"Multimorbiditas adalah kondisi dua atau lebih penyakit diidap oleh seseorang yang lebih sering terjadi diantara pasien geriatri dan pasien penyakit kronis lainnya. Adanya beberapa penyakit yang sedang diobati akan memengaruhi jumlah dari jenis obat yang digunakan untuk indikasi tersebut. Penggunaan obat yang banyak secara teratur disebut dengan polifarmasi. Polifarmasi cenderung terjadi pada pasien dengan penyakit kompleks seperti hipertensi, diabetes mellitus, dan penyakit kejiwaan yang disertai komorbid. Penggunaan beberapa obat ini harus dipantau dengan teliti untuk menghindari risiko reaksi obat yang tidak diinginkan (ROTD) dan meningkatkan health outcome pasien. Apotek Roxy merupakan fasilitas kesehatan yang melayani resep polifarmasi setiap harinya. Pengawasan pasien dengan resep polifarmasi dapat meningkatkan keselamatan pengguna. Penelitian ini dilakukan untuk mendokumentasikan dan mengkaji resep polifarmasi yang dilayani Apotek Roxy pada periode April 2023. Resep yang digunakan di-sampling dengan metode purposive sampling dengan syarat resep memiliki jumlah jenis lebih dari lima obat. Resep dikaji dalam aspek administratif, farmasetik, dan klinis. Ditemukan pada lima resep polifarmasi memiliki masalah dalam aspek administratif dan klinis. Drug related problem didapatkan diantara lain adanya interaksi obat.

Multimorbidity is a condition in which a patient has two or more illness, usually occured in geriatric and chronic illness patients. Multiple condition needs to be treated and affecting the amount of drug type patients need to consume. Regularly consuming substantial amount of drug is called polypharmacy. Polypharmacy tend to occur in patients with complex disease such as hypertension, DM, or mental illness with comorbid. Multiple drug type used to treat diseases need to be monitored thorough to avoid adverse drug reaction and increase patient’s health outcome. Apotek Roxy is a health facility that attend to polypharmacy prescriptions on the daily basis. Monitoring of polypharmacy patients can increase user’s safety. This study was dedicated to document and assess polypharmacy prescriptions throughout April 2023. Prescription sampling was done by purposive sampling method for prescriptions with at least five types of medication. Prescriptions were assessed in three aspects; administration, pharmaceutic, and clinical. In five selected polypharmacy prescriptions, incompabilities were found in administation and clinical aspects. Possible drug related problems are found, such as drug interactions."

"Tanggung jawab seorang apoteker dalam pelayanan klinis salah satunya adalah memastikan bahwa terapi yang diperoleh pasien sudah optimal. Apoteker harus dapat mengidentifikasi adanya permasalahan terkait obat serta memberikan solusi dan mencegah terjadinya hal yang tidak diharapkan tersebut. Masalah terkait obat yang mungkin terjadi di antaranya interaksi obat dan efek samping obat. Interaksi obat dapat disebabkan karena pengobatan polifarmasi atau penggunaan beberapa macam obat (lima atau lebih) secara bersamaan setiap hari. Untuk mencegah masalah terkait obat yang mungkin terjadi, dilakukan pengkajian analisis efek samping dan interaksi obat terhadap resep polifarmasi. Metode yang digunakan dalam tugas khusus ini adalah deskriptif retrospektif menggunakan resep yang dilayani oleh Apotek Roxy Depok pada bulan April 2023. Sampel resep yang memenuhi kriteria selanjutnya dianalisis efek samping obat dan interaksinya menggunakan pangkalan data seperti MedScape dan drugs.com interaction checker. Hasil dari pengkajian tiga resep polifarmasi yang diperoleh, ditemukan potensi efek samping yang didominasi dengan efek samping umum pada gangguan gastrointestinal termasuk nyeri perut, diare, konstipasi dan muntah, serta gangguan lain seperti sakit kepala, kelelahan berlebihan, edema, dan mialgia. Berdasakan analisis interaksi obat, ditemukan potensi interaksi major sebesar 5,55%, interaksi moderate 77,78% dan interaksi minor 16.67%.

One of the pharmacist's responsibilities in clinical services is to ensure that therapy received by the patient is optimal. Pharmacists must be able to identify drug-related problems and provide solutions to prevent adverse events. Potential drug-related problems include drug interactions and drug adverse effects. Drug interactions can occur due to polypharmacy or the use of multiple medications (five or more) simultaneously on a daily basis. To prevent potential drug-related problems, an analysis of adverse effects and drug interactions for polypharmacy prescriptions was conducted. This study employed a descriptive retrospective methodology using prescriptions filled at Roxy Depok Pharmacy in April 2023. Eligible prescriptions were further analyzed for drug side effects and interactions using databases such as MedScape and drugs.com interaction checkers. The analysis of three polypharmacy prescriptions revealed potential side effects predominantly involving common gastrointestinal adverse events, including abdominal pain, diarrhea, constipation, and vomiting, as well as other effects such as headache, excessive fatigue, edema, and myalgia. Drug interaction analysis identified major interactions at 5.55%, moderate interactions at 77.78%, and minor interactions at 16.67%."

"Apoteker dituntut harus memiliki kompetensi yang memadai dalam menjalankan tugas dan tanggung jawab kefarmasian di apotek. Oleh karena itu, calon apoteker harus dipersiapkan dengan baik sebelum memegang tugas dan tanggung jawab tersebut. Salah satu program untuk mempersiapkan calon apoteker yaitu melalui Praktik Kerja Profesi Apoteker (PKPA). Pelaksanaan PKPA dilakukan mulai tanggal 3-31 Juli 2023 di Apotek Roxy Depok. Metode yang digunakan dalam penyusunan laporan tugas khusus ini menggunakan metode berupa studi literatur dan penelusuran terkait interaksi dan efek samping obat yang dapat terjadi pada resep polifarmasi pada periode bulan Juli 2023. Berdasarkan 5 resep yakng dikaji, hasil penelitian menunjukkan adanya interaksi yang bersifat moderat pada resep 2,4 dan 5. Oleh karena itu interaksi tersebut perlu dimonitor agak tidak membahayakan keselamatan pasien. Apoteker memiliki peran penting untuk memberikan konseling bagi pasien yang mendapatkan resep polifarmasi serta berusahan untuk melakukan kerja sama yang baik dengan dokter pembuat resep dalam rangka memberikan pengobatan yang efektif dan aman bagi pasien.Peran apoteker dalam meminimalkan risiko obat yang membahayakan bagi pasien sangat diperlukan. Apoteker dapat memaksimalkan konseling terhadap pasien untuk menyampaikan efek samping yang dapat terjadi terkait dengan obat serta penanganannya. Selain itu apoteker memiliki peran penting untuk mencegah adanya interaksi yang membahayakan pasien terkait polifarmasi yakni dengan memaksimalkan kolaborasi dengan dokter pembuat resep terkait pengobatan yang efektif bagi pasien.

Pharmacists are required to have adequate competence in carrying out pharmaceutical duties and responsibilities in pharmacies. Therefore, prospective pharmacists must be well-prepared before assuming these tasks and responsibilities. One program aimed at preparing prospective pharmacists is through the Pharmacist Professional Internship Program (PKPA). The PKPA was conducted from July 3-31, 2023, at Roxy Pharmacy Depok. The method used in preparing this specific task report employed literature review and investigation methods related to drug interactions and side effects that may occur in polypharmacy prescriptions during the July 2023 period. Based on the analysis of five prescriptions, the study results showed moderate drug interactions in prescriptions 2, 4, and 5. Therefore, these interactions need to be monitored to avoid endangering patient safety. Pharmacists play a crucial role in counseling patients receiving polypharmacy prescriptions and strive to collaborate effectively with prescribing doctors to ensure effective and safe treatment for patients. The pharmacist's role in minimizing the risk of dangerous drug effects for patients is essential. Pharmacists can maximize counseling for patients to convey potential drug-related side effects and their management. Additionally, pharmacists play a critical role in preventing dangerous interactions related to polypharmacy by maximizing collaboration with prescribing doctors regarding effective patient treatment."

"Terapi Sulih Hormon (HRT) estrogen dan progesteron digunakan untuk mengatasi gejala menopause dan untuk pencegahan osteoporosis. Terapi ini bermanfaat signifikan jika digunakan pada wanita dengan gejala dalam 10 tahun pertama setelah menopause atau pada usia di bawah 60 tahun. Penggunaannya dalam dosis tinggi dan jangka panjang dapat menimbulkan risiko kanker endometrium dan payudara. Apoteker dalam farmasi klinik bertanggung jawab memastikan resep hormon diatur dengan tepat untuk mencegah kesalahan pengobatan dengan melakukan pengkajian resep. Pengkajian resep dilakukan pada resep hormon dan kontrasepsi pasien yang masuk di Apotek Roxy Sawangan pada periode April 2023 dengan menelaah aspek administratif, aspek farmasetik dan aspek klinis. Kajian aspek administratif  pada resep 1: tidak ada SIP, paraf dokter dan berat badan pasien; dan pada resep 2 : tidak ada alamat dan berat badan pasien. Analisis farmasetik pada resep 1 menunjukkan kekeliruan penulisan cara pemberian untuk terapi cyclo progynova dan tidak ditemukan penyimpangan pada resep 2. Pada pengkajian aspek klinis resep 1 ditemukan ketidaksesuaian aturan pakai pada resep 1 dan cara pemakaian belum lengkap pada resep 2. Pengkajian lebih lanjut terhadap resep-resep hormon untuk indikasi lainnya perlu dilakukan untuk pencegahan masalah terkait obat dan kesalah pengobatan yang lebih baik.

Hormone Replacement Therapy (HRT) estrogen and progesterone is used to treat menopausal symptoms and for the prevention of osteoporosis. This therapy is of significant benefit if used in women with symptoms in the first 10 years after menopause or under the age of 60 years. Its use in high doses and long term can increase the risk of endometrial and breast cancer. Pharmacists in clinical pharmacy are responsible for ensuring hormone prescriptions are managed appropriately to prevent medication errors by reviewing prescriptions. Prescription reviews were carried out on hormone and contraceptive prescriptions for patients admitted to the Roxy Sawangan Pharmacy in the April 2023 period by examining administrative aspects, pharmaceutical aspects and clinical aspects. Study of administrative aspects of prescription 1: there is no SIP, doctor's initials and patient's weight; and in prescription 2: no patient's address and weight. Pharmaceutical analysis on prescription 1 showed an error in writing the administration method for cyclo progynova therapy and no irregularities were found in prescription 2. In assessing the clinical aspects of prescription 1, it was found that the instructions for use in prescription 1 were inconsistent and the instructions for use were incomplete in prescription 2. Further assessment of the prescriptions Prescribing hormones for other indications is necessary for better prevention of drug-related problems and medication errors."

Partisipan penelitian terdiri dari apoteker yang bekerja di Apotek Roxy Beji, dan konteksnya adalah pada periode bulan Juli saat program Praktek Kerja Profesi Apoteker (PKPA). Temuan menunjukkan bahwa skrining resep, khususnya untuk polifarmasi, sangat penting dalam mencegah masalah terkait obat dan kesalahan pengobatan. Proses skrining membantu menghindari salah tafsir, kegagalan komunikasi, dan kesalahan pengobatan di antara para profesional layanan kesehatan, yang pada akhirnya meningkatkan keselamatan pasien dan kualitas hidup. Prescription writing plays a crucial role in ensuring optimal communication between doctors, pharmacists, and nurses to enhance patient safety. Errors in prescription writing are common and can lead to medication errors, which can be fatal for patients. Screening prescriptions, especially for polypharmacy, is essential to prevent drug-related problems and medication errors.Research Problem and Aim: The research problem focuses on the importance of screening prescriptions, particularly for polypharmacy, to prevent medication errors and improve patient outcomes. The aim of the study is to investigate the impact of prescription screening on reducing medication errors and enhancing patient safety in a pharmacy setting.Methodology: The research design involves conducting prescription screening, including administrative, pharmaceutical, and clinical screening, to ensure the legality of prescriptions and minimize treatment errors. The study participants consist of pharmacists working at Apotek Roxy Beji, and the context is within the period of July during a Professional Work Practice of a Pharmacist (PKPA) program. The findings indicate that prescription screening, especially for polypharmacy, is essential in preventing drug-related problems and medication errors. The screening process helps in avoiding misinterpretations, communication failures, and medication errors between healthcare professionals, ultimately improving patient safety and quality of life. The study highlights the significance of prescription screening in reducing medication errors and enhancing patient safety. Implementing rigorous screening protocols in pharmacies can help prevent drug-related problems and improve treatment outcomes for patients."

"Hipertensi merupakan faktor risiko terhadap kerusakan organ penting seperti otak, jantung, ginjal, retina, pembuluh darah besar (aorta) dan pembuluh darah perifer (konsensus hipertensi). Oleh sebab itu seringkali pasien hipertensi menerima dua atau lebih obat (polifarmasi) sehingga terdapat kemungkinan resiko interaksi antar obat yang dapat mempengaruhi hasil terapi. Pemantauan interaksi obat perlu dilakukan pada resep pasien yang menerima terapi polifarmasi. Terapi antihipertensi dilakukan dalam jangka panjang sehingga perlu dilakukan pemantauan agar diperoleh target terapi dan mencegah terjadinya reaksi obat yang tidak diinginkan (ROTD). Apotek merupakan salah satu fasilitas pelayanan kesehatan yang memberikan pelayanan kefarmasian salah satunya adalah pengkajian dan pelayanan resep. Analisa interaksi obat pada resep hipertensi di Apotek Roxy Pondok Labu dilakukan berdasarkan informasi yang terdapat pada leaflet obat serta pengecekan interaksi pada beberapa situs drug interaction checker. Dari tiga resep yang dipilih merupakan resep yang ditebus pada bulan Maret-April 2023 ditemukan total 9 interaksi obat berdasarkan signifikansi dan 9 interaksi obat berdasarkan mekanisme kerja obat. Berdasarkan tingkat signifikansi interaksi obat yang ditemukan sebanyak 9 interaksi dimana interaksi moderate sebagai interaksi yang paling banyak muncul dengan persentase 66,67 % atau 6 dari total 9 interaksi. Berdasarkan mekanismenya, interaksi obat yang ditemukan sebanyak 9 interaksi dimana interaksi farmakodinamik yang paling banyak muncul dengan persentase 55,56 % atau 5 dari total 9 interaksi.

Hypertension is a risk factor for damage to important organs such as the brain, heart, kidneys, retina, large blood vessels (aorta) and peripheral blood vessels (hypertension consensus). Therefore, hypertension patients often receive two or more drugs (polypharmacy) so that there is a possible risk of interactions between drugs that can affect the results of therapy. Monitoring of drug interactions needs to be done on the prescriptions of patients receiving polypharmacy therapy. Antihypertensive therapy is carried out in the long term so that monitoring is necessary to obtain therapeutic targets and prevent unwanted Adverse Drug Reaction (ADR). Pharmacy is one of the health care facilities that provides pharmaceutical services, one of which is prescription assessment and service. Analysis of drug interactions in hypertension prescriptions at Roxy Pondok Labu Pharmacy was carried out based on information contained in the drug leaflet and checking interactions on several drug interaction checker sites. Of the three prescriptions selected, which were prescriptions filled in March-April 2023, a total of 9 drug interactions based on significance and 9 drug interactions based on drug mechanism of action were found. Based on the level of significance of drug interactions found as many as 9 interactions where moderate interactions as the most common interaction with a percentage of 66.67% or 6 out of a total of 9 interactions. Based on the mechanism, 9 drug interactions were found where pharmacodynamic interactions were the most common with a percentage of 55.56% or 5 out of a total of 9 interactions."

"Penyakit infeksi merupakan penyakit yang dapat disebabkan oleh mikroorganisme patogen, seperti bakteri, virus, parasit atau jamur. Terapi untuk mengobati penyakit infeksi antara lain antibakteri/ antibiotik, antijamur, antivirus, antiprotozoa. Penggunaan obat antibiotik bukan tanpa risiko, terlebih jika masyarakat mengkonsumsi obat ini sembarangan. Penggunaan obat antibiotik harus dengan resep dokter agar hasilnya semakin optimal. Resep dari dokter tersebut harus memuat informasi yang jelas agar apoteker maupun petugas kefarmasian dapat menyiapkan obat dengan baik, maka dari itu perlu dilakukan pengkajian resep terlebih dahulu untuk menghindari terjadinya kesalahan dalam menyediakan dan memberikan obat kepada pasien. Pada tugas khusus ini dilakukan pengkajian resep terhadap tiga resep obat yang mengandung antibiotik di Apotek Roxy Pondok Labu. Pada kegiatan pengkajian resep, terdapat kajian terkait administrasi yang berisikan nama, umur, jenis kelamin, berat badan pasien, nama dokter, nomor Surat Izin Praktik (SIP), alamat, nomor telepon, paraf, dan tanggal penulisan resep. Kajian kesesuaian farmasetik yang berisikan bentuk sediaan, kekuatan sediaan, stabilitas dan kompatibilitas. Kajian pertimbangan klinis yang memuat informasi ketepatan indikasi dan dosis obat, aturan dan lama penggunaan obat, duplikasi atau polifarmasi, reaksi obat yang tidak diinginkan, kontraindikasi, dan interaksi obat. Berdasarkan data dan analisis yang telah dilakukan, dapat hasil menunjukkan bahwa kegiatan pengkajian tiga resep obat antibiotik dapat terdokumentasikan dengan baik. Tiga resep yang dikaji secara administratif, kesesuaian farmasetik, dan pertimbangan klinis cukup lengkap dan memberikan informasi yang cukup untuk apoteker maupun tenaga kefarmasian lainnya untuk menyediakan dan memberikan obat yang sesuai kepada pasien.

Therapy to treat infectious diseases includes antibacterial/antibiotic agents, antifungal, antiviral, and antiprotozoal medications. The use of antibiotics is not without risks, especially if people use these drugs carelessly. Antibiotics also must be prescribed by a doctor, ensuring optimal results. The prescription from the doctor must contain clear information so that pharmacists and pharmacy staff can prepare the medicine properly, therefore it is necessary to review the prescription first to avoid errors in providing and administering the medicine to the patient. In this special assignment, a prescription review was carried out for three prescription drugs containing antibiotics at the Apotek Roxy Pondok Labu. In the prescription review activity, there is a study related to administration which contains the patient's name, age, gender, weight, doctor's name, Practice License number (SIP), address, telephone number, initials and date of writing the prescription. Pharmaceutical suitability studies containing dosage form, dosage strength, stability and compatibility. A study of clinical considerations that includes information on the accuracy of indications and drug doses, rules and duration of drug use, polypharmacy, unwanted drug reactions, contraindications and drug interactions. Based on the data and analysis that has been carried out, the results show that the review activities for three antibiotic drug prescriptions can be well documented. The three prescriptions reviewed administratively, pharmaceutical suitability, and clinical considerations are quite complete and provide sufficient information for pharmacists and other pharmaceutical personnel to provide and administer appropriate medications to patients."
