"Skripsi ini membahas kecelakaan kerja yang terjadi di job site ADMO PT. Saptaindra Sejati pada tahun 2006 - 2008. Penelitian ini merupakan penelitian deskriptif analitik, dimana penulis memberikan gambaran kecelakaan kerja yang terjadi serta melakukan analisis terhadap penyebab kecelakaan dengan menggunakan Loss Causation Model untuk mengetahui penyebab langsung, penyebab dasar, serta kurangnya pengendalian dari pihak manajemen.
Hasil penelitian menyarankan pihak perusahaan untuk meningkatkan pelaksanaan pelatihan serta melakukan evaluasi terhadap materi dan metode pelatihan yang sudah digunakan. Selain itu, perlu dilakukan pengawasan berupa inspeksi terdokumentasi pada area tambang untuk mengetahui adanya kecenderungan tindakan dan kondisi berbahaya agar tindakan perbaikan dapat segera dilakukan.

The focus of this study is occupational accident that happened at ADMO coal mining project of PT. Saptaindra Sejati on 2006 - 2008. This research try to explain the details of occupational accident and analyze the causes of accident by using Loss Causation Model to know immediate causes, basic causes and lack of control from management.
The result of research, suggest company to increase training implementation and commit evaluation to the subject matter training and training method that already used. Besides that, the company need to do documented inspection at mining area to know is there any tendency of unsafe act and unsafe condition so that improvement can be immediately execute.

"Banyaknya kendaraan yang ada di Jakarta rnenyebabkan terjadinya kemacetan yang ada pada ruas-ruas jalan ibukota. Ketersediaan prasarana jalan yang hampir jenuh untuk dibangun membuat hampir tidak tertampungnya kendaraan yang ada. Inilah salah satu yang melatarbelakangi mengapa diperlukan mass rapid transit (busway) atau angkutan yang dapat mengangkut masyarakat dalam jumlah banyak, aman serta nyaman. Dalam perkembangannya sampai dengan saat , keberadaan jalur busway masih sangat rawan terhadap kecelakaan karena jalur tersebut memiliki interaksi dengan jalan umum dan masih rendahnya kesadaran masyarakat pengguna jalur urnurn untuk tidak rnenggunakan jalur busway tersebut.
Menurut data yang didapat penulis, kecelakaan yang terjadi pada koridor I sarnpai koridor VII sampai dengan bulan Juni 2008 sebanyak 96 kejadian. Inilah yang melatarbelakangi mengapa penulis tertarik untuk menganalisa penyebab kecelakaan yang terjadi dan memberikan solusi pencegahan berdasarkan penyebab tersebut Analisa yang akan digunakan oleh penulis adalah penyajian data tabulasi sederhana berdasarkan laporan polisi yang ada, danjuga analisa deskriptif berdasarkan kasus dan kondisi lapangan yang terkait di dalarnnya. Penulis juga rnenggunakan data sekunder berupa Laporan Polisi yang didapat dan Ditlantas Polda Metro Jaya. Dan penelitian yang dilakukan, penulis melihat adanya peningkatan angka kecelakaan yang terjadi sejak awal pengoperasian busway tahun 2004 sampai dengan bulan Juni 2008. Beberapa faktor yang menjadi penyebab dan kecelakaan tersebut antara lain kelalaian pengguna jalan kaki dan keberadaan tata letak separator serta nambu jalan.
Berdasarkan analisa penulis terhadap kasus kecelakaan yang terjadi pencegahan yang dapat dilakukan adalah pernenintah memberikan pengedukasian dan memberikan infonmasi yang berhubungan terhadap peningkatan kesadaran keselarnatan berkendaraan kepada pengemudi busway, pengendara kendaraan benmotor serta masyanakat luas, serta mempenbaiki tata letak separator dan rambu yang ada.

Many vehides at hkarta cause traffic jam to some highway at the dty. The Availability of the existing infrastructure that almost surfeited, makes the vehides almost can not handled. This is why the mass rapid transit is needed or the transportation that can transfer a lot of people with safe and convertible. The growth rate of bus way shows that until now the bus way still potentially to cause an accident because bus way have interaction with the public road and the awareness of the people to not use the bus way is still in lower.
According to the latest data, the accident which happened at l st - 7 th corridor are 96 case. This is why the writer interested to analyzed the cause of acddent and give the prevention based on that cause. Analyze that the writer used are the simple tabulation presentation data based police report and also descriptive analysis based on case and road condition. The writer also used secondary data in the form of Police Report which is got from Directorate of Highway Metro Jaya Police. From the research that have been done, the writer sees the value of acddent is increasing which happened since the l st bus way operated in 2004 until ..Line 2008. Sme of the factors that cause that accident are laxity user hiker and the arrangement location of arbiter road or disposition of traffic light.
Based on the writer analyze to every accident case that has been happened, the prevention that can be doing by the government are to educate and give information related to increasing of awareness safety in riding to all busway driver, motorcydist and to the community, and also fix the disposition of arbiter road and traffic light that has been existing.

"ABSTRAK
Industri penerbangan sipil di Indonesia telah dinominasikan oleh beberapa lembaga survei sebagai industri dengan tingkat keselamatan terendah di dunia. Banyaknya kecelakaan penerbangan sipil di Indonesia disebabkan oleh sistem manajemen keselamatan yang buruk. Menurut Maintenance Error Decision Aid (MEDA), saat ini, 80% kecelakaan penerbangan disebabkan oleh kesalahan manusia (pilot, pengontrol lalu lintas udara, mekanik, dll). Hasil ini berbeda dibandingkan dengan tahun-tahun awal industri penerbangan yang 80% kecelakaan disebabkan oleh kegagalan mesin. Oleh karena itu, kita harus menemukan metode yang paling tepat untuk menganalisis kecelakaan penerbangan untuk mencegah terulangnya hal itu. Penerbangan sipil berjadwal di Indonesia hampir mewakili semua penerbangan sipil karena masih belum umumnya industri general aviation di Indonesia. Oleh karena itu, penelitian ini mengusulkan model analisis dan investigasi kecelakaan yang dimodifikasi berdasarkan swiss cheese model untuk mengidentifikasi faktor manusia dan organisasi yang terkait dalam kecelakaan penerbangan sipil berjadwal. Model ini akan terdiri dari kategori dan subkategori yang dikembangkan oleh model sebab-akibat yang dikombinasikan dengan hukum dan peraturan yang berlaku serta praktik sistem manajemen keselamatan di industri penerbangan sipil berjadwal di Indonesia. Model yang diusulkan diharapkan dapat menganalisis kecelakaan penerbangan sipil terjadwal dengan lebih baik dan jelas serta membantu manajemen untuk mengambil tindakan keselamatan yang diperlukan untuk mencegah terulangnya kecelakaan.

ABSTRACT
Civil aviation industry in Indonesia has been nominated by some survey institutes to be the lowest in safety rating in the world. This is caused by poor safety management system which lead to many civil aviation accidents in Indonesia. According to Maintenance Error Decision Aid (MEDA), nowadays, 80% of aviation accident are due to human error (pilots, air traffic controllers, mechanics, etc). This result differ compared to the early years of the aviation industry which is 80% of accident are caused by machine failure. Therefore, we have to find the most appropriate method to analyze an aviation accident in order to prevent its reccurence. In Indonesia, scheduled civil aviation almost represent all civil aviation in the country. Therefore, This research proposed a modified accident analysis and investigation model based on swiss cheese model to identify the human and organizational factors involved in scheduled civil aviation accidents. The model will be consist of categories and subcategories which is developed by classic ancient causation models combined with the laws and regulation in Indonesia and a safety management system practices in the scheduled civil aviation industry. The proposed model is expected to be able to analyze scheduled civil aviation accident better and clearer and help the management to take a safety action needed to prevent the recurence of accidents."

It will also presents a template and accident database focusing on fatal electrocutions and falls in the construction industry. Data has been stored in MySQL database, and a program editor has been employed to customize the program for accident analysis. An accident analysis form connected to the database has been developed, along with an accident report in qualitative format and meaningful categories to provide analyzable data. The program can be used as soon as an inspector finishes preparing an accident report; accessing the database minimizes the lead time for translating qualitative reports into analyzable data, so that the Council for Labor Affairs, corporate personel, and inspectors can have real time information about accident trends and patterns for the prevention of repetitive accidents."
