"ABSTRAK
Bayi prematur sangat sensitif terhadap stimulus nyeri. Tujuan dari studi kasus ini adalah menganalisis efektivitas penerapan asuhan keperawatan pada bayi dengan gangguan pemenuhan kebutuhan kenyamanan melalui intervensi stimulasi multisensori menggunakan model Comfort Kolcaba di ruang perinatologi. Lima kasus terpilih menunjukkan terjadinya gangguan kenyamanan dan nyeri akut pada bayi prematur akibat terpapar dengan prosedur invasif. Intervensi dilakukan dengan mengkaji skor nyeri, pemberian stimulasi multisensori, serta perlibatan orang tua dalam intervensi melalui sentuhan dan kontak kulit ke kulit. Intervensi keperawatan berdasarkan evidence based nursing melalui tindakan stimulasi multisensori dilakukan untuk menurunkan skor nyeri. Hasil menunjukkan bahwa intervensi berbasis teori Comfort Kolcaba efektif untuk meningkatkan kenyamanan. Disarankan agar teori Comfort Kolcaba dapat diterapkan dalam pemberian asuhan keperawatan pada bayi prematur.

ABSTRACT
Premature infants are particularly sensitive to pain stimulus. The purpose of this case study is to analyze the effectiveness of nursing care in infants with impaired fulfillment of comfort through multisensory stimulation intervention in the perinatology room using Comfort Kolcaba model. Five cases selected show disruption of comfort and acute pain in preterm infants from exposure to invasive procedures. The intervention is done by assessing pain scores, multisensory stimulation, as well as the involvement of parents in the intervention through touch and skin-to-skin contact. Nursing interventions based on evidence based nursing through multisensory stimulation measures taken to decrease the pain score. The results show that the theory-based intervention Comfort Kolcaba effective to increase comfort. Application of Comfort Kolcaba theory can be applied in the provision of nursing care in premature infants. ;;"

"Nyeri merupakan stimulus yang dapat merusak perkembangan otak bayi prematur. Developmental care merupakan salah satu strategi untuk mengurangi respon nyeri akibat prosedur invasif. Penelitian ini bertujuan untuk mengetahui pengaruh developmental care terhadap respons nyeri akut pada bayi prematur yang dilakukan prosedur invasif. Penelitian menggunakan rancangan quasi experimental non equivalent control group, before and after design, dengan jumlah sampel 42 bayi prematur terdiri dari 21 responden sebagai kelompok kontrol dan 21 sebagai kelompok intervensi yang dipilih dengan teknik sampling accidental sampling. Pada penelitian ini didapatkan hasil ada perbedaan selisih skor respons nyeri akut antara kelompok yang diberikan developmental care dan kelompok kontrol (p= 0,000; α= 0,05), developmental care menurunkan skor nyeri sebesar 2,05 poin. Developmental care perlu diterapkan dalam memberikan asuhan keperawatan pada bayi prematur. Penelitian lanjutan tentang pengaruh developmental care terhadap perkembangan bayi prematur pada dari aspek kognitif, bahasa, dan motorik perlu dilakukan.

Pain is a stimulus that can damage the developing brains of premature babies. Developmental care is one strategy in reduce pain caused by invasive procedure. The objective of this research was to determine the effects of developmental care on acute pain response in premature babies who had been performed invasive procedure. This study used quasiexperiment research design non equivalen control group, before and after design, involving 42 sample consisted of 21 premature babies in both control and intervention groups, technical sampling used accidental sampling. This study founded that there was a significant difference in change score of acute pain response between on intervention and control groups (p= 0.000; α= 0.05), developmental care decreased 2,05 point of pain scores). Developmental care was recommended to be applied in nursing care for premature babies. Further research on effect developmental care on development of premature babies influence cognitive, languanges, and motor skills aspects need to be designed."

"Latar belakang: Asuhan nutrisi optimal untuk bayi prematur bertujuan mengoptimalkan tumbuh kembang dan peningkatkan kualitas hidup bayi prematur. Asuhan nutrisi berupa penilaian masalah nutrisi, menentukan kebutuhan nutrisi, dan pemantauan pertumbuhan membutuhkan kurva pertumbuhan. Saat ini tidak ada kurva pertumbuhan standar untuk bayi prematur. Tujuan: Membandingkan laju pertumbuhan setiap minggu hingga maksimal usia 37 minggu pada bayi prematur yang diberikan nutrisi berdasarkan berat badan ideal menurut kurva Intergrowth-21st dan kurva Fenton, serta mengetahui hubungan antara sepsis, respiratory distress syndrome, riwayat hambatan laju pertumbuhan intrauterin dengan laju pertumbuhan berat badan . Metode: Pnelitian ini adalah uji klinis ajak tersamar ganda pada 93 bayi prematur usia gestasi 33-36 minggu. Perhitungan kebutuhan nutrisi dan pemantauan asupan dilakukan setiap hari serta pemantauan antropometri berupa berat badan, panjang badan, dan lingkar kepala dilakukan minimal satu kali dalam seminggu. Analisis uji-t dilakukan pada sebaran data normal Hasil: Pada bayi prematur yang diberikan nutrisi berdasarkan berat badan ideal menurut kurva Intergrowth-21st dan kurva Fenton tidak berbeda bermakna dalam hal laju kenaikan berat badan, panjang badan, dan lingkar kepala setiap minggu hingga maksimal usia 37 minggu. Pemberian nutrisi berdasarkan berat badan ideal menurut kurva Intergrowth-21st dibandingkan menggunakan kurva Fenton memberikan hasil lebih banyak bayi prematur yang mencapai berat badan lahir dalam waktu kurang dari 7 hari (33% vs 19%) dan lebih banyak bayi prematur yang tumbuh liniear sesuai dengan trajektori pertumbuhan yang harus dicapai, dengan indeks berat badan 39% vs 20%, panjang badan 55% vs 36%, dan lingkar kepala 59% vs 48%. Riwayat hambatan pertumbuhan intrauterin memiliki hubungan bermakna dengan laju pertumbuhan berat badan pada pasien yang mendapatkan nutrisi berdasarkan berat badan ideal menurut kurva Intergrowth-21st(p 0,003, IK 95% 2,326-10,239) Simpulan: Penggunaan kurva Intergrowth-21st lebih baik dibandingkan kurva Fenton dalam menentukan kebutuhan kalori dan pemantauan pertumbuhan bayi prematur, sehingga mencageha terjadinya hambatan laju pertumbuhan.

Background: Nutrition implementation in premature babies goals are to optimize growth development and increase life quality. Nutritional implementation program including nutritional assessment, determine nutritional necessity or nutritional therapy, and monitor of premature babies' growth with standardized growth curve. Until recently, there is no standardize growth curve for premature baby. Objective: To compare growth velocity every week until maximum 37 weeks corrected age in premature infants who has been given nutritional implementation based on their ideal weight using Intergrowth-21 curve or Fenton curve. And, to correlate growth velocity and factors such as sepsis, respiratory distress syndrome, and intrauterine growth retardation. Methods: This is randomized double blinded clinical trial on 93 premature babies with gestational age range within 33-36 weeks. Daily nutritional intake and monitoring intake toleration were done. Anthropometric monitoring data were taken minimum every week. T-test analysis was used on normal data distribution. Results: There are no significant difference in weekly growth velocity (body weight, length, head circumference) in premature babies until 37 weeks corrected age who is given nutritional intake based on ideal body weight of Intergrowth-21st curve compare to Fenton curve. Intergrowth-21st curve shows more premature babies achieving babies' birth weight in less than 7 days (33% vs 19%) and more premature babies have linier growth according to their trajectory growth goals, with body weight index 39% vs 20%, body length 55% vs 36%, and head circumference 59% vs 48%. Intra uterine growth retardation and growth velocity has significant correlation in patients received nutrition implementation based on targeted ideal body weight according to  Intergrowth-21st curve (p 0,003, IK 95% 2,326-10,239). Conclusion: Intergrowth 21st curve has better result in determining nutritional necessity and monitoring growth of premature babies compare to Fenton curve in order to prevent slower growth velocity in premature babies."

"Penelitian ini dilakukan untuk mengetahui pengaruh perawatan metode kanguru (PMK) terhadap fungsi fisiologis pada bayi prematur yang terpasang ventilasi mekanik. Penelitian ini menggunakan desain randomized control trial (RCT) yang melibatkan 50 responden bayi prematur yang terpasang ventilasi mekanik dan dibagi menjadi dua kelompok, yaitu kelompok kontrol (perawatan rutin rumah sakit) 25 responden dan kelompok intervensi (bayi dilakukan PMK) 25 responden sesuai kriteria inklusi. PMK dilakukan tiga hari berturut-turut selama satu jam/hari. Hasil analisis uji perbedaan menujukkan adanya perbedaan saturasi oksigen secara bermakna pada saat dilakukan PMK (p=0.000; α=0,05) dan frekuensi pernapasan intra (p= 0,007), dan post (p= 0,027) pada kelompok kontrol dan kelompok intervensi. Berdasarkan hasil tersebut, adanya pengaruh perawatan metode kanguru terhadap fungsi fisiologis pada bayi prematur yang terpasang ventilasi mekanik. Intervensi ini diharapkan dapat dilanjutkan sebagai prosedur di ruang NICU karena sudah terbukti dapat mempercepat weaning ventilator.

his study was conducted to determine the effect of the kangaroo method of care (PMK) on the physiological function of mechanically ventilated premature infants. This study used a randomized control trial (RCT) design involving 50 premature infants who were mechanically ventilated and divided into two groups, namely the control group (routine hospital care) 25 respondents and the intervention group (babies undergoing PMK) 25 respondents according to the inclusion criteria. . PMK is carried out three days in a row for one hour/day. The results of the analysis of the difference test showed that there was a significant difference in oxygen saturation during PMK (p=0.000; α=0.05) and intra-respiratory frequency (p= 0.007), and post (p= 0.027) in the control group and the intervention group. Based on these results, there is an effect of kangaroo treatment on physiological function in mechanically ventilated premature infants. It is hoped that this intervention can be continued as a procedure in the NICU room because it has been proven to speed up ventilator weaning"

"Janin dan bayi prematur menghabiskan sebagian besar waktunya untuk tidur di dalam dan di luar rahim. Tidur dianggap sebagai aktivitas penting pada periode neonatal, serupa dengan pernapasan dan nutrisi. Kebisingan merupakan salah satu penyebab gangguan pola tidur pada bayi prematur di ruang perawatan intensif neonatal dan berdampak pada gangguan tumbuh kembang. Studi ini memberikan gambaran penerapan Model Adaptasi Callista Roy dalam asuhan keperawatan pada lima kasus bayi prematur dengan risiko gangguan pola tidur. Desain yang digunakan adalah studi kasus yang didapatkan dari lima kasus terpilih. Teori Adaptasi Callista Roy mampu memfasilitasi perawat untuk menggali masalah keperawatan pada bayi prematur secara komprehensif. Pemberian edukasi Bundles Alert to Alarm kepada seluruh perawat neonatal menggunakan media pitstop dapat menurunkan angka kebisingan diruang perawatan neonatal. Penerapan teori Adaptasi Callista Roy dan penerapan Bundles allert to alarm dapat direkomendasikan untuk diterapkan dalam asuhan keperawatan yaitu dapat memfasilitasi istirahat tidur bayi prematur yang dirawat di ruang perawatan neonatal.

Fetuses and premature babies spend most of their time sleeping inside and outside the womb. Sleep is considered an important activity in the neonatal period, similar to breathing and nutrition. Noise is one of the causes of disturbed sleep patterns in premature babies in the neonatal intensive care room and has an impact on growth and development disorders. This study provides an overview of the application of the Callista Roy Adaptation Model in nursing care in five cases of premature babies at risk of sleep pattern disorders. The design used is a case study obtained from five selected cases. Callista Roy's Adaptation Theory is able to facilitate nurses to comprehensively explore nursing problems in premature babies. Providing Bundles Alert to Alarm education to all neonatal nurses using pitstop media can reduce noise levels in the neonatal care room. The application of Callista Roy's Adaptation theory and the application of Bundles alert to alarm can be recommended for application in nursing care, namely it can facilitate sleep for premature babies who are cared for in the neonatal care room."

Rekomendasi penelitian developmental care perlu diterapkan dalam memberikan asuhan keperawatan pada bayi prematur dan diharapkan ada penelitian lanjutan tentang pengaruh developmental care terhadap perkembangan bayi prematur di kemudian hari baik dari aspek kognitif, bahasa maupun motorik.

"Perpisahan akibat perawatan yang dilakukan pada bayi prematur berpengaruh terhadap proses bonding attachment. Penelitian kualitatif dengan pendekatan fenomenologi deskriptif pada tujuh partisipan ini menggunakan teknik Purpossive Sampling yang bertujuan untuk mengeksplorasi secara mendalam pengalaman orang tua menerima perilaku caring perawat dalam memfasilitasi bonding attachment bayi prematur di Ruang NICU. Pengumpulan data dilakukan dengan wawancara mendalam dan dianalisis dengan metode Colaizzi.
Hasil analisis data didapatkan tujuh tema yaitu proses peningkatan pengetahuan, mampu melakukan perawatan terhadap bayinya, respon ibu terhadap tindakan perawatan yang diberikan, termotivasi dalam melakukan perawatan bayi prematur, terpenuhinya kebutuhan bayi selama dilakukan perawatan, keterlibatan dalam asuhan keperawatan dan kepuasan terhadap perawatan. Penelitian ini diharapkan dapat meningkatkan asuhan keperawatan dan bonding attachment pada bayi prematur.

Separation between parents and premature babies in intensive care unit affects in bonding attachment process. This study was a qualitative research design with descriptive phenomenology approach took 7 participants used Purpossive Sampling Technique. This study aims to explored mother's experience received nursing caring in facilitated bonding attachment of premature babies. Data were collected with indepht interview and analized with Colaizzi method.
The results of data analysis got seven themes: knowledge improving process; capable to cared their babies; mother's respons with nursing care; motivated to cared their premature babies, the premature babies needed was fullfiled well; participated in nursing care; and nursing care satisfaction. The result is expected to be input in improving nursing care and bonding attachment in premature babies.

"[ABSTRAK
Perawatan bayi prematur di unit perawatan intensifdapat menimbulkan stimulasi berlebihan bahkan stres fisik pada bayi akibat prosedur invasif yang rutin dilakukan.Studi fenomenologi ini bertujuan untuk mengetahui pengalaman perawat dalam mengambil keputusan etik untuk pelaksanaan tindakan invasif pada bayi prematur.Partisipan berjumlah 7 orang perawat di ruang perinatologi yang dipilih secara purposive sampling. Hasil penelitian mengidentifikasi empat tema: (1)masalah etik yang ditemui dalam pelaksanaan prosedur invasif, 2) kebutuhan perawat untuk mampu mengambil keputusan etik, 3) hambatan dalam mengambil keputusan, dan 4) perilaku etis saat pelaksanaan prosedur invasif. Penelitian selanjutnya diharapkan dapat mengevaluasi secara kuantitatif kemampuan pengambilan keputusan etik perawat dalam pelaksanaan tindakan invasif selama proses asuhan keperawatan pada bayi prematur.

"Pengetahuan dan keterampilan perawat dalam melakukan posisi prone pada bayi prematur memiliki peran penting karena apabila tidak dilakukan dengan tepat dapat berpengaruh terhadap pertumbuhan dan perkembangan bayi tersebut. Penelitian ini dilakukan untuk mengetahui efektivitas video posisi prone berbasis telefon pintar terhadap pengetahuan, sikap dan keterampilan perawat dalam melakukan posisi prone dengan tepat. Penelitian ini menggunakan desain randomized control trial dengan melibatkan 58 perawat NICU-Perinatologi pada salah satu Rumah Sakit Rujukan di Jakarta yang diambil menggunakan teknik purposive sampling. Hasil penelitian menunjukkan intervensi video posisi prone berbasis telefon pintar terbukti efektif meningkatkan pengetahuan, sikap dan keterampilan perawat dalam melakukan posisi prone pada bayi prematur (p<0,001, α<0,05). Penelitian memberikan saran supaya hasil penelitian ini dapat menjadi rujukan optimalisasi pengetahuan, sikap, dan keterampilan perawat dengan pemanfaatan teknologi informasi yaitu video tujuh langkah melakukan posisi prone dengan tepat berbasis telefon pintar yang dirancang dengan menarik dan memungkinkan akses secara fleksibel.

Nurses' knowledge and skills in prone positions on premature babies have an important role because if it is not done properly, it can affect the growth and development of the baby. This study was conducted to determine the effectiveness of smartphone-based prone position videos on nurses' knowledge, attitudes, and skills in performing the prone position correctly. This study used a randomized control trial design involving 58 NICU-Perinatology nurses at a Referral Hospital in Jakarta taken using a purposive sampling technique. The results showed that the smartphone-based prone position video intervention improved nurses' knowledge, attitudes, and skills in performing positions. prone in premature infants (p<0.001, <0.05). The study provides suggestions so that the results of this study can be a reference for optimizing the knowledge, attitudes, and skills of nurses by utilizing information technology, namely a seven-step video of doing the correct prone position based on a designed smartphone attractively and allows flexible access."
