Abstrak :
Pengelolaan sediaan farmasi di puskesmas terdiri dari serangkaian tahapan yang dimulai dari perencanaan hingga pencatatan dan pelaporan serta pemantauan dan evaluasi pengelolaan sediaan farmasi dan BMHP. Proses pencatatan dan pelaporan merupakan elemen penting dalam penatalaksanaan obat baik yang ketika obat diterima, disimpan, didistribusikan, serta yang digunakan di Puskesmas, termasuk pencatatan dan pelaporan dalam pembuatan Laporan Pemakaian dan Lembar Permintaan Obat (LPLPO). LPLPO merupakan suatu laporan atau format yang berisi tentang penggunaan dan pengeluaran obat. LPLPO dibuat oleh puskesmas pada setiap bulannya yang berfungsi untuk monitoring pemakaian dan penerimaan obat setiap bulan dengan tujuan dapat menjaga ketersediaan obat pada puskesmas dalam jangka waktu tertentu. Berdasarkan hasil pengamatan terkait data yang tertera pada kartu stok masing-masing obat yang ada di Puskesmas Kecamatan Kramat Jati sesuai dengan data yang terdapat pada Laporan Pemakaian dna Lembar Permintaan Obat (LPLPO) Periode Januari hingga Maret Tahun 2023 Puskesmas Kecamatan Kramat Jati. Daftar sepuluh penggunaan obat terbanyak di Puskesmas Kecamatan Kramat Jati Periode Januari hingga Maret tahun 2023 antara lain Parasetamol 500 mg, Metformin 500 mg, DMP Kombinasi/ Alpara tab, Amlodipin 10 mg, Tablet Tambah Darah (Rematri), Vitamin B Kompleks, Deksametason, Guaifenesin, Cetirizin, dan Vitamin A 200000 IU. ...... Management of pharmaceutical supplies at community health centers consists of a series of stages starting from planning to recording and reporting as well as monitoring and evaluating the management of pharmaceutical supplies and BMHP. The recording and reporting process is an important element in drug management, both when drugs are received, stored, distributed, and used at the Community Health Center, including recording and reporting in making Laporan Pemakaian dan Lembar Permintaan Obat (LPLPO). LPLPO is a report or format that contains information about the use and dispensing of drugs. The LPLPO is made by the puskesmas every month which functions to monitor the use and receipt of medicines every month with the aim of maintaining the availability of medicines at the puskesmas within a certain period of time. Based on the results of observations related to the data listed on the stock card for each drug in the Kramat Jati District Health Center, it is in accordance with the data contained in the Laporan Pemakaian dan Lembar Permintaan Obat (LPLPO) for the period January to March 2023 at the Kramat Jati District Health Center. The list of the ten most commonly used drugs at the Kramat Jati District Health Center for the period January to March 2023 includes Paracetamol 500 mg, Metformin 500 mg, DMP Combination/Alpara tab, Amlodipine 10 mg, Blood Increase Tablets (Rheumatism), Vitamin B Complex, Dexamethasone, Guaifenesin , Cetirizine, and Vitamin A 200000 IU.

Praktik Kerja Profesi Apoteker di Suku Dinas Kesehatan Kota Administrasi Jakarta Timur bertujuan agar dapat memahami peranan, tugas, dan tanggung jawab apoteker di suku dinas kesehatan serta memiliki pengetahuan tentang tugas pokok dan fungsi suku dinas kesehatan di bidang farmasi. Selain itu calon apoteker juga diharapkan dapat memiliki wawasan, pengetahuan, keterampilan, dan pengalaman praktis dalam melakukan pekerjaan dan memiliki gambaran nyata tentang permasalahan kefarmasian di suku dinas kesehatan. Tugas Khusus berjudul Rekapitulasi Laporan Pemakaian dan Lembar Permintaan Obat LPLPO Periode Desember 2015-November 2016 di Wilayah Kota Administrasi Jakarta Timur. Tugas khusus ini bertujuan untuk mengkaji 20 jenis obat yang paling banyak digunakan berdasarkan LPLPO periode Desember 2015 hingga November 2016 di Wilayah Kota Administrasi Jakarta Timur.

Internship at Regional Health Department of East Jakarta was aimed to understand the role, duties, and responsibilities of pharmacists in the regional health department and have knowledge about the main tasks and functions of the regional health department in pharmaceutical field. In addition, the student of apothecary profession also expected to have insight, knowledge, skills, and practical experience in doing the job and have a real picture of pharmaceutical problems in regional health department. Special assignment entitled Recapitulation of Usage Report and Drug Demand Sheet LPLPO for December 2015 November 2016 in East Jakarta Region. This special assignment aims to examine the 20 most widely used drugs based on LPLPO period December 2015 to November 2016 in East Jakarta Region.

Internship at East Jakarta Sub Department for Health Month Period October 2017 aims to understand the duties and responsibilities of pharmacists in government institutions of the Directorate of Food Inspection and Certification of National Agency of East Jakarta Sub Department for Health, having the insight, knowledge, skills and practical experience to undertake pharmaceutical practice Government agencies, can also have the insight of pharmaceutical practice issues in government institutions such as the East Jakarta Sub Department for Health. The special assignment given is ldquo;Recapitulation of Medicine Use Report and Request Form in 10 Community Health Center of East Jakarta Month Period August 2017 in East Jakarta Sub Department for Health rdquo;. The purpose of this special assignment is having the insight of the consumption report from Community Health Center of East Jakarta.

Salah satu faktor penunjang kelancaran pelayanan kesehatan di puskesmas adalah tersedianya obat yang cukup untuk masyarakat yang melakukan pengobatan di puskesmas. Ketersediaan obat yang optimal dapat direncanakan dari proses perencanaan termasuk proses pencatatan dan pelaporan. Proses perencanaan kebutuhan obat tiap tahunnya dilakukan dengan sistem berjenjang (bottom-up) dari puskesmas yang berada di kelurahan hingga ke kecamatan. Tiap puskesmas akan membuat dan melaporkan Laporan Pemakaian dan Lembar Permintaan Obat (LPLPO). Formulir ini berfungsi untuk memonitoring pemakaian dan permintaan obat setiap bulan dan dapat diketahui profil penggunaan obat yang paling banyak hingga paling sedikit digunakan di puskesmas sehingga dapat disesuaikan dengan pola penyakit yang tersebar di masyarakat wilayah kerja puskesmas tersebut. Penelitian ini bertujuan untuk mengetahui kesesuaian kartu stok obat dengan LPLPO dan mengetahui profil 10 (sepuluh) obat terbanyak di Puskesmas Kecamatan Kramat Jati Periode Oktober-Desember 2022 sehingga dapat membantu dalam proses perencanaan dan permintaan untuk periode selanjutnya. Pengambilan data menggunakan metode retrospektif dengan melihat LPLPO Puskesmas Kecamatan Kramat Jati Periode Oktober-Desember 2022. Hasil dari penelitian ini berupa 10 (sepuluh) profil penggunaan obat terbanyak dari 228 obat di Puskesmas Kecamatan Kramat Jati Periode Oktober-Desember 2022 dengan urutan sebagai berikut: Metformin 500 mg; Parasetamol 500 mg; Calcium lactate 500 mg; Guaifenesin 100 mg; Chlorpheniramine maleate (CTM) 4 mg; Amlodipin 5 mg; Deksametason 0,5 mg; Vitamin B Kompleks; Glimepirid 2 mg; dan Vitamin B6 10 mg. ...... One of the factors supporting the smooth operation of healthcare services at health centers is the availability of sufficient medications for the community seeking treatment. Optimal medication availability can be planned through processes including recording and reporting. The annual medication planning process is conducted in a hierarchical (bottom-up) system from health centers in villages to those in districts. Each health center will prepare and report the Medication Usage Report and Request Form (LPLPO). This form functions to monitor monthly medication usage and requests, allowing the identification of the most to least used medications at the health center, which can then be adjusted according to the disease patterns prevalent in the community served by the health center. This study aims to determine the conformity of medication stock cards with LPLPO and to identify the profile of the ten most used medications at the Kramat Jati District Health Center for the period of October-December 2022, thereby assisting in the planning and requesting process for the next period. Data collection was conducted retrospectively by reviewing the LPLPO of the Kramat Jati District Health Center for the period of October-December 2022. The results of this study include the profile of the ten most used medications out of 228 at the Kramat Jati District Health Center for the period of October-December 2022, in the following order: Metformin 500 mg; Paracetamol 500 mg; Calcium lactate 500 mg; Guaifenesin 100 mg; Chlorpheniramine maleate (CTM) 4 mg; Amlodipine 5 mg; Dexamethasone 0.5 mg; Vitamin B Complex; Glimepiride 2 mg; and Vitamin B6 10 mg.

Abstrak :
This research is intended to identify the description of the performance and factors that effect the performance of the drugs personnel in managing report of drug request utilization (LPLPO). The research method use cross-sectional design. The research location are all community health centers that available in distrcit of Padang Pariaman and district of Agam with respondents of 46 drug personnel of community health centers. Data collection is done by filling-up questionaire, and continued with monthly checking of documents of report of drug request utilization (LPLO). Results of the research indicate that performance of the drug personnel of the community health is 67,4%. The results of research regarding the description of performance level of drug personnel of the community health centers is viewed in terms of internal and external factors. Regarding significant variables after chi-square test, the internal factor related with the performance of drug personnel is the work tenure and level of education, while knowledge does not show significant relationship. Within the external factors, only training is significantly related to the performance. While othe factors, i.e., remuneration, support of coworkers, supervision, and work load do not have significant relationship. From the multivariate analysis the most dominant variable is education. The suggestion from the result of this reseacrh is that it is ecxpected that the province needs to recommend personnel recruitment to manage the drug at the community health centers with formal education of assitant pharmacist. the suggestion for districts is that refreshing training for drug personnel needs to be done periodic meetings need to be done.

Pharmacist Internship Program (PKPA) held in Suku Dinas Kesehatan Jakarta Timur and Puskesmas Kecamatan Ciracas, East Jakarta. PKPA activity is intended that students can see directly pharmacist activities that take place in a health center, able to understand the role, duties and responsibilities of pharmacists in health centers, as well as having a vivid description of the problem and the work of pharmacy practice at the center. Intenship in East Jakarta Sub Department of Health aims to understand the roles, duties, and responsibilities of Pharmacist in the Health Office, having knowledge about the main tasks and functions of Health Resources Section, especially those related to pharmacy, gaining skills and practical experience in undertaking pharmaceutical work in government and having a clear picture of the pharmaceutical problems that can arise in government. The specific assignment given is Monitoring of Pharmacy Report Form 148 from 10 Puskesmas at Sub District of East Jakarta into EisDinkes Format of Period January - March 2018. The purpose of this specific assignment is to enable the prospective pharmacist to know the LPLPO reporting data and compare it with the reporting system Eis Dinkesin the period January - March 2018 in 10 puskesmas at sub-district of East Jakarta.

Antibiotik merupakan obat yang digunakan pada infeksi yang disebabkan oleh bakteri. Penggunaan antibiotik perlu dimonitoring karena penggunaan yang berlebihan dapat meningkatkan terjadinya resistensi. Evaluasi penggunaan obat secara kuantitatif dapat dilakukan menggunakan metode ATC/DDD (ATC/Anatomical Therapeutic Chemical, DDD/Defined Daily Dose) yang merupakan sistem klasifikasi dan pengukuran penggunaan obat. Tujuan penelitian ini adalah mengetahui lima antibiotik yang paling banyak digunakan di Puskesmas Kecamatan Matraman dan di seluruh jaringan Puskesmas wilayah Kecamatan Matraman pada tahun 2022 dengan metode ATC/DDD. Data pemakaian antibiotik didapatkan dari Laporan Penggunaan dan Lembar Permintaan Obat (LPLPO). Lima antibiotik yang paling banyak digunakan di Puskesmas Kecamatan Matraman tahun 2022 yaitu amoksisilin kaplet 500 mg (59.2%), ciprofloxacine tablet 500 mg (10.7%), cefadroxil kapsul 500 mg (7.6%), amoksisilin sirup kering 125 mg/5 mL (5.6%), dan thiamfenikol kapsul 500 mg (4.6%). Sementara lima antibiotik yang paling banyak digunakan di seluruh jaringan Puskesmas wilayah Kecamatan Matraman tahun 2022 yaitu amoksisilin kaplet 500 mg (58.0%), ciprofloxacine tablet 500 mg (11.0%), cefadroxil kapsul 500 mg (6.8%), amoksisilin sirup kering 125 mg/5 mL (4.8%), dan isoniazid tablet 300 mg (3.1%). ......Antibiotics are drugs used to treat infections caused by bacteria. The use of antibiotics needs to be monitored because excessive use can increase the occurrence of resistance. Quantitative evaluation of drug use can be done using the ATC/DDDD (ATC = Anatomical Therapeutic Chemical; DDD = Defined Daily Dose) method, which is a classification and measurement system for drug use. The purpose of this study is to find out the five most widely used antibiotics in the Matraman District Health Center and in the entire Matraman District Health Center network in 2022 using the ATC/DDDD method. Antibiotic usage data is obtained from the Drug Use Report and Request Sheet. The five most widely used antibiotics at the Matraman District Health Center in 2022 are amoxicillin capsules 500 mg (59.2%), ciprofloxacine tablets 500 mg (10.7%), cefadroxil capsules 500 mg (7.6%), amoxicillin dry syrup 125 mg/5 mL (5.6%), and thiamphenicol capsules 500 mg (4.6%).
