Tugas khusus yang diberikan berjudul Pengkajian Resep Diagnosis Myalgia berdasarkan Peraturan Menteri Kesehatan RI No.30 Tahun 2014 di Puskesmas Kecamatan Jatinegara. Tujuan penyusunan tugas khusus ini untuk mengetahui hasil pengkajian resep diagnosis myalgia di Puskesmas Kecamatan Jatinegara berdasarkan Permenkes RI nomor 30 tahun 2014.

Abstrak :
Apotek adalah sarana pelayanan kefarmasian tempat dilakukan praktik kefarmasian oleh Apoteker. Pengkajian resep merupakan salah satu standar pelayanan kefarmasian yang dilakukan di Apotek. Pengkajian resep dilakukan untuk melakukan analisa adanya permasalahan terkait obat dan apabila ditemukan masalah terkait obat harus segera dikonsultasikan kepada dokter penulis resep. Tujuan dilakukannya pengkajian resep untuk mencegah permasalahan terkait obat sebelum obat diserahkan kepada pasien sehingga tidak terjadi medication error yang berdampak pada kegagalan terapi dan efek obat yang tidak diharapkan yang merugikan pasien. Penelitian ini bertujuan untuk mengetahui kelengkapan administrasi, kesesuaian farmasetik dan kesesuaian klinis pada peresepan di Apotek Kimia Farma Menteng Huis. Dalam penelitian ini, dilakukan pengkajian resep Pasien Rujuk Balik (PRB) pasien asma dengan mengkaji secara administratif, farmasetik dan klinis. Pada persyaratan administratif, didapatkan ketidaklengkapan berat badan pasien dan paraf dokter. Pada pengkajian secara farmasetik, resep yang tertulis sudah sesuai bentuk dan kekuatan sediaannya. Pada pengkajian secara klinis, terdapat duplikasi dan interaksi pada beberapa obat. Dapat disimpulkan bahwa masih banyak ketidaksesuaian dalam penulisan resep yang diamati sehingga harus dikonsultasikan kepada dokter penulis resep. ......Pharmacy is a pharmaceutical service facility where pharmacists practice pharmacy. Prescription review is one of the standard pharmaceutical services carried out at pharmacies. Prescription review is carried out to analyze drug-related problems and if a drug-related problem is found, a doctor who prescribes it must be consulted immediately. The purpose of reviewing prescriptions is to prevent drug-related problems before the drugs are dispensed to patients so that medication errors do not occur which result in therapy failure and unexpected drug effects that harm the patient. This study aims to determine the completeness of administration, pharmaceutical suitability and clinical suitability for prescriptions at the Kimia Farma Pharmacy, Menteng Huis. In this study, the prescription review of Back-Referral Program (PRB) was carried out in asthma patient by examining administrative, pharmaceutical and clinical aspects. In administrative requirements, incomplete patient weight and doctor's initials were obtained. In the pharmaceutical study, the written prescription have appropriate dosage forms and dosage strengths. In clinical studies, there are duplications and interactions with several drugs. It can be concluded that there are still many observed discrepancies in prescription writing, so a doctor who prescribes it must be consulted.

Berdasarkan Pedoman tentang Standar Pelayanan Kefarmasian di Apotek, pengkajian resep merupakan salah satu aspek dari kegiatan farmasi klinis. Risiko terjadinya kesalahan atau medication error yang paling besar adalah prescribing error dengan persentase sebesar 99,12% yang dapat terjadi baik pada aspek administratif resep, aspek farmasetik, dan aspek klinis seperti polifarmasi atau interaksi obat. Adanya polifarmasi pada resep akan meningkatkan risiko terjadinya interaksi obat. Laporan ini disusun untuk menganalisa adanya masalah terkait obat pada resep polifarmasi baik dari aspek administratif, farmasetik, dan klinis. Pengkajian resep dilakukan terhadap 2 resep dengan polifarmasi mayor dan minor yang masuk ke Apotek Roxy Depok pada periode November 2022. Terdapat beberapa hal yang harus dilengkapi kembali terkait data berat badan pasien, nomor telepon dokter, dan paraf dokter untuk aspek administratif. Pada aspek farmasetik resep 1 perlu ditanyakan kembali kepada dokter yang meresepkan terkait bentuk sediaan ramipril dan kekuatan sediaan gliquidon. Pada aspek klinis resep 1 perlu ditambahkan terkait aturan penggunaan allopurinol, serta melakukan konfirmasi kembali kepada dokter yang meresepkan bahwa terdapat interaksi mayor yang perlu diwaspadai pada resep 2. ......Based on the Guidelines on Standard for Pharmaceutical Services in Pharmacies, prescription screening is an aspect of clinical pharmacy activities. The greatest risk of medication error is prescribing error with a percentage of 99.12% which can occur both in the administrative aspects of the prescription, pharmaceutical aspects, and clinical aspects such as polypharmacy or drug interactions. The presence of polypharmacy in the prescription will increase the risk of drug interactions. This report was prepared to analyze drug-related problems in polypharmacy prescriptions in administrative, pharmaceutical, and clinical aspects. The prescription review was conducted on 2 prescriptions with major and minor polypharmacy that entered Apotek Roxy Depok in the November 2022 period. There are several things that must be completed again regarding patient’s weight data, doctor's telephone number, and doctor's initials for administrative aspects. In the pharmaceutical aspect, prescription 1 needs to be asked again to the prescribing doctor regarding the dosage form of ramipril and the strength of the gliquidon preparation. In the clinical aspect, prescription 1 needs to be added regarding the rules for using allopurinol, as well as confirming again to the prescribing doctor that there are major interactions that need to be watched out for in prescription 2.

Abstrak :
Pengkajian resep merupakan kegiatan farmasi klinik yang meliputi pengkajian admininstratif, kesesuaian farmasetis, dan kesesuaian klinis untuk meminimalkan terjadinya kesalahan pengobatan. Penelitian ini bertujuan untuk mengetahui gambaran kepatuhan dari petugas farmasi dan menganalisa hubungan karakteristik individu, motivasi, kepemimpinan, imbalan dan suber daya terhadap kepatuhan dari petugas farmasi dalam melaksanakan pengkajian resep di Rumah Sakit Umum Ganesha Tahun 2018. Penelitian ini adalah penelitian kuantitatif dengan desain penelitian cross sectional dengan menggunakan kuisioner yang disebar ke seluruh petugas farmasi. Total jumlah sampel sebanyak 31 orang petugas farmasi. Hasil penelitian menyatakan lebih dari setengah petugas farmasi memiliki kepatuhan kurang terhadap pelaksanaan pengkajian resep. Variabel yang mempengaruhi kepatuhan petugas farmasi dalam melaksanakan pengkajian resep adalah masa kerja p=0,003, kepemimpinan p=0,017 dan imbalan p=0,001. Perlu dilakukan manajemen SDM yang baik, evaluasi pemberian imbalan dan membuat kebijakan terkait reward serta punishment yang adil bagi petugas farmasi. Selain itu, rutin dilaksanakannya pelatihan dan pengawasan untuk mengevaluasi hasil kerja dari petugas farmasi dalam melaksanakan pengkajian resep.Kata Kunci :Kepatuhan, pengkajian resep, petugas farmasi.

Prescription screening is a clinical pharmacy activity contain of an administrative screening, pharmaceutics compatibility and clinic compatibility to minimalize medication errors. The aims of this study is to understand compliance overview from the pharmacy officer and analyze the relationship of individual characteristic, motivation, leadership, reward and resources with compliance from pharmacy officer in order to do screening prescription at Ganesha General Hospital in 2018. The method of this study was a cross sectional quantitative study using questionnaire to 31 pharmacy officer as a sample. The result of the study showed that more than half of the pharmacy officer have less compliance to prescription screening activity. The variables that affected pharmacy officer compliance in doing prescription screening was working period p 0,003, leadership p 0,017 and reward p 0,001. An effective human resource management, reward evaluation, a fair reward and punishment policyfor pharmacy officer are needed. In addition, routine training and supervision are also important to evaluate the performance of pharmacy officer in doing prescription screening.Key Words Compliance, prescription screening, Pharmacy officer.

Pengkajian resep adalah hal yang perlu dilakukan untuk melihat apakah pengobatan yang dilakukan bersifat rasional. Hal ini perlu diperhatikan khususnya pada pasien jantung koroner dan hipertensi karena pasien umumnya mengkonsumsi jumlah obat yang cukup banyak. Pengkajian resep dilakukan di Apotek Kimia Farma 348 Dramaga pada periode bulan Januari – Maret 2023. Telaah dilakukan dengan memperhatikan tiga aspek, yaitu telaah administratif, farmasetik, dan klinis pada 20 resep. Berdasarkan hasil telaah yang dilakukan, didapatkan hasil, yaitu pada aspek telaah administratif, terdapat beberapa syarat yang belum terpenuhi, seperti kurangnya informasi mengenai jenis kelamin, berat badan, dan usia pasien yang dapat diatasi dengan melakukan konfirmasi kembali pada rumah sakit dan dokter atau pasien yang bersangkutan; pada aspek telaah farmasetik memenuhi seluruh syarat; dan pada aspek telaah klinis terdapat beberapa syarat yang belum terpenuhi, seperti adanya polifarmasi dan kemungkinan interaksi obat. Permasalahan ini dapat diatasi dengan memberikan edukasi pada pasien untuk memberikan jeda konsumsi obat serta memberikan informasi mengenai pentingnya ketepatan waktu konsumsi obat untuk mendapatkan efek terapi yang maksimal. ......Prescription review needs to be done to see whether patient’s treatment is rational. This needs to be considered especially in patients with coronary heart disease and hypertension because patients generally consume a large number of drugs. A prescription review was carried out at Kimia Farma 348 Dramaga Pharmacy in the period January - March 2023. The review was carried out by paying attention to three aspects, namely administrative, pharmaceutical and clinical reviews on 20 recipes. Based on the results of the review carried out, the results were obtained, namely the aspect of administrative review, there were several conditions that had not been fulfilled, such as a lack of information regarding the patient's gender, weight, and age which could be overcome by confirming to the hospital, doctor, or patient; pharmaceutical review aspect meets all requirements; and in clinical aspect there are several requirements that have not been fulfilled, such as the existence of polypharmacy and the possibility of drug interactions. Abstrak :
Infeksi Saluran Kemih (ISK) adalah infeksi bakteri yang sering terjadi dan salah satu alasan paling umum untuk penggunaan antibiotik. Salah satu masalah yang kini menjadi perhatian utama adalah masalah resistensi antimikroba, akibat penggunaan antibiotik secara tidak bijak, baik oleh masyarakat maupun tenaga kesehatan. Tujuan analisis resep antibiotik di Apotek Roxy Ciledug adalah pengkajian resep sesuai persyaratan Peraturan Menteri Kesehatan RI No. 73 Tahun 2016, analisis kesesuaian terapi berdasarkan Tatalaksana ISK dan Genitalia Pria 2021 oleh Ikatan Ahli Urologi Indonesia dan EAU Guidelines on Urological Infections 2022, dan memberikan rencana tindak lanjut untuk penyelesaian masalah terkait obat yang terjadi atau berpotensi terjadi. Penelitian ini bersifat observasional dengan pengambilan sampel secara retrospektif. Kriteria inklusi yang digunakan yaitu pasien dengan peresepan antibiotik oral maupun topikal. Kriteria eksklusi yaitu resep antibiotik telemedicine. Resep antibiotik yang digunakan sebagai sampel dalam penelitian ini ditetapkan secara simple random sampling. Jumlah sampel yang digunakan yaitu satu resep. Pasien dengan peresepan antibiotik yang masuk di Apotek Roxy Ciledug pada periode Juli 2022 berjumlah 87 pasien. Resep yang digunakan adalah resep antibiotik azithromycin, doxycycline, metronidazole. Hasil pengkajian menunjukkan resep telah sesuai persyaratan administratif, tidak ada bentuk sediaan menurut kajian farmasetik, terdapat efek samping obat dan tidak tepat dosis menurut kajian aspek klinis. Tindak lanjut mengenai masalah terkait potensi efek samping obat yaitu pemberian informasi obat kepada pasien, sedangkan masalah terkait interaksi obat yaitu pemberian jeda waktu penggunaan obat. ......Urinary Tract Infection (UTI) is a common bacterial infection and one of the most common reasons for taking antibiotics. One of the problems that has now become the main concern is antimicrobial resistance, due to the unwise use of antibiotics, either by the public or health workers. The purpose of analyzing antibiotic prescriptions at Apotek Roxy Ciledug is to review prescription according to Peraturan Menteri Kesehatan RI No. 73 Tahun 2016, analysis of the suitability of therapy based on the Tatalaksana ISK dan Genitalia Pria 2021 by the Indonesian Association of Urologists and the EAU Guidelines on Urological Infections 2022, and provides a follow up plan for solving DRP that occur or potentially to occur. This study is an observational study with retrospective sampling. The inclusion criteria were patients prescribed oral or topical antibiotics. The exclusion criteria were telemedicine antibiotic prescriptions. Antibiotic prescriptions used as sample in this study were determined by simple random sampling. The number of samples used is one prescription. Patients with antibiotic prescriptions who entered the Apotek Roxy Ciledug in July 2022 totaled 87 patients. The prescription used was azithromycin, doxycycline, metronidazole. The results showed that the prescription met administrative requirements, there were no dosage forms according to pharmaceutical studies, there were drug side effects and the dosage was incorrect according to clinical aspects. Follow up regarding problems related to potential drug side effects is providing drug information to patients, while problems related to drug interactions is providing drug use interval.

Abstrak :
Pelayanan obat atas resep dokter merupakan salah satu dari pelayanan kefarmasian di Apotek. Resep yang sesuai harus memuat cukup informasi yang memungkinkan tenaga kefarmasian yang bersangkutan mengerti obat yang akan diberikan kepada pasiennya. Resep juga harus ditulis dengan jelas agar terhindar dari salah persepsi antara dokter dengan tenaga kefarmasian yang bersangkutan. Namun pada kenyataanya, masih ada beberapa permasalahan yang ditemui di dalam peresepan. Permasalahan dalam peresepan obat tersebut disebut juga Medication error. Medication error merupakan kejadian yang dapat merugikan pasien akibat pemakaian obat yang seharusnya dapat dicegah dalam pengobatan. Upaya dalam pencegahan terjadinya kesalahan pemberian obat (Medication error) adalah pengkajian resep pada resep yang diterima dari pasien. Metode penelitian yang digunakan dalam penyusunan laporan tugas khusus ini yaitu pengambilan data secara retrospektif dimana dilakukannya pengamatan dan mencatat terkait dengan aspek administrasi, aspek farmasetik, dan aspek klinis dari resep tersebut dan di ceklis pada form yang telah dibuat. Berdasarkan hasil Praktik Kerja Profesi Apoteker di Apotek Kimia Farma 352 Margonda yang telah dilakukan oleh peneliti dapat disimpulkan bahwa Ketepatan resep pada pasien gastritis berdasarkan aspek administrasi masih terdapat beberapa kekurangan sehingga diperlukan kelengkapan dalam resep pada aspek administrasi, Ketepatan resep pada pasien gastritis berdasarkan aspek farmasetik telah memenuhi persyaratan standar pelayanan kefarmasian, namun terdapat beberapa resep yang masih belum memenuhi persyaratan sehingga kelengkapannya perlu diperhatikan kembali, dan Ketepatan resep pada pasien gastritis berdasarkan aspek klinis telah memenuho persyaratan standar pelayanan kefarmasian seluruhnya sehingga harus dipertahankan seterusnya seperti ini. ......Drug service on a doctor's prescription is one of the pharmaceutical services at the pharmacy. Prescriptions must also be written clearly to avoid misunderstandings between the doctor and the pharmacist concerned. But in fact, there are still some problems encountered in the prescription. Problems in prescribing these drugs are also called Medication errors. Medication error is an event that can harm the patient due to the use of drugs that should be prevented in treatment. Efforts to prevent medication errors from occurring are reviewing prescriptions on prescriptions received from patients. The research method used in the preparation of this special assignment report was retrospective data collection where observations and notes were made related to the administrative, pharmaceutical, and clinical aspects of the prescription and checked on the form that had been made. Based on the results of the Pharmacist Professional Work Practice at Kimia Farma 352 Margonda Pharmacy which has been carried out by researchers, it can be concluded that the accuracy of prescriptions for gastritis patients based on administrative aspects still has some deficiencies, so completeness of prescriptions is needed on administrative aspects. Accuracy of prescriptions for gastritis patients based on pharmaceutical aspects has been meet the standard requirements for pharmaceutical services, but there are several prescriptions that still do not meet the requirements so that their completeness needs to be considered again, and the accuracy of prescriptions for gastritis patients based on clinical aspects has fulfilled the standard requirements for pharmaceutical services in its entirety so it must be maintained onwards like this.

Pengkajian resep merupakan salah satu bagian dari pelayanan farmasi klinik yang dilakukan apoteker mulai dari pengkajian administratif, farmasetik serta klinis sebelum diracik. Apoteker sebagai mitra kerja dokter harus memahami dan mengkaji resep yang berpotensi menimbulkan kesalahan pengobatan melalui kajian terhadap kejadian medication error sesuai yang tercantum pada Standar Pelayanan Kefarmasian. Tujuan dari pengkajian resep ini yakni untuk mengetahui kerasionalan penggunaan obat ditinjau dari indikator pola peresepan berdasarkan kelengkapan administrasi dan indikator potensi medication error resep polifarmasi di Apotek Kimia Farma 11 Bandung. Kajian ini dilakukan selama Bulan Agustus 2020 yang bertempat di Apotek Kimia Farma 11 Bandung, Jl. WR. Supratman No. 72, Bandung. Kajian dilakukan selama Bulan Agustus 2020 terhadap 50 lembar resep yang berasal dari Klinik Kimia Farma Supratman dan di luar klinik, dengan melihat kejelasan penulisan terkait obat, kelengkapan resep serta gambaran mengenai interaksi obat pada 2 resep (polifarmasi). Dari hasil pengamatan, ditemukan banyak kelengkapan penulisan resep yang rendah berupa informasi nomor izin praktek dokter/SIP (48%), usia (46%), berat badan (4%) dan alamat pasien (22%). Dari 2 kajian resep secara klinis, terdapat interaksi obat antara Clopidogrel dengan Curcumin/Piracetam (resiko pendarahan) juga dengan obat antikolesterol Simvastatin/Artovastatin (penurunan kadar Clopidogrel). ......Prescription assessment is one part of clinical pharmacy services performed by pharmacists, starting from administrative, pharmaceutical, and clinical assessments before formulation. Pharmacists as a doctor’s work partner must understand prescriptions that have the potential to cause medication errors through a review of its incidence as stated in the Pharmaceutical Service Standards. The purpose of this prescription review is to determine the rationality of drug use in terms of prescribing pattern indicators based on administrative completeness and potential indicators of polypharmacy prescription medication errors at Kimia Farma 11 Pharmacy in Bandung. This study was conducted during August 2020 at Kimia Farma 11 Bandung Pharmacy, JalanWR. Supratman No. 72, Bandung. The study was carried out during August 2020 on 50 prescription sheets originating from the Kimia Farma Supratman Clinic and outside the clinic, by looking at the clarity of writing related to drugs, completeness of prescriptions, and an overview of drug interactions on two polypharmacy prescriptions. From the observations, it was found that there were many low completeness in writing prescriptions in the form of information on the doctor’s practice license number (48%), age (46%), body weight (4%) and the patient’s address (22%). From two clinical prescription studies, there were drug interactions between Clopidogrel and Curcumin/Piracetam (risk of bleeding) as well as the anti-cholesterol drug Simvastatin/Artovastatin (decreased levels of Clopidogrel).

Peraturan Menteri Kesehatan RI Nomor 73 tahun 2016 menetapkan standar pelayanan kefarmasian di Apotek, yang bertujuan untuk meningkatkan mutu kehidupan pasien. Pelayanan kefarmasian telah berkembang dari orientasi produk menjadi berorientasi pada pasien. Pengkajian resep adalah aspek operasional penting dalam pelayanan kefarmasian, yang bertujuan untuk mengidentifikasi dan menyelesaikan permasalahan terkait obat sebelum disiapkan. Dalam pengkajian resep, terdapat tiga aspek yang diperhatikan: administratif, farmasetik, dan klinis. Tujuan penelitian ini adalah melakukan pengkajian pada resep pasien dengan diagnosis osteoarthritis di Apotek Kimia Farma 049 untuk mengidentifikasi kemungkinan kesalahan pengobatan dan memberikan saran. Ditemukan bahwa resep pasien osteoarthritis memiliki beberapa permasalahan administratif, seperti ketidaklengkapannya, tetapi dari segi farmasetik dan klinis sudah sesuai. Kesimpulan dari pengkajian adalah bahwa meskipun ada permasalahan administratif pada resep, obat yang diresepkan sudah sesuai dengan tatalaksana osteoarthritis. Saran yang diberikan adalah untuk memperhatikan aspek administratif yang kurang lengkap pada resep dan melakukan konfirmasi kepada pasien atau dokter jika diperlukan. ...... Pharmaceutical services have evolved from product-oriented to patient-oriented. Prescription assessment is a crucial operational aspect in pharmaceutical services, aiming to identify and resolve medication-related issues before preparation. In prescription assessment, three aspects are considered: administrative, pharmaceutical, and clinical. The aim of this study is to assess prescriptions for patients diagnosed with osteoarthritis at Kimia Farma Pharmacy 049 to identify potential medication errors and provide recommendations. It was found that prescriptions for osteoarthritis patients had some administrative issues, such as incompleteness, but were adequate in terms of pharmaceutical and clinical aspects.
