"This book is designed with two overriding objectives, to help readers understand the impact of vision impairment in people living daily with diabetes rather than considering diabetic retinopathy solely as a medical problem, and to explore what we know and don't know about the ways diabetes affect the eye. "ABSTRAK
Teknologi pelepasan obat jangka panjang diinginkan untuk mengobati penyakit mata, termasuk penyakit retinopati diabetik. Obat ini umumnya disampaikan melalui intraokular menggunakan injeksi intravitreal karena ketidakefektifan dan hambatan dari metode pemberian obat lainnya. Namun, frekuensi injeksi yang tinggi dapat menyebabkan beberapa kerugian, seperti ketidaknyamanan pasien dan beberapa komplikasi. Oleh karena itu, diperlukan sistem yang sempurna untuk mendapatkan pelepasan jangka panjang dan terkontrol. Untuk membentuk sistem seperti itu, Poly Lactic Acid PLA nanopartikel polimer digunakan untuk membungkus obat dexamethasone. Persiapan nanopartikel menggunakan metode penguapan pelarut-emulsifikasi. Selain itu, untuk meningkatkan stabilitas dan interaksi dengan asam hyaluronic dan vitreous, surfaktan kationik DDAB didodecyldimethylammonium bromide ditambahkan untuk modifikasi permukaan. Variasi surfaktan yang digunakan adalah DDAB 0,5 dan PVA-DDAB-0,5 . Uji rilis dilakukan selama 24 hari, dengan interval sampling 48 jam T = 35 C . Data menunjukkan bahwa stabilitas DDAB di permukaan dapat mencapai lebih lama dalam PLA-PVA-DDAB 0,5 dibandingkan dengan PLA-DDAB 0,5 . Setelah 24 hari, pelepasan kumulatif PLA-PVA-DDAB 0,5 mencapai 67,53 sementara PLA-DDAB 0,5 mencapai 89,2.

ABSTRACT
Long term drug release technology is desirable to treat ocular diseases, including diabetic retinopathy disease. The drug is commonly delivered via intraocular using intravitreal injecton due to ineffectiveness and obstacles of other drug delivery methods. However, the high frequency of injection can lead to several disadvantages, such as the patients rsquo inconvenience and several complications. Therefore, a perfect system to obtain long term and controlled release is required. To establish a such system, Poly Lactic Acid PLA polymer nanoparticles is used to encapsulate dexamethasone drug. The preparation of the nanoparticles uses emulsification solvent evaporation method. Moreover, to increase stability and interaction with the hyaluronic acid and vitreous, cationic surfactant DDAB didodecyldimethylammonium bromide is added for surface modification. Variations of surfactant used are DDAB 0.5 and PVA DDAB 0.5 . Release test was conducted for 24 days, with sampling interval of 48 hours T 35 C . The data show that the DDAB stability in the surface can reach longer in PLA PVA DDAB 0.5 comparing to PLA DDAB 0.5 . After 24 days, PLA PVA DDAB 0.5 cumulated release reached up to 67.53 while PLA DDAB 0.5 reached up to 89.2. "

"Bayi prematur adalah bayi yang lahir dengan usia kehamilan kurang dari 37 minggu yang memiliki sistem saraf dan organ-organ yang belum sempurna sehingga lebih beresiko mengalami berbagai masalah kesehatan. Salah satu masalah kesehatan yang dapat terjadi adalah pada organ mata yang merupakan organ penting dalam perkembangan bayi. Retinopathy of Prematurity (ROP) merupakan salah satu penyakit mata yang terjadi pada bayi prematur yang disebabkan oleh pembentukan pembuluh darah retina yang tidak normal. Proses diagnosis yang dilakukan oleh dokter mata belum bisa mengatasi kenaikan jumlah kasus ROP, sehingga disini penulis menggunakan pendekatan deep learning untuk melakukan klasifikasi tingkat keparahan ROP pada citra fundus retina. Metode deep learning yang digunakan adalah Convolutional Neural Network (CNN) dengan arsitektur ResNet50. Data yang digunakan pada penelitian ini merupakan data sekunder yang diperoleh dari online database Kaggle berupa 90 data citra fundus retina yang terbagi atas 38 citra bukan penderita ROP, 19 citra penderita ROP Stage 1, 22 citra penderita ROP Stage 2, dan 11 citra penderita ROP Stage 3. Pada tahap persiapan data, dilakukan perbaikan kontras citra menggunakan Contrast Limited Adaptive Histogram (CLAHE) dan image masking. Kemudian dilakukan resize citra menjadi ukuran 224×224. Data kemudian diaugmentasi menggunakan teknik flip horizontal dan rotation agar data menjadi lebih banyak yang kemudian dibagi menjadi 80% data training dan 20% data testing. Dari 80% data training, diambil 20% untuk data validation. Training model dilakukan menggunakan model dengan arsitektur ResNet50 dengan hyerparameter model yaitu batch size 64, learning rate 0.001, dan epoch sebanyak 30, fungsi optimasi Adam (Adaptive moment estimation), dan fungsi loss categorical cross entropy. Proses modelling dilakukan sebanyak 5 kali percobaan dan berhasil memperoleh nilai rata-rata kinerja training model sebesar 99.714% dan 92.85% pada akurasi training dan akurasi validation-nya, selain itu diperoleh nilai 0.01864 dan 0.18434 pada loss training dan loss validation. Sedangkan rata-rata kinerja testing model berhasil memperoleh akurasi testing sebesar 97.352%, testing loss sebesar 0.0986374, dan AUROC sebesar 0.0955. Selain melakukan evaluasi kinerja, peneliti juga akan menggunakan GradCAM untuk menampilkan visualisasi ciri-ciri yang dianggap penting untuk nantinya membantu dokter dalam mengevaluasi ROP.

Premature infants are babies born with a gestational age of less than 37 weeks, and they have underdeveloped nervous systems and organs, making them more susceptible to various health issues. One of the health problems that can occur involves the eye, which plays a crucial role in the baby's development. Retinopathy of Prematurity (ROP) is one of the eye diseases that affects premature infants and is caused by abnormal blood vessel formation in the retina. The current diagnostic processes performed by ophthalmologists have not been effective in addressing the increase in ROP cases. Therefore, in this study, the author employs a deep learning approach to classify the severity of ROP in retinal fundus images. The deep learning method utilized is the Convolutional Neural Network (CNN) with the ResNet50 architecture. The research data consists of 90 retinal fundus images obtained from the online database Kaggle, comprising 38 images of non-ROP cases, 19 images of ROP Stage 1, 22 images of ROP Stage 2, and 11 images of ROP Stage 3. In the data preparation phase, the image contrast is enhanced using Contrast Limited Adaptive Histogram (CLAHE) and image masking techniques. Subsequently, the images are resized to 224×224 dimensions. Data augmentation is performed using horizontal flip and rotation techniques to increase the dataset, which is then split into 80% training data and 20% testing data. From the 80% training data, 20% is further allocated for validation data. The model is trained using the ResNet50 architecture with hyperparameters set to batch size 64, learning rate 0.001, and 30 epochs. The optimization function used is Adam (Adaptive Moment Estimation), and the loss function is categorical cross-entropy. The modeling process is repeated five times, and the average performance of the training model is achieved at 99.714% for training accuracy and 92.85% for validation accuracy, with training and validation losses of 0.01864 and 0.18434, respectively. As for the average performance of the testing model, the testing accuracy is 97.352%, the testing loss is 0.0986374, and the AUROC (Area Under the Receiver Operating Characteristic) is 0.0955. In addition to evaluating the model's performance, the researcher also employs GradCAM to visualize important features, which can assist doctors in evaluating ROP cases.

"Retinopati diabetik merupakan komplikasi mikrovaskular akibat hiperglikemi pada diabetes melitus, komplikasi tersebut memberikan dampak bagi pasien berupa penurunan fungsi penglihatan sehingga dapat mempengaruhi kualitas hidup. Tujuan dari penelitian ini adalah untuk mengidentifikasi faktor-faktor yang berhubungan dengan kualitas hidup pasien retiopati diabetik.Metode penelitian ini adalah potong lintang/cross sectional dengan pendekaran observasi analitik.Jumlah responden sebanyak 160 pasien. Hasil penelitian menunjukan bahwa faktor – faktor yang tidak berhubungan dengan kualitas hidup adalah jenis kelamin (p=0,617) dan kepuasan pengobatan (P=0,106). Faktor-faktor yang berhubungan dengan kualitas hidup pasien retinopati diabetik adalah usia (p=0,002), lama menderita DM (p=0,005), derajat keparahan retinopati diabetik (p=0,0001), stress (p=0,045), dukungan keluarga (p=0,024), status fungsional (p=0,046). Pasien retinopati diabetik yang memiliki usia kurang dari 50 tahun, lama menderita DM lebih dari 10 tahun, derajat DR NPDR) dan dukungan keluarga baik berpeluang mengalami kualitas hidup baik sebesar 40%. Hasil analisis multivariat menunjukan faktor yang paling dominan berhubungan dengan kualitas hidup pasien retinopati diabetik adalah dukungan keluarga dengan nilai OR= 4,172(CI 95%= 1,860; 16,414). Dengan penelitian ini di harapkan dapat menjadi acuan bagi perawat dalam mengembangkan pola asuhan keperawatan pada pasien gangguan penglihatan terkait kualitas hidup pasien.

Diabetic retinopathy is a microvascular complication due to hyperglycemia in diabetes mellitus, this complication affects the patient in the form of decreased visual function so that it can affect the quality of life. The purpose of this study was to identify factors related to the quality of life of diabetic retiopathy patients. The method of this study is cross sectional / cross sectional with an analytic observation approach. The number of respondents was 160 patients. The results showed that factors not related to quality of life were gender (p = 0.617) and treatment satisfaction (P = 0.106). Factors related to the quality of life of diabetic retinopathy patients were age (p = 0.002), duration of DM (p = 0.005), severity of diabetic retinopathy (p = 0.0001), stress (p = 0.045), family support ( p = 0.024), functional status (p
= 0.046). Diabetic retinopathy patients who have a age of less than 50 years, long suffering from diabetes more than 10 years, level of severity is DR (NPDR) and good family support have the opportunity to experience a good quality of life of 40%. The results of multivariate analysis showed that the most dominant factor related to the quality of life of patients with diabetic retinopathy was family support with an OR value of 4.172 (95% CI = 1.860; 16.414). With this research, it is hoped that it can become a reference for nurses in developing patterns of nursing care for vision impaired patients regarding the quality of life of patients

"Diabetes melitus (DM) merupakan ancaman serius bagi pembangunan kesehatan dan pertumbuhan ekonomi nasional serta merupakan penyebab penting timbulnya kecacatan dan kematian. Dari semua kasus DM, DM tipe 2 mencakup lebih dari 90% dari semua pasien diabetes. Nefropati diabetik dan retinopati diabetik merupakan komplikasi mikroangiopati pada DM tipe 2 yang paling ditakuti dan keduanya sering ditemukan bersamaan. Perkembangan lanjut dari keduanya menyebabkan gagal ginjal tahap akhir dan kebutaan. Tujuan dari penelitian ini adalah untuk mengetahui kadar albumin urin dalam membedakan retinopati diabetik dan non retinopati diabetik.
Penelitian potong lintang ini terdiri dari 100 subyek yang terbagi atas kelompok retinopati diabetik 50 orang dan non retinopati diabetik 50 orang dari populasi DM tipe 2. Penderita didiagnosis DM tipe 2 oleh dokter Divisi Metabolik Endokrin Departemen Ilmu Penyakit Dalam Rumah Sakit Ciptomangunkusumo. Untuk retinopati diabetik dan non retinopati diabetik, diagnosis dilakukan dengan foto fundus pada pupil yang didilatasi oleh dokter Divisi Retina Departemen Ilmu Penyakit Mata Rumah Sakit Ciptomangunkusumo. Pada kedua kelompok dicatat data karakteristik subyek dan dilakukan pemeriksaan kadar albumin urin.
Kadar albumin urin pada kelompok retinopati diabetik lebih tinggi secara bermakna dibandingkan pada kelompok non retinopati diabetik (303,41±11,14 mg/g kreatinin vs 28,14±4,90 mg/g kreatinin, p <0,001). Nilai cut-off kadar albumin urin untuk membedakan retinopati diabetik dan non retinopati diabetik adalah 118 mg/g kreatinin dengan sensitivitas 72%, spesifisitas 78%, nilai duga positif 77%, nilai duga negatif 74%, rasio kemungkinan positif 3,27 dan rasio kemungkinan negatif 0,36.
Kami menyimpulkan pemeriksaan kadar albumin urin dapat dipakai untuk membedakan retinopati diabetik dan non retinopati diabetik.

Diabetes mellitus (DM) is a worldwide public health concern as they impose enormous medical, economic and social costs on both patient and the health care system. Together they contribute to serious morbidity and mortality. Type 2 DM affects more than 90% of all DM cases. Diabetic nephropathy and diabetic retinopathy are the two most dreaded complications of diabetes and frequently found together. Progression of both is the leading cause of end-stage renal disease and blindness. The aim of this study is to investigate albumin urine level in distinguishing diabetic retinopathy and non-diabetic retinopathy.
This cross-sectional study consisted of 100 respondents, in which 50 of them were categorized as diabetic retinopathy and 50 as non-diabetic retinopathy. The patients were diagnosed with type 2 DM by a doctor from Endocrinology Metabolic Division of Internal Medicine Department at Ciptomangunkusumo Hospital. Meanwhile diabetic retinopathy and non-diabetic retinopathy were diagnosed by ophthalmologist from Retina Division of Eye Medicine Department at Ciptomangunkusumo Hospital. Baseline characteristics of both groups were recorded and the albumin urine level was measured.
The albumin urine level in diabetic retinopathy group was significantly higher than that in the non-diabetic retinopathy group (303,41±11,14 mg/g kreatinin vs 28,14±4,90 mg/g kreatinin, p <0,001). The albumin urine level cut-off value used to distinguish diabetic retinopathy and non-diabetic retinopathy was 118 mg/g creatinine with sensitivity of 72%, specificity of 78%, positive predictive value of 77%, , negative predictive value of 74%, positive likelihood ratio of 3,27, and negative likelihood ratio of 0,36.
We conclude that albumin urine level test can be utilized to distinguish diabetic retinopathy from non-diabetic retinopathy.

"Latar Belakang: Beberapa penelitian terakhir menunjukkan adanya hubungan antara diabetes melitus tipe 2 (DMT2) dengan kejadian hipotiroid subklinis (HSK). Penelitian lain menunjukkan bahwa pada DMT2 yang disertai HSK, angka kejadian retinopati ternyata lebih tinggi dibanding pada DMT2 yang tanpa disertai HSK. Pasien HSK sendiri diketahui mempunyai risiko tinggi terhadap kejadian dislipidemia. Bagaimana hubungan antara dislipidemia dengan retinopati pada pasien DMT2 dengan HSK, sampai saat ini masih belum diketahui.
Tujuan: Mengetahui proporsi HSK pada pasien DMT2, hubungan antara HSK dengan kontrol glukosa darah, HSK dengan dislipidemia, serta hubungan antara dislipidemia dengan kejadian retinopati pada pasien DMT2 dengan HSK.
Metode: Desain penelitian yang digunakan adalah potong lintang. Sampel adalah pasien dewasa yang sudah didiagnosis DMT2 minimal 1 tahun, yang berobat ke poliklinik rawat jalan Divisi Metabolik Endokrin RSCM yang memenuhi kriteria inklusi. Data-data yang dikumpulkan adalah kontrol glukosa (HbA1c), profil lipid (kolesterol total, LDL, HDL, trigliserida), TSHs, fT4 dan data retinopati. Data diambil dari rekam medis maupun pemeriksaan laboratorium.
Hasil: Proporsi penyakit HSK pada pasien DMT2 sebesar 7.2 % dan sebagian besar berusia di atas 60 tahun. Tidak didapatkan perbedaan proporsi antara lakilaki dan perempuan. Dari analisis didapatkan pasien DMT2 dengan kontrol gula darah yang buruk (HbA1c >7) memiliki risiko 3,664 kali lebih besar mengalami HSK dibanding dengan pasien DMT2 yang gula darahnya terkontrol baik (p:0,010). Pada pasien DMT2 dengan HSK yang disertai dislipidemia, risiko terkena retinopati 2,76 kali lebih besar dibanding pasien tanpa dislipidemia (p:0,014).
Simpulan: Terdapat hubungan bermakna antara HSK dengan kontrol gula darah (HbA1c) pada pasien DMT2. Terdapat hubungan antara HSK dan dislipidemia pada pasien DMT2. Terdapat hubungan antara dislipidemia dengan kejadian retinopati pada pasien DMT2 dengan HSK.

Background: Some recent studies suggest that there is a link between type 2 diabetes mellitus (T2DM) and the incidence of subclinical hypothyroid (SCH). Other studies have shown that if a T2DM is accompanied SCH, the incidence of retinopathy was higher than in the T2DM without SCH. SCH patients themselves are known to have a high risk of occurrence of dyslipidemia. The the relationship between the incidence of dyslipidemia and retinopathy in patients with T2DM with SCH, is still unknown.
Objective: To determine the proportion of SCH in patients with T2DM, the relationship between SCH and glycemic control (HbA1c), SCH with dyslipidemia, and dyslipidemia with the incidence of retinopathy in T2DM patients with SCH.
Methods: The study design used is cross sectional. Sample were adult patients who have been diagnosed with T2DM at least 1 year, who went to the outpatient ward of Metabolic Endocrine Division, Cipto Mangunkusumo Hospital. Collected data include glycemic control (HbA1c), lipid profile (total cholesterol, LDL, HDL, triglycerides), TSHs, FT4 and retinopathy data. Data were retrieved from medical records and laboratory tests.
Results: The proportion of SCH in patients with T2DM 7.2%, and mostly aged over 60 years. There were no differences in the proportion between men and women. From the analysis reveals the T2DM patients with poor blood sugar control (HbA1c >7) had 3.664 times greater risk of developing SCH compared with T2DM patients with well-controlled blood sugar (p:0.010). In patients with T2DM with SCH accompanied dyslipidemia, retinopathy risk 2.76 times greater than patients without dyslipidemia (p:0.014).
Conclusion: There is a significant relationship between the SCH and glycemic control in patients with T2DM, SCH and dyslipidemia and also between dyslipidemia and retinopathy in T2DM patients with HSK.

"ABSTRAK
Tujuan tesis ini adalah mengetahui pengaruh suplementasi sitikolin 1000 mg per hari selama 4 minggu terhadap hasil elektroretinografi pada pasien NPDR non-proliferative diabetic retinopathy . Desain penelitian ini adalah uji klinis acak terkontrol tersamar ganda. Tiga puluh delapan mata yang memenuhi kriteria inklusi dan eksklusi dirandomisasi untuk masuk ke dalam kelompok plasebo P-NPDR atau sitikolin 1000 mg S-NPDR . Pada akhir penelitian didapatkan 18 mata pada kelompok sitikolin dan 16 mata pada kelompok plasebo. Keluaran primer penelitian ini adalah nilai amplitudo P50 dan N95 PERG within group dan intergroup yang dinilai pada baseline dan 4 minggu paska pemberian intervensi. Analisis hasil didapatkan pada S-NPDR didapatkan perbaikan nilai rerata amplitudo N95 sebelum terapi, 4.85 1.9-10.3 V, dan setelah terapi, 5.7 1.9-17.1 V, P = 0.04 . Terdapat kecenderungan perbaikan amplitudo P50 yang lebih baik pada kelompok T-NPDR dan perbaikan amplitudo N95 yang lebih baik pada S-NPDR yang tidak bermakna secara statistik P = 0.45; P = 0.35.

ABSTRACT
The purpose of this study was to determine the effect of citicoline 1000 mg oral supplementation given for 4 weeks on electroretinography abnormalities in patients with NPDR non proliferative diabetic retinopathy . The study design was a double blind randomized controlled clinical trial. Thirty eight patients who matched the inclusion and exclusion criteria were randomized into two groups the plasebo P NPDR and citicoline C NPDR . In the end, there were 18 eyes in citicoline group and 16 eyes in plasebo group. The primary outcome was P50 and N95 amplitude in PERG within group and intergroup which were taken at the baseline and 4 weeks after treatment. Results at the end of treatment, the N95 amplitude in C NPDR showed improvement, 4.85 1.9 10.3 V, before treatment to 5.7 1.9 17.1 V, after treatment with P 0.04. In P NPDR showed positive trend in P50 amplitude while in C NPDR showed positive trend in N95 amplitude, but these values were not statistically significant P 0.45 P 0.35.

"Tekanan darah tinggi pada retina Hypertensive Retinopathy merupakan penyakit yang timbul akibat tingginya tekanan darah yang mengalir pada pembuluh darah retina, mengakibatkan penebalan dinding pembuluh darah, sehingga debit aliran darah pada retina berkurang. Komplikasi yang timbul dari penyakit ini beragam dan membahayakan, mulai dari oklusi pembuluh darah retina, kerusakan saraf mata, bahkan kebutaan. Skripsi ini membahas tentang pendeteksian tekanan darah tinggi pada retina, sehingga dapat digunakan sebagai media untuk membantu diagnosis dan pencegahan penyakit tekanan darah tinggi pada retina Hypertensive Retinopathy . Pendeteksian dilakukan dengan menganalisa gambar retina Fundus Image pasien dengan metode Principal Component Analysis PCA dan Backpropagation Neural Network BNN , sehingga outputnya berupa klasifikasi citra ke salah satu dari dua golongan; yaitu retina normal dan retina dengan tekanan darah tinggi. Dari hasil perancangan diperoleh tingkat akurasi pengujian dan pengujian neural network hingga 85,5 dan 63,6 .

Hypertensive Retinopathy is a disease caused by high blood pressure flowing in the retinal blood vessels, resulting in thickening of blood vessel walls and reduced blood flow in the retina. Complications arising from these diseases are diverse and dangerous, ranging from retinal vein occlusion, nerve eye damage, even blindness. This paper discusses the detection of high blood pressure in the retina, so it can be used as a medium to help diagnosis and prevention of Hypertensive Retinopathy disease. Detection is done by analyzing the patient 39 s retinal image Fundus Image with Principal Component Analysis PCA method and Backpropagation Neural Network BNN , so that the output is image classification to one of two classes namely the normal retina and retina with high blood pressure. The result shows that this proposed model have leaning and testing accuracy up to 85,5 and 63,6 ."
