"Praktik keperawatan residensi yang dilakukan di Rumah Sakit Persahabatan dengan kekhususan respirasi diharapkan dapat mengatasi masalah pernapasan. Selama menjalani proses residensi, residen berperan menjadi seorang Clinical Care Manajer (CCM) yang bertugas sebagai konsultan keperawatan bagi staf keperawatan dan pemberi terapi keperawatan kepada pasien, sebagai peneliti serta pendidik dalam rangka pemberi asuhan keperawatan untuk meningkatkan mutu atau kualitas asuhan keperawatan. Dalam memberikan asuhan keperawatan, residen menerapkan model adaptasi Roy baik pada kasus kelolaan dengan diagnosa sarkoma dinding dada maupun pada 30 kasus resume, pada kasus resume ini didapatkan diagnosa medis terbanyak adalah kanker paru, diagnosa keperawatan terbanyak pola napas tidak efektif dan intervensi keperawatan yang paling banyak adalah manajemen airway. Penerapan model adaptasi Roy menunjukkan peran perawat sebagai pemberi asuhan keperawatan dalam membantu individu, baik dalam kondisi sakit maupun pada proses penyembuhan dan peningkatan pemeliharaan kesehatan di setiap mode fisiologis, konsep diri, fungsi peran dan interdependen. Selama melakukan asuhan keperawatan, masalah keperawatan yang banyak muncul yaitu pada mode fisiologis berupa masalah gangguan oksigenasi. Hasil analisis praktik residensi keperawatan didapatkan bahwa asuhan keperawatan menggunakan pendekatan model adaptasi Roy bertujuan untuk membantu individu sesegera mungkin beradaptasi terhadap stimulus baik stimulus internal maupun stimulus eksternal. Pada penerapan Evidence Base Nursing (EBN), residen melakukan penerapan teknik relaksasi benson pada pasien dengan PPOK yang bertujuan untuk mengurangi derajat dyspnea dan kecemasan. Hasil penerapan EBN menunjukan bahwa relaksasi benson signifikan dalam menurunkan derajat dyspnea dan kecemasan pasien PPOK. Pada proyek inovasi, dilakukan program peningkatan kemampuan perawat dalam melakukan perawatan trakeostomi pada pasien dewasa dengan 6 elemen bundle trakeostomi yang telah terstandar, berbasis bukti, sistematis dan terstruktur. Tujuan dari inovasi ini untuk meningkatkan kemampuan dan pengetahuan perawat dalam melakukan perawatan trakeostomi dan terciptanya booklet bundle tracheostomy sebagai pedoman standar intervensi perawatan trakeostomi.
Residency nursing practice carried out at Persahabatan Hospital with a respiratory specialty is expected to be able to overcome respiratory problems. During the residency process, the resident plays the role of a Clinical Care Manager (CCM) who serves as a nursing consultant for nursing staff and provides nursing therapy to patients, as a researcher and educator in the context of providing nursing care to improve the quality of nursing care. In providing nursing care, residents apply Roy's adaptation model both in cases managed with a diagnosis of chest wall sarcoma and in 30 resume cases, in these resume cases the most medical diagnoses were lung cancer, the most nursing diagnoses were ineffective breathing patterns and the most nursing interventions is airway management. The application of Roy's adaptation model shows the role of nurses as providers of nursing care in helping individuals, both in sick conditions and in the healing process and improving health maintenance in every physiological, self-concept, role and interdependent mode. During nursing care, nursing problems that often arise are in the physiological mode in the form of problems with oxygenation disorders. The results of the analysis of nursing residency practice showed that nursing care using the Roy adaptation model approach aims to help individuals adapt as quickly as possible to stimuli, both internal and external stimuli. In the application of Evidence Base Nursing (EBN), residents apply the Benson relaxation technique to patients with COPD which aims to reduce the degree of dyspnea and anxiety. The results of applying EBN show that Benson relaxation is significant in reducing the degree of dyspnea and anxiety in COPD patients. In the innovation project, a program was carried out to increase nurses' abilities in carrying out tracheostomy care for adult patients with 6 elements of a tracheostomy bundle that were standardized, evidence-based, systematic and structured. The aim of this innovation is to increase the ability and knowledge of nurses in carrying out tracheostomy care and to create a tracheostomy bundle booklet as a standard guideline for tracheostomy care interventions."

ABSTRAK Keperawatan respirasi merupakan salah satu area khusus dimana perawat memiliki peran dalam memberikan asuhan keperawatan berdasarkan kebutuhan pasien. Perawat spesialis respirasi memiliki peran lebih yang berbeda, dengan menerapkan konsep dan teori keperawatan dalam penanganan kasus pada pasien gangguan pernapasan. Peran ners spesialis sebagai klinisi ahli, pendidik, manajer kasus, konsultan, dan peneliti untuk merencanakan atau memperbaiki kualitas keperawatan bagi klien dan keluarganya. Teori keperawatan yang digunakan dalam merawat pasien (satu kasus kelolaan dan 30 kasus resume) dengan menggunakan model adaptasi Roy. Peran sebagai peneliti diterapkan dengan melakukan latihan breathing retraining pada pasien asma. Sementara peran untuk memperbaiki kualitas pelayanan salah satunya adalah menerapkan inovasi bagi pasien tuberkulosis dalam mencegah terjadinya putus obat.

"Laporan praktik spesialis Keperawatan Medikal Bedah bertujuan untuk melakukan analisis praktik residen dalam mengimplementasikan peran perawat spesialis sehingga dapat memiliki kemampuan yang belandaskan ilmu-ilmu pengetahuan dan praktik profesional keperawatan. Peran perawat spesialis sebagai pemberi asuhan keperawatan diimplementasikan pada pasien-pasien dengan gangguan sistem perkemihan yang terdiri dari satu kasus kelolaan utama yaitu pasien dengan urolithiasis dan 30 kasus resume. Selain itu, peran perawat spesialis sebagai pendidik dan konselor juga diimplementasikan dalam mengelola pasien kelolaan utama dan resume. Peran perawat spesialis sebagai peneliti dilakukan dengan membuat litelature review sebagai bagian dari pelaksanan Evidanve Based Nursing Practice. Hal ini dilakukan dengan membuat litelature review mengenai akupresur pada titik Hugo untuk mengatasi nyeri penusukan akses arteriovenous fistula pada pasien hemodialisis dan abdominal massage untuk mengatasi konstipasi pada pasien di Intensive Care Unit. Peran sebagai inovator diimplementasikan dengan melakukan proyek inovasi berupa Buku Harian Pasien HD untuk mengendalikan Interdialytic Weight Gains (IDWG), dimana dalam pelaksanaannya residen menggunakan pendekatan interpesonal dan leadership.

Dalam memberikan asuhan keperawatan pada kasus kelolaan pasien dengan Bronkiektasis dan 30 kasus resume menggunakan teori Henderson 14 kebutuhan dasar manusia yang menunjukkan peran perawat sebagai pemberi asuhan keperawatan dalam membantu individu baik dalam keadaan sehat maupun sakit, melalui usahanya melakukan berbagai aktivitas guna mendukung kesehatan dan penyembuhan individu atau proses meninggal dengan damai. Masalah keperawatan yang banyak muncul yaitu tentang pemenuhan kebutuhan bernapas normal. Penerapan EBN cold application dan latihan napas dan batuk untuk menurunkan tingkat nyeri pada pasien terpasang chest tube. Pelaksanaan proyek inovasi pengembangkan video edukasi monitoring dan perawatan di rumah pasien terpasang WSD sebagai media self-management bagi perawat ruang rawat dan pasien yang terpasang WSD. Hasil analisis praktik residensi keperawatan didapatkan bahwa asuhan keperawatan menggunakan pendekatan teori Henderson bertujuan untuk membantu sesegera mungkin kemandirian pasien. Penerapan cold application dan latihan napas dan batuk dapat menurunkan skor nyeri pasien terpasang chest tube. Proyek inovasi video edukasi monitoring dan perawatan di rumah pasien terpasang WSD sebagai media self-management meningkatkan pengetahuan perawat dan kemandirian pasien.

Pendekatan teori keperawatan dalam pelaksanaan pemberian asuhan keperawatan merupakan salah satu tujuan dari pelaksanan program ners spesialis keperawatan medikal bedah. Teori keperawatan dapat diterapkan dalam berbagai masalah yang dihadapi oleh pasien, seperti halnya masalah pada pasien dengan gangguan sistim perkemihan. Pasien dengan masalah gangguan pada sistim perkemihan akan mengalami berbagai perubahan pada sistim keseimbangan tubuh, sehingga tubuh memerlukan suatu proses adaptasi. Pendekatan teori adaptasi Roy, melalui mekanisme koping, sistim regulator dan kognator dapat mengatasi perilaku adaptif maupun maladaptif yang dialami oleh pasien berdasarkan hasil pengkajian perilaku dan stimulus. Pelaksanaan penerapan Evidence Based Nursing: melalui pemberian intervensi keperawatan non farmakologi: Aerobic Exercise Intradialytic pada pasien Chronic Kidney Disease Stage V yang menjalani hemodialisis, terbukti dapat meningkatkan pencapaian adekuasi dialisis pasien. Hasil penerapan proyek inovasi penggunaan model the activated health education melalui media edukasi flipchart mengenai terapi pengganti ginjal terbukti berpengaruh dan mampu meningkat pengetahuan pasien mengenai terapi pengganti ginjal.
The concept of nursing care implementation is one of the objectives medical nursing specialist medical programs. "Praktik residensi keperawatan medikal bedah adalah bagian dari pendidikan profesi yang mempunyai tujuan untuk menghasilkan perawat spesialis. Hasil analisis praktik menunjukkan bahwa model adaptasi Roy dapat digunakan sebagai pendekatan dalam asuhan keperawatan pasien dengan gangguan sistem pernapasan untuk mengembangkan mekanisme adaptif pasien. Pemberian posisi condong kedepan dan Latihan nafas pursed lips breathing dapat menurunkan sesak napas pada pasien dengan penyakit paru. Selain itu, Latihan Breathing Exercise : ACBT Dalam Meningkatkan Airway Clearance Pada Pasien mendapat evaluasi yang positif dan dapat diterapkan di tatanan klinis sebagai inovasi pengkajian dan manajemen keperawatan mandiri pada pasien PPOK selama di rumah.
Medical surgical nursing residency practice is part of professional education that aims to produce specialist nurses. Specialist nurses are expected to carry out their role in providing nursing services in cases with respiratory system disorders. The role is in line with the competencies to be achieved during residency practice in the form of providing nursing care, implementing evidence-based nursing practice and innovation projects. Nursing care was applied to one primarily managed patient with a case of COPD and 30 resume cases using Roy's adaptation model approach. The application of scientific evidence-based nursing actions carried out is the provision of a forward leaning position and pursed lips breathing exercises in dyspnea patients with lung disease. The innovation project applied is the Development of Self Management with Video Breathing Exercise: ACBT in Increasing Airway Clearance in Patients with Chronic Obstructive Pulmonary Disease (COPD). The results of the practice analysis show that Roy's adaptation model can be used as an approach in nursing care of patients with respiratory system disorders to develop patient adaptive mechanisms. Forward leaning and pursed lips breathing exercises can reduce shortness of breath in patients with lung disease. In addition, Breathing Exercise: ACBT in Increasing Airway Clearance in Patients received a positive evaluation and can be applied in clinical settings as an innovation in self-assessment and nursing management in COPD patients while at home."

"Program spesialis keperawatan medikal bedah khususnya neurologi dimaksudkan untuk menjadikan seorang perawat spesialis neurosains yang berperan dalam pemberi asuhan keperawatan lanjut, melakukan pembuktian ilmiah, dan agen pembaharu. Asuhan keperwatan yang dilaksanakan pada kasus stroke hemoragik dan 30 pasien dengan gangguan neurologis menggunakan pendekatan Model adaptasi Roy. Perilaku maladaptif yang paling sering ditemui pada mode adaptasi fisiologis adalah ketidakefektfan perfusi serebral. Intervensi keperawatan yaitu monitor tekanan intrakranial bertujuan untuk meningkatkan adaptasi pasien dalam meningkatkan perfusi jaringan cerebral. Penerapan EBN tentang penilaian kualitas tidur pada pasien stroke menggunakan Pittsburgh Sleep Quality Index. Tiga puluh pasien menunjukkan bahwa sebagian besar paien mengalami kualitas tidur buruk. Program inovasi keperawatan berupa penerapan perawatan pasca craniotomy di ruang NCCU, HCU dan rawat inap bedah saraf.

Nursing innovation program in the form of post craniotomy care application in the NCCU, HCU and neurosurgery inpatients."
