"Chronic hepatitis due to hepatitis B virus (HBV) or hepatitis C virus (HCP) is still a major problem in terms of progressive liver damage, prevention and therapy in most parts ofthe world. Unfortunately, to date, there is still no specific and effective therapy for HBV. No therapy can be given to carrier; non-replicative and asymptomatic patients of chronic HBV infection. Lamivudine or alpha-interferon can be used for treatment of compensated chronic hepatitis B infection with significant increase of aminotransferase. Approximately 40 % of patients can have seroconversion with this form of therapy. Chronic hepatitis D virus injection can be treat with alpha-interferon and in the final stage, may undergo liver transplantation. For chronic hepatitis C virus infection, alpha-interferon with ribavirin have been shown to have a better efficacy than afpha-interferon alone where the efficacy can reach 39-49 %."

"ABSTRAK
Pendahuluan:
Interferon gamma (IFN-γ) merupakan sitokin penting dalam upaya mengeliminasi M. tuberculosis. Kadar IFN-γ pada pasien tuberkulosis (TB) ditemukan meningkat dan akan mengalami penurunan setelah menjalani terapi. Penelitian ini dilakukan untuk mengetahui hubungan antara kadar IFN-γ serum dan derajat kepositifan sputum basil tahan asam (BTA).
Metode:
Penelitian ini merupakan penelitian observasional dengan metode rancangan potong lintang (cross sectional study). Penelitian dilakukan di RS. Wahidin Sudirohusodo dan RS. Labuang Baji Makassar mulai Juni 2013 sampai Maret 2014. Sampel dipilih dengan metode convenience accidental sampling. Sampel yang dianalisis berupa plasma penderita TB paru dan orang sehat di masyarakat yang diukur dengan teknik analisis Enzyme Linked Immunosorbent Assay (ELISA). Data yang diperoleh dianalisis dengan uji Mann-Whitney dan Kruskal-Wallis menggunakan SPSS for windows versi 17.0.
Hasil:
Jumlah sampel yang diteliti sebanyak 88 subjek dengan 50 subjek TB paru dan 38 subjek kontrol. Berdasarkan pemeriksaan sputum BTA, ditemukan 4 (8%) subjek dengan BTA negatif, 7 (14%) subjek dengan 1 sampel BTA positif, 17 (34%) subjek dengan 2 sampel BTA positif, dan 22 (44%) subjek dengan 3 sampel BTA positif. Didapatkan kecenderungan peningkatan kadar IFN-γ seiring meningkatnya derajat kepositifan sputum BTA. Terdapat perbedaan bermakna kadar serum IFN-γ dengan derajat kepositifan sputum BTA pada pasien TB paru kasus baru.
Simpulan:
Kadar IFN-γ serum cenderung meningkat seiring dengan bertambahnya derajat kepositifan sputum BTA."

"Latar Belakang : Wabah pneumonia akibat Corona Disesase 2019 (COVID-19) masih menjadi perhatian nasional maupun global. Wanita hamil termasuk dalam kelompok risiko tinggi/rentan terhadap infeksi COVID-19. Adanya badai sitokin akibat infeksi COVID-19 menyebabkan gejala klinis yang semakin berat. Zinc sebagai salah satu mikronutrien penting yang berpengaruh dalam regulasi produksi sitokin oleh sistem imun tubuh. Kondisi defisiensi zinc pada wanita hamil yang terinfeksi COVID-19 dapat menyebabkan disregulasi dari sistem imun dan menghasilkan badai sitokin yang mengakibatkan sindrom ancaman gagal napas akut dan kematian. Tujuan : Meneliti hubungan kadar zinc serum maternal terhadap kadar sitokin pro- inflamasi interleukin-6 (IL-6) dan interferon gamma (IFN-) pada wanita hamil yang terkonfirmasi COVID-19 dengan gejala ringan-sedang dan berat. Metode : Penelitian observasional dengan desain studi cross-sectional. Subyek penelitian adalah wanita hamil yang terkonfirmasi COVID-19 dengan gejala ringan-sedang dan berat. Faktor yang diteliti meliputi kadar zinc, IL-6 dan IFN- serum maternal dan hubungannya kadar zinc serum terhadap kadar IL-6 dan IFN- serum maternal.
Hasil : Jumlah total subyek sebanyak 48 orang dibagi menjadi 28 subyek untuk kelompok wanita hamil yang terkonfrimasi COVID-19 gejala ringan-sedang dan 20 subyek dengan gejala berat. Rata-rata usia responden 30,61 tahun untuk kelompok gejala ringan-sedang dan 32,9 tahun untuk kelompok gejala berat. Rata-rata usia kehamilan pada kelompok gejala ringan-sedang lebih tua dibanding kelompok gejala berat (38,1 minggu vs 34,5 minggu).Lama perawatan kelompok dengan gejala berat lebih lama dibanding kelompok gejala ringan-sedang. 60% subyek dari kelompok gejala berat berakhir dengan kematian maternal. Semua subyek dalam penelitian ini mengalami defisiensi zinc. Tidak terdapat perbedaan bermakna kadar zinc serum maternal pada kedua kelompok yaitu 54,0 (34-78) μg/dl untuk kelompok gejala ringan-sedang dan 52,0 (38-97) μg/dl untuk kelompok gejala berat. Terdapat perbedaan bermakna antara kelompok gejala ringan-sedang dengan kelompok gejala berat terhadap kadar IL-6 serum (5,8 (1,5-69,6) pg/ml vs 18,6 (3,8-85,3) pg/ml) dan kadar IFN- serum (0,9 (0,1-16,8) pg/ml vs 9,0 (0,9-21,1) pg/ml). Tidak ada korelasi antara kadar zinc serum maternal dengan kadar IL-6 dan IFN- serum maternal. Kesimpulan : Kadar zinc serum maternal tidak berbeda bermakna diantara kedua kelompok penelitan. Kadar IL-6 dan IFN- serum kelompok gejala berat lebih tinggi dibanding kelompok gejala ringan-sedang. Tidak ada korelasi antara kadar zinc serum dengan kadar IL-6 dan IFN- serum maternal.

Bacground : The outbreak of Corona Disesase 2019 (COVID-19) is still a national and global concern. Pregnant women are included in the highrisk/susceptibility group for COVID-19 infection. The presence of a cytokine storm due to COVID-19 infection causes increasingly severe clinical symptoms. Zinc is one of the important micronutrients that influence the regulation of cytokine production by the immune system. Zinc deficiency in pregnant women with confirmed COVID-19 can cause dysregulation of the immune system and produce a cytokine storm that results in acute respiratory distress syndrome and maternal death. Objective: To evaluate the relationship between maternal serum zinc levels and the pro- inflammatory cytokines interleukin-6 (IL-6) and interferon-gamma (IFN-) in pregnant women with confirmed COVID-19 with mild-moderate and severe symptoms. Methods: A cross-sectional was adopted in this study. The subjects of the study were pregnant women with confirmed COVID-19 with mild-moderate and severe symptoms. We measure the maternal serum zinc levels, serum IL-6 and IFN- levels, then we evaluate the relation between the maternal serum zinc levels and the maternal serum IL- 6 and IFN- levels.
Results: The total number of subjects was 48 patiens, divided into 28 subjects for the pregnant women with confirmed COVID-19 with mild-moderate symptoms and 20 subjects with severe symptoms. The average age of the respondents was 30.61 years for the mild-moderate group and 32.9 years for the severe group. The mean gestational age in the mild-moderate group was older than in the severe one (38.1 weeks vs. 34.5 weeks). The length of stay of subjects with severe symptom was longer than the mild-moderate group. 60% cases from the severe group ended in maternal death. All the participants in this study suffered zinc deficiency. There was no significant difference in maternal serum zinc levels between the two study groups (54.0 (34-78) g/dl in mild-moderate group vs 52.0 (38-97) g/dl in severe group). There was a significant difference between mild- moderate vs severe groups in which the serum IL-6 levels were (5.8 (1.5-69.6) pg/ml vs 18.6 (3.8-85.3) pg/ ml) and the serum IFN- levels were (0.9 (0.1-16.8) pg/ml vs. 9.0 (0.9- 21.1) pg/ml). There is no correlation between maternal serum zinc level and maternal serum IL-6 and IFN- levels. Conclusion: The maternal serum zinc levels were not significantly different between the two study groups. The maternal serum IL-6 and IFN- levels in the severe group were higher than in the mild-moderate group. There is no correlation between maternal serum zinc level and maternal serum IL-6 and IFN- levels.

"Chronic hepatotropic viruses commonly evade the antiviral defence systems of the body and cause a long - lasting persistent infection. The prolonged nature of the infection ensures that every infected person has ample opportunity to transmit the virus to others, allowing many millions of people world-wide to become infected. Three viruses commonly cause chronic hepatitis B virus, hepatitis C virus and hepatitis Delta virus.
Virus specific CD8 T cells of the host, represent the main effector cells against viral infection. Where as the antiviral cytokinex have a major role in the control of viral replication (non-cytolytic inhibition).
To cause persistent infection, a virus must avoid the host defences and that hepatotropic viruses have developed elaborate strategies to achieve this. In the case of hepatitis B virus, two proteins are involved in the inhibition of the host defences; those are the core protein that has been shown to inhibit the production of interferon and the polymerase protein has been shown to inhibit its effect. Where as in the case of hepatitis C virus, the NS5A and E2 protein reduce the effect of interferon by inhibiting the antiviral kinase.
In order to survive and persistent in the liver, the hepatotropic viruses must be able to avoid both arms of the immune system, either by mutation of viral proteins or by preventing activation of the immune system."

"Ruang lingkup dan metodologi: Glukokortikoid telah lama digunakan sebagai antiiflamasi dan imunosuppresan. Penggunaan yang panjang dengan dosis tinggi menyebabkan efek samping yang cukup serius. Dewasa ini telah dikembangkan berbagai pembawa obat yang dapat membawa obat langsung ke target obat atau ke reseptornya. Dengan menginkorporasikan obat ke pembawa obat, contohnya liposom, efek samping sistemik dapat ditekan. Purwaningsih dkk telah berhasil membuat sediaan baru yakni Liposom-metilprednisolon palmitat (L-MPLP). Penelitian ini bertujuan untuk mempelajari efek farmakologi dari L-MPLP, terutama efek antiinflamasinya. Parameter yang dilihat adalah penurunan produksi interferon gamma pada kultur limfosit T setelah distimulasi dengan concanavalin A secara in vitro maupun in vivo. Hasil dan Kesimpulan : Terjadi penurunan kadar interferon gamma setelah pemberian L-MPLP secara in vivo pada dosis 2, 8 dan 16 mg/kgBB secara signifikan dibandingkan kontrol tanpa perlakuan sedangkan pemberian MPL tidak terjadi penurunan kadar interferon gamma. Pada kultur in vitro, pemberian L-MPLP maupun MPL pada kadar 5.10 -1, 5.10-2 dan 5.10-3 keduanya mampu menekan produksi interferon gamma, dimana penekanan oleh L-MPLP lebih baik dibanding MPL secara signifikan.

The Improving of Methylprednisolone Palmitate Potency After Incorporated With Liposome. An Antiinflammation Study In Culture Of Mice?s Splenocytes. Glucocorticoid has been used as an antiinflammatory and immuno-suppresive drug. Longterm utilisation at high dose of glucocorticoid is associated with serious side effects. In recent years, many attempts have been performed in searching the appropiate vehicles to deliver the drug directly into the target organ or the receptor. By incorporating the drug into its vehicle such as liposome, the systemic side effect can be minimized. Purwaningsih et al has successfully synthetized a novel preparation of liposome methylprednisolone palmitate (L-MPLP). The aim of the study was to learn the pharmacological effect of L-MPLP, especially on antiinflammatory effect of this novel preparation, compared with the standard methylprednisolone (MPL). The parameter was the potency of L-MPLP in reducing gamma-interferon production in T-lymphocyte culture after stimulation with concanavalin A in vitro as well as in vivo. Gamma-interferon was assayed by ELISA method. The reduction of gamma interferon, in vivo, after the administration of L-MPLP at the dose of 2,8 and 16 mg/kgBW respectively, showed significant difference than a control group, while MPL did not. The addition of both L-MPLP and MPL in in vitro culture at the concentration of 5.10-3, 5.10-2 and 5.10-1 mM have proved to suppress the gamma-interferon production, where the suppression of L-MPLP was more effective than MPL, significantly."

"Infeksi virus hepatitis B (VHB) merupakan masalah kesehatan global. Terapi yang ada saat ini hanya berdampak minimal terhadap covalently closed circular deoxyribonucleic acid (cccDNA), sehingga eradikasi sulit dicapai. Matriks Metaloproteinase-9 (MMP-9) berperan dalam meningkatkan replikasi VHB, namun belum ada penelitian yang mengevaluasi peran penghambat MMP-9 terhadap replikasi VHB. Kultur primer hepatosit diperoleh dari Tupaia javanica dan diinfeksi dengan VHB manusia, kemudian dibagi menjadi dua kelompok, yaitu kontrol dan intervensi. Kelompok intervensi diberikan penghambat MMP-9 dosis 1 nM, 3 nM, dan 7 nM. Peripheral blood mononuclear cells (PBMC) manusia diperoleh dari pasien hepatitis B kronik, kemudian dibagi menjadi dua kelompok, yaitu kontrol dan intervensi. Kelompok intervensi diberikan penghambat MMP-9 dosis 3 nM. Dilakukan pengukuran HBsAg, DNA VHB, cccDNA, MMP-9, type-1 IFN receptor 1 (IFNAR1), dan interferon-β (IFN-β) sebelum dan 72 jam setelah pemberian intervensi pada kedua kelompok. Pada kultur primer hepatosit T. javanica yang diinfeksi VHB manusia, pemberian penghambat MMP-9 dosis 1 nM, 3 nM, dan 7 nM secara konsisten menurunkan kadar HBsAg, DNA VHB, cccDNA, dan MMP-9. Dosis 3 nM meningkatkan kadar IFNAR1, sedangkan dosis 7 nM meningkatkan kadar IFN-β. Dosis 3 nM menunjukkan efek yang lebih optimal dibandingkan dosis lainnya. Pada PBMC manusia dengan infeksi VHB, pemberian penghambat MMP- 9 dosis 3 nM menurunkan kadar HBsAg, DNA VHB, cccDNA, dan MMP- 9, serta meningkatkan kadar IFN-β, namun menurunkan kadar IFNAR1. Studi ini menunjukkan bahwa pemberian penghambat MMP-9 dapat menurunkan kadar HBsAg, DNA VHB, cccDNA, dan MMP-9, serta meningkatkan kadar IFN-β pada kultur primer hepatosit T. javanica dan PBMC manusia.

Hepatitis B virus (HBV) infection remains a significant global health concern. Current therapies have minimal impact on covalently closed circular deoxyribonucleic acid (cccDNA), making HBV eradication difficult. Matrix Metalloproteinase-9 (MMP-9) enhance HBV replication, but its inhibition has not been studied. Primary hepatocyte cultures were obtained from Tupaia javanica and infected with human HBV, then divided into control and intervention groups. The intervention groups were treated with MMP-9 inhibitors at doses of 1 nM, 3 nM, and 7 nM. Human peripheral blood mononuclear cells (PBMC) were isolated from chronic hepatitis B patients and divided into control and intervention groups. The intervention groups received MMP-9 inhibitors at dose of 3 nM. Measurements of HBsAg, HBV DNA, cccDNA, MMP-9, type-1 IFN receptor 1 (IFNAR1), and interferon-β (IFN-β) were performed before and 72 hours after intervention in both groups. In T. javanica primary hepatocyte culture infected with human HBV, MMP-9 inhibitors at doses of 1 nM, 3 nM, and 7 nM consistently decreased levels of HBsAg, HBV DNA, cccDNA, and MMP-9. The 3 nM dose increased IFNAR1 levels, while the 7 nM dose increased IFN-β levels. The 3 nM dose demonstrated the most optimal effects. In human PBMC with HBV infection, MMP-9 inhibitor at dose of 3 nM decreased levels of HBsAg, HBV DNA, cccDNA, MMP-9, increased IFN-β levels, but reduced IFNAR1 levels. This study shows that administration of MMP-9 inhibitors reduced levels of HBsAg, HBV DNA, cccDNA, and MMP-9, while increased IFN-β levels in T. javanica primary hepatocyte cultures and human PBMC."

"Interferon (IFN) merupakan sitokin yang diproduksi oleh berbagai tipe sel sebagai respon rangsangan terhadap stimulasi virus, bakteri, parasit, sel tumor, atau antigen lain. Interferon α termasuk kelompok IFN tipe I yang mempunyai berbagai efek biologis yang meliputi antiviral, antitumor dan juga sebagai immunoterapetik. Penelitian ini bertujuan untuk mensintesis protein rekombinan human IFN α2a melalui sistem ekspresi pada bakteria E. coli BL21(DE3). Pada gen human ifn α2a dilakukan penambahan situs pemotongan enzim restriksi Nco I dan Xho I menggunakan metode PCR, kemudian dilanjutkan dengan proses ligasi ke vektor pET-32b(+) dan selanjutnya ditransformasikan pada E. coli DH5α.
Hasil sekuensing menunjukkan bahwa vektor rekombinan (pET-32b(+)-IFN α2a) memiliki urutan nukleotida yang benar. Vektor rekombinan ini selanjutnya ditransformasikan ke dalam E.coli BL21(DE3). Klon transforman yang diperoleh dikultur dan diinduksi dengan penambahan IPTG 1 mM sehingga mengekspresikan protein rekombinan human IFN α2a. Dari hasil isolasi, diperoleh protein rekombinan human IFN α2a dalam bentuk protein terfusi sehingga mempermudah proses deteksi dan purifikasi. Protein dikarakterisasi melalui metode SDS PAGE dilanjutkan dengan Western blot dan pewarnaan CBB. Pita protein rekombinan human IFNα2a yang diperoleh berukuran 36 kDa. Hasil maksimal ditunjukkan ekspresi pada suhu 37⁰C dengan waktu inkubasi 5 jam setelah induksi.

Interferon (IFN) is a cytokine produced by various cell types as a response of stimulation to viruses, bacteria, parasites, tumor cells, or other antigens. Interferon α type I IFN groups have various biological effects, including antiviral, antitumor and immunotherapeutic. The aim of this research is to synthesize recombinant human IFN α2a proteins through bacterial expression systems in E. coli BL21 (DE3). Addition genes of human IFN α2a, which are restriction enzyme cutting sites for Nco I and Xho I, are added through PCR method. This step is followed by ligation process to the pET-32b(+) vector and then transformed into E. coli DH5α.
The recombinant vector (pET-32b(+)-IFN α2a) has a nucleotide right sequence after it was being sequenced, after was transformed into E. coli BL21 (DE3). Obtained transformant clones were cultured and induced by addition of IPTG 1 mM to produce the expression of recombinant human IFN α2a proteins. As result of isolation process, recombinant protein of human IFN α2a are collected in fused protein thus can simplify the detection and purification method. The proteins are characterized by the SDS PAGE method followed by Western blot and CBB staining. The results show that the recombinant human IFN α2a protein bands are exactly 36 kDa. The maximum expression results were obtained at 37⁰C with 5 hours incubation after induction process."

"Latar Belakang: Infeksi tuberkulosis laten ITBL merupakan ancaman bagi para petugas kesehatan terutama yang sehari-harinya kontak dengan penderita Tuberkulosis TB. Infeksi TB telah dapat dideteksi sejak lebih dari 100 tahun yang lalu dengan uji tuberkulin tuberculin skin test, TST. Sebagai alternatif terhadap uji tuberkulin saat ini telah tersedia pemeriksaan in vitro berupa pemeriksaan interferon gamma release assay IGRA.
Tujuan: Penelitian ini bertujuan untuk membandingkan TST dan IGRA dalam mendiagnosis TB laten pada petugas kesehatan di Rumah Sakit Umum Pusat Persahabatan Jakarta.
Metode: Penelitian dilakukan dengan disain potong lintang.
Hasil: Prevalens ITBL pada petugas kesehatan di RSUP Persahabatan adalah 66. Sejumlah 67 subjek dilakukan pemeriksaan IGRA dan TST dengan hasil 27 40 subjek dengan hasil pemeriksaan IGRA positif, 42 63 subjek dengan hasil pemeriksaan TST positif dan 44 66 subjek dengan hasil pemeriksaan IGRA dan atau TST positif, dengan kesesuaian sedang ?=0,459. Tidak ada hubungan antara usia dan parut BCG dengan hasil pemeriksaan TST maupun IGRA.
Kesimpulan: proporsi ITBL berdasarkan TST lebih besar dibadingkan IGRA dengan kesesuaian sedang

Introduction: Latent tuberculosis infection LTBI is a threat to the healthcare workers, especially who close contact with tuberculosis TB patients. Tuberculosis infection has been detected since more than 100 years ago with the tuberculin test TST. As an alternative to the tuberculin test now is currently available in vitro examination of interferon gamma release assay IGRA.
Objective: to compare TST and IGRA in the diagnosis of LTBI among healthcare workers in Persahabatan HospitalJakarta.
Method: The study is conducted with a cross sectional design.
Results: The prevalence of latent TB among health care workers in Persahabatan general Hospital was 66 .Of the 67 subjects examined, there were 27 40 subjects with IGRA positive, 42 63 subjects with TST positive and 44 66 subjects with IGRA and or TST positive, with moderate agreement 0,459 . There was no correlation between age and BCG scar with the results of the TST or IGRA.
Conclusion: High proportion of LTBI more positive with TST compare to IGRA, with moderate agreement
