperawatan dapat meningkatkan pengetahuan dan motivasi dari perawat untuk mengembangkan diri dalam memberikan konseling kepada pasien gagal ginjal terminal.
ABSTRACT
This final paper is an analysis report of residency medical surgical nursing practice consisting of case management of urinary system disorder with Orem Self Care approach, application of evidence based nursing using massage at LI 4 point to reduce pain in AV fistula cannulation, innovation projects on seminars about socialization of counseling and nursing counselors in end stage renal disease patients. The Orem self care approach believes that self care is something that can be learned by involving the patients existing abilities based on his illness. Massage at LI 4 point is effective to reduce pain of AV fistula cannulation in hemodialysis patients, easy to apply and without side effects, so it can applied by nurses as nonpharmacologic pain management interventions to reduce pain in AV fistula cannulation. Seminars about counseling and nursing counselors can improve the knowledge and motivation of nurses to provide a counseling to end stage renal disease patients."

"Praktik residensi di ruang ortopedi untuk memberikan asuhan keperawatan pada gangguan muskuloskeletal menggunakan teori keperawatan Self Care Orems, menerapkan praktik keperawatan berdasarkan pembuktian, dan melakukan inovasi keperawatan. Kasus kelolaan utama pada gangguan sistem muskuloskeletal adalah osteomielitis kronis. Masalah keperawatan paling banyak adalah nyeri dan mobilitas fisik untuk universal self care requisites serta kurang pengetahuan untuk developmental self care requisites. Intervensi keperawatan berdasarkan pembuktian yang telah diterapkan adalah cold compression therapy yang dapat meningkatkan proses penyembuhan dengan indikator penurunan nyeri dan edema, serta peningkatan rentang gerak sendi. Inovasi keperawatan yaitu penggunaan Clinical Practice Guidline untuk melakukan asuhan keperawatan berdasarkan clinical pathway pada pasca ORIF ekstremitas bawah. Perawat dapat menerapkan teori Self Care Orems, cold compression therapy, serta melakukan kegiatan inovasi untuk meningkatkan kompetensi perawat dan kualitas pelayanan keperawatan.
The advanced clinical practice in orthopaedic ward involved the planning of nursing care for musculoskeletal disorders patients with Orem?s Self Care theory, the implementation of evidenced based nursing practice, and the implentation of nursing innovations. The primary case management of the musculoskeletal system disorders is chronic osteomyelitis. Most nursing problems for the universal selfcare requisites are pain and physical mobility, while for the developmental self care requisites is knowledge deficit. The evidence based nursing intervention implemented was cold compression therapy. It can improve the healing process with the indicators of decreased pain and edema, and increased range of motion. The nursing innovation implemented was the use of Clinical Nursing Practice Guidline in giving nursing care based on the clinical pathways post-ORIF lower extremities. Nurses can apply the Orem?s Self Care Theory, cold compression therapy, and innovation activities to improve their competency and also quality of nursing services."

"Praktik residensi keperawatan medikal bedah yang dilakukan selama dua semester telah memberikan asuhan keperawatan pada pasien dengan gangguan respirasi, melakukan implementasi evidence based nursing dan dan proyek inovasi serta pengabdian masyarakat yang dilakukan di RSUP Fatmawati dan RSUP Persahabatan. Peran pemberian asuhan keperawatan pada kasus kelolaan utama yaitu PPOK eksaserbasi grup D dan 30 pasien resume menggunakan teori keperawatan self care Orem, teori ini digunakan karena masalah respirasi adalah masalah kronik, teori ini dapat diimplentasikan oleh perawat spesialis untuk meningkatkan perawatan diri pasien (self care) sesuai dengan kemampuannya untuk dapat meningkatkan kualitas hidup pasien. Implementasi EBN berupa balloon blowing exercise terbukti mampu mengurangi keluhan sesak napas pada pasien CLRD, mudah dilakukan, tidak memerlukan waktu lama, murah dan tidak menimbulkan efek samping. Penerapan proyek inovasi pengembangan self management: written action plan dengan media booklet terbukti dapat meningkatkan manajemen diri pasien PPOK. Booklet dapat membantu baik keluarga maupun pasien PPOK dalam mengetahui kondisi PPOK pasien serta melakukan manajemen yang tepat sesuai dengan kondisi pasien. Pengabdian masyarakat berupa edukasi kepada pasien TB Paru tentang TB Paru dan manajemen terapi pengobatannya terbukti dapat meningkatkan pengetahuan pasien, hal ini kemudian diharapkan dapat menurunkan angka putus obat pasien TB Paru.
The medical surgical nursing residency practice which was carried out for two semesters has provided nursing care to patients with respiratory disorders, implemented evidence based nursing and and innovation projects as well as community service carried out at Fatmawati Hospital and Friendship Hospital. The role of providing nursing care in the main managed cases, namely COPD exacerbation group D and 30 patients resumed using Orem's self care nursing theory, this theory is used because respiratory problems are chronic problems, this theory can be implemented by specialist nurses to improve patient self care according to its ability to improve the patient's quality of life. The implementation of EBN in the form of balloon blowing exercise is proven to be able to reduce complaints of shortness of breath in CLRD patients, is easy to do, does not take long, is inexpensive and does not cause side effects. The implementation of self-management development innovation projects: written action plans with booklet media has been proven to improve COPD patient self-management. Booklets can help both families and COPD patients in knowing the patient's COPD condition and carrying out appropriate management according to the patient's condition. Community service in the form of educating pulmonary TB patients about pulmonary TB and management of treatment therapy has been proven to increase patient knowledge, this is then expected to reduce the dropout rate of pulmonary TB patients."

Sebagai manajer kasus, residen mengelola kasus utama pasien tuberculosis dan diabetes mellitus, dan 30 kasus resume dengan menggunakan Teori Self-Care Orem. Masalah keperawatan utama yang paling sering muncul adalah ketidakefektifan bersihan jalan napas. Sebagai pemberi layanan, residen telah menerapkan intervensi keperawatan berdasarkan pembuktian yaitu oral hygiene dengan chlorhexidine 0,12% yang dapat mencegah terjadinya Ventilator Associated Pneumonia (VAP). Inovasi keperawatan yang dilakukan yaitu modifikasi Water Seal Drainage (WSD) 1 botol yang telah mendapatkan respon yang baik dari para ahli dibidangnya untuk meningkatkan keselamatan pasien dan keselamatan pekerja atau petugas kesehatan dalam proses asuhan keperawatan.ABSTRACT The purpose of medical-surgical nurse resident in respiratory system clinical practice is to perform three roles of a nurse specialist as a case manager, healthcare provider and innovator. "Praktik klinik residensi keperawatan medikal bedah pada pasien osteomielitis bertujuan untuk mengembangkan dan mengimplementasikan peran perawat spesialis sebagai pemberi asuhan keperawatan profesional, edukator, peneliti, dan pembaharu inovator. Peran pemberi asuhan keperawatan dilakukan dengan menggunakan pendekatan Teori Self Care Orem pada pasien Infected union of left tibia post debridement and bone graft application from iliac wing dan 30 pasien lainnya dengan berbagai gangguan sistem muskuloskeletal. Peran sebagai edukator dan peneliti dilakukan oleh perawat spesialis terhadap pasien dan juga rekan sejawat di tatanan pelayanan rumah sakit. Penerapan EBN (Evidence Based Nursing) yaitu dengan mengaplikasikan intervensi diaphragmatic breathing exercise pada pasien pasca operasi Total Knee Replacement untuk mengurangi nyeri dan kecemasan. Proyek inovasi juga dilakukan sebagai perwujudan peran sebagai pembaharu atau inovator yaitu melalui pelaksanaan berupa Pengkajian Risiko DVT skala Autar dalam rangka meningkatkan kualitas asuhan keperawatan profesional di rumah sakit.
The clinical practice of medical-surgical nursing residency in osteomyelitis patients aims to develop and implement the role of specialist nurses as professional nursing care providers, educators, researchers, and reformers innovators. The role of nursing care provider is carried out using the Orem Self Care Theory approach in patients with infected union of left tibia post debridement and bone graft application from iliac wing and 30 other patients with various musculoskeletal system disorders. The role of educator and researcher is carried out by specialist nurses to patients and also colleagues in hospital service settings. The implementation of EBN (Evidence Based Nursing) is by applying diaphragmatic breathing exercise interventions in postoperative Total Knee Replacement patients to reduce pain and anxiety. Innovation projects are also carried out as a manifestation of the role as a reformer or innovator, namely through the implementation of the Autar scale DVT Risk Assessment in order to improve the quality of professional nursing care in the hospital."

"ABSTRAK
Analisis praktik residensi keperawatan medikal bedah terdiri dari pengelolaan kasus gangguan sistem endokrin terutama diabetes melitus dengan pendekatan teori Self Care Orem, penerapan evidence based nursing dan melakukan proyek inovasi. Asuhan keperawatan menggunakan pendekatan teori self care berfokus untuk meningkatan kemampuan pasien dalam melakukan perawatan diri secara mandiri sehingga komplikasi lanjut dapat dicegah. Laporan ini bertujuan untuk menganalis penggunaan teori self care Orem dalam melakukan asuhan keperawatan pada pasien dengan diabetes melitus dengan menggunakan metode studi kasus dan literatur review. Hasil analisis didapatkan bahwa sebagian besar pasien diabetes melitus mempunyai masalah keperawatan ketidakstabilan glukosa darah, manajemen terapeutik tidak efektif, perluasan infeksi, dan gangguan keseimbangan nutrisi kurang dari kebutuhan tubuh. Intervensi keperawatan keperawatan yang dilakukan berfokus pada self care pasien diantaranya pengontrolan glukosa darah, manajemen diet, dan pengendalian infeksi. Teori self care Orem dapat diterapkan pada pasien diabetes melitus dalam memandirikan pasien dalam mengendalikan diabetesnya.
ABSTRACT
Analysis of medical surgical nursing residency practice consists of case management of endocrine system disorder especially diabetes mellitus with Orem Self Care theory approach, the application of evidence based nursing and innovation project. Nursing care using the self care theory approach focuses on improving the ability of patients in self-care self-care so that further complications can be prevented. This report aims to analyze the use of Orem self care theory in performing nursing care in patients with diabetes mellitus using case study and review literature methods. The analysis results obtained that most patients with diabetes mellitus have nursing problems of blood glucose instability, ineffective therapeutic management, extent of infection, and disturbance of nutrient balance less than body needs. Nursing care nursing interventions focused on the patient 39;s self-care such as blood glucose control, diet management, and infection control. Orem 39;s self care theory can be applied to diabetes mellitus patients in establishing patients in controlling their diabetes. "

"Analisis praktik residensi keperawatan medikal bedah merupakan karya ilmiah akhir ners spesialis yang menggambarkan pelaksanaan praktik spesialis keperawatan dalam meningkatkan kualitas pelayanan dan pendidikan berkelanjutan. Analisis ini terdiri dari asuhan keperawatan pada gangguan sistem perkemihan dengan kasus utama End Stage Renal Disease dan 30 kasus resume menggunakan pendekatan model adaptasi Roy, penerapan evidence-based nursing tentang latihan intradialisis Range of Motion untuk menurunkan fatigue dan hipertensi post hemodialisis, dan penerapan proyek inovasi tentang upaya pemenuhan durasi hemodialisis untuk mencapai adekuasi. Teori model adaptasi Roy bertujuan untuk mempertahankan, memodifikasi atau mengubah stimulus sehingga pasien dapat adaptif terhadap kondisi kesehatannya. End Stage Renal Disease merupakan salah satu kasus gangguan sistem perkemihan akibat kerusakan pada fungsi ginjal yang dapat menyebabkan komplikasi sistemik dan mengancam kehidupan serta kualitas hidup seseorang. Hipertensi menjadi salah satu penyebab tersering dari timbulnya kerusakan pada ginjal. Latihan ROM intradialisis dapat menurunkan level fatigue dan tekanan darah post hemodialisis yang dapat dilakukan pasien secara mandiri selama dialisis berlangsung. Pemenuhan durasi hemodialisis menjadi salah satu cara yang dapat dilakukan untuk meningkatkan bersihan ureum (Kt/V) sehingga adekuasi hemodialisis dapat tercapai.
Analysis of medical surgical nursing residency practice is the final scientific work of specialists describing the implementation of specialist nursing practice in improving the quality of services and continual education. This analysis consisted of nursing care in urinary system disorders with major cases of End Stage Renal Disease and 30 resume cases using Roy's adaptation model approach, application of evidence-based nursing about Range of Motion intradialysis exercises to reduce fatigue and post-hemodialysis hypertension, and implementation of innovations project about efforts to meet hemodialysis duration to achieve adequate. Roy's adaptation model theory aims to maintain, modify or change the stimulus so that patients can be adaptive to their health conditions. End Stage Renal Disease is one case of urinary system disorders due to damage the kidney function that can cause systemic complications and threaten the life and quality of life of a person. Hypertension is one of the most common causes of kidney damage. Intradialysis ROM exercises can reduce levels of fatigue and post hemodialysis blood pressure that patient can conduct independently during dialysis. Hemodialysis duration fullfillment is one of the ways that can be done to increase urea clearance (Kt / V) so that hemodialysis adequacy can be achieved."

Tujuan utama memberikan asuhan keperawatan secara komprehensif pada pasien kasus Spinal Cord Injury dengan trauma melalui pendekatan keperawatan model teori self-care Orem. Asuhan berupa pengelolaan 1 kasus kelolaan uatam dan 30 resume yang terdiri 3 bagian kasus besar yaitu kasus Spine, ekstremitas atas, dan ekstremitas atas dengan menggunakan teori model keperawatan Self-Care Orem yang menekankan tentang kebutuhan perawatan diri. Pencegahan terjadinya luka tekan dengan pemberian Aloe vera gel sebagai bagian dari tindakan mandiri keperawatan berdasarkan bukti penelitian ilmiah. Proyek inovasi berupa pengkajian resiko DVT dengan menggunakan Autar Scale adalah bentuk pengembangan dari intervensi keperawatan untuk pencegahan resiko terjadinya DVT pada pasien dengan kasus muskuloskletal. Tujuan seluruh kegiatan yang dilakukan selama kegiatan residensi adalah mewujudkan asuhan keperawatan yang komprehensif berdasarkan bukti ilmiah terkini.

The care is in the form of managing 1 main case and 30 resumes consisting of 3 major case sections namely the Spine, upper extremity and upper extremity cases using a theoretical model including Orem's Self Care who is depressed about the need for self-care. Prevention of pressure sores by administering Aloe Vera Gel as part of an independent act of kindness based on scientific research evidence. The innovation project in the form of DVT risk assessment using the Autar Scale is a form of developing interventions to prevent the risk of DVT in patients with musculoskeletal cases. The aim of all activities carried out during the residency activities is to realize comprehensive group care based on the latest scientific evidence."

Selain itu juga dilakukan penerapan praktek berbasis bukti dengan pengkajian 3 menit kaki diabetesdi poliklinik penyakit dalam diharapkan dapat menjadi skrining terhadap pencegahan terjadinya ulkus diabetik. Selain penerapan Evidence Based Nursing Practice juga dilakukan penerapan inovasi promosi kesehatan dengan tujuan untuk memandirikan pasien dalam melakukan perawatan diri sehari-hari dalam mencegah terjadinya komplikasi diabeti yang dilakukan dipoliklinik penyakit dalam RS. Ciptomangunkusumo, Jakarta. Diharapkan dengan penerapan asuhan keperawatan dengan menggunakan pendekatan Self Care Orem, serta penerapan pengkajian 3 menit kaki diabetik dan promosi kesehatan dapat menjadi acuan dalam penerapan asuhan keperawatan yang komprehensif dan terintegrasi, sehingga pada akhirnya diharapkan dapat meningkatkan kulaitas asuhan keperawatan.
Diabetes mellitus is a chronic disease in the endocrine system of the most prevalent in health care system. Nurses in providing nursing care can use various methods for approach , one of them with models of Orem Self Care approach.
That uses approach for diabetic patients through independent diabetes management. Nursing specialist of Medical-Surgical practice for endocrine conduct an analysis of the application models of Orem Self Care approach in the provision of nursing care activities. It also made the application of evidencebased practice with an assessment of the diabetic foot in 3 minutes in outpatient
with diabetic, with the aims to be screened foot problems for prevention of diabetic ulcers.
In addition to the application of evidence -based nursing practice is also carried
out the implementation of inovation prgramme health promotion. The purpose of
this programme is promote patient ability to be independent for perform daily
self-care and prevention from diabetic complication in Cipto Mangunkusumo
Hospital at Jakarta. The cxpectation of implementation of nursing care by using Orem Self Care Approach, 3 minutes diabetic foot assessment and health promotion can be a reference in the implementation of nursing care is comprehensive and integrated , which in turn is expected to improve the quality of nursing care ."

"Karya ilmiah akhir ini disusun untuk memberikan gambaran tentang pelaksanaan praktek residensi keperawatan medikal bedah yang telah dilaksanakan selama dua semester. Kegiatan utama yang telah dilaksanakan diantaranya memberikan asuhan keperawatan pada pasien dengan gangguan sistem perkemihan, praktek keperawatan berbasis pembuktian, dan inovasi keperawatan. Asuhan keperawatan yang dilakukan pada pasien dengan gangguan sistem perkemihan menggunakan konsep teori Self Care Orem.
Masalah keperawatan yang paling sering terjadi pada pasien dengan gangguan perkemihan diantaranya kelebihan volume cairan, intoleran aktifitas, ketidakseimbangan nutrisi kurang dari kebutuhan tubuh. Untuk mengatasi masalah tersebut telah dilakukan berbagai intervensi keperawatan berdasarkan sistem keperawatan sesuai dengan kemampuan pasien dalam memenuhi kebutuhannya. Penerapan praktek keperawatan berbasis pembuktian dilaksanakan dengan memberikan latihan fisik saat hemodialisis untuk meningkatkan adekuasi hemodialisis. Tujuan latihan fisik ini adalah untuk meningkatkan pengeluaran ureum yang merupakan zat toksik dalam tubuh, sehingga adekuasi dicapai secara optimal.
Hasil menunjukkan adekuasi hemodialisis meningkat sebesar 70,2 %. Inovasi keperawatan dilakukan dengan melaksanakan edukasi manajemen pasien hemodialisi dengan memberikan booklet pada pasien yang menjalani hemodialisis. Hasil program inovasi menunjukkan bahwa pengetahuan pasien meningkatkan. Dapat disimpulkan bahwa teori Self Care yang diterapkan pada pasien dengan gangguan perkemihan mengoptimalkan kemampuan pasien dalam mempertahankan kebutuhannya.
Disarankan agar teori Self Care Orem ini dapat diterapkan dalam memberikan asuhan keperawatan pada pasien dengan gangguan sistem perkemihan, latihan fisik saat hemodialisis dapat diberikan sebagai terapi keperawatan dalam meningkatkan adekuasi hemodialisis serta melakukan edukasi pada pasien untuk meningkatkan pengetahuannya sehingga kesehatan yang optimal dapat tercapai.
This final scientific work is made to provide an overview of the implementation of medical-surgical nursing practice residency which has been held for two semesters. The main activities have been implemented, including providing nursing care to patients with disorders of the urinary system, evidence-based nursing practice, and nursing innovation. Nursing care in patients with urinary system disorders using theoretical concepts Orem Self Care.
Nursing problems often occur in patients with urinary disorders, such excess fluid volume, activity intolerance, imbalance nutritional less, than body requirements. To overcome these problems have been made based nursing interventions nursing system, in accordance with the patient's ability to meet their needs. Application of evidence-based nursing practice implemented by giving physical exercise to improve the adequacy of current hemodialysis. The purpose of this exercise is to increase spending urea which is a toxic substance in the body, so that the adequacy achieved optimally.
Results indicate adequacy of hemodialysis increased by 70.2%. Nursing Innovation is done by carrying out management education hemodialysis patient by providing a booklet in patients undergoing hemodialysis. The results innovation program showed that the patient knowledge improves. It can be concluded that the theory of Self Care is applied in patients with urinary disorders, optimizing the patient's ability to sustain its needs.
It is suggested that the theory of Orem's Self Care can be applied in providing nursing care to patients with disorders of the urinary system, physical exercise while hemodialysis can be given as therapy to improve the adequacy of hemodialysis nursing and educating patients to improve knowledge so that optimum health can be achieved."
