As a vital part of patient care delivery, patient safety culture contributes to the quality of care provided by nurses. Safe patient care is positively linked to the attitudes of nurses. This study aimed to assess the perception of nurses working in a newly established teaching hospital. A cross-sectional study involving 194 nurses from three different units was conducted by using a 24-item Hospital Survey of Patient Safety Culture. Data on gender, working unit, age, years of working, and attendance in workshops on patient safety were also collected. The majority of the nurses had a positive total score of patient safety culture. The lowest score was 76 (63%), and the highest score was 120 (96%). The awareness on patient safety culture significantly differed between gender, years of working, and working units. Post-hoc comparisons using Tukey’s HSD test yielded a significant difference between nurses from critical care units and those from medical and surgical units. The mean score and total positive score on awareness on patient safety culture of the former were higher than those of the latter. Overall, the majority of the staff nurses in International Islamic University Malaysia Medical Center had a positive total score on awareness on patient safety culture. Awareness on patient safety, which is considered crucial worldwide, should be enhanced to influence the development of a positive patient safety culture within hospitals. This implementation would directly develop high-quality care to patients and positively impact health organizations.

Latar Belakang: Selama proses pengobatan berlangsung dapat terjadi kesalahan yang mungkin disebabkan oleh tenaga kesehatan maupun pasien itu sendiri. Kesalahan medikasi adalah kejadian pemberian obat yang dapat menimbulkan cedera kepada pasien yang seharusnya dapat dicegah selama dalam kontrol tenaga kesehatan, dan pasien itu sendiri. Salah satu peran penting perawat di layanan kesehatan yaitu memperhatikan prinsip tujuh benar obat saat memberikan obat kepada pasien. Dari data insiden keselamatan pasien di rumah sakit X tahun 2015 - 2017 tercatat ada 35 kasus insiden keselamatan pasien yang 14 kasus diantaranya adalah kasus kesalahan pemberian obat di rawat inap. Hal ini tidak sesuai dengan standar yang diatur oleh Kemenkes RI tahun 2008 yang menyatakan bahwa insiden di rumah sakit seharusnya zero accident. Penelitian ini bertujuan untuk menganalisa faktor faktor yang mempengaruhi penerapan prinsip keselamatan pasien oleh perawat dalam proses pemberian obat di bangsal rawat inap rumah sakit X. Metode Penelitian: Penelitian ini merupakan penelitian kualitatif. Data primer diperoleh melalui wawancara mendalam dengan 10 orang informan dan observasi. Data yang terkumpul dianalisa untuk mengetahui penyebab kesalahan dan kemungkinan perbaikannya. Hasil Penelitian: Dari penelitian terlihat bahwa faktor yang memicu terjadinya kesalahan pemberian obat di ruang rawat inap rumah sakit X berasal dari kondisi laten yaitu kurangnya jumlah staff keperawatan, turnover yang tinggi dan tidak tersedianya SPO 7 benar pemberian obat. Faktor lingkungan tempat kerja yang kurang memadai, adanyapekerjaan tambahan perawat, kurangnya supervisi dari pimpinan, panduan yang kurangjelas serta tidak adanya farmasi klinik merupakan faktor kontributor yang dapat mempengaruhi terjadinya insiden keselamatan pasien dimana salah satunya adalah kejadian medication error di unit rawat inap rumah sakit X saat ini. Kesimpulan: Faktor-faktor yang mempengaruhi penerapan prinsip keselamatan pasien dalam pemberian obat di rawat inap rumah sakit X antara lain adalah faktor leadership, kurangnya jumlah SDM pelayan kesehatan, tingginya turnover perawat, faktor lingkungan tempat kerja yang kurang kondusif, tidak tersedianya SPO pemberian obat dengan prinsip 7 benar, sosialisasi yang tidak dilakukan secara konsisten dan kontinyu serta tidak berjalannya program diklat atau pelatihan. ......Background: During the treatment of patients held can occur some mistakes, may becaused by health workers and patients itself. Medication error is the incident of a drug provision that can inflicting injury to the patient which can be prevent during in control of health workers, and patients itself. One important role nurses in health services is taking into account the seven right principle while giving medicines to patients. From the data of patient safety incident in hospital X during 2015-2017 there are 35 cases of patient safety incidents when 14 cases of them were cases of medicine error in inpatients ward. It is not conforming to standard arranged by the ministry of Health of Indonesia in 2008 stating that the incident at the hospital supposed to zero accident. This study aimed to analyse factors influencing the application of patient safety by nursesin the process of administering medication in inpatients ward at the X hospital. Methods: The research is the qualitative study. The primary data was obtained through indepth interviews with 10 informants and observation. The data collected were analysed todetermine the cause of error and the possibility of improvement. Results: From this research it can be seen that factors that can be triggered a mistake in theprovision of a medicine in the inpatient ward X hospital derived from latent condition suchas the number of nursing staff, high turnover, the lack of operational standard procedure of 7 right medication administration. Environmental factor such as workplace was inadequate, the additional job for nurses, a lack of supervision from leaders, an unclear guidelines and the absence of clinical pharmacy are the contributor factor that can affect patent safety incident where one of them is the medication error incident in inpatient ward. Conclusion: Factors influencing the occurrence of the application of patient safety at the X hospital among others are leadership, shortage of human resources, the high turnover of nurses, the less condusive of an environmental workplace, the lack of operational standard procedure of 7 right principle in administering medicine, socialization that has not been carried out consistently and continuously as well as the training courses program.

Motivasi dan kinerja perawat dapat mempengaruhi pelayanan kepada pasien termasuk didalamnya kepuasan pasien. Penelitian ini bertujuan untuk mengetahui motivasi dan kinerja perawat pelaksana terhadap kepuasan pasien ruang rawat inap di RS. Desain penelitian ini merupakan deskriptif korelasi dengan pendekatan potongan lintang (cross-sectionl) menggunakan sampel sebanyak 75 responden yang dipilih dengan teknik acak secara sederhana (simple random sampling). Instrumen yang digunakan terdiri dari data motivasi, kinerja perawat dan kepuasan pasien. Hasil penelitian menunjukan sebagian besar perawat mempunyai motivasi kurang 52%. Sebagian besar mempunyai kinerja baik 56% dan sebagian pasien puas terhadap pelayanan sebanyak 57,3%. Hasil dari penelitian menunjukan bahwa tidak ada hubungan antara motivasi perawat dengan kinerja perawat(p>0,05), hasil dari penelitian menunjukan bahwa tidak ada hubungan antara motivasi dan kepuasan pasien (p>0,05), dan tidak ada hubungan antara kinerja dan kepuasan pasien (p>0,05). Hasil penelitian menyarankan upaya-upaya peningkatan motivasi dan kinerja perawat yang lebih sejahtera yang dapat diterapkan serta diharapkan dapat meningkatkan tingkat kepuasan pasien. ...... Motivation and nurses can affect the performance of services to patients including patient satisfaction. This study aimed to determine the motivation and performance of nurses on patient satisfaction wards in hospital inpatient room. This study design was a descriptive correlation with the approach cross section (cross-sectionl) used a sample of 75 respondents were selected by random techniques (simple random sampling). The instrument used consisted of motivation of data, the performance of nurse and patient satisfaction. The results showed the majority of nurses are motivated less 52%. Most have a good performance and a 56% majority of patients are satisfied with the services as much as 57.3%. The results of the study showed that there was no relationship between motivation and performance of patients (p> 0.05), there was no relationship between motivation and patient satisfaction (p> 0.05), and there was no relationship between performance and patient satisfaction (p> 0.05). The results of the study suggested efforts to increase motivation and performance more prosperous nurses that can be applied and is expected to increase the level of patient satisfaction.
