"Comorbidity and complexity of cases seen in clinical work form a basis for discounting the applicability and generality of evidence-based treatments (EBTs). The authors evaluated treatment outcomes in 2 samples of clinically referred children who met criteria for oppositional defiant disorder (n = 183; 42 girls, 141 boys; ages 3-14) or conduct disorder (n = 132; 35 girls, 97 boys; ages 7-14) but varied in comorbidity (up to 5 additional disorders). In addition to comorbidity, 4 domains of case complexity were evaluated: scope and severity of child dysfunction, socioeconomic disadvantage, parent and family functioning, and barriers that emerged during treatment. Comorbidity was associated with greater therapeutic change. Children who varied in comorbidity did not differ on outcome measures at the end of treatment. Complexity was either unrelated or positively related to therapeutic change. As an exception, perceived barriers were associated with less child improvement, but, even with high barriers, effect sizes for these children were large. The findings suggest that comorbidity or complexity of cases does not necessarily influence outcome or limit the applicability of EBTs."

"Karya Ilmiah Akhir ini merupakan laporan akhir setelah praktik residensi peminatan Keperawatan Medikal Bedah. Laporan akan menguraikan satu kasus utama dari pengelolaan terhadap 31 kasus yang dikelola, Evidence Based Nursing Practice (EBNP) dan Inovasi. Perubahan signifikan didapatkan pada hasil penerapan asuhan keperawatan menggunakan model adaptasi Roy, pasien mampu beradaptasi lebih baik terhadap masalah yang dihadapi tanpa tergantung pada keberadaan stimulus sebagai pencetus masalah. EBNP yang akan dilaporkan adalah penggunaan teknik relaksasi untuk mengatasi nyeri dan kecemasan post operasi gangguan muskuloskeletal, dengan hasil tindakan ini efektif untuk menurunkan nyeri tetapi tidak menunjukkan penurunan besar untuk kecemasan. Kegiatan inovasi yang dikerjakan yaitu program discharge planning untuk pasien perioperatif muskuloskeletal memberikan hasil yang baik dalam menurunkan kecemasan dan kepatuhan terhadap prosedur yang dijalankan.

This is a final report of Medical Surgical Nursing Specialist practice. It contains the explanation of one case of 31 managed cases, Evidence Based Nursing Practice (EBNP) and Innovation project. There is a significance outcomes after application of the Roy adaptation model in nursing care practice, patients could adapt better to the problems independently to existence of stimuli as the problem precipitator. The reported EBNP was implementation of relaxation technique for reducing musculoskeletal post operative pain and anxiety, this intervention was effective to reduce pain but not showing significant effect for anxiety. The innovation project, a discharge planning program for musculoskeletal perioperative patients showing a good result in reducing anxiety and promoting compliance to the procedure.

"Karya ilmiah akhir ini merupakan laporan praktik residensi keperawatan medikal bedah yang telah dilaksanakan selama dua semester. Pelaksanaan praktik residensi meliputi praktikan sebagai pemberi asuhan keperawatan langsung, sebagai peneliti dimana melaksanakan tindakan keperawatan mandiri dengan basis pembuktian ilmiah serta melakukan proyek inovasi sebagai upaya pengembangan intervensi keperawatan. Asuhan keperawatan pasien yang mengalami gangguan sistem perkemihan menerapkan teori adaptasi Roy dikarenakan gangguan sistem perkemihan yang dikelola memunculkan perubahan perilaku pasien sehingga memerlukan pengelolaan asuhan keperawatan yang profesional agar pasien mampu beradaptasi dengan adaptif sesegera mungkin.
Masalah keperawatan yang sering ditemukan pada yaitu kelebihan volume cairan tubuh aktual/resiko, ketidakseimbangan nutrisi kurang, gangguan mobilitas fisik, kerusakan integritas kulit aktual, nyeri, cemas dan koping tidak efektif. Tindakan keperawatan berbasis pembuktian ilmiah terapi musik dalam menurunkan kecemasan pasien pre operatif urologi, hasil penerapan diperoleh terapi musik menurunkan kecemasan dengan rata-rata penurunan kecemasan 5,7.
Hasil proyek inovasi menunjukan secara umum pasien memiliki persepsi terhadap edukasi dengan booklet interaktif yang sudah dilakukan adalah sangat baik dan baik untuk digunakan. Disarankan model adaptasi Roy dapat diterapkan pada pasien dengan gangguan sistem perkemihan terutama penyakit kronis sistem perkemihan, terapi musik dapat dilakukan untuk menurunkan kecemasan pasien pre operatif urologi dan edukasi dengan booklet interaktif sebagai media penyampaian edukasi perawatan diri pada pasien yang menjalani hemodialisis rutin adalah tepat mengenai sasaran dan sesuai dengan yang dibutuhkan oleh pasien.

The purpose of this final scientific report is to describe series of activities in clinical practice residency in medical surgical nursing that had been conducted for 2 semesters. Three main activities of residency practician were providng of direct nursing care on urinary system disorders, implementing evidence based as a researcher and innovating projects as the development of nursing intervention efforts. Practician applied Roy's adaptation theory to manage nursing care of patients with urinary system disorders due to run led to changes in patient behavior that required professional management of nursing care so that the patient was able to adapted as soon possible.
The common nursing problems in patinet with urinary system disorders were actual/risk excess fluid volume, imbalanced nutrition: less than body requirements, impaired physical mobility, actual/risk impaired skin integrity, pain, anxiety and ineffective coping. The implemntation of evidence-based nursing practice that music therapy in reducing preoperative anxiety urology patients, the result that grouping following music therapy reduced anxiety score averages between the grouping with an average decline of 5,7. The innovation projects was used interactive booklet for education patient undergoing on hemodialysis.
The result of innovation showed generally the perception of patient was very well and good to be used interactive booklet for education to improved management self. Suggested Roy adaptation model can be applied to patients with chronic urinary diseases especially chronic kidney disease, music therapy can be done to reduce preoperative anxiety urology and interactive booklet is the right media to educate patients undergoing hemodialysis.
