[ABSTRAK
Magnetic resonance imaging selanjutnya disebut MRI merupakan peralatan radiologi diagnostik yang tidak mengandung radiasi pengion. Hal tersebut tidak berarti menjadikan alat ini bebas dari potensi bahaya. Penelitian ini ditujukan untuk mendapatkan deskripsi, menganalisa dan menilai penerapan keselamatan lingkungan pada fasilitas MRI 1.5T di wilayah DKI Jakarta. Penelitian didesain dengan metode campuran dengan tehnik purposive sampling. Dilaksanakan pada 4 fasilitas MRI 1.5T di wilayah DKI Jakarta dengan jumlah responden sebanyak 25 (dua puluh lima) orang radiografer yang bertugas di pelayanan MRI. Data kuantitatif diuji dengan menggunakan uji statistik nonparametrik yaitu Cochran’s Q dan Kruskalwalis. Nilai Cochran’s Q hitung>Chi square tabel (66.495>36.415) sehingga H0 ditolak atau terdapat perbedaan pemahaman radiografer. Uji statistik kruskalwalis menunjukkan nilai mean rank antar rumah sakit sumber data bervariasi. Data kualitatif terkait ketersediaan perangkat keselamatan menunjukkan standar American Collage of Radiology (ACR) lebih aplikatif dibandingkan standar Keputusan Menteri Kesehatan (KMK) RI No. 410/MENKES/SK/III/2010 tentang standar pelayanan radiologi diagnostik di sarana pelayanan kesehatan. Keempat rumah sakit sumber data telah memenuhi standar ACR sedangkan standar KMK 410/2010 hanya dapat dipenuhi oleh sebagian. Kepatuhan petugas dalam praktek aman penanganan pasien mandiri di empat rumah sakit sumber data telah memenuhi standar ACR dengan nilai bervariasi.

This study aimed to get a description, analyze and assess the implementation of environmental safety at 1.5T MRI facility in Jakarta. The research is designed with a mix method with purposive sampling technique. Held on 4 (four) 1.5T MRI facilities in Jakarta. The number of respondents as many as 25 (twenty five) radiographers who served in MRI services. The quantitative data were tested using nonparametric statistical test that Cochran's Q and Kruskalwalis . Cochran's Q value count > Chi square table (66 495> 36 415) so that H0 is rejected or there is a difference of understanding radiographer. Kruskalwalis statistical test shows the mean rank among hospitals varied data sources. The qualitative data related to the availability of safety devices demonstrate the American Collage Of Radiology (ACR) standards more applicable than Keputusan Menteri Kesehatan (KMK) RI No. 410/MENKES/SK/III/2010 standard. The fourth hospital data sources meet the standard ACR while KMK 410/2010 standard can only be met by the majority. ABSTRAK International Labour Organization (ILO) telah menerbitkan standar praktis pencegahan kecelakaan besar dalam industri dimana telah diadopsi oleh pemerintah dengan peraturan No.: Kep-84/PPK/X/2012 tentang Tata Cara Penyusunan Dokumen Pengendalian Bahaya Besar dan Menengah. Kilang LPG sebagai salah satu instalasi yang memiliki potensi bahaya besar (major hazard) harus dilengkapi dengan penyusunan Dokumen Pengendalian Potensi Bahaya Besar untuk memastikan aspek K3 terpenuhi. Dalam penyusunan dokumen ini akan dilakukan proses verifikasi lapangan oleh inspektur. Dalam pelaksanaan proses verifikasi lapangan maka perlu dilakukan terlebih dahulu kajian terhadap penilaian risiko yang akan dihadapi oleh inspektur dalam proses pemeriksaan perlatan utama dan pendukung pada kilang LPG. Hasil penilaian risiko yang dilakukan dengan menggunakan teknik matriks konsekuensi dan probabilitas (risk index) sesuai dengan ISO 31010:2009 terdapat 12 tabel penilaian risiko dengan 82 potensi risiko K3. Dari 12 kegiatan pemeriksaan peralatan utama dan pendukung terdapat peringkat risiko (Risk Rating) Tinggi sebanyak 9 jenis kegiatan pemeriksaan atau sebesar 75%, dan sebanyak 3 jenis kegiatan berperingkat Menengah atau sebesar 25%. Kegiatan yang memiliki peringkat risiko Tinggi adalah pemeriksaan pada obyek/peralatan yang memiliki risiko kebakaran atau ledakan karena gas atau kondensat. Hasil upaya pengendalian risiko menunjukkan bahwa seluruh risiko sebanyak 82 potensi risiko dapat diturunkan peringkat risikonya menjadi risiko yang dapat diterima (Accepted) yaitu Rendah dan Sangat Rendah. Metode pengendalian risiko dilakukan dengan pengendalian adminstratif dan penggunaan Alat Pelindung Diri (APD).
ABSTRACT International Labour Organization (ILO) has published a Code of Practice on Prevention of Major Industrial Accidents which has been adopted by the Government of Republic Indonesia regulation No.: Kep-84/PPK/X/2012 on Procedures for Document Preparation to Major and Medium Hazard Control. LPG plant is one of the installation that has a potential for major hazard and should be arranged the Document of Major Hazard Controll to ensure safety aspects are achieved. In preparing this document, it must be verified by the independent inspectors. Along of the verification process, the inspector will be exposed to high risk condition. The results of the risk assessment carried out by using a risk index technique in accordance with ISO / IEC 31010 there were 12 tables of risk assesment result and with 82 of the potential risk of K3. There are 9 tables (75%) of inspection activities that has High Risk Rating and 3 tables (25%) that has Medium Risk Rating. The inspection activities that have a high risk rating is the examination of the object / equipment that has a risk of fire or explosion due to gas or condensate. After risk controlled has applied, the final risk rating for 82 potentail risk to be accepted ( Low and Very Low Risk). Risk rating has been reduced by using the methods of administrative control and Personal Protective Equipment (PPE).

ABSTRAK
Kendaraan roda empat dapat mengalami perilaku understeer atau oversteer ketika berbelok. Perilaku tersebut menunjukkan ketidakstabilan pada kendaraan yang dapat terjadi ketika kendaraan di laju dengan kecepatan tinggi diatas permukaan jalan dengan koefisien gesek yang rendah. Ketidakstabilan ini dapat menjadi potensi bahaya ketika berkendara.Desain pengendali prediktif bertingkat dengan model gerak kendaraan double track, diajukan dalam skripsi ini untuk mengatasi perilaku understeer dan oversteer. Perancangan pengendali dimulai dari mendapatkan data masukkan dan keluaran pergerakkan kendaraan. Kemudian dengan metode least square bertingkat, didapatkan matrik matrik model identifikasi bertingkat. Model identifikasi tingkat pertama digunakan untuk mendapatkan nilai eror estimasi keluaran, sedangkan model identifikasi tingkat kedua digunakan sebagai model pengendali prediktif bertingkat.Pada akhir penelitian, desain pengendali prediktif bertingkat diuji melalui simulasi untuk melihat kemampuan pengendali yang telah dirancang.
ABSTRACT
Oversteer and understeer could be experienced by each of four wheel vehicle. The behaviours show the instability of the vehicle, and might be happened because of high velocity of the vehicle and low friction coefficient of the road. The instability could be one of the potential risks in driving the vehicle.The design of multistage predictive control with double track vehicle model is proposed in this research to handle understeer and oversteer behaviours. The design started from collecting the related input and output. Then the multistage least square method is used to find the matrix used in multistage identification model. The first stage of identification model is used to get prediction error that happened while estimating the output. The second level of identification model is used as multistage predictive control model.In the end of research, the multistage predictive control is tested through simulation to check the performance of the controller.

Penelitian ini tentang health risk assessment pada kegiatan produksi yang dilakukan di area Stasiun Pengumpul A (SP A) dan Stasiun Pengumpul Utama B (SPUB) PT X tahun 2014. Penilaian risiko kesehatan dilakukan dengan dengan menganalisis nilai potensi dari bahaya yang ada (P), Frekuensi pajanan (F), Durasi (D), Risiko Awal (RA), Upaya Perlindungan (UP) dan Risiko Risidual (RR) untuk mengetahui level risiko kesehatan yang ada pada setiap kegiatan kerja. metode penelitian ini adalah semikuantitatif dengan melakukan observasi dan pengumpulan data. Hasil dari penilaian risiko menunjukkan bahwa pada kegiatan di area SP A yaitu pengukuran volume tangki di tangki produksi 7 (benzene, toluene, xylene, hexane, pasta gasoline), di tangki 4 dan 6 (pasta gasoline), pengoprasian pompa transfer, compressor (bising) memiliki risiko kesehatan tinggi. Sedangakn di area SPU B pajanan kebisingan pada pompa transfer memiliki risiko medium. ......This Health Risk Assessment research that was held at Oil Station A (SP A) and Primary Oil Station B (SPU B) PT X Years 2014. Health risk assessment was conducted by analyzing the value potential of hazard (P), Frequency rate of exposure (F), Duration (D), level of risk (RA), Prevention means (UP) and Residual Risk (RR) from every job activity. This study is a semi quantitative method by observation and data collection. The result of this risk assessment indicated that in SP A activity, measuring the volume of tank in tank production 7 (benzene, toluene, xylene, hexane, gasoline pasta), in tank 4 and 6 (gasoline past), in transfer pump and compressor (noisy) has a high risk. While in the area SPU B noise exposure in transfer pump that have a medium risk.
