"Skizofrenia merupakan salah satu gangguan psikosis yang mengacaukan kejiwaan seseorang berupa sikap, pikiran, dan perilaku. Orang dengan gangguan jiwa berhak untuk mendapatkan pelayanan kesehatan jiwa sesuai dengan standar pelayanan kesehatan jiwa. Salah satu cara untuk menangani masalah kejiwaan tersebut adalah dengan menggunakan terapi farmakologi dengan menggunakan antipsikotik untuk mengatasi gejala yang dialami. Antipsikotik adalah golongan obat untuk mengendalikan dan mengurangi gejala psikosis yang bisa dialami oleh penderita gangguan mental. Salah satu peran penting apoteker adalah memberikan pharmaceutical care yang komprehensif dengan mendukung penggunaan obat yang benar dan rasional, mengidentifikasi, mencegah, dan mengatasi adanya drug related problem, serta melakukan monitoring dan evaluasi pengobatan sesuai dengan standar pelayanan yang berlaku. Salah satu bentuk pelayanan farmasi klinik di apotek adalah melakukan pengkajian resep baik dari aspek administratif, farmasetik, dan klinis. Selama masa pengobatan dengan obat-obat antipsikotik dan antidepresan, diperlukan monitoring terkait kepatuhan pengobatan, manfaat terapeutik, dan efek samping.

One form of clinical pharmacy services in pharmacies is to conduct prescription assessments from the administrative, pharmaceutical, and clinical aspects. During the treatment period with antipsychotic and antidepressant drugs, monitoring is required regarding medication adherence, therapeutic benefits, and side effects."

"Bioburden merupakan parameter penilaian penting dalam kualitas produk steril. Dengan mengetahui adanya beban cemaran mikro maka akan dapat diketahui gambaran mutu produk sediaan steril sebelum dan setelah melewati proses sterilisasi nantinya. PT. Harsen Laboratories sendiri merupakan industri manufaktur farmasi di Indonesia dengan produksi sediaan injeksi terbesar di Indonesia dimana untuk menjamin kualitas sediaan injeksi yang diproduksi ialah dengan menjalankan pengujian bioburden pada tiap-tiap produk injeksi. Prosedur penyusunan protap bioburden dimulai dari suprevisor Andev menyusun dan merevisi prosedur pengujian bioburden kemudian diperiksa oleh Manajer RnD sub departemen Andev dan Packdev untuk memastikan prosedur pengujian bioburden dapat berjalan dengan baik. Protap pengujian bioburden dapat dilakukan dengan 2 metode, yaitu metode membran filtrasi dan metode cawan tuang dimana kedua metode ini harus dilakukan oleh analis QC terlatih dan mengikuti prosedur yang terlampir secara jelas di dalam protap dan lembar kerja pengujian bioburden.Penyakit kardivoaskular ialah gangguan yang terjadi pada jantung dan pembuluh darah. Permasalahan yang sering timbul terkait penyakit ini ialah pengobatan yang bervariasi terutama dari sisi penggunaan obat yang beragam dimana keragaman dari efek samping dan interkasi pada peresepan obat-obatan tersebut juga meningkat. Sebagai seorang tenaga kesehatan yang profesional di bidang farmasi, apoteker berkewajiban untuk mengkaji terlebih dahulu resep yang masuk untuk menghindari kesalahan pemberian obat-obatan. Frekuensi masuknya resep untuk pengobatan penyakit kardiovaskular cukup banyak terutama di Apotek Roxy Biak yang terletak di pusat ibukota, yaitu kota Jakarta Pusat. Hasil skrining resep kardiovaskular pada penulisan laporan ini ialah kajian administratif dalam penulisan resep yang diterima di apotek Roxy Biak sudah cukup lengkap, namun ada beberapa informasi yang kurang dimana apoteker dapat menggali informasi lewat komunikasi untuk memverifikasi resep. Kesesuaian farmasetik dari obat-obatan kardiovaskular yang diberikan juga sudah cukup jelas dan pertimbangan klinik yang harus dipantau dari resep kardiovaskular yang masuk ialah harus memantau efek samping obat dan memantau terapi obat yang dikonsumsi pasien.

Harsen Laboratories itself is a pharmaceutical manufacturing industry in Indonesia with the largest production of injection preparations in Indonesia where to ensure the quality of the injection preparations produced is to carry out bioburden testing on each injection product. The procedure for preparing the bioburden protocol starts with the Andev supervisor compiling and revising the bioburden testing procedure and then being checked by the Andev and Packdev sub-department RnD Managers to ensure the bioburden testing procedure runs properly. The procedure for testing bioburden can be carried out by 2 methods, which is the membrane filtration method and the pour plate method where both methods must be carried out by a trained QC analyst and follow the procedures clearly attached in the protocol and the bioburden testing worksheet.

Pelayanan kefarmasian di Apotek terbagi kedalam dua jenis yaitu pengelolaan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai serta pelayanan farmasi klinis. Mengingat banyaknya ketidaksesuaian antara resep yang diberikan oleh dokter kepada pasien, maka tugas khusus ini dilakukan untuk melakukan observasi terkait pelayanan farmasi klinis di Apotek Biak khususnya terkait ketepatan pelayanan resep serta kesesuaian data dan kelengkapan resep yang diterima di Apotek Roxy sesuai dengan ketentuan Standar Pelayanan Kefarmasian yang berlaku. Hal ini diharapkan dapat mencegah terjadinya kesalahan pemberian obat (medication error). Pembuatan laporan tugas khusus Praktik Kerja Profesi Apoteker (PKPA) di Apotek Roxy Biak diawali dengan mengumpulkan beberapa data berupa sampel resep bulan Februari 2023 dan dilakukan analisis resep berdasarkan telaah aspek administratif, farmasetik, dan klinis. Berdasarkan hasil kegiatan yang telah dilakukan, pelayanan farmasi klinis yang dilakukan di Apotek Roxy termasuk pelayanan dan pengkajian resep serta dispensing obat yang dilakukan sudah tepat dan sesuai dengan Standar Pelayanan Kefarmasian di Apotek, serta Resep obat yang diterima dan dilayani di Apotek Roxy biak sudah sesuai dengan kelengkapan data yang tepat berdasarkan pengkajian administrative, farmasetika, dan klinis.

"Apotek sebagai sarana pelayanan praktik kefarmasian bertanggung jawab dalam memberikan pelayanan farmasi klinik, termasuk pengkajian dan pelayanan resep. Sebanyak tiga resep yang memiliki jumlah obat minimal tiga dan terdapat pengobatan diabetes pada periode bulan Januari 2023 dan melakukan studi literatur untuk menilai rasionalitas peresepan agen antidiabetes. Seluruh resep dianalisis berdasarkan aspek administratif, farmasetik dan farmakologis. Dari ketiga resep yang ditelaah secara administratif, farmasetik dan klinis, ditemukan beberapa kekurangan aspek administratif seperti usia pasien dan paraf dokter. Secara farmasetik dan klinis, seluruh obat yang diresepkan rasional, dosis dan aturan pakai tepat. Formulir pengkajian resep sebaiknya digunakan untuk mendokumentasikan kelengkapan aspek administratif, farmasetik maupun klinis dari resep-resep yang dilayani. Peran aktif apoteker untuk menyampaikan informasi terkait obat dan saran non farmakologis juga penting untuk meningkatkan kepatuhan pasien dalam penggunaan obat.

Rational prescribing includes the right indication, drug selection, dosage, time of administration, route and method of administration to reduce the incidence of medication errors such as adverse effects. Diabetes is a chronic disease that occurs when the pancreas does not produce enough insulin or cannot use the insulin it produces effectively. The main management of diabetes is controlling glucose levels to achieve optimal glycemic levels with antidiabetic drugs. In this report, analysis on prescriptions of diabetic drugs was carried out on prescriptions received by Roxy Biak Pharmacy in January 2023. Three prescriptions containing a minimum of three antidiabetic agents were sampled, followed by literature study to assess rational drug prescription. All prescriptions were analyzed based on administrative, pharmaceutical and pharmacological aspects. Of the three prescriptions that were reviewed several administrative aspects were found to be lacking such as the patient's age and doctor's initials. All prescriptions fulfilled the pharmaceutical and clinical aspects whereby all the drugs prescribed with rational dosages. A prescription review form should be used to document the completeness of the administrative, pharmaceutical and clinical aspects of the prescriptions. The active role of pharmacists in providing drug-related information and non-pharmacological treatments is also pivotal to increase patient adherence in drug use."

"Obat dengan indikasi gangguan gastrointestinal termasuk dalam 10 besar obat yang paling banyak diresepkan. Seringkali, pasien dengan keluhan gangguan gastrointestinal diresepkan beberapa obat secara bersamaan. Obat-obat yang digunakan untuk pengobatan penyakit gastrointestinal perlu diperhatikan, karena dapat mengubah fisiologi saluran gastrointestinal yang mempengaruhi farmakokinetik obat. Maka dari itu dilakukan pengkajian resep yang juga termasuk bagian dari pelayanan farmasi klinik di apotek yang bertujuan untuk mengetahui kesesuaian resep untuk penyakit gastrointestinal berdasarkan kajian administratif, kesesuaian farmasetik, dan pertimbangan klinis. Resep yang dikaji pada laporan ini diambil dari Apotek Roxy Biak bulan April 2023. Sebanyak tiga resep kasus diambil untuk laporan ini, berisi obat-obat untuk gangguan gastrointestinal yang diinstruksikan dokter untuk diracik. Hasil pengkajian resep menunjukkan, pada aspek administratif data pasien masih terdapat ketidaklengkapan, Pada aspek farmasetik, ada beberapa obat yang harus diperhatikan stabilitasnya jika diracik, terutama sediaan obat bersalut. Pada aspek klinis, diketahui terdapat resep yang memiliki masalah terkait dosis yang kurang tepat dan interaksi obat. Obat untuk gangguan gastrointestinal yang menyebabkan interaksi dengan obat oral secara farmakokinetik, sebaiknya tidak diracik menjadi satu sediaan, melainkan diberi instruksi waktu pemakaian obat.

Therefore, a prescription screening is carried out, which is also part of the clinical pharmacy service at the pharmacy and aims to determine the suitability of prescriptions for gastrointestinal diseases based on administrative studies, pharmaceutical suitability, and clinical considerations. The prescriptions studied in this report were taken from Roxy Biak Pharmacy in April 2023. A total of three case prescriptions were taken for this report, containing drugs for gastrointestinal disorders that the doctor instructed to be compounded. The results of the prescription screening show that in the administrative aspect, patient data is still incomplete. In the pharmaceutical aspect, there are several drugs whose stability must be considered when formulated, especially coated drug preparations. In the clinical aspect, there are prescriptions that have problems related to inappropriate dosage and drug interactions. Drugs for gastrointestinal disorders that cause interactions with oral drugs pharmacokinetically should not be mixed into one preparation, but should be given instructions on when to use the drug."

"Tuberkulosis (TBC) merupakan penyakit infeksi pernapasan menular dengan persentase kasus kematian yang tinggi di Indonesia. Provinsi DKI Jakarta masuk dalam wilayah zona merah kasus TBC disebabkan oleh beberapa faktor, salah satunya yaitu terkait pemberian Obat Anti Tuberkulosis (OAT) kepada pasien penderita TBC. OAT diresepkan oleh dokter terhadap pasien penderita TBC dan tidak menutup kemungkinan terjadi kesalahan pengobatan. Kesalahan pengobatan kepada pasien dapat menyebabkan resistensi hingga kematian. Tugas akhir ini ditulis untuk meninjau resep OAT dari dokter yang diterima oleh Apotek Roxy Biak dengan mengkaji resep OAT pada bulan Januari 2021 secara administratif, farmasetik, dan klinis. Penulis berharap tugas akhir yang telah disusun dapat menunjukkan kesesuaian resep OAT yang diterima oleh Apotek Roxy Biak terhadap kaidah penulisan resep sesuai dengan aturan yang berlaku.

Tuberculosis (TB) is an infectious respiratory disease with a high percentage of deaths in Indonesia. DKI Jakarta Province is included in the red zone of TB cases due to several factors, one of which is related to the administration of Anti Tuberculosis Drugs (OAT) to patients with TB. OAT is prescribed by doctors to patients with TB and does not rule out the possibility of medication errors. Medication errors to patients can lead to resistance or even death. This final project was written to review OAT prescriptions from doctors received by Roxy Biak Pharmacy by reviewing OAT prescriptions in January 2021 administratively, pharmaceutically, and clinically. The author hopes that the final project that has been written can show the suitability of the OAT prescription received by the Roxy Biak Pharmacy to the prescription writing rules in accordance with the regulations."

"Diabetes adalah penyakit kronis yang disebabkan karena kekurangan insulin akibat dari gangguan sekresi insulin. Diabetes banyak dialami oleh masyarakat khususnya pada masyarakat dengan pola hidup yang sudah berubah yaitu diabetes tipe 2. Sehingga kerasionalan resep untuk obat diabetes menjadi penting,. Metformin merupakan first line terapi untuk DM tipe 2, baik metformin generik maupun merek dagang. Analisa resep dilakukan dengan mengambil sampel resep yang mengandung metformin (baik generik dan merek yaitu glucophage XR). Lalu resep generik dan merek dibandingkan jumlah pengeluarannya dalam 1 bulan, setelah itu dilakukan analisis mengenai aspek administratif, aspek farmasetik dan aspek klinis serta dibandingkan dengan guideline terapi yang sudah ada. Hasilnya perbandingan pengeluaran metformin generik dan bermerek yaitu 27 : 1, dan dari resep yang dianalisis sudah sesuai dengan guideline terapi dan rasional.

Metformin is the first line therapy for DM type 2, both in generic or trademark metformin. Prescription analysis was carried out by taking prescription samples containing metformin (both generic and brand, that is glucophage XR). Then the generic and brand prescriptions were compared with the amount spent in 1 month, after that an analysis was carried out regarding administrative, pharmaceutical and clinical aspects and compared with the existing therapeutic guidelines. The result was a comparison of generic and branded metformin is 27:1, and the analyzed prescriptions is rational and already appropiate with guidelines therapeutic."

"Kegiatan pelayanan farmasi klinik yang dilaksanakan di apotek salah satunya mencakup pengkajian dan pelayanan resep. Resep yang dilayani di apotek cukup beragam, mulai dari resep untuk penyakit akut maupun kronis seperti diabetes.Tujuan dari skrining dan analisis resep obat antidiabetes adalah untuk mengetahui obat antidiabetes yang diresepkan oleh dokter, mengetahui jumlah resep yang mengandung obat antidiabetes, serta menilai kerasionalan penggunaan obat antidiabetes pada resep yang dilayani di Apotek Roxy Biak selama periode Juli 2022. Dari seluruh resep yang masuk selama periode tersebut, resep yang mengandung obat antidiabetes dicatat dan dikumpulkan data, kemudian dipilih dua resep untuk dikaji kelengkapan resep berdasarkan aspek administratif, farmasetik, serta klinis. Pengkajian 2 (dua) resep antidiabetes di Apotek Roxy Biak dilihat dari aspek administratif, farmasetika dan klinis sudah sesuai. Namun pada aspek administratif penulisan umur dan berat badan pasien tidak dituliskan. Aspek administratif terkait umur dan berat badan perlu dikonfirmasi kembali kepada pasien atau keluarga pasien yang mengambil obat di apotek, hal ini penting untuk mengkaji ketepatan pengobatan yang akan diterima oleh pasien.

Of all prescriptions received during this period, prescriptions containing antidiabetic drugs were recorded and data collected, then two recipes were selected to review the completeness of the prescription based on administrative, pharmaceutical, and clinical aspects. The review of 2 (two) antidiabetic prescriptions at the Roxy Biak Pharmacy from an administrative, pharmaceutical and clinical perspective was appropriate. However, in the administrative aspect, the patient's age and weight were not written down. Administrative aspects related to age and weight need to be confirmed again with the patient or the patient's family who picks up the drug at the pharmacy, this is important to assess the accuracy of the treatment that the patient will receive."

"Pengkajian resep termasuk kedalam bagian dari kegiatan farmasi klinis di Apotek yang merupakan bagian dari Pelayanan Kefarmasian yang langsung dan bertanggung jawab kepada pasien berkaitan dengan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai dengan maksud mencapai hasil yang pasti untuk meningkatkan kualitas hidup pasien. Kegiatan pengkajian Resep meliputi administrasi, kesesuaian farmasetik dan pertimbangan klinis. Penyakit infeksi di Indonesia masih termasuk dalam sepuluh penyakit terbanyak. Peresepan antibiotik di Indonesia yang cukup tinggi dan kurang bijak akan meningkatkan kejadian resistensi. Berdasarkan gambaran dari peresepan antibiotik yang cukup tinggi, maka diperlukan pengkajian terhadap skrining administrasi dan farmasetik resep antibiotik untuk mencegah terjadinya kesalahan pengobatan yang dapat merugikan pasien di Apotek Roxy Biak. Tujuan penulisan tugas khusus praktik kerja profesi apoteker ini yaitu pengkajian resep obat untuk dua resep antibiotik di apotek Roxy Biak pada bulan juli 2022 dari segi kajian administratif, klinis dan farmasetik. Hasil yang didapat yaitu obat yang ditebus pada Apotik Roxy Biak masih memiliki kekurangan pada kelengkapan kajian administratif, yaitu Berat Badan (BB), umur pasien, dan alamat pasien, paraf dan nomor telepon dokter. Dari segi kajian farmasetik dan klinis, resep antibiotik yang ditebus di Apotek Roxy Biak telah memenuhi persyaratan.

The purpose of writing this special assignment for the work practice of the pharmacist profession is to review drug prescriptions for two antibiotic prescriptions at the Roxy Biak pharmacy in July 2022 in terms of administrative, clinical and pharmaceutical studies. The results obtained were that the drugs that were redeemed at the Roxy Biak Pharmacy still had deficiencies in the completeness of administrative studies, namely body weight (BB), patient's age, and patient's address, initials and doctor's telephone number. Berdasarkan pengkajian dari aspek farmasetik, kedua resep tidak ada penyimpangan, namun dari aspek administratif  tidak terdapat berat badan pasien dan paraf pembuat resep dan dari aspek klinis terdapat adanya duplikasi dan interaksi antar obat yang perlu dimonitor. Hal tersebut perlu dikonfirmasi kembali untuk mencegah terjadinya kesalahan pemberian obat.

Clinical pharmacy services are an important part of the pharmacy's commitment to provide excellent service. Prescription assessment is an important part of clinical pharmacy services to determine problems related to prescription drugs such as antihypertensive drugs. The purpose of this review is to assess the rationality of prescriptions in administrative, pharmaceutical, and clinical aspects of antihypertensive drug prescriptions served by Roxy Biak Pharmacy during April 2023. Of all the incoming prescriptions, two prescriptions were selected to be studied for administrative, pharmaceutical, and clinical aspects. Based on the assessment from the pharmaceutical aspect, there are no irregularities in the two prescriptions, but from the administrative aspect there is no patient weight and prescription-making paraphrasing and from the clinical aspect there is duplication and interaction between drugs that need to be monitored. This needs to be reconfirmed to prevent the occurrence of medication errors."
