Abstrak :
This undergraduate thesis covers policies, phenomena, and critical situations in Indonesian economy during the period of 1980 through 2005. This long period is divided into two periods: policy reform era in 1988-1997 and post crisis era in 1998-2005. In order to be able to properly identify the most binding constraint of Indonesian economy and to compare it to other economies, the method used is fixed effect model or least square dummy variables. The empirical results and narrative analysis suggest that the most binding constraint during the two periods is educational attainment. The most interesting result is the meager influence of institutions quality on economic activity during the New Order Era but increasing after it. Another staggering result is that the financial and banking deregulations implied by access to sound money were negatively related to growth. The results also indicate that Indonesia needs to sharpen its expenditure on productive sectors. In addition, democracy needs good treatment in the long term.

ABSTRAK
Background: diagnostic of pulmonary TB in HIV patients is a problem due to non specific clinical features, or radiological appearance. HIV patients with CD4≤200 cells/mL infected with M. tuberculosis have less capacity in containing M. tuberculosis, developing granulomas, casseous necrosis, or cavities. This condition is caused by weakend inflammatory which later reduced sputum production and may cause false negative result. This study aimed to assess differences in the positivity level of acid fast bacilli (AFB) and cultures of M. tuberculosis from non-bronchoscopic sputum (spontaneous and induced sputum) compared to bronchoscopic sputum (bronchoalveolar lavage) in HIV positive patients suspected pulmonary tuberculosis with CD4<200 cells/μL. Methods: this cross sectional study was conducted in adult HIV patients treated in Hasan Sadikin Hospital with CD4≤200 cells/μL suspected with pulmonary tuberculosis by using paired comparative analytic test. All patients expelled sputum spontaneously or with sputum induction on the first day. On the next day, bronchoalveolar lavage (BAL) was performed. The two samples obtained from two methods were examined by AFB examination with staining Ziehl Neelsen (ZN) and cultured of M. tuberculosis on solid media Ogawa on all patients. Positivity, sensitivity and increased sensitivity of AFB and culture of M. tuberculosis in the non bronchoscopic and bronchoscopic groups were compared. Results: there were differences in the positivity level of AFB with ZN staining between non-bronchoscopic and bronchoscopic groups which were 7/40 (17.5%) vs 20/40 (50.0%) (p<0.001). The differences between the cultures of non-bronchoscopic and bronchoscopic groups were 16/40 (40.0%) vs 23/40 (57.5%) (p=0.039). Bronchoscopic sputum increased the positivity level of the ZN AFB examination by 32.5% (from 17.5% to 50.0%) as well as on culture examination by 17.5% (from 40.0% to 57.5%). Conclusion: Bronchoalveolar lavage can improve the positivity level of smears and cultures in patients suspected of pulmonary TB in HIV patients with CD4<200 cells/μL.

The sequencing of the human genome opened a new world of biomarkers. Is it possible to tell by a person's genetic signature how they would respond to a particular drug? his is the dream of "personalized medicine". At the moment as many as half of all drugs do not work for the people who take them. Such pharmacogenomics could also reduce the size and cost of clinical tests by allowing pharma firms to select the most suitable patients. But the reality is rather different. The difficulty lies in proving that they reliably correlete with clinical outcome. Companies that had placed their faith in genomics are now caught in a dilemma: to produce information only or new drugs. As the cost of gene sequencing has fallen, firms have rushed to offer genetic tests directly to consumers, often raising grand expectations. There now seems to be a backlash. Doctors have complained about being bypassed. Most generic tests directly do not provide conclusive evidence of this risk of disease Some academics also remain deeply skeptical about genomics being used in medicine. In addition , personalised medicine would be economic folly for firms who would need to develop a special pill for every patient. Traditionally firms sold conventional one - size - all drugs. Perhaps a better way would be mass customization.

This book reviews the use of theranostic nanoparticles for cancer applications over the past decade. First, it briefly discusses the challenges and limitations of conventional cancer treatments, and presents an overview of the use of nanotechnology in treating cancer. These introductory chapters are followed by those exploring cancer diagnosis and a myriad of delivery methods for nanotherapeutics. The book also addresses multifunctional platforms, treatment monitoring, and regulatory considerations. As a whole, the book aims to briefly summarize the development and clinical potential of various nanotheranostics for cancer applications, and to delineate the challenges that must be overcome for successful clinical development and implementation of such cancer theranostics.
