"As a vital part of patient care delivery, patient safety culture contributes to the quality of care provided by nurses. Safe patient care is positively linked to the attitudes of nurses. This study aimed to assess the perception of nurses working in a newly established teaching hospital. A cross-sectional study involving 194 nurses from three different units was conducted by using a 24-item Hospital Survey of Patient Safety Culture. Data on gender, working unit, age, years of working, and attendance in workshops on patient safety were also collected. The majority of the nurses had a positive total score of patient safety culture. The lowest score was 76 (63%), and the highest score was 120 (96%). The awareness on patient safety culture significantly differed between gender, years of working, and working units. Post-hoc comparisons using Tukey’s HSD test yielded a significant difference between nurses from critical care units and those from medical and surgical units. The mean score and total positive score on awareness on patient safety culture of the former were higher than those of the latter. Overall, the majority of the staff nurses in International Islamic University Malaysia Medical Center had a positive total score on awareness on patient safety culture. Awareness on patient safety, which is considered crucial worldwide, should be enhanced to influence the development of a positive patient safety culture within hospitals. This implementation would directly develop high-quality care to patients and positively impact health organizations."

"No blaming culture merupakan bagian dari budaya keselamatan pasien. Penelitian ini bertujuan untuk mengidentifikasi hubungan no blaming culture dengan pelaksanaan keselamatan pasien oleh perawat di instalasi rawat inap, RSUP Fatmawati Jakarta. Metode penelitian ini menggunakan design deskriptif korelasi dengan pendekatan cross sectional. Sampel penelitian berjumlah 152 perawat, pemilihan sampel dilakukan dengan consecutive sampling.
Instrumen penelitian terdiri dari 3 bagian kuisioner yaitu kuisoner data demografi, kuisioner no blaming culture dan kuisioner pelaksanaan keselamatan pasien. Analisis data menggunakan uji chi-square, yang menunjukkan ada hubungan yang bermakna antara no blaming culture dengan pelaksanaan keselamatan pasien oleh perawat di rumah sakit (p=0,003, CI: 1,376-5, 118).
Penelitian ini merekomendasikan penerapan no blaming culture di RS dan diharapkan penelitian selanjutnya akan mengidentifikasi hal-hal yang dapat meningkatkan pelaporan kesalahan oleh di RS.

No blaming culture is a part of patient safety culture. This study aims to determine the relationship of no blaming culture with patient safety implementation by nurses at inpatient installation in Fatmawati Hospital Jakarta. This study uses descriptive correlation design withcross-sectional. Sample was 152 nurses, which is selected by consecutive sampling.
This study uses 3 questionnaire namely demographic questionnaire, patient safety implementation questionnaire and no blaming culture questionaire. This study showed there was significant correlation between no blaming culture with patient safety implementation by nurses in hospitals (p = 0.003, CI 1,376-5,118).
This study recommended the implementation of ~uu blaming culture in hospital to increase patient safety and for further research it is expected to identify the way to improve error reporting by nurse.

"Penerapan keselamatan pasien di Rumah Sakit Setia Mitra belum berjalan dengan optimal maka perlu pengukuran budaya keselamatan pasien tertutama di kalangan perawat. Penelitian ini bertujuan untuk mengetahui gambaran persepsi perawat tentang budaya keselamatan pasien dan bagaimana penerapan sasaran keselamatan pasien di Rumah Sakit Setia Mitra. Penelitian deskriptif ini dilakukan dengan pendekatan kuantitatif dan kualitatif. Populasi adalah seluruh perawat yang ada di Rumah Sakit Setia Mitra yaitu 77 perawat dengan sampel 68 perawat.
Hasil penelitian menunjukkan persepsi perawat tentang budaya keselamatan pasien di Rumah Sakit Setia Mitra yaitu 72.1% baik. Dimensi paling banyak dipersepsikan baik yaitu dimensi operan sebesar 75% dan paling sedikit dipersepsikan baik adalah dimensi dukungan manajemen sebesar 1.5%. Secara statistik usia (p value = 0.048), masa kerja (p value = 0.016) dan jabatan perawat (p value = 0.049) memiliki hubungan dengan persepsi perawat terhadap budaya keselamatan pasien (p<0.05).
Pada penerapan sasaran keselamatan pasien yaitu 55.15% memahami dengan baik bagaimana penerapan sasaran keselamatan pasien di Rumah Sakit Setia Mitra. Disarankan agar tim keselamatan pasien yang ada lebih dioptimalkan tugas dan fungsinya sehingga budaya keselamatan pasien di rumah sakit dapat terus ditingkatkan.

It is necessary to measure patient safety culture especially among nurses. This study aims to describe the nurses perception about patient safety culture and how the implementation of patient safety goals at Setia Mitra Hospital. This descriptive research conducted with quantitative and qualitative approaches. Population in this study is all nurses in Setia Mitra hospital that consist of 77 nurses with sample 68 nurses.
The result of study shows the nurse's perception about patient safety in Setia Mitra hospitals is 72.1% good. Dimension with the most good nurse perception is hospital handoffs and transitions dimension by 75% and at least perceived good is management support dimension of 1.5%. Statistically, age (p value = 0.048), work period (p value = 0.016) and the level of nurses (p value = 0.049) have a relationship with the nurses perception on patient safety culture (p <0.05).
In the implementation of patient safety goals in Setia Mitra Hospital, namely 55.15% with a good understanding. It is recommended that patient safety team in the Setia Mitra Hospital further optimized its duties and functions so that the patient safety culture in hospitals can be improved.

"Keselamatan pasien merupakan hal yang sangat mendasar dalam pelayanankepada pasien di rumah sakit. Sebagai langkah awal dalam upaya meningkatkankeselamatan pasien di rumah sakit adalah dengan mengukur budaya keselamatanpasien. Tujuan penelitian ini adalah untuk mengetahui hubungan kepemimpinantransformasional, kerjasama tim, dan kesadaran individual dengan budaya keselamatanpasien di Rumah Sakit Prima Medika RSPM Denpasar. Metode yang digunakanadalah metode campuran mix method. Pendekatan kuantitatif menggunakan jenispenelitian potong-lintang cross-sectional. Kuesioner dibagikan kepada sampelsebanyak 218 responden. Pada pendekatan kualitatif dilakukan penelusuran lebih lanjutterhadap hal-hal yang dirasa masih belum terjawab, untuk melengkapi penjelasan hasilpenelitian kuantitatif.
Analisis statistik menggunakan Structural Equation Modelling SEM, dengan program STATA-SE 12.1. Wawancara mendalam dilakukan denganDireksi RSPM dan pegawai yang terkait, untuk konfirmasi hasil penelittian. Hasilanalisis menunjukkan variabel independen yang saling berhubungan yaituKepemimpinan Transformasional, Kesadaran Individual, dan Kerjasama Tim, danketiganya berhubungan secara bemakna dengan variabel dependen BudayaKeselamatan Pasien p.

Patient safety is very basic in the service to patients in the hospital. As a firststep in improving patient safety in hospitals is by measuring the patient 39 s safety culture.The purpose of this research is to know the relationship of transformational leadership,teamwork, and individual awareness to patient safety culture at Prima Medika Hospital RSPM Denpasar. The method used is mix method. The quantitative approach usescross sectional research. Questionnaires were distributed to a sample of 218respondents. In a qualitative approach, further searches on things that remainunanswered, to complement the explanation of the results of quantitative research.
Statistical analysis using Structural Equation Modeling SEM, with STATA SE 12.1program. In depth interviews were conducted with the RSPM Board of Directors andrelevant employees, to confirm the results of the study. The results of the analysis showthat the independent variables are Transformational Leadership, Individual Awareness,and Team Cooperation, and all three are related significantly to the dependent variableof Patient Safety Culture p.

"Keselamatan dan kepuasan pasien merupakan indikator kualitas pelayanan belum terpenuhi secara optimal. Model budaya belajar perawat membentuk perawat kompeten dalam penerapan sasaran keselamatan pasien. Penelitian bertujuan mengidentifikasi budaya belajar tentang keselamatan pasien, mengembangkan model budaya belajar perawat dan mengidentifikasi pengaruhnya terhadap kompetensi perawat, keselamatan dan kepuasan pasien. Desain operasional riset, tahap pertama kualitatif fenomenologi dengan sembilan partisipan, tahap kedua pengembangan model, dan tahap ketiga desain quasi eksperiment pre-post test design with control group dengan 139 responden perawat dan 240 responden pasien.
Hasil penelitian tahap pertama didapat sembilan tema yaitu arti keselamatan, persiapan penerapan, fokus keselamatan, implemetasi, masalah, tahapan budaya belajar, aktivitas pembelajaran dan dukungan pengembangan budaya belajar. Tahap kedua diperoleh model budaya belajar "SAYA perawat" tentang sasaran keselamatan pasien dengan komponen, perawat; Sistem dukungan, aktivitas pembelajaran dan tahapan budaya belajar. Tahap ketiga model budaya belajar "SAYA Perawat" tentang sasaran keselamatan pasien berpengaruh secara bermakna terhadap kompetensi perawat dalam penerapan sasaran keselamatan pasien (p 0,0001), keselamatan (p 0,0001) dan kepuasan pasien (p 0,0001). Terdapat hubungan bermakna kompetensi perawat dalam penerapan keselamatan pasien dengan keselamatan (p 0,0001) dan kepuasan pasien (p 0,0001). Model budaya belajar ini agar dapat diimplementasikan di ruang perawatan untuk memenuhi keselamatan dan kepuasan pasien.

Safety and patient satisfaction which are indicators of quality cannot be optimaly fulfilled. Learning culture models about patient safety form nurses competent in the application of patient safety. This study aims to identify nurses learning culture about patient safety, develop a model of nurse learning culture and identify its affect on nurses competency, patient safety and satisfaction. The research design used operational research, the first phase used qualitative phenomenology with nine participants, the second stage is the development of the model, and the third stage is the quasi experimental design pre-post test design with control group with 139 nurses and 240 patient respondents.
The results of the first phase of research obtained nine themes, namely the meaning of safety, preparation for application, focus on safety, implementation, problems, stages of learning culture, learning activities and support for the development of a learning culture. The second stage was obtained learning culture models "SAYA Nurses" about patient safety goals is fit and valid there have components, nurses as the subject of development; support systems, learning activities and the learning culture stage . The third stage of the learning culture model "SAYA Nurses about patient safety goals has a significant effect on nurses' competence (p 0,0001), patient safety (p 0,0001) and satisfaction (p 0,0001). There is a significant relationship between nurses' competence with patient safety (p 0,0001) and satisfaction (p 0,0001). The learning culture models to be implemented in the ward to safety and satisfaction.

"Tesis ini membahas mengenai sejauh mana budaya keselamatan pasien pada residen. Desain penelitian cross sectional dengan analisis mixed method. Subjek penelitian seluruh residen dengan instrumen kuisioner HSOPS. Hasil penelitian menyimpulkan dimensi budaya keselamatan pasien yang paling rendah adalah dimensi respon tidak menghukum terhadap kesalahan (18%). 12 dimensi keselamatan pasien tidak dipengaruhi oleh jenis kelamin dan tingkat kelas residen (p> 0,05), tetapi sebagian besar dipengaruhi oleh asal departemen.

The tesis focus about the patient safety culture among the resident. This is a cross sectional design research with mix method analysis where all the residen of Medical Faculty of Sriwijaya University as the research subject with HSOPS quesionaire as an instrument. The lowest culture dimension is nonpunitive response to errors (18%). The twelve dimension of patient safety did not influenced by the gender and class level of resident (p>0.05), but majority influenced by the departement catagory"

"Keselamatan pasien merupakan hal yang baru dalam pelayanan kesehatan. Akreditasi Rumah Sakit versi 2012 mewajibkan keselamatan pasien sebagai prinsip utama pelayanan kesehatan di Rumah Sakit. Rumah Sakit Gigi dan Mulut, meskipun berbeda dari Rumah Sakit lainnya, tetap diwajibkan untuk menerapkan Akreditasi Rumah Sakit versi 2012. Penelitian ini untuk mengetahui gambaran iklim keselamatan pasien, tindakan manajemen dan kinerja keselamatan pasien menurut dokter gigi di RSGM X Kota Cimahi. Dengan mengambil satu populasi dokter gigi RSGM X sebagai data penelitian, didapatkan hasil yakni persepsi keselamatan pasien di RSGM X yang rendah dikarenakan persepsi iklim keselamatan pasien, tindakan manajemen dan sasaran keselamatan yang rendah. Diharapkan institusi dapat memberikan pelatihan secara berkala.

Patient safety is a new thing in healthcare. Indonesia National Hospital Accreditation 2012 version requires that patient safety as a key principle of healthcare at the Hospital. Dental Hospital, though different from other hospitals, was still required to implement the Indonesia National Hospital Accreditation version 2012. This study was to describe the climate of patient safety, action management and patient safety peception of dentist in RSGM X Cimahi. By taking RSGM X dentist population as research data, showed that patient safety perception in RSGM X is low because of low patient safety climate, management actions and patient safety goals. It is expected that institutions can provide regular training of patient safety."

"Budaya keselamatan pasien merupakan pondasi utama dalam pelaksanaan keselamatan pasien. Penelitian ini bertujuan untuk mengetahui pengaruh pemberdayaan champion keselamatan pasien terhadap penerapan budaya keselamatan pasien di Rumah Sakit Haji Jakarta.
Metode penelitian ini menggunakan pre-experiment design: pretest-posttest without control group design, sampel yang digunakan 81 perawat. Data analisis dengan menggunakan Mc Nemar.
Hasil menunjukkan terdapat peningkatan persentasi penerapan budaya keselamatan pasien setelah program pemberdayaan champion keselamatan pasien dengan pengaruh yang tidak bermakna secara statistik (p= 0,451; CI= 0.084-0.928).
Penelitian ini merekomendasikan perlunya pengembangan champion keselamatan pasien dengan memperhatikan kriteria kelayakan sebagai champion keselamatan pasien serta membangun program pemberdayaan champion keselamatan pasien berkelanjutan sesuai kebutuhan rumah sakit.

The culture of patient safety is the main foundation in the implementation of patient safety. This study aimed to determine the influence of the patient safety champion empowerment on the application of patient safety culture in Jakarta Hajj Hospital.
This research method used pre-experimental design: pretest-posttest without control group design, the sample was 81 nurses. Data were analyzed using Mc Nemar test.
The results showed there was an increase in the percentage of the implementation of a patient safety culture after the program of patient safety champions empowerment had been implemented, which was not statistically significant (p = 0.451; CI = 0084-0928).
It is recommended to develop the patient safety champions by taking into account the eligibility criteria as a patient safety champion and to build a sustainable program of patient safety champion empowerment that suitable to the needs of the hospital.

"Budaya keselamatan memiliki peran penting dalam mewujudkan pelayanan keperawatan yang aman bagi pasien. Masih ditemui masalah terkait budaya keselamatan pasien dan sikap pelaporan insiden keselamatan pasien. Budaya keselamatan pasien dapat berhubungan dengan sikap perawat dalam pelaporan insiden. Penelitian ini bertujuan mengetahui hubungan budaya keselamatan pasien dengan sikap perawat dalam pelaporan insiden keselamatan pasien. Penelitian Cross Sectional menggunakan cluster sampling ini dilakukan dengan pengisian kuesioner yang melibatkan 400 perawat di tiga rumah sakit umum daerah di tiga kabupaten Derah Istimewa Yogyakarta.
Hasil didapatkan adanya hubungan yang signifikan antara budaya keselamatan pasien dengan sikap perawat dalam pelaporan insiden keselamatan pasien p=0,005 . Hasil regresi linear menunjukkan variabel yang paling mempengaruhi sikap pelaporan secara berurutan yaitu jabatan, budaya keselamatan pasien, level kompetensi, masa kerja, dan usia perawat R2=0,892.
Kesimpulan adalah bahwa budaya keselamatan pasien memiliki peran penting terkait sikap perawat dalam pelaporan insiden keselamatan pasien, upaya untuk memperkuat budaya keselamatan pasien dapat memperbaiki sikap perawat dalam pelaporan insiden keselamatan pasien.
Rekomendasi yang diberikan yaitu perbaikan pengaturan staf di rumah sakit, penyelenggaraan pelatihan atau diskusi rutin sebagai tindak lanjut dari pelaporan insiden, menghilangkan budaya menyalahkan terkait pelaporan insiden, memberikan apresiasi kepada perawat yang bersedia melaporkan insiden, menumbuhkan budaya saling mendukung antar perawat dalam pelaporan insiden keselamatan pasien.

Safety culture has important role in realizing a safe nursing service for patients. Problems related to patient safety culture and patient safety incident reporting are still encountered. The safety culture of the patient may relate to the nurse 39 s attitude in incident reporting. This study aims to determine the relationship of patient safety culture with the attitude of the nurses in reporting patient 39 s safety incidents. Cross sectional study using cluster sampling was conducted by filling a questionnaire involving 400 nurses at three regional public hospitals in three districts in the province of Yogyakarta special region.
The result shows that there is a significant correlation between patient safety culture and nurse attitude in reporting patient 39 s safety incident p 0,005 . Linier regression result shows consecutively that their position, patient safety culture, level of competence, year of service and age affect their attitude in reporting an accident R2 0,892.
The conclusion is that the patient safety culture has an important role in the nurse 39 s attitude in reporting the patient 39 s safety incident, efforts to strengthen the patient 39 s safety culture could improve the nurse 39 s attitude in reporting the patient 39 s safety incident.
Recommendations include improvements in hospital staffing, regular training or regular discussions as a follow up to incident reporting, eliminating a culture of incident reporting error, giving appreciation to nurses willing to report incidents, fostering a mutually supportive culture among nurses in reporting patient safety incidents.

"Pengembangan Budaya Keselamatan Pasien di RSUD H Abdul Manap Kota Jambi Salah satu pendekatan untuk meningkatkan keselamatan pasien adalah membangun budaya keselamatan pasien. Budaya keselamatan pasien sendiri merupakan salah satu aspek dari budaya organisasi, karena itu, untuk mengembangkannya diperlukan pengkajian budaya organisasi agar terjadi perubahan yang mendorong upaya peningkatan keselamatan pasien. Penelitian ini bertujuan menganalisis budaya keselamatan pasien dan mengidentifikasi profil organisasi untuk menentukan langkah strategis pengembangan budaya keselamatan pasien di RSUD H. Abdul Manap Kota Jambi. Penelitian cross-sectional, mix method dengan survei kepada sebanyak 190 tenaga klinis yang langsung berhubungan dengan pasien yaitu tenaga medis, tenaga keperawatan dan tenaga penunjang pelayanan medis dengan menggunakan kuesioner HSOPSC (Hospital Survey on Patients Safety Culture) yang dikembangkan oleh AHRQ (Agency for Healthcare Research and Quality) dalam mengukur Budaya Keselamatan Pasien dan OCAI (Organization Culture Assessment Instrument) untuk mengidentifikasi profil budaya organisasi. Dilakukan analisis korelasi antara kedua temuan. Dilakukan pula Focus Group Discussion (FGD) untuk mengidentifikasi hambatan dan harapan dan kemudian dibahas dalam Consencus Decission Making Group (CDMG) jajaran manajemen sebagai kesepakatan tentang rencana tindak lanjut. Pembelajaran organisasi dan perbaikan berkelanjutan dan kerjasama dalam unit merupakan dimensi budaya terkuat sementara staffing, respons non-punitive terhadap kesalahan terlapor, frekuensi pelaporan kejadian dan jumlah kejadian yang dilaporkan merupakan dimensi budaya kurang yang perlu mendapatkan intervensi. Budaya Clan adalah jenis budaya organisasi yang paling dominan, tetapi bukan merupakan budaya yang kuat karena selisih skor dengan budaya Hierarki hanya satu (1) poin. Hasil korelasi antara dimensi pada budaya keselamatan menunjukkan adanya hubungan positif bermakna. Sedangkan korelasi antara tipe budaya organisasi dengan dimensi budaya keselamatan pasien menunjukkan hasil yang bervariasi. Strategi mutu budaya Clan dan budaya Hierarki dipakai sebagai panduan untuk perubahan dalam pengembangan budaya keselamatan pasien di RSUD H. Abdul Manap. Secara keseluruhan budaya keselamatan pasien termasuk dalam kategori budaya sedang, dimensi budaya pelaporan merupakan yang terlemah. Upaya pengembangan budaya keselamatan pasien memerlukan komitmen pimpinan, pemberdayaan staf dan pengembangan sistem. Diperlukan penelitian lebih lanjut untuk menganalisis berbagai faktor dalam pengembangan budaya pelaporan.

Building Patient safety culture is the first step in the improvement of patient safety. As patient safety culture is one aspect of organizational culture, therefore, to develop it, an assessment of organizational culture is needed to make changes that encourage efforts to improve patient safety. This study aims to analyze the patient safety culture and identify organizational profiles to determine the strategic steps of developing a patient safety culture at H. Abdul Manap Hospital in Jambi City. Methods: This study was cross-sectional, mix method. A survey was conducted on 190 clinical staff who were directly faced to patients namely medical doctors, nursing staffs and medical service support personnels using the HSOPSC (Hospital Survey on Patient Safety Culture) questionnaire developed by AHRQ (Agency for Healthcare Resesarch and Quaity) and OCAI (Organization Culture Assessment Instrument). Correlation analysis between the two findings was carried out. Focus Group Discussion (FGD) was carried out to identify obstacles and expectations in the implementation of patient safety culture, and its results will be discussed in the managements Consensus Decisions Making Group as an agreement that will be taken as an action plan Results: Organizational learning and continuous improvement and Teamwork within units are the strongest cultural dimensions while staffing, non-punitive response to errors, frequency of events reported and “number of events reported are dimensions of culture that need to be intervened. Clan culture is the most dominant type of organizational culture, but it is not a strong culture because the difference in scores with the Hierarchy culture was only one (1) point. The results of the correlation between the dimensions of safety culture indicate a significant positive. While the correlation between the type of organizational culture and the dimensions of patient safety culture, shows varying result. The quality strategy of Clan culture and hierarchical culture are used as a guide for changes in the development of a patient safety culture at H. Abdul Manap Hospital. Conclusion: In general, the patient safety culture belongs to the moderate level. The reporting culture becomes the weakest dimensuons. A managements commitment and staff empowerment as well as system development are all needed on the development of a patient safety culture. Further research is required to investigate various factors to develop the reporting culture."
