Abstrak :
Latar belakang: Tingkat keparahan cedera ginjal iskemia-reperfusi (I/R) berhubungan erat dengan tingginya angka kesakitan dan kematian. Hasil penelitian terdahulu pada manusia dan hewan telah mebuktikan bahwa Neutrophil Gelatinase Associated Lipocalin (NGAL) dapat mendeteksi dan memprediksi terjadinya cedera ginjal I/R dini. Tujuan penelitian ini adalah untuk membuktikan bahwa peningkatan kadar NGAL serum dan urin berhubungan dengan kerusakan epitel tubuli ginjal pada tikus yang mengalami iskemia reperfusi dini. Metode: Peneltian ini menggunakan 28 ekor tikus Sprague-Dawley jantan sebagai hewan model, dikelompokkan dalam 4 kelompok: sham 4 jam (Sham 4), sham 8 jam (Sham 8), iskemia 10 menit reperfusi 4 jam (I/R 4), dan iskemia 10 menit reperfusi 8 jam (I/R 8). Analisis kadar kreatinin serum diperiksa dengan metode Jaffe, sedangkan NGAL serum dan urin menggunakan metode ELISA Direct Sandwich. Evaluasi tingkat kerusakan jaringan ginjal dilakukan secara semi kuantitatif pada sediaan histologi dengan pulasan HE. Deskripsi kelainan tingkat seluler ginjal diperjelas melalui evaluasi menggunakan mikroskop elektron dan Imunohistokimia (IHK). Hasil: Kadar NGAL serum berkorelasi bermakna dengan tingkat kerusakan ginjal (ρSpearman NGAL serum = 0,701, p < 0,001), juga kadar NGAL urin berkorelasi bermakna dengan tingkat kerusakan ginjal (ρSpearman = 0,689, p < 0,001). Tingkat ekspresi NGAL lebih tinggi pada kelompok I/R dibanding sham (t-test, t = -26635,046, p < 0,001), juga tingkat kerusakan ginjal tikus (t-test, t = -5,028, p < 0,001), dan kadar NGAL serum dan urin pada kelompok I/R berbeda nyata dibanding sham (Mann-Whitney, U = 0, p < 0,001). Pada cutoff point 136,95 ng/mL dan 58,69 ng/mL berturut ? turut untuk NGAL serum dan urin diperoleh sensitivitas = 1, spesifisitas = 1. Kesimpulan: Peningkatan kadar NGAL serum dan urin berkorelasi dengan kerusakan epitel tubuli ginjal pada tikus yang mengalami cedera ginjal iskemia reperfusi dini.
Background: The severity of ischemia-reperfusion (I/R) kidney injury is highly correlated with mortality and morbidity rate. Research on human and animal prove that NGAL predicts kidney injury at early phase. The objective of this study is to prove that the increase in serum and urinary NGAL are correlated with kidney tubular epithelial damage, and this increase has occurred in initiation phase, indicated by rat kidney histopathology in an early I/R model. Methods: Twenty eight male Sprague-Dawley rats were divided into 4 groups: 4 hour sham (Sham 4), 8 hour sham (Sham 8), 10 minute ischemia 4 hour reperfusion (I/R 4) and 10 minute ischemia 8 hour reperfusion (I/R 8). Blood, urine and kidney samples were collected. Serum creatinine level was analyzed with Jaffe method, while serum and urinary NGAL level were analyzed with direct sandwich ELISA method. Evaluation of kidney damage were measured semi quantitatively in tissue stained with HE. Further evaluation to confirm cellular changes on kidney was performed by electron microscope and immunohistochemistry. Results: Serum NGAL was found significantly correlated with degree of kidney tissue damage (ρSpearman NGAL serum = 0.701, p < 0.001), also urinary NGAL (ρSpearman = 0.689, p < 0.001). NGAL expression differs significantly between I/R group and sham (t-test, t = -26635.056, p < 0.001), also kidney damage (t-test, t = -5.028, p < 0.001), and serum and urinary NGAL levels (Mann-Whitney, U = 0, p < 0.001). With cutoff points of 136.95 ng/mL and 58.69 ng/mL subsequently for serum and urinary NGAL , it is found that sensitivity = 1, specificity = 1. Conclusion: Elevation of serum and urinary NGAL are significantly correlated with epithelial tubular kidney damage on rat undergoing early ischaemia reperfusion.

Penyakit kusta masih menjadi masalah kesehatan di dunia terutama di Indonesia sebagai negara dengan prevalensi tertinggi ketiga di dunia. Gejala klinis dan pemeriksaan penunjang yang ada sulit untuk menegakkan diagnosis definitif kusta pausibasilar (PB) dengan basil tahan asam (BTA) negatif. Diperlukan uji alternatif seperti real-time polymerase chain reaction berbasis probe TaqMan yang sensitif dan spesifik untuk mendeteksi Mycobacterium leprae. Tujuan penelitian ini adalah mendeteksi M. leprae pada pasien kusta PB dengan basil tahan asam negatif, dengan menggunakan real-time polymerase chain reaction (PCR) berbasis probe TaqMan pada spesimen kerokan jaringan kulit dan jaringan biopsi kulit. Sebanyak 24 pasien yang menjadi sampel penelitian. Setiap pasien dilakukan pengambilan spesimen kerokan jaringan kulit dari cuping telinga dan lesi kulit dan jaringan biopsi kulit. Uji TaqMan real-time PCR memberikan positivitas sebesar 20,8%, 25% dan 95,8% pada spesimen kerokan jaringan kulit cuping telinga, lesi kulit dan pada jaringan biopsi kulit. Positivitas hasil pemeriksaan patologi anatomi (PA) yang menunjukkan kusta sebesar 79,2%. Hal ini menunjukkan bahwa pemeriksaan TaqMan real-time PCR mampu meningkatkan kemampuan diagnosis pemeriksaan PA sebesar 16,6%. yang selama ini menjadi pemeriksaan baku emas kusta. Spesimen yang dianjurkan untuk digunakan pada pemeriksaan TaqMan real-time PCR adalah jaringan biopsi kulit. ...... Leprosy is still considered as a major health problem worldwide especially in Indonesia as the third highest prevalence in the world. The clinical symptoms are often similar with other skin diseases and the current available laboratory methods to establish definitive diagnosis of paucibacillary leprosy with negative acid-fast bacilli is difficult. Therefore, it is important to apply an alternative assay such as real-time polymerase chain reaction-based TaqMan probe for sensitive and specific detection of Mycobacterium leprae. The aim of this study is to detect M. leprae in paucibacillary leprosy with negative acid-fast bacilli using real-time PCR-based TaqMan probe in slit skin and skin biopsy tissue specimens. A total of 24 patients were selected as sample. From each patient it was taken slit skin specimen from the ear lobe and skin lesions and skin biopsy tissue specimen. The TaqMan real-time PCR showed 20,8%, 25% and 95,8% positivity in slit skin of earlobes, slit skin lession and skin biopsy tissue respectively. Histopatology examination result showed 79,2% positivity of leprosy. It showed that TaqMan real-time PCR could improve the diagnosis by 16,6% compared to histopathology which considered as gold standard. Specimen recommended to be used is skin biopsy tissue.

Kasus ?Primitive Neuro Ectodermal Tumor? (PNET) sangat jarang dan sangat sukar didiagnosis. Sebuah kasus PNET didiagnosis dengan teknik histopatologi dan pemeriksaan imunohistokimia. Seorang bayi laki-laki umur 4 bulan diperiksakan ke rumah sakit dengan benjolan pada dinding dada sejak bayi tersebut berumur 3 hari. Benjolan tersebut makin lama makin membesar hingga akhirnya mencapai diameter ± 10 cm, selanjutnya penderita dibawa ke klinik. Benjolan tersebut terfiksir pada dinding dada dengan batas tidak tegas, pada kulit diatas tumor tampak dua ulkus. Selanjutnya tumor tersebut didiagnosis sebagai suatu hemangioma. Secara makroskopis tumor berukuran 17 x 13 x 5,5 cm, berbatas tidak tegas, berwarna putih dan lunak. Secara mikroskopis massa tumor terdiri atas sel-sel berukuran kecil yang tidak berdiferensiasi, berbentuk bulat-oval, dengan inti hiperkromatik, dan sebagian membentuk struktur roset, Homer-Wright di antara bagian lainnya yang difus. Mitosis 7/10 HPF, nekrosis minimal kurang dari 25%. Gambaran ini sesuai dengan suatu ?malignant small round sel tumor?, Pada pemeriksaan imunohistokimia dengan panel antibodi meliputi Vimentin, NSE, Chromogranin dan CD99 menunjukkan Vimentin positif lemah-sedang, NSE negatif-positif lemah, Chromogranin negatif-positif lemah dan CD99 positif lemah-sedang. Secara keseluruhan, berdasarkan pemeriksaan makroskopis, histopatologik, dan imunohistokimia disimpulkan sebagai suatu ?Malignant Small Round Cell Tumor? yang sesuai dengan PNET / ES (Ewing?s sarcoma) yang perlu di konfirmasi dengan pemeriksaan sitogenetik. Primitive Neuro Ectodermal Tumor (PNET) is rare and difficult to diagnose. A case of PNET was diagnosed based on histopathological and immunohistochemical findings. A 4-month-old infant was admitted to the hospital with a tumor on the midline of his chest wall since he was 3 days old. The tumor was fixed on the chest wall and had ill-defined margin, enlarged over time and reached more than 10 cm in diameter when he was brought to a clinician. Two small ulcers were seen on the skin overlying the tumor. It was diagnosed as soft tissue tumor suggestive of a hemangioma. The tumor was 17 x 13 x 5.5 cm in size, white colored and firm to the touch. Microscopic examination revealed malignant small round cells with round to ovoid nuclei, coarse chromatin and scanty cytoplasm. Most cells were arranged in a solid pattern with scattered Homer-Wright rosettes. The mitotic count was 7/10 HPF, and necrosis was minimal (less than 25%). On immunohistochemical examination, the cells showed weak to moderate immunoreactivity to Vimentin and CD99, but showed negative to weak positive reactivity to NSE and Chromogranin. Based on the clinical features, gross findings, histopathologic and immunohistochemical examinations, the case was diagnosed as a malignant small round cell tumor consistent with PNET / ES (Ewing?s Sarcoma). To confirm the diagnosis, cytogenetic examination is suggested. (Med J Indones 2007; 16:108-12).

ABSTRAK
Tuberkulosis (TB) orbita adalah kondisi yang jarang terjadi. Tuberkulosis kelenjar lakrimalis atau dakrioadenitis adalah kelainan pada TB orbita yang sangat jarang terjadi, bahkan di negara-negara endemik dimana kejadian TB paru dan luar paru memiliki prevalensi yang sangat tinggi. Seorang wanita usia 48 tahun dengan diplopia, secara perlahan-lahan mengalami pembengkakan tanpa rasa sakit dibagian atas dan bawah kelopak mata. Tidak ada riwayat demam atau penyakit paru. Histopatologi menunjukan adanya peradangan granulomatosa dengan sel raksasa datia langhans yang berasal dari kelenjar lakrimalis. Studi mikrobiologi untuk menunemukan BTA menunjukkan hasil yang negatif. Pasien merespon baik terhadap pengobatan antituberkulosis. TB kelenjar lakrimalis tidak mudah untuk didiagnosis karena pada umumnya tidak terdapat TB aktif secara sistemik muncul bersamaan. Walaupun TB kelenjar lakrimalis merupakan manifestasi TB yang sangat jarang, tetapi penting untuk dikenali keberadaannya, khususnya ketika pasien berasak dari daerah endemik seperti Indonesia.

ABSTRAK
Helicobacter pylori diperkirakan menginfeksi lebih dari setengah populasi orang dewasa. Deteksi dini H.pylori sangat diperlukan untuk mencegah berkembangnya infeksi menjadi keganasan lambung. Bentuk coccoid dari H. pylori sulit dideteksi dengan kultur dan histopatologi, namun dapat terdeteksi dengan metode molekuler seperti real-time PCR. Salah satu gen yang dapat digunakan sebagai gen target real-time PCR yaitu 16S rRNA, yang diketahui spesifik dan juga digunakan untuk menganalisis kekerabatan antar strain. Analisis ini bermanfaat untuk melihat penyebaran infeksi H.pylori di dunia. Penelitian ini merupakan penelitian eksperimental laboratorium. Metode pengambilan sampel yang digunakan yaitu, metode consecutive sampling. Biopsi diambil dari 2 antrum dan 2 korpus pada 42 penderita dispepsia untuk pemeriksaan real-time PCR dan histopatologi. Optimasi kondisi real-time PCR meliputi uji volume cetakan DNA, sensitifitas dan spesifisitas teknik, kemudian dilanjutkan aplikasi pada sampel klinis dari biopsi lambung. Delapan dari 11 sampel yang positif dilakukan sekuensing dan analisis filogenetik. Hasil optimasi diperoleh suhu annealing 64?C, konsentrasi primer 0,8 ?M dan konsentrasi probe 0,6 ?M. Ambang batas deteksi real-time PCR untuk mendeteksi jumlah DNA minimal H.pylori yaitu 46 bakteri. Spesifisitas uji reaksi silang real-time PCR ini tidak menunjukkan adanya reaksi silang dengan mikroorganisme lain. Proporsi positif hasil pemeriksaan real-time PCR sebesar 26,2 , sedangkan histopatologi sebesar 11,9 . Pemeriksaan real-time PCR mampu meningkatkan diagnosis sebesar 14,3 dibandingkan pemeriksaan histopatologi. Hasil sekuensing dan analisis filogenetik menunjukkan bahwa strain H.pylori dari sampel memiliki kekerabatan dengan strain Taiwan, India, dan Australia. ABSTRACT
Helicobacter pylori infection is estimated infect almost half of the adult population in the world. Early detection of H.pylori is needed to prevent the development of infections into gastric malignancies. The coccoid form of H. pylori is difficult to detect using culture and histopathology but it can be detected by molecular methods such as real time PCR. One of the genes that can be used as a real time PCR target gene is 16S rRNA, which is known to be specific and also used to analyze closely related strain. This analysis were useful to showed the spread of H.pylori infection in the world.This study is an experimental laboratory. The sampling method used is the consecutive sampling method. Biopsy was taken from 2 antrum and 2 corpus in 42 patients with dyspepsia for real time PCR and histopathology examination. Optimization real time PCR conditions include DNA template volume testing, sensitivity and specificity of the technique, followed by application of clinical samples from gastric biopsy. Eight of the 11 positive samples were sequenced and analyzed for phylogenetics pattern. The optimization result obtained annealing temperature 64 C, primer concentration was 0,8 M and probe concentration was 0,6 M. Limit detection of the DNA was 46 bacteria. The specificity of the PCR 39 s real time indicate that there was no cross reaction with other microorganisms. The positive proportion of PCR real time examination was 26.2 , while histopathology was 11.9 . A real time PCR examination was able to improve the diagnosis by 14,3 compared to histopathology examination. Sequencing and phylogenetic analysis results showed that our strain were closely related to Taiwan, India and Australia strains. Latar Belakang Obesitas merupakan masalah kesehatan pada abad ke-21 dan berkaitan erat dengan peningkatan masalah kesehatan seperti non-alcoholic fatty liver disease (NAFLD). Patofisiologi obesitas tersebut melibatkan makrofag proinflamatori M1 (salah satu marker adalah Itgax atau CD11c) dalam perkembangan NAFLD. Dalam kasus yang lebih berat, NAFLD dapat berkembang menjadi fibrosis liver yang melibatkan TGF-β1. Meskipun pengobatan antiobesitas telah beredar dimasyarakat, namun obat tersebut dapat memiliki kontraindikasi dan efek samping masing-masing. Acalypha indica (Ai) merupakan salah satu tanaman herbal yang memiliki efek antihiperlipidemia dan antiobesitas. Penelitian ini akan menganalisis efek Ai terhadap perkembangan kerusakan liver melalui Itgax sebagai marker makrofag proinflamatori, TGF-β1 dan histopatologi liver pada tikus obesitas. Metode Uji reverse transcription-quantitative polymerase chain reaction (RT-qPCR) dilakukan untuk mengukur ekspresi Itgax dan TGF- β1 serta pemeriksaan histologi untuk menilai ada/tidaknya inflamasi dan persentase rerata luas area sinusoid pada organ liver tersimpan tikus obesitas dengan perlakuan: (1) diet normal (kontrol normal), (2) diet tinggi fruktosa dan kolesterol (DTFK) (kontrol negatif), (3) DTFK + Gemfibrozil (kontrol positif), serta (4) DTFK + ekstrak Ai (DTFK + Ai). Hasil Ekspresi Itgax pada kelompok tikus DTFK+Ai (1,580 + 0,836) terbukti lebih rendah dibanding dengan kelompok kontrol negatif (1,880 + 1,293), ekspresi TGF-β1 pada kelompok tikus DTFK+Ai (1,340 + 0,706) terbukti lebih rendah dibanding dengan kelompok kontrol negatif (1,850 + 1,358), walaupun tidak berbeda bermakna secara statistik. Di lain pihak, terjadi perbaikan inflamasi pada kelompok tikus DTFK+Ai dibanding dengan kontrol negatif, dan persentase rerata luas area sinusoid pada kelompok tikus DTFK+Ai (3,286 + 0,138) terbukti lebih tinggi dibanding dengan kontrol negatif (2,654 + 0,165). Kesimpulan Pemberian ekstrak akar Ai dapat menurunkan ekspresi Itgax dan TGF-β1, memperbaiki inflamasi pada liver serta meningkatkan persentase luas rerata sinusoid liver tikus obesitas, walaupun memerlukan penelitian lebih lanjut. ......Introduction Obesity is a health problem in the 21st century and is closely related to an increase in health problems such as non-alcoholic fatty liver disease (NAFLD). The pathophysiology of obesity involves proinflammatory M1 macrophages (one of the markers is Itgax or CD11c) in the development of NAFLD. In more severe cases, NAFLD can progress to liver fibrosis involving TGF-β1. Even though anti-obesity treatments have been circulating in the community, these drugs can have their own contraindications and side effects. Acalypha indica (Ai) is a herbal plant that has antihyperlipidemia and antiobesity effects. This study will analyze the effect of Ai on the development of liver damage through Itgax as a marker for proinflammatory macrophages, TGF-β1 and liver histopathology in obese mice. Method The reverse transcription-quantitative polymerase chain reaction (RT-qPCR) test was carried out to measure the expression of Itgax and TGF-β1 as well as histology examination to assess the presence/absence of inflammation and the average percentage of sinusoid area in the stored liver organ of obese mice treated with: (1) diet normal (normal control), (2) high fructose and cholesterol diet (DTFK) (negative control), (3) DTFK + Gemfibrozil (positive control), and (4) DTFK + Ai extract (DTFK + Ai). Results Itgax expression in the DTFK+Ai group of mice (1.580 + 0.836) was proven to be lower than the negative control group (1.880 + 1.293), TGF- β1 expression in the DTFK+Ai group of mice (1.340 + 0.706) was proven to be lower than the negative control group. (1.850 + 1.358), although it is not statistically significantly different. On the other hand, there was an improvement in inflammation in the DTFK+Ai mice group compared to the negative control, and the average percentage of sinusoid area in the DTFK+Ai mice group (3.286 + 0.138) was proven to be higher than the negative control (2.654 + 0.165). Conclusion Administration of Ai root extract can reduce the expression of Itgax and TGF-β1, improve inflammation in the liver and increase the percentage of the average area of liver sinusoids in obese mice, although further research is needed.

ABSTRACT
Background: early detection of H. pylori is essential to prevent the development of infections into gastric malignancies. The coccoid form of H. pylori is difficult to detect either by culture or histopathology; however, it can be detected using molecular methods, such as real-time PCR. The study was expected to provide new information on the development of H. pylori detection. Methods: a cross-sectional study was conducted at the Gastrointestinal Endoscopy Center of Cipto Mangunkusumo Hospital between October 2016 and August 2017. The sampling method used was consecutive sampling. Biopsy from gastric antrum and corpus were performed in 64 patients. We collected 2 specimens from each site to be examined using real-time PCR and histopathology. Initially, we conducted real-time PCR optimization followed by application of clinical samples from gastric biopsy. Data analysis using McNemars χ2 and Kappa tests. Results: the real-time PCR showed 25% positivity, while the positive proportion of histopathological examination was 14%. Real-time PCR has a sensitivity and specificity 88.9% dan 85.5%, respectively. The McNemars x2 test showed that there is significantly different (p=0.039) between the two tests; kappa value (p=0.561). Conclusion: the real-time PCR examination is more sensitive than histopathology. This technique can improve diagnosis by 11% compared to histopathological examination.

Pada organisme aerob, oksigen direduksi sempurna menjadi air di akhir rantai pernafasan di mitokondria yang menyediakan energi untuk menjaga fungsi normal set. Tetapi di dalam mitokondria ada molekul oksigen yang direduksi sebagian membentuk superoksida. Superoksida adalah radikal bebas, senyawa kimia yang mempunyai elektron tidak berpasangan. Radikal bebas sangat penting untuk banyak proses biologi seperti untuk melawan mikroorganisme yang patogen. Namun radikal bebas dapat merusak bila tidak dikontrol dan menyebabkan stres oksidatif. Stres oksidatif terjadi pula di jantung saat direoksigenasi setelah hipoksia, cedera yang terjadi disebut reperfusion injury. Jantung akan kehilangan fungsinya terutama kontraktilitasnya. Penelitian ini bertujuan untuk mengetahui efek kurkumin pada cedera reperfusi. Kurkumin adalah antioksidan yang mempunyai 2 cincin fenol. Kurkumin diduga' dapat mencegah ceders reperfusi setelah hipoksia jantung yang dilakukan pads model the isolated working heart. Penelitian ini menggunakan marmut jantan yang terbagi menjadi 3 kelompok, yaitu kelompok kontrol, kelompok kurkumin 0,25 1.tM, clan kelompok kurkumin 0,5 µM. Efek proteksi kurkumin dilihat dengan mengukur aliran darah koroner, aliran darah aorta, tekanan sistolik dan pengamatan histopatologi jantung. Aliran darah koroner meningkat selama reoksigenasi setelah 60 menit hipoksia, dengan kurkumin aliran darah koroner cenderung menurun. Aliran darah aorta dan tekanan sistolik menurun selama reoksigenasi setelah hipoksia 60 menu, dan cenderung meningkat setelah pemberian kurkumin. Histopatologi jantung yang direoksigenasi setelah hipoksia 60 menit memperlihatkan perubahan pada sel jantung. Kurkumin dosis 0,25 p.M cenderung iebih balk clan pada dosis 0,5 p.M dalam memproteksi jantung selama reoksigenasi setelah hipoksia 60 menit.

However, in the same mitochondrial respiratory chain, oxygen is "partially reduced" to form super oxide. Super oxide is a free radical, a chemical species with an unpaired electron. Free radicals are essential for many normal biological processes, i.e., they are essential in the response of tissue to invading microorganisms. However, they can be destructive if they are not tightly controlled. The oxidative stress also changes the heart in reoxygenation after hypoxia, which is called as reperfusion injury. The heart loses its function, especially contractility. The present study was conducted to find out the effect of curcumin in reoxygenation injury. Curcumin is an antioxidant, a symmetrical compound with two phenyl rings. It is hypothesized that curcumin is active to reduce reperfusion injury after hypoxia in the heart, assayed by the working guinea pig heart model. Three groups of male guinea pigs were used in this study, consisting of control group, curcumin 0, 25 µM, and curcumin 0,5 pM groups. The protective effect of curcumin was investigated by measuring coronary flow, aortic flow and systolic pressure, and by observing the histopathology of the heart. Reoxygenation of the heart after 60 minutes of hypoxia resulted in increased coronary flow, and curcumin decreased the coronary flow. Aortic flow and systolic pressure decreased during reoxygenation after 60 minutes of hypoxia, and curcumin increased aortic flow and systolic pressure. histopathology of the heart after reoxygenation and 60 minutes of hypoxia showed changes of the myocardium. Curcumin 0,25 µM was better than curcumin 0,50 p.M as a protective agent during reoxygenation after 60 minutes of hypoxia.

Latar belakang: Spektrum plasenta akreta (SPA) adalah plasentasi abnormal yang dapat menyebabkan perdarahan masif yang mengancam nyawa wanita hamil dan kasusnya telah meningkat dalam beberapa dekade terakhir. Diagnosis dan pembedaan SPA prenatal sangat krusial untuk merencanakan manajemen multidisiplin. Magnetic Resonance Imaging (MRI) plasenta sekuens Half-Fourier Acquisition Single-shot Turbo spin Echo (HASTE) adalah pencitraan prenatal yang mempunyai resolusi baik dengan minimal artefak gerakan fetus dan ibu sehingga dapat mengevaluasi derajat invasi plasenta abnormal. Metode: Studi asosiatif dilakukan secara potong lintang melibatkan pasien dengan diagnosis SPA secara histopatologi di RSUP Fatmawati pada periode Januari 2017 hingga Februari 2023 yang menjalani pemeriksaan MRI plasenta sekuens HASTE. MRI plasenta dievaluasi secara blinded (pasien akan diberi kode dan identitas pasien disembunyikan ketika evaluasi dilakukan) oleh peneliti dan dikonfirmasi oleh dokter radiologi urogenital. Sediaan histopatologi dibaca ulang oleh dokter patologi anatomi secara blinded yang tidak mengetahui hasil USG dan MRI plasenta. Uji hipotesis menggunakan uji McNemar. Hasil: SPA berdasarkan MRI plasenta ditemukan plasenta akreta 17%; plasenta inkreta 53,2%; plasenta inkreta dan perkreta 14,9%; plasenta perkreta 14,9%. SPA berdasarkan hasil histopatologi yaitu plasenta akreta 18%; plasenta inkreta 51,1%; plasenta perkreta 10,6%. Kesesuaian MRI plasenta dengan histopatologi pada plasenta akreta R=0,138; plasenta inkreta dengan R=0,203; plasenta perkreta R=0,189. Kesimpulan: Tidak ditemukan kesesuaian antara MRI plasenta sekuens HASTE dan histopatologi yang kemungkinan disebabkan oleh signal to noise ratio (SNR) pada MRI plasenta tidak optimal; spesimen histopatologi yang diambil dari derajat invasi plasenta yang berbeda dan kurangnya informasi lokasi pengambilan spesimen; spesimen tidak berwujud vesika urinaria tetapi bagian perlekatan sehingga mungkin spesimennya tidak jelas; kurang komunikasi aktif antara operator dengan dokter patologi anatomi. ......Background: Placenta accreta spectrum (PAS) is abnormal placentation associated with severe, life-threatening maternal hemorrhage that increased in last few decades. Prenatal differentiation of placenta accreta spectrum (PAS) is essential for planning multidisciplinary management. Half-Fourier Acquisition Single-shot Turbo spin Echo (HASTE) sequence of preoperative placental Magnetic Resonance Imaging (MRI) is prenatal imaging with good resolution, minimal artifacts of fetus and pregnant woman movement to evaluate abnormal placentation. Methods: Associative cross-sectionally study involved patients with PAS histopathology diagnosis at the General Hospital of Fatmawati in the period January 2017 to February 2023 who underwent HASTE sequence of placental MRI. Placental MRI evaluated blinded (patient with coded and hidden identity) by investigator and confirmed by genitourinary radiologist. Histopathology sample were reviewed by pathologist blinded (did not know results of ultrasound nor placental MRI). The compatibility of placental MRI will be assessed with histopathology findings. Test hypothesis using the McNemar’s test. Results: PAS according placenta MRI: placenta accreta 17%; placenta increta 53,2%; placenta increta and percreta 14,9%; placenta percreta 14,9%. PAS according histopathology are placenta accreta 18%; placenta increta 51,5%; placenta percreta 10.6%. The compatibility of placental MRI with histopathology the highest was placenta increta (R = 0.203) followed by placenta percreta (R = 0.189) and placenta accreta (R = 0.138). Conclusions: There was no compatibility found between HASTE sequence of preoperative placental MRI and histopathology which may be due to suboptimal signal to noise ratio (SNR) on placental MRI; histopathological specimen taken from different degree of placental invasion and lack information regarding location of specimens; specimen attached to chorionic villi not in form of urinary bladder; lack active communication between operator and pathologist.
