"Abstrak
Heart failure has become a public health problem with increasing incidence and prevalence. Many patients first came to the primary care and managed by general practitioner. Accurate diagnosis is essential to provide good management of heart failure. However, symptoms and signs alone are often neither sufficient nor specific to confirm the diagnosis. Some studies show that heart failure patients are still incorrectly diagnosed and inadequately treated, despite the availability of current guidelines. Inadequacy of facilities is the main obstacle in diagnosing and managing heart failure, especially in developing countries like Indonesia. The medications recommended for treating heart failure are still under-prescribed. This review discuss about the challenges of diagnosis and management of heart failure in primary care."

"Masih terjadi kekurangan sumber daya manusia dokter pada sistem layanankesehatan di indonesia maupun di Provinsi Sumatera Selatan. Ketersediaan dokterdipengaruhi produksi, sebaran, dan pengembangan karir. Penelitian ini bertujuanuntuk mengetahui preferensi pilihan karir dokter internsip di Provinsi SumateraSelatan dan faktor-faktor yang berhubungan dengan pilihan karir tersebut. Faktoryang diamati adalah pendapatan, kemudahan mencari kerja, prestise, persepsimasa depan, jam kerja reguler, jam kerja fleksibel, pekerjaan tanpa jaga malam,cita-cita dan wilayah kerja.Penelitian survey deskriptif dengan pendekatan kuantitatif, cross-sectional.Pengumpulan data dilakukan menggunakan kuesioner dengan skala likert.Analisis data menggunakan Aplikasi Statistik.Populasi penelitian adalah dokter peserta program internsip di ProvinsiSumatera Selatan. Sampel yang digunakan sebanyak 112 orang yang memenuhikriteria inklusi. Hasil penelitian mendapatkan 73,2 responden menginginkandokter spesialis sebagai pilihan karirnya, 18,8 responden menginginkan dokterumum sebagai pilihan karirnya sementara 8 memilih karir lain-lain. Didapatkanjenis kelamin, asal universitas dan daerah asal tidak berhubungan dengan pilihankarir. Variabel pendapatan, kemudahan mencari kerja, jam kerja yang fleksibel,dan wilayah kerja pada uji hipotesis berhubungan dengan pilihan karir, sedangkanvariabel prestise, beban kerja, masa depan, masa studi dan cita-cita tidak terbuktiberhubungan dengan pilihan karir.Dari hasil temuan diatas disimpulkan terdapat kecenderungan dokter untuklebih memilih dokter spesialis sebagai pilihan karirnya. Pilihan karir berhubungandengan pendapatan, kemudahan mencari kerja, jam kerja dan wilayah kerjanya.Untuk itu disarankan agar tidak terjadi ketimpangan dalam tenaga kerja dokterdapat dibuat kebijakan untuk membuat karir sebagai dokter umum lebih menarikdengan melakukan intervensi terhadap faktor-faktor yang berhubungan sepertipeningkatan pendapatan atau kepastian kerja bagi dokter umum.

There is still a shortage of human resources physicians on health caresystems in Indonesia as well as in Southern Sumatera Province. The availabilityof doctors influenced by the production, distribution, and career development.This study aims to find out the preferences of career choice of doctor internsip inSouthern Sumatera Province and factors related to career choice. Factors observedare income, ease of job search, prestige, future perception, regular working hours,flexible working hours, work without night shift, goals and work areas.Descriptive survey research with quantitative approach, cross sectional. Thedata were collected using questionnaires with Likert scale. Data analysis usingStatistical Application.The study population is the doctor of internsip program participants inSouthern Sumatera Province. The sample used was 112 people who fulfilled theinclusion criteria. The study found that 73.2 of respondents wanted a specialistas a career choice, 18.8 of respondents wanted a general practitioner as theircareer choice while 8 chose another career. Gender, origin of university andarea of origin not related to career choice. Revenue factors, easy job search,flexible working hours, and work areas on hypothesis testing are related to careerchoice, while prestige factors, workload, future, study period and goals are notproven to be related to career choice.From the results of the above findings concluded there is a tendency doctorsto prefer a specialist as a career choice. Career options relate to income, ease ofjob search, working hours and work area. It is suggested that in order not tohappen imbalance in the workforce of doctors, policy can be made to make acareer as a general practitioner more interesting by intervening on related factorssuch as increasing income or working field certainty for general practitioners."
