ABSTRACT
Purpose To evaluate the status of surgical training in Japan through a national level needs assessment. Methods A survey was sent to all 909 graduating residents (GRs) and their 611 program directors (PDs) for the year 2016. A working group of surgical educators from around the country was formed under the education committee of the Japan Surgical Society. The survey items were developed by consensus of this working group. The survey investigated the knowledge and problems of the current curriculum, and the status of the current residency training. Results The response rates were 56,3% of the GRs and 76,8% of the PDs. Among the participants, 47,6% of the GRs and 29,4% of the PDs believed that the residency curriculum did not match the clinical experience. Over 80% of the GRs and PDs agreed on the importance of training outside of the OR, whereas only 13% of the GRs had received such training regularly. Trainees also reported a lower satisfaction rate about the opportunity to train outside of the OR. Conclusion This national level needs assessment of surgical training in Japan identified several gaps in the curriculum. These results provide valuable data to assist the ongoing efforts for surgical residency curriculum improvement.

ABSTRAK
Latar belakang Lulusan fakultas kedokteran harus mampu bertugas sebagai dokter layanan primer, sesuai Standar Kompetensi Dokter Indonesia. Karena itu, ilmu kedokteran komunitas mempunyai posisi strategis dalam menyelesaikan masalah kesehatan. Kebutuhan dokter layanan primer akan kompetensi ilmu kedokteran komunitas seharusnya sesuai dengan kompetensi yang diberikan melalui kurikulum fakultas kedokteran. Penelitian ini bertujuan untuk melihat kesesuaian kompetensi ilmu kedokteran komunitas yang ada dalam SKDI dengan kebutuhan dokter puskesmas dan kompetensi ilmu kedokteran komunitas yang tertuang dalam kurikulum Program Studi Kedokteran Universitas Batam (Prodi KUB)

Metode Desain penelitian potong lintang dilaksanakan menggunakan kuesioner dengan skala Likert, untuk mengetahui kesesuaian tujuh belas (17) kompetensi kedokteran komunitas dalam SKDI dengan kebutuhan kompetensi kedokteran komunitas di puskesmas. Selain itu, jugadianalisis kesesuaian kompetensi ilmu kedokteran komunitas SKDI dalam Prodi kedokteran KUB oleh suatu tim ahli. Hasil kuesioner kebutuhandihubungkan dengan hasil analisis kurikulum tim.

Hasil Dari 84 dokter puskesmas ada 72 (85,7%) memenuhi syarat. Kompetensi dalam Prodi KUB telah mencakup ke tujuh belas butir kompetensi ilmu kedokteran komunitas SKDI dengan pencapaian kompetensi antara 2-4. Dokter puskesmas membutuhkan semua kompetensi dalam SKDI dengan nilai Likert antara 4,50 – 5,00 dari penilaian 1-6. Didapat korelasi positif lemah tidak bermakna, antara kompetensi kedokteran komunitas dalam kurikulum Prodi KUB dengan kebutuhan kompetensi yang dirasakan dokter puskesmas (r= 0,267; p = 0,30).

Simpulan Kompetensi kedokteran komunitas kurikulum Prodi KUB telah sesuai dengan materi kompetensi SKDI. Kebutuhan kompetensi kedokteran komunitas yang dirasakan dokter puskesmas belum ditunjang dengan tingkat kompetensi dalam kurikulum Prodi KUB.
ABSTRACT
Background A medical graduate should be able to carry out duties as a primary care phycisian in accordance with Indonesian Competency Standard (ICS). In line with this, the role of community medicine (CM) science is strategic in resolution of health problems especially in the health center. Community Medicine competency is gained from the medical school curriculum. This study aim to asses the relevance between CM competency in curriculum of Faculty of Medicine in Batam (KUB) to the needs of primary health care doctor in carrying out the task in health center.

Method A cross sectional study with questionaire (Likert scale 1-6) used to determine the compatibility of primary care doctor needs of CM competency with the seventeen competencies in ICS. The relevance of CM competencies in the KUB curriculum with the ICS was also analyzed by an expert team from KUB. The result of the questionaire then was correlate with the analysis result of the team.

Result Among 84 health center doctors in Batam, 72 (85,7%) were elligible for this study. Respondents mentioned that all CM competencies in the ICS were needed in their role in health center, with a range of 4.5 – 5.0 Likert scale. Analysis of KUB curriculum reveal that all CM competencies in ICS were covered, and the level varies between 2-4. There was a positive weak, not significant correlation between the need of CM according to the doctors and the competencies in the KUB curriculum (r= 0,267; p = 0,30).

Conclusion Community Medicine competency in KUB curriculum is in conformity with the ICS. High need of CM competency felt by the primary doctor in Batam has not been supported by the curriculum above.
