"Praktik kerja profesi apoteker ini dilakuakn di Rumah Sakit Universitas Indonesia dan Apotek Roxy Sawangan. Laporan Praktik Kerja Profesi Apoteker ini memberikan informasi mengenai peran apoteker dalam kegiatan pelayanan kefarmasian di rumah sakit dan apotek. Apoteker di rumah sakit dan apotek berperan penting dalam memberi pelayanan kepada pasien yang berkaitan dengan sediaan farmasi untuk dapat mencapai hasil yang pasti untuk meningkatkan mutu kehidupan pasien. Selain itu, apoteker bertanggung jawab dalam kegiatan pengelolaan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai, serta bertanggung jawab dalam memberikan pelayanan farmasi klinik kepada pasien. Laporan praktik kerja ini disusun menggunakan metode pengkajian, pengamatan secara langsung, dan studi literatur. Hasil dari kegiatan ini menunjukkan bahwa apoteker di rumah sakit turut berperan penting dalam pelayanan sediaan kefarmasian untuk pasien rawat inap dan dapat dievaluasi menggunakan waktu tunggu pelayanan resep sesuai dengan standar yang ditetapkan. Sedangkan hasil dari kegiatan praktik kerja di apotek, menunjukkan bahwa apoteker berperan dalam melakukan skrining resep secara administrasi, farmasetik, dan klinis. Hal tersebut bertujuan untuk memberikan pelayanan secara maksimal kepada pasien dan mencegah adanya kesalahan dalam pemberian terapi.

This pharmacist profession practice is carried out at the University of Indonesia Hospital and Roxy Sawangan Pharmacy. This Pharmacist Professional Practice Report provides information on the role of pharmacists in pharmaceutical service activities in hospitals and pharmacies. Pharmacists in hospitals and pharmacies play an important role in providing services to patients related to pharmaceutical preparations to be able to achieve definite results to improve the quality of life of patients. In addition, pharmacists are responsible for the management of pharmaceutical preparations, medical devices, and medical consumables, and are responsible for providing clinical pharmacy services to patients. This work practice report was prepared using the method of assessment, direct observation, and literature study. The results of this activity indicate that pharmacists in hospitals play an important role in the service of pharmaceutical preparations for inpatients and can be evaluated using the waiting time for prescription services in accordance with established standards. Meanwhile, the results of practical work activities in pharmacies indicate that pharmacists play a role in screening prescriptions in administration, pharmaceutical, and clinical ways. It aims to provide maximum service to patients and prevent errors in therapy."

"Formularium Rumah Sakit merupakan daftar Obat yang disepakati staf medis, disusun oleh Komite/Tim Farmasi dan Terapi (KFT) yang ditetapkan oleh Pimpinan Rumah sakit. Penyusunan Formularium Rumah Sakit mengacu kepada Formularium Nasional, panduan praktik klinis Rumah sakit serta mempertimbangkan hasil evaluasi penggunaan obat di Rumah Sakit. Rumah Sakit Universitas Indonesia sendiri telah memiliki Formularium dan dapat dijadikan sebagai bahan belajar bagi calon apoteker tentang manajemen obat di Rumah Sakit. Sama hal nya dengan manajemen di Rumah Sakit, Apotek juga memiliki manajemen dalam pengelolaannya. Apotek yang berkembang dan maju tidak lepas dari manajemen pengelolaan obat yang baik dan terkendali. Perencanaan dan pengadaan obat merupakan kunci utama dan tahap awal yang penting dalam menentukan keberhasilan suatu apotek. Analisis Pareto ABC merupakan salah satu cara pengendalian persediaan dengan mengelompokkan persediaan menjadi 3 klasifikasi berdasarkan nilai investasi barang yaitu kelompok A, B dan C. Sistem perencanaan dan pengadaan obat di Apotek Roxy Sawangan selama ini belum menggunakan metode analisa pareto ABC namun menggunakan metode konsumsi data penjulan obat harian. Hal tersebutlah yang kemudian dianalisa serta dituangkan kedalam laporan kerja praktik profesi apoteker ini.

The University of Indonesia Hospital itself has a formulary and can be used as learning material for prospective pharmacists about drug management in hospitals. the same thing with management in hospitals, pharmacies also have management in their management. Pharmacies that develop and progress cannot be separated from good and controlled drug management. Planning and procurement of drugs is the main key and an important initial stage in determining the success of a pharmacy. Pareto ABC analysis is one way of controlling inventory by grouping inventory into 3 classifications based on the investment value of goods, namely groups A, B, and C. The planning and procurement system for drugs at Roxy Sawangan Pharmacy has not used the Pareto ABC analysis method but uses the sales data consumption method for daily medicines. Then analyzed and poured into this report on the internship pharmacist's professional."

Praktik Kerja Profesi Apoteker di PT Anugerah Pharmindo Lestari bertujuan untuk mempelajari kegiatan distribusi obat dan alat kesehatan dalam ruang lingkup Pedagang Besar Farmas (PBF), mempelajari peran dan fungsi apoteker di PBF secara professional, dan secara khsus melakukan identifikasi permasalahan pada proses komplain dan retur produk kemudian mengembangkan inovasi yang mendukung proses komplain dan retur pada aplikasi eZRx PT Anugerah Pharmindo Lestari. Praktik Kerja Profesi Apoteker di Apotek Roxy Jatibaru bertujuan untuk mempelajari kegiatan pelayanan kefarmasian di apotek, mempelajari peran dan fungsi apoteker di apotek, dan secara khusus menganalisis penyimpangan dalam penyimpanan obat berdasarkan aspek farmakologi di Apotek Roxy Jatibaru.

Pharmacist Internship at Roxy Jatibaru Pharmacy aims to study the activities of pharmaceutical services in pharmacies, study the roles and functions of pharmacists in pharmacies, and specifically analyze deviations in drug storage based on pharmacological aspects at Roxy Jatibaru Pharmacy."

"Waktu tunggu pelayanan obat (racikan dan non racikan) dan kehilangan penjualan obat atau obat lost sales dapat mempengaruhi kepuasan pasien/pelanggan. Indikator ini termasuk dalam standar pelayanan minimal (SPM) farmasi rumah sakit yang diatur dalam Permenkes No. 129 tahun 2008. Rumah Sakit Universitas Indonesia (RSUI) memiliki salah satu pelayanan yaitu depo (Departemen Pengambilan Obat) yang memberikan pelayanan BPJS Kesehatan. Pengkajian evaluasi data waktu tunggu dan obat lost sales pasien BPJS di Depo rawat jalan RSUI dalam periode 3 bulan bertujuan untuk melatih kemampuan dan keterampilan para calon apoteker agar dapat meningkatkan juga menjaga kualitas SPM sebagai standar mutu pelayanan sehingga dapat mencegah kerugian finansial rumah sakit serta penurunan kepuasan pelanggan. Seluruh data dikaji dengan penelitian observasional yang bersifat deskriptif dan hasilnya disesuaikan dengan kriteria SPM. Dari evaluasi ini diketahui jika waktu tunggu pelayanan obat non racikan tidak memenuhi standar SPM tetapi obat racikan memenuhi dan tingkat obat lost sales pasien BPJS sebanyak 69% yang disebabkan oleh peresepan yang tidak sesuai dengan Formularium Nasional (Fornas). Pentingnya kualitas SPM di rumah sakit sama halnya dengan di apotek salah satunya yaitu Apotek Roxy Pondok Labu. Di apotek ini penggunaan obat yang banyak dibeli pasien salah satunya adalah obat antidiabetik. Berdasarkan analisa penggunaan obat antidiabetik periode september 2021 dengan cara pengkajian yang sama seperti sebelumnya diketahui bahwa penggunaan obat terbanyak dipegang oleh obat DM (Diabetes Melitus) golongan guanida yang termasuk obat generik dosis tunggal yaitu metformin 500 mg sebanyak 8.770 tablet dengan persentase 51,49 % dengan tingginya penggunaan obat jenis ini maka perlu ketersedian barang yang banyak untuk menghindari terjadinya kekosongan stok yang dapat menyebabkan terjadinya lost sales.

Respond time drug services (prescription and non prescription) and lost drug sales can affect patient/customer satisfaction. This indicator is included in the hospital pharmacy Minimum Services Standard (MSS) which is regulated in Permenkes number 129 of 2008. The University of Indonesia Hospital (RSUI) has one service, namely a Depot (Drug Collection Department) which provides BPJS Health services. The study of evaluation of respond time and lost drug sales of BPJS patients at outpatient depots RSUI for a period of 3 months aims to train the abilities and skills of prospective pharmacists in order to improve and maintain the quality of MSS as a service quality standard so as to prevent hospital financial losses and decrease customer satisfaction. All data were examined by observational research method by descriptive and the results were adjusted according to the MSS criteria. From this evaluation, it is known that the respond time for non prescription drugs does unqualified the MSS standards but the prescription drugs qualified and the level of drug lost sales for BPJS patients is 69% due to prescribing that is not in accordance with the National Formulary (Fornas). The importance of the quality level of MSS in hospitals is the same as in pharmacies, one of which is the Roxy Pondok Labu Pharmacy. In this pharmacy, the use of drugs that many patients buy is antidiabetic drugs. Based on the analysis of the use of antidiabetic drugs for the period of September 2021, with same method as before, it was found that the most drug use was held by DM (Diabetes Melitus) drugs of the guanide group, which included single dose generic drugs, namely metformin 500 mg as many as 8.770 tablets with a percentage of 51.49% with a high number of drug use. With this type, it is necessary to have a lot of availability to avoid stock emptiness which can cause lost sales."

"Telah dilakukan penelitian sebagai tugas khusus dalam Praktik Kerja Profesi Apoteker. Penelitian di RS Universitas Indonesia yaitu mengevaluasi penggunaan obat pasien Jaminan Kesehatan Nasional (JKN) di instalasi farmasi rawat jalan. Hasil didapatkan dari metode analisis ABC, dan disimpulkan bahwa pada pemakaian obat peserta JKN di rawat jalan terdapat 19 obat Fast Moving, 626 obat Slow Moving, dan 384 obat-obat dead stock. Praktik kerja di BPOM tentang peran apoteker khususnya di Direktorat Pengawasan Obat Tradisional dan Suplemen Kesehatan, yaitu melakukan pengawasan terhadap informasi dan promosi, fasilitas produksi dan distribusi, serta keamanan dan mutu dari obat tradisional dan suplemen kesehatan. Penelitian di Apotek Roxy Sawangan yaitu menganalisis data penjualan obat antihipertensi periode bulan Mei – Juli 2021. Hasil didapatkan bahwa amlodipine 5 mg merupakan antihipertensi dengan penjualan tertinggi. Penelitian di PT. Anugerah Pharmindo Lestari yaitu penambahan fitur pendaftaran klien pada aplikasi eZRX sebagai pengembangan digitalisasi yang dapat mempermudah klien maupun staff dalam proses pendaftaran.

Research has been carried out as a special task in the Pharmacist Professional

Practice. Research at the Indonesia University Hospital is to evaluate the use of drugs for outpatients pharmacy installations with the National Health Insurance (JKN). The results were obtained from the ABC analysis method, and it was concluded that in the outpatient use of JKN participants' drugs there were 19 fast moving , 626 slow moving, and 384 dead stock of drugs. Work practices at BPOM regarding the role of pharmacists, especially in the Directorate of Supervision of Traditional Medicines and Health Supplements, namely supervising information and promotion, production and distribution facilities, as well as the safety and quality of traditional medicines and health supplements. Research at the Roxy Sawangan Pharmacy is analyzed data on sales of antihypertensive drugs for the period May – July 2021. The results showed that 5 mg amlodipine was the antihypertensive with the highest sales. Research at PT. APL is the addition of a client registration feature on the eZRX application as a digitalization development that can make it easier in the client registration process.

"Praktik kerja profesi apoteker merupakan suatu tahapan yang wajib dilakukan oleh seorang calon Apoteker. Kegiatan praktik kerja profesi apoteker ini memiliki tujuan untuk memberikan gambaran mengenai tugas dan kewajiban seorang apoteker di suatu instansi baik itu industri farmasi, rumah sakit, puskesmas bahkan apotek. Dalam praktik kerja profesi apoteker di industri farmasi, seorang apoteker yang berperan pada departemen marketing mempunyai peranan dalam meningkatkan branding produk obat yang dilandasi dengan pengetahuan farmasi. Tidak hanya itu, peran apoteker sebagai marketing pun akan tetap dilakukan di area kerja apotek, rumah sakit. Di apotek, seorang apoteker dapat meningkatkan penjualan produknya dengan memanfaatkan tipe komunikasi yang mudah dipahami dan dimengerti oleh pasien atau target pasarnya. Peran apoteker baik di industri farmasi ataupun apotek haruslah didasarkan atas eight stars of pharmacist, yakni (i) Care giver; (ii) Decision maker; (iii) Communicator; (iv) Leader; (v) Manager; (vi) Long-life learner; (vii) Teacher; (viii) Researcher. Dari kedelapan peran apoteker menurut WHO tersebut, seorang apoteker dalam menjadikan delapan peran tersebut menjadi bagian dalam setiap langkah dan tindakan yang dilakukan saat bekerja sebagai apoteker, baik itu di ruang lingkup industri farmasi atau pelayanan farmasi.

"Medication error merupakan kejadian yang merugikan pasien akibat pemakaian obat selama dalam penanganan tenaga kesehatan, yang sebetulnya dapat dicegah. Pengkajian resep merupakan hal yang penting dalam mencegah terjadinya kesalahan pemberian obat (medication error). Pengkajian resep wajib dilakukan oleh apoteker pada alur pelayanan resep yang meliputi kajian administratif, kajian kesesuaian farmasetik, dan kajian pertimbangan klinis. Data yang terdapat pada tugas khusus ini diambil berdasarkan resep pada periode November 2021 di Apotek Roxy Pitara. Pengumpulan data dilakukan dengan cara mengklasifikasikan resep yang diterima oleh Apotek Roxy Pitara berdasarkan tanggal diterimanya resep masuk dan kemudian diambil secara acak sebanyak tiga resep. Selanjutnya dilakukan pengkajian terhadap tiga resep yang telah diambil secara acak, meliputi kajian administratif, kajian kesesuaian farmasetik, dan kajian pertimbangan klinis. Hasil pengkajian resep menunjukkan bahwa tidak terdapat resep yang memenuhi seluruh aspek administratif. Seluruh resep tidak mencantumkan berat badan pasien dan paraf dokter. Dari hasil kajian kesesuaian farmasetik, terdapat dua resep (resep 2 dan resep 3) yang memenuhi seluruh aspek kesesuaian farmasetik. Satu resep lainnya (resep 1) tidak memenuhi aspek kesesuaian farmasetik karena tidak mencantumkan kekuatan sediaan. Dari hasil kajian pertimbangan klinis, tidak terdapat resep yang memenuhi seluruh aspek pertimbangan klinis. Dua resep (resep 1 dan resep 3) tidak memenuhi aspek pertimbangan klinis karena tidak mencantumkan berat badan pasien anak, sehingga tidak dapat diketahui ketepatan dosis obat yang diberikan serta terdapat satu resep (resep 2) yang memiliki interaksi obat. Industri farmasi adalah badan usaha yang memiliki izin dari menteri kesehatan untuk melakukan kegiatan pembuatan obat atau bahan obat. Industri farmasi wajib memenuhi persyaratan CPOB. Salah satu area yang terdapat di industri farmasi adalah area produksi. Terdapat beberapa hal yang perlu diperhatikan pada area produksi, diantaranya adalah sistem tata udara yang memegang peran penting dalam industri farmasi untuk memberikan perlindungan terhadap lingkungan pembuatan produk, memastikan produksi obat yang bermutu serta memberikan lingkungan kerja yang nyaman bagi personil. Setiap industri farmasi perlu melakukan pemantauan terhadap sistem tata udara. Hasil dari pemantauan didokumentasikan dalam bentuk tren data. Metode yang digunakan dalam penelitian ini adalah metode retrospektif yang diperoleh dari data BAS (Building Automatic System) meliputi data suhu, kelembaban nisbi, dan perbedaan tekanan udara di Lantai 4 Area Produksi Solid (Kelas E) Non Sefalosporin PT Mahakam Beta Farma periode Januari – Juni 2021. Pengumpulan data dilakukan dengan cara mencatat data suhu, kelembaban nisbi, dan perbedaan tekanan udara. Selanjutnya dilakukan pengolahan data yang dilakukan dengan cara memplotkan data ke dalam borang statistik (Microsoft Excel) dan dilanjutkan dengan pembuatan grafik untuk mengetahui pola serta pergerakan tren dari data suhu, kelembaban nisbi, dan perbedaan tekanan udara yang telah dipantau serta posisi dari data terhadap control limit atau spesifikasinya. Kemudian dilakukan evaluasi yang berisi hasil review dan evaluasi terhadap data yang diperoleh serta dibuat kesimpulan. Hasilnya sebagian besar memenuhi persyaratan, baik suhu, kelembaban nisbi, maupun perbedaan tekanan udara dengan rincian masing-masing yaitu suhu ≤25 ESCIC, kelembaban nisbi ≤65%, dan perbedaan tekanan udara ≥5 Pa. Namun, terdapat dua data yang melewati rentang specification limit, yaitu data kelembaban nisbi AHU 4.7 dan data perbedaan tekanan udara AHU 4.4, sehingga insiden/deviasi harus dibuat.

Medication error is an adverse event of the patient due to the use of drugs during the handling of health workers, which can actually be prevented. Prescription assessment is important in preventing medication errors. Prescription assessment must be carried out by pharmacists in the flow of prescription services that include administrative studies, pharmaceutical suitability studies, and clinical consideration studies. The data contained in this particular task were taken on the basis of prescriptions in the November 2021 period at the Apotek Roxy Pitara. The data collection was done by classifying the prescriptions received by Apotek Roxy Pitara based on the date of incoming prescriptions and then randomly taken three prescriptions. Furthermore, an assessment of three prescriptions that have been taken randomly, including administrative studies, pharmaceutical suitability studies, and clinical consideration studies. The results of the prescription review show that there is no prescription that meets all administrative aspects. The entire prescription does not list the patient's weight and the doctor's signature. From the results of the study of pharmaceutical suitability, there are two prescriptions (prescription 2 and prescription 3) that meet all aspects of pharmaceutical suitability. Other prescription (prescription 1) does not meet the pharmaceutical suitability aspect because it does not include the strength of the preparation (dosage). From the results of the clinical consideration study, there is no prescription that meets all aspects of clinical consideration. Two prescriptions (prescription 1 and prescription 3) do not meet the aspect of clinical consideration because it does not include the weight of the pediatric patient, so it cannot be known the accuracy of the dose of the drug given and there is one prescription (prescription 2) that has drug interactions.

The pharmaceutical industry is a business entity that has a license from the minister of health to carry out drug manufacturing activities or medicinal materials. The pharmaceutical industry is obliged to meet the requirements of GMP. One of the areas in the pharmaceutical industry is the production area. There are several things that need to be considered in the production area, including air conditioning systems that play important role in the pharmaceutical industry to provide protection for the product manufacturing environment, ensure quality drug production and provide comfortable working environment for personnel. Every pharmaceutical industry needs to monitor the air system.

The results of monitoring are documented in the form of data trends. The method used in this study is a retrospective method obtained from BAS (Building Automatic System) data including temperature, relative humidity, and air pressure differences data on the 4th floor of the Non Cephalosporin Solid Production Area (Class E) of PT Mahakam Beta Farma for the January – June 2021 period. Data collection is done by recording temperature, relative humidity, and air pressure differences data. Furthermore, data processing is done by plotting the data into a statistical form (Microsoft Excel) and followed by making graphs to determine the pattern and trend movement of temperature, relative humidity, and air pressure differences data that have been monitored and the position of the data against the control limit or specifications limit. Then conducted an evaluation that contains the results of the review and evaluation of the data obtained and made conclusions. The results mostly meet the requirements, both temperature, relative humidity, and air pressure differences with the details of each temperature is ≤25 ESCIC, relative humidity ≤65%, and air pressure difference ≥5 Pa. However, there are two data that pass through the specification limit range, namely relative humidity data AHU 4.7 and air pressure differences data AHU 4.4, so the incident / deviation must be made."

Praktik Kerja Profesi Apoteker di PT Anugerah Pharmindo Lestari bertujuan untuk mengamati kegiatan distribusi obat dan bahan medis habis pakai dalam ruang lingkup Pedagang Besar Farmasi (PBF), mengetahui peran dan fungsi apoteker di PBF secara profesional, serta secara khusus melakukan identifikasi permasalahan pada proses pendaftaran customer baru kemudian mengembangkan inovasi pada eZView PT Anugerah Pharmindo Lestari. Praktik Kerja Profesi Apoteker di Apotek Kimia Farma 382 bertujuan untuk menganalisis kegiatan pelayanan kefarmasian di apotek secara umum, mengetahui peran dan fungsi apoteker di apotek secara profesional, serta secara khusus menganalisis kerasionalan peresepan terhadap pasien diabetes melitus di Apotek Kimia Farma 382.

Oleh karena itu, calon apoteker dituntut untuk menjalani praktik profesi sebagai bekal dan pengalaman untuk memahami peran seorang apoteker dalam pelayanan dan industri. Praktik Kerja di Apotek Roxy, PT Pharos Indonesia, dan Puskesmas Kecamatan Pasar Rebo Periode Bulan September – Desember tahun 2021. Melalui proses PKPA pada berbagai tempat, diharapkan calon apoteker dapat memperoleh ilmu yang berguna nantinya dalam dunia pekerjaan.

ANUGERAH PHARMINDO LESTARI, dengan tujuan untuk mengetahui dan memahami secara mendalam terkait penerapan CDOB dan segala aspek terkait industri distributor obat. Tugas khusus pada PKPA di PT. ANUGERAH PHARMINDO LESTARI terkait desain gudang penyimpanan obat.
