Abstrak :
Penelitian ini membahas tentang faktor-faktor yang berhubungan dengan pencatatan pelaporan insiden keselamatan pasien oleh dokter dan Komite Mutu Keselamatan Pasien Manajemen Risiko (KMKPMR) di RS Permata Depok tahun 2017. Tujuan dari penelitian ini adalah untuk mengetahui faktor pendukung dan penghambat dalam pencatatan pelaporan insiden keselamatan pasien di RS Permata Depok tahun 2017. Jenis penelitian ini adalah kualitatif menggunakan case study dengan metode wawancara mendalam, telaah data sekunder dan telaah dokumen. Hasil penelitian menunjukkan bahwa pengalaman kerja, pengetahuan tentang pencatatan pelaporan insiden keselamatan pasien, kerja rangkap, komunikasi, pengambilan keputusan dan penghargaan berhubungan dengan faktor pencatatan insiden keselamatan pasien oleh dokter, sedangkan pengalaman kerja, komunikasi dan penghargaan berhubungan dengan faktor pelaporan insiden keselamatan pasien oleh KMKPMR. ......The aim of this research is to identify the supporting and inhibiting factors relating to the recording and reporting the incidence of patient's safety at Permata Depok Hospital in 2017. Type of research is qualitative using case studies with in-depth interview, secondary data review and document review. The results of this study are work experience, knowledge, work load, communication, decision making and reward related to the factors of recording the incidence of patient's safety by the doctors, while work experience, communication and reward are related the factors of reporting the incidence of patient's safety by The Hospital Committee of Patient Safety.

This dissertation is a study in diachronic perspective on the impact of recording technologies, more specifically cassette and video compact disc (VCD), on Indonesian local cultures and societies. It examines how modern reproduced sound, which is constantly proliferating and multiplying up to today through various (social) media, but initially facilitated by recording media technology through the agency of regional recording industries, has influenced the contours of Indonesian local cultures. The study relates Indonesia?s first encounter with recording technology, examines the nature and cultural ramifications of the expansion of recording technology among Indonesia?s ethnic groups, and looks at its engagement with other media. As a case study, the West Sumatran recording industry is explored, along with the commercial cassettes and VCDs it has produced.

Skripsi ini membahas mengenai analisis penerapan norma penghitungan penghasilan neto berdasarkan asas sistem pemungutan perpajakan equlity dan ease of administration atas kegiatan usaha Artis dan faktor yang dijadikan acuan dalam memilih metode pencatatan dan pembukuan Artis penyanyi dan perancang busana. Penelitian ini merupakan penelitian kuantitatif dengan desain deskriptif. Hasil temuan dari penelitian ini adalah asas keadilan dan kesederhanaan terhadap wajib pajak orang pribadi kegiatan usaha artis baik penyanyi maupun perancang busana sudah terpenuhi dimana setiap wajib pajak diberikan keistimewaan untuk memilih menyelenggarakan pembukuan atau norma penghitungan penghasilan neto sebagai kewajiban untuk memenuhi perpajakannya, serta faktor-faktor memilih salah satu metode penghitungan pencatatan atau pembukuan. Temuan penelitian juga menyarankan agar terus dilakukannya penyesuaian atas ketentuan peraturan mengenai Norma Penghitungan Penghasilan Neto yang diatur dalam Keputusan Direktorat Jenderal Pajak Nomor KEP-536/PJ./2000, sehingga dapat mendekati dengan keadaan sebenarnya. ...... This thesis discusses the analysis of the application of the norm calculation is based on the principle of net income tax collection system equlity and ease of administration on business activities Artist and factors referenced in choosing the method of recording and bookkeeping Artists singer and designer. This research is quantitative descriptive method. The findings of this study are the principles of fairness and simplicity to the individual taxpayer singer and designer has been fulfilled which every taxpayer is given the privilege to choose to keep books or norm calculation of net income to meet their tax obligation, as well as factors choose one method of calculating the recording or bookkeeping. The study finding also suggest that continued to do an adjustment to the provisions of the deemed profit stipulated in the Directorate General of Taxation Number KEP-536/PJ./2000, so as to approach the real situation.

Laporan magang ini bertujuan untuk membahas mengenai evaluasi atas pencatatan Piutang Usaha milik PT X. Fokus pembahasan terletak pada aktivitas magang terkait Piutang Usaha dan proses pencatatan akun Piutang Usaha PT X yang bergerak di bidang properti. Berdasarkan hasil evaluasi atas akun piutang usaha, pencatatan yang telah dibuat PT X atas akun piutang usaha telah sesuai dengan teori dan standar yang berlaku. Selain itu, laporan magang ini bertujuan untuk merefleksikan diri atas pengalaman Penulis selama melakukan kegiatan magang dan mengevaluasi diri untuk mempersiapkan diri di dunia kerja. Melalui refleksi diri ini, Penulis memahami bahwa komunikasi dalam suatu pekerjaan sangatlah penting agar pekerjaan bisa menjadi lebih efisien. The focus of the discussion lies in the internship activities and recording process related to the accounts receivable by PT X, which is a property company. Based on the results of the evaluation on accounts payable, the presentation that has been made by PT X accounts receivable is in accordance with the prevailing theory and standards. In addition, this internship report aims to reflect on the author's experience during internships and evaluate the author to prepare for the working world. Through this self-reflection, the author understands that communication in a workplace is very important so that work can be more efficient. The author also realizes that many things related to the author's self-potential still have to be developed, such as time management and accuracy which will be useful for the author in facing the working world.

Abstrak :
Koordinasi berbagai aspek dalam sistem informasi psikotropika membutuhkan dukungan sistem informasi manajemen psikotropika yang dapat menghasilkan informasi yang lengkap, akurat dan tepat waktu. Sistem informasi ini harus mampu menyediakan laporan-laporan baik rutin maupun khusus, yang terkait dengan impor produksi dan distribusi psikotropika. Studi evaluasi sistem pencatatan dan pelaporan psikotropika di Ditwas Napza Badan Pengawas Obat dan Makanan guna mengevaluasi laporan psikotropika untuk mendapatkan suatu laporan yang tepat waktu dan akurat. Pendekatan kajian dengan evaluasi sistem laporan dimana data dikumpulkan melalui, telaah dokumen, pengamatan kegiatan dan fasilitas terkait, dan wawancara dengan pengelola laporan psikotropika. Hasil penelitian menunjukkan bahwa laporan psikotropika belum tepat waktu dan akurat. Kurang tepat dan akuratnya laporan psikotropika disebabkan oleh mekanisme penyampaian laporan memerlukan waktu panjang sesuai dengan birokrasi yang ada dan dipengaruhi juga oleh fasilitas dan dana yang sangat terbatas. Untuk mendapatkan laporan yang tepat waktu dan akurat perlu dilakukan intervensi, faktor strategik yang perlu diintervensi adalah membuat mekanisme pengiriman baru dengan menggunakan fasilitas faximile langsung kepada Kepala Tim Pengelola Laporan dan penyediaan dana oleh Badan POM, membuat peraturan baru yang mengatur sistem pelaporan psikotropika.

The Evaluation on the Psychotropic Reporting and Recording System : The Study in National Agency of Drug and Food Control, 2002The coordination in the aspects of the psychotropic information system needs the supporting of the psychotropic management information system that is produced completely, accurately, and the up to date information. The information system must be able to supply routine and non-routine reporting, which is related with import, production, and distribution of psychotropic. The evaluation study on the psychotropic reporting and recording system in Directorate of Narcotics, Psychotropic and Addictive Substance Control, National Agency of Drug and Food Control, were aimed to evaluate of psychotropic report to get the up to date and accurate report. The assessment approach used reporting system evaluation, which data were collected to proceed i.e. to analyze the documents, to observer the related activities and facilities, and interviews to the psychotropic report officers. The results of research study have revealed that the psychotropic report is not be up to date and accurate yet. They are caused by the report-delivery mechanism inquired long time according the bureaucratic procedures and the report facilities and financial recourses are very limited. In order to acquire the up to date and accurate report has needed the intervention. The strategic factors are needed the intervention, which developed the new report-delivery mechanism used facsimile directly to Head of Reporting Handler Team and to supply budget by National Agency of Drug and Food Control, to establish the new regulation in the psychotropic reporting system.
