ABSTRAK
In prosthodontics, mandibular bone density plays an important role in the success of treatment. Generally, the aging process will be accompanied by decreasing mandibular bone density. This is more obvious in post menopausal women with low or absent estrogen production. Relationship between mandibular bone density and bone density in other parts of the body is still debatable. The aim of this study was to evaluate this relationship. Periapical radiographs and bone densitographs of 114 post menopausal women were evaluated in a cross sectional study. The results showed a relationship between age and amount of teeth present with decreasing mandibular bone density, and a significant correlation between mandibular bone density and lumbal or femur bone density.

MiR-21 diketahui berperan dalam proliferasi dan diferensiasi osteoklas, namun peran ekspresi miR-21 serum pada osteoporosis masih belum jelas. Penelitian sebelumnya mendapatkan bahwa ekspresi miR-21 serum berkorelasi positif dengan densitas mineral tulang pada penderita osteoporosis pascamenopause, tetapi penelitian tersebut tidak menganalisis faktor-faktor lainnya yang terlibat dalam osteoporosis pascamenopause.Penelitian ini bertujuan untuk mengetahui peran ekspresi miR-21 serum, konsentrasi RANKL, OPG, TGF- ? 1, sklerostin, rasio RANKL/OPG, kalsium serum dan aktivitas fisis terhadap densitas mineral tulang belakang pada perempuan pascamenopause hipoestrogenik dengan osteoporosis dibandingkan dengan tanpa osteoporosis, dengan point of interest pada ekspresi miR-21 serum.Penelitian ini dilakukan dengan disain uji potong lintang komparatif, di RSUD Ulin Banjarmasin, pada bulan Agustus 2015 sampai Juli 2016. Subjek dibagi menjadi 2 kelompok yaitu perempuan pascamenopause hipoestrogenik dengan osteoporosis dan tanpa osteoporosis. Pengambilan sampel dilakukan dengan metode consecutive. Pemeriksaan ekspresi miR-21 serum menggunakan metode absolute quantification real-time PCR. Analisis statistik menggunakan uji korelasi Spearman, Mann-Whitney U test dan regresi linear berganda.Subjek dibagi menjadi 2 kelompok yaitu perempuan pascamenopause hipoestrogenik dengan osteoporosis PMOP dan tanpa osteoporosis PMNOP masing-masing sebanyak 60 subjek. Median ekspresi miR-21 serum pada PMOP lebih tinggi secara bermakna dibandingkan dan PMNOP p = 0,001 . Ekspresi miR-21 serum, RANKL, rasio RANKL/OPG dan aktivitas fisis berkorelasi bermakna dengan nilai BMD pada PMOP. Aktivitas fisis sedang berkorelasi negatif bermakna dengan ekspresi miR-21 serum pada PMOP dan PMNOP. Analisis regresi linear berganda menggunakan metode backward stepwise mendapatkan persamaan regresi linear: BMD = 1,373 - 0,085 Ln.miR-21 - 0,176 Log 10.RANKL R2 = 52,5 .Simpulan. Ekspresi miR-21 serum pada perempuan pascamenopsuse hipoestrogenik dengan osteoporosis terbukti lebih tinggi dibandingkan dengan tanpa osteoporosis. Ekspresi miR-21 serum terbukti memberikan pengaruh negatif terhadap nilai BMD tulang belakang pada perempuan pascamenopause hipoestrogenik dengan osteoporosis sebesar 8,5 , dengan persamaan regresi linear BMD = 1,373 - 0,085 Ln.miR-21 - 0,176 Log10.RANKL. Persamaan ini dapat menjelaskan nilai BMD tulang belakang sebesar 52,5 . Kata kunci: BMD, miR-21 serum, Osteoporosis, Pascamenopause
ABSTRACT
MiR-21 is known to play a role in osteoclast proliferation and differentiation, but the role of serum miR-21 expression in osteoporosis remains unclear. Previous research found that serum miR-21 expression was positively correlated with bone mineral density in postmenopausal osteoporosis patients, but the study did not analyze other factors involved in postmenopausal osteoporosis.This study aimed to determine the role of serum miR-21 expression, concentration of RANKL, OPG, TGF- ? 1, sclerostin and serum calcium, RANKL/OPG ratio, and physical activity on bone mineral density of spine in hypoestrogenic postmenopausal women with osteoporosis compared to no osteoporosis, with point of interest on the expression of serum miR-21.This study was conducted by comparative cross sectional design, conducted at Ulin General Hospital Banjarmasin, from August 2015 until July 2016. The subjects were divided into 2 groups of hypoestrogenic postmenopausal women with osteoporosis and without osteoporosis. Sampling was done by consecutive method. Examination of serum miR-21 expression using absolute quantification real-time PCR method. Statistical analysis using Spearman correlation test, Mann-Whitney U test and multiple linear regression.The subjects were divided into 2 groups of hypoestrogenic postmenopausal women with osteoporosis PMOP and without osteoporosis PMNOP each as many as 60 people. Median of serum miR-21 expression at PMOP group was significantly higher compared to PMNOP group p = 0.001 . Serum miR-21 expression, RANKL, RANKL/OPG ratio and physical activity were significantly correlated with BMD values ? ? ? ?in PMOP group. Moderate physical activity was significantly negative correlated with serum miR-21 expression. Multiple linear regression multivariate analysis using backward stepwise method obtained linear regression equation BMD = 1,373 - 0,085 Ln.miR-21 - 0,176 Log10.RANKL R2 = 52,5 .Conclusion. Serum miR-21 expression in hypoestrogenic postmenopausal women with osteoporosis has been shown to be higher compared with no osteoporosis. Serum miR-21 expression proved to have a negative effect on spinal BMD values in hypoestrogenic postmenopausal women with osteoporosis of 8.5 , with linear regression equation BMD = 1.373 - 0.085 Ln.miR-21 - 0.176 Log10.RANKL. During three months ,August 1991 until November 1991, the randomized clinical trial study was conducted to elucidate the effect of calcium supplementation on calcium metabolism to prevent bone loss ip postmenopausal women. Thirty four postmeno- pausal women age 55 to 65 years old from RN. 01, Panjunan village, Astanaanyar subdistrict, Bandung regency West Java who had undergone menopause at least five years since last menses were recruited to the study. The subjects were randomly assigned to the treatment (1000 mg calcium/day) and the control groups in a double blind designs. Thirty one subjects who remained in the study consist of sixteen treatment subjects and fifteen control subjects were included in the final data analysis. Baseline data of the study showed no a significant difference for all variables, therefore baseline data were homogenous. Based on two x 24 hours dietary recall methods all subjects had low dietary calcium intake (291.5l mg/ day and 289.32 mg/day). After treatment, based on the laboratory tests show that the serum calcium concentration was significantly increased in the treatment group (p<0.001) and was not significant difference (p=O.596) in the control group. Serum calcium was a significant difference (p<0.001) between the treatment and the control group. Parathyroid hormone level was significantly decreased in the treatment group (p<0.05) and was significantly increased in the control group (p<0.01). There was significant difference (p<0.00l) between the treatment and the control group. Bone loss of radius and ulna were determined by z-ray standard measurement were significantly reduced (p<0.001 and p<0.01, respectively) after 1000 mg calcium supplementation. Alkaline phosphatase was significantly decreased (p<0.001) in the treatment group and was significantly increased in the control group. There was significant difference (p<0.001) between the treatment and the control group. Urinary ca}cium creatinine ratio was not significantly increased (pI0.764 and p=0.0BB) and was not significant difference (p=0.067) in all subjects after treatment. This study support the hypothesis that calcium supplementation can maintain calcium balance, even improved bone density in postmenopausal women.

ABSTRAK Latar Belakang: Gangguan kognitif merupakan salah satu masalah pada aging population berkaitan dengan perubahan neuroendokrin pascamenopause. Gangguan kognitif minimal (minimal cognitive impairment/MCI) merupakan kondisi peralihan fungsi kognitif antara penuaan normal dan demensia. Tujuan: Untuk mengetahui hubungan kadar kisspeptin dan kadar hCG hipofisis dengan gangguan kognitif minimal pada perempuan pascamenopause. Metodologi: MCI ditentukan dengan sistem skoring yang terdiri dari status diabetes melitus, toleransi glukosa terganggu, dislipidemia, Geriatric Depression Scale (GDS), uji Rey Osterrieth Complexion Figure (ROCF), uji digit span backward dan uji Trail Making Test B. Studi potong lintang terhadap 181 perempuan pascamenopause usia £ 65 tahun, terbagi 2 kelompok yaitu dengan MCI 90 orang dan tanpa MCI 91 orang. Hasil: Analisis terhadap usia, lama menopause, indeks massa tubuh, lama pendidikan, kadar kisspeptin dan kadar hCG hipofisis. Kadar kisspeptin menunjukkan perbedaan bermakna anatar kedua kelompok (p<0,001). Kadar kisspeptin dan lama menopause berkorelasi positif dengan skor MCI (r=0,607 dan r=0,542; berurutan). Namun, tidak ada perbedaan kadar hCG hipofisis antara kedua kelompok (p=0,664), dan skor MCI tidak berkorelasi dengan kadar hCG hipofisis (p=0,398; r=0,06). Kesimpulan: Kadar kisspeptin signifikan lebih tinggi pada perempuan pascamenopause dengan MCI, dan menunjukkan korelasi positif. Sementara kadar hCG hipofisis tidak berbeda di antara kedua kelompok dan tidak menunjukkan korelasi.
ABSTRACT Backgroud: Cognitive impairment is one of problems among elderly women due to neuroendocrine alteration after menopause. Minimal cognitive impairment (MCI) is a transition state of cognitive function between normal aging and dementia. Aims: To investigate relationship between kisspeptin and pituitary hCG with MCI in postmenopausal women. Methods: MCI was determined by scoring; with diabetic status, glucose intolerant, dyslipidemia, Geriatric Depression Scale (GDS), Rey Osterrieth Complexion Figure (ROCF), digit span backward, and Trail Making Test (TMT) B. Using cross-sectional study, 181 postmenopausal women £ 65 years old, were grouped into with and without MCI; 90 and 91 women, respectively. Results: Data was analysed to their ages, span of menopause, body mass index (BMI), education grade, kisspeptin and hCG level. Kisspeptin level had significantly different among the groups (p<0.001). There was a positive relationship between kisspeptin level and span of menopause to MCI score (R=0.607 and R= 0.542, respectively). Pituitary hCG level, however, showed no difference among the groups. Moreover, MCI score showed no relationship to hCG level (p=0.398; R=0.063). Conclusions: Kisspeptin level was significantly higher among postmenopausal women with MCI, and showed a positive relationship. While pituitary hCG had no difference among the groups, and showed no relationship.

Hipertensi merupakan masalah kesehatan masyarakat yang prevalensinya cukup tinggi. Kenaikan prevalensi sejalan dengan bertambahnya usia khususnya pada wanita yang telah memasuki masa menopause. Obesitas sering terjadi pada wanita usia pertengahan dibanding pria, hal ini menjadi penyebab mengapa berat badan sering mempengaruhi tekanan darah pada wanita. Penelitian ini untuk mengetahui hubungan kegemukan dengan hipertensi pada wanita postmenopause dengan melakukan analisis data sekunder: studi kohor faktor risiko penyakit tidak menular di kelurahan Kebon Kalapa, kec. Bogor Tengah, Kota Bogor tahun 2011. Penelitian dilakukan dengan disain Cross Sectional. Hasil: Proporsi responden yang mengalami kegemukan 74,6% dan hipertensi 52,4%. Prevalens rasio (PR) hipertensi 1,51 kali lebih besar terjadi pada responden yang gemuk (95% CI: 1,12-2,04, p value = 0,003). Analisis multivariat dengan Cox Regression yaitu setelah dikendalikan dengan variabel confounding: umur, pendapatan keluarga dan riwayat penyakit kronis, maka PR hipertensi pada reponden yang gemuk sebesar 1,38 kali lebih tinggi dibandingkan dengan responden yang berat badan normal (95% CI: 0,92?2,07). Kesimpulan: kegemukan pada wanita postmenopause dapat meningkatkan risiko hipertensi dan dipengaruhi oleh faktorfaktor risiko lain seperti umur, riwayat penyakit kronis dan kondisi sosial ekonomi, sehingga perlu dilakukan antisipasi sejak dini dengan meningkatkan perilaku hidup sehat dan pendidikan kesehatan bagi masyarakat khususnya wanita.
Hypertension is a public health problem that prevalence is quite high. The increase in prevalence with age , especially in women who have entered menopause. Obesity is common in middle-aged women than men, and this is also the reason why weight frequently affects blood pressure in women than men. Objective:To determine the relationship of obesity with hypertension in postmenopausal women with secondary data analysis: the baseline cohort study of risk factors for non-communicable diseases in Kebon Kalapa, Central Bogor, Bogor City in 2011. Methods: Cross sectional study design. Results: The proportion of overweight is 74.6 % and 52.4 % for hypertension . Prevalence ratios ( PR ) hypertension 1.51 times greater in obesity ( 95 % CI : 1.12 to 2.04 , p value = 0.003). Multivariate analysis using Cox Regression. Upon controlled potential confounding variable is the variable age , family income and a history of chronic disease , the prevalence rate of hypertension in obese respondents was 1.38 times higher compared with those who had normal weight (95 % CI is 0.92-2.07). Conclusion: Obesity in postmenopausal women may increase the risk of increased blood pressure , and is also influenced by other risk factors such as age , history of chronic disease and socioeconomic conditions , so it needs to be done early anticipation by increasing healthy behavior and health education for the community , especially women.

Abstrak :

Menopause menyebabkan hipoestrogenisme dan mengakibatkan penuaan kulit. Fitoestrogen dari biji T. foenum-graecum (klabet) diharapkan dapat mengatasi penuaan kulit pascamenopause. Penelitian ini bertujuan untuk mengetahui konsentrasi optimal ekstrak klabet dalam menstimulasi sekresi COL1A1 dan COL3A1, cara kerjanya melalui jalur reseptor estrogen α (REα) atau REβ, dan peran klabet dalam mengurangi kerutan serta meningkatkan ketebalan kulit wajah.

Studi in vitro menggunakan human dermal fibroblast (HDF) tua yang diperoleh dari kultur sel fibroblas kulit perempuan pascamenopause dan HDF muda dari prepusium, dilakukan di Laboratorium Universitas YARSI. Sekresi COL1A1 dan COL3A1 diperiksa dengan ELISA lalu ditambah antagonis REa dan b. Penelitian in vivo merupakan uji klinis acak tersamar ganda berdurasi 12 minggu, di RSCM, Januari–November 2019. Subjek 50 perempuan pascamenopause yang dibagi dua: kelompok perlakuan mendapat krim klabet 5% dan kelompok plasebo mendapat krim dasar. Skor kerutan dahi, crow’s feet, dan nasolabial diukur dengan skoring atlas skin aging Bazin dan tebal dermis dengan high resolution ultrasound (HRU) 18 MHz.

Ekstrak klabet 2 µg/mL merupakan konsentrasi optimal dalam meningkatkan sekresi COL1A1 dan COL3A1 pada HDF tua dan muda dibandingkan tanpa perlakuan dan 5 nM estradiol. Uji inhibisi menunjukkan hambatan REα 50%; REβ dan RE α,β sampai 75%. Penurunan skor kerutan dahi, crow’s feet, dan nasolabial berbeda bermakna pada kedua kelompok minggu ke-4, ke-8, dan ke-12.  Peningkatan selisih skor kerutan dahi, crow’s feet, dan nasolabial tidak bermakna pada kedua kelompok antara baseline, minggu ke-4, ke-8, dan ke-12. Ketebalan dermis meningkat bermakna pada minggu ke-4 dibandingkan baseline. Pada minggu ke-8 dibandingkan minggu ke-4 dan pada minggu ke-12 dibandingkan minggu ke-8, ketebalan dermis menurun bermakna pada kedua kelompok. Peningkatan selisih ketebalan dermis pada minggu ke-4 dan ke-8 tidak bermakna.

Simpulan: Ekstrak klabet 2 µg/mL meningkatkan COL1A1 dan COL3A1 lebih banyak dibandingkan kelompok tanpa perlakuan dan estradiol 5 nM. Klabet bekerja terutama melalui REβ. Penurunan skor kerutan kulit dan peningkatan ketebalan kulit wajah tidak bermakna dibandingkan plasebo. Diduga, krim klabet konsentrasi 5% tidak tepat untuk mengurangi kerutan dan menambah ketebalan kulit. Perlu penelitian lebih lanjut untuk mengetahui konsentrasi optimal klabet sebagai terapi penuaan kulit wajah pascamenopause.

Hypoestrogenism in menopause leads to skin aging, for which phytoestrogen originated from the seeds of T. foenum-graecum (fenugreek) is expected to be a solution. This study aims to show the effect of phytoestrogen from fenugreek extract in stimulating COL1A1 and COL3A1 through its mechanism of action on estrogen receptor (ER) ERa or ERb and its role in diminishing facial wrinkles and increasing dermal thickness.

The in vitro study was carried out in YARSI University Laboratory throughout March 2017–March 2018. This study looks into the effect of fenugreek extract on collagen level secretion in old and young human dermal fibroblast (HDF) compared to control. The in vivo study was a randomized, double-blind, 12-week trial conducted in RSCM from January–November 2019. Fifty postmenopausal women divided into two groups: the intervention group was given 5% fenugreek cream while the placebo group was given base cream.

The in vitro study showed that a concentration of 2 mg/mL was the optimal dose to stimulate COL1A1 and COL3A1 secretion in both old and young HDF compared to control (no treatment) and 5 nM estradiol. The inhibition test demonstrated suppression of ERa by 50%; ERb and ERa,b by up to 75%, indicating that the fenugreek activates both receptors, especially ERb. However, the success of the in vitro study did not translate into the in vivo study. Both the intervention group and the placebo group were able to achieve statistically significant in facial wrinkle scores from all focus areas without any significant disparity between both groups at all timepoints. Dermal thickness of facial skin showed similar results for both groups with significant improvements in the 8thweek compared to baseline and significant decrease by the 12thweek.

Conclusion: Fenugreek extract with a concentration of 2 mg/mL increased COL1A1 and COL3A1 secretion more potently compared to control and estradiol 5 nM. However, the decrease in facial skin wrinkles scores and the increase in dermal thickness were not significant compared to placebo. We suspect that a concentration of 5% was not adequate for the expected antiaging effects. Further studies are necessary to determine a more appropriate fenugreek concentration to permit clinical use as a postmenopausal antiaging therapy.
