"Penilaian Kinerja Puskesmas (PKP) adalah suatu upaya untuk melakukan penilaian hasil kerja/prestasi puskesmas yang dilakukan secara menyeluruh dari ketiga aspek yaitu aspek program, aspek manajemen dan aspek mutu/kualitas pelayanan.
Dalam tesis ini, penilaian kinerja puskesmas hanya dilakukan pada kinerja 6 program pelayanan upaya kesehatan wajib puskesmas (basic six) yaitu 1) Pelayanan KIA/KB dengan 7 indikator kinerja, 2) Pelayanan Pengobatan dengan 2 indikator kinerja, 3) Pelayanan Perbaikan Gizi dengan 7 Indikator kinerja, 4) Pelayanan P3M dengan 8 indikator kinerja, 5) Pelayanan Kesehatan Lingkungan dengan 3 indikator kinerja, 6) Pelayanan Promosi Kesehatan dengan 4 indikator kinerja. Total indikator kinerja semuanya ada 31 indikator yang didasarkan pada indikator SPM Bidang Kesehatan.
Metodologi yang digunakan dalam Tesis ini adalah metode System Depelovment LM: Cycle (SDLC) yang terdiri dari : Perencanaan dan analysis sistem, design system, implementasi system dan perawatan system Sedangkan manajemen database menggunakan konsep Data Base Manajemen System (DBMS), dimana untuk menilai apakah hasil pengembangan system berjalan dengau baik, maka dilakukan uji coba prototype di Laboratorium Komputer Fakultas Kesehatan Masyarakat Universitas Indonesia.
Input data menggunakan laporan SP3 puskesmas berikut suplemennya dan diproses dengan menggunakan aplikasi program SI-PKP yang secara otomatis dapat menghasilkan output berupa informasi klasifikasi kinerja puskesmas yang disajikan dalam bentuk tabel dan grafik. PKP pada Tesis ini hanya dilakukan terhadap 20 UPTD Puskesmas (50%) dari total 40 UPTD Puskesmas yang ada di Kabupaten Bogor dengan hasil penelitian menunjukkan bahwa sebanyak 7 UPTD Puskesmas (35%) masuk klasifikasi Baik (warna hijau), 6 UPTD Puskesmas (30%) masuk kategori cukup (warm kuning) dan 7 UPTD Puskesmas (35%) masuk klasifikasi kurang (warna merah).
Informasi yang dihasilkan tersebut kiranya dapat dijadikan dasar bagi manajemen untuk intervensi program atau reward dan punishment. Manajemen masih perlu juga melakukan pengembangan system ini, sehingga betul-betul dapat diaplikasikan dan memenuhi kebutuhan system secara keseluruhan.

Performance Assessment at Primary Health Care (PKP) is an effort for doing assessment of job result or achievement at Primary Health Care which has been done by totally from three aspects including program, management and service quality aspect.
In this thesis, performance assessment at Primary Health Care is only done by performance of 6 service programs of nmndatory health effort at Primary Health Care (basic six) such as I) KIA/KB service by 7 performance indicators, 2) Medical service by 2 performance indicators, 3) Nutrition improvement by 7 performance indicators, 4) P3M service by 2 performance indicators, 5) Environment safety service by 8 performance indicators, 6) Health promotion service by 4 performance indicators. All of performance indicators are 31 indicators which based on indicator of service standard at Health Department.
Methodologies which are used in this Thesis are System Development Life Cycle (SDLC) consisting: Planning and analysis system, design system, implementation system and care system While database management used a Data Base Management System (DBMS) concept for assessing development result system has been run better, so it has been done by prototype test at Computer Laboratory of Public Health in Indonesia University.
Data Input used SP3 reports at Primary Health Care and their supplements and then they were processed by using SI-PKP program application of program which can spend output in the form of performance classification infomation at Primary Health Care which were presented in the form of tables and graphics. PKP on this thesis was only done to 20 UPTD at Primary Health Care (50%) from total of 40 UPTD at Primary Health Care in Bogor district by study result indicated that amount of 7 UPTD at Primary Health Care (35%) were a good classification (green color), 6 UPTD at Primary Health Care (30%) were medium category (yellow color) and 7 UPTD at Primary Health Care (35%) were less classification (red color).
From information above presumably can be made based on management for program intervention or reward and punishment. Management still needs to do this system development, so it can be implemented seriously and fulfill a requirement system as a whole.

"Penelitian ini bersifat kuantitatif dengan desain studi cross sectional. Data yang dikumpulkan berupa data primer, diperoleh dengan wawancara terhadap ibu-ibu yang melahirkan pada tahun 2008/2009. Dari hasil penelitian didapatkan kesimpulan bahwa pendidikan, pendapatan, sikap, pengetahuan, dan dukungan suami berhubungan secara signiiilcan dengan perilaku ibu dalam pemilihan penolong persalinan, sedangkan umur, paritas, pekerjaan, riwayat ANC dan Jarak tidak berhubungan secara signifikan. Pendidikan merupakan faktor dominan setelah dikontrol oleh sikap, dukungan suami dan riwayat ANC, (P = 0,001,011 = 4,555). Puskesmas disarankan untuk menlngkatlcan kualitas layanan konseling kepada ibu hamil dan pcmberian pendidikan kesehatan mengenai pentingnya pertolongan persalinan oleh tenaga kesehatan.

This research is a quantitative design with a cross sectional study. Data collected as primary data, obtained with the interviews of the mothers who give birth in the year 2008/2009. From the research results obtained the conclusion that education, income, attitudes, knowledge, and support the husband is significantly associated with maternal behavior in the selection of auxiliary labor, while age,parity, employment, ANC history and distance does not significantly related. Education is a factor controlled by the dominant attitude, support her husband and ANC history, (P = 0.00 l , OR = 4.555). Publik health centre recommended to improve the quality of counseling services to pregnant women and providing health education about the importance of labor by the help of health workers."

"Kesadaran masyarakat akan pentingnya kesehatan terus berkembang seiring denganmeningkatnya tingkat pendidikan dan status kehidupan sosial. Untuk meningkatkanpelayanan kesehatan yang bermutu, nyaman, dan berorientasi pada kepuasan konsumen,pemerintah sebagai penyedia layanan kesehatan dituntut untuk membenahi sistempelayanan yang bersifat layanan publik. Untuk mendukung program pembangunankesehatan dengan meningkatkan pelayanan kepada masyarakat, maka dibentuk BadanLayanan Umum BLU di setiap Puskesmas. Di kabupaten Bogor pada tahun 2017sudah ditetapkan 19 puskesmas untuk dilakukan penilaian adminsitratif sebagai syaratpenetapan menjadi Badan Layanan Umum Daerah BLUD . Penelitian ini bertujuanuntuk melihat kesiapan Puskesmas dari segi masukan yaitu sumber daya yang dimiliki,yaitu sumber daya manusia, anggaran, sarana, serta peraturan untuk ditetapkan menjadiBLUD. Selain itu, dilakukan juga analisis untuk mengetahui bagaimana manajemenpuskesmas berupa proses pengaturan organisasi dan penetapan tujuan dalam persiapanpenetapan BLUD. Faktor luar puskesmas juga mempunyai pengaruh dalam kesiapanpuskesmas dalam penerapan BLUD. Dari hasil penelitian ini akan diketahui informasimendalam tentang kesiapan administratif penerapan BLUD Puskesmas, serta faktor apasaja yang menjadi penghambat dalam proses persiapan dalam penerapan BLUDPuskesmas di Kabupaten Bogor.

Public awareness of the importance of health continues to grow along with increasinglevels of education and social life status. To improve the quality of health services,comfortable, and consumer oriented, the government as a healthcare provider isrequired to fix the service system that is public service. To support health developmentprograms by improving services to the community, a Local Public Service BLU isestablished in every Public Health Puskesmas . In Bogor regency in 2017, there are 19public health centers have been set up for administrative assessment as a condition ofdetermination to become the Local Public Service Agency BLUD . This study aims tosee the preparedness of Puskesmas in terms of input that is resources, namely human,budget, facilites, and regulations to be established into BLUD. In addition, analysis isalso conducted to find out how the management of puskesmas in the form oforganizational arrangement process and goal setting in preparation of the determinationof BLUD. The outside factors of the puskesmas also have an influence in the puskesmasreadiness in applying BLUD. From the results of this study will be known in depthinformation about the administrative readiness of the application BLUD Puskesmas, aswell as any factors that hamper the preparation process in the application of BLUDPuskesmas in Bogor Regency. "

"Tesis ini membahas tentang analisis keadilan (fairness) pembagian jasa pelayanan kapitasi didalam puskesmas dan antar puskesmas serta pendapat dinas kesehatan tentang biaya administrasi dana kapitasi kabupaten Bogor tahun 2019. Penelitian ini adalah penelitian kualitatif dengan design case study. Hasil penelitian menujukkan bahwa keadilan secara internal sudah adil. Sedangkan keadilan secara ekternal belum adil dan keadilan antar instansi belum adil. Sehingga perlu ada survey gaji untuk menetapkan besaran point tenaga kerja professional dan tenaga kerja manajemen. Selain itu, perlu ada penelitian lebih lanjut terkait penyesuaian resiko berdasarkan usia, jenis kelamin dan geografi untuk menetapkan besaran kapitasi antar puskesmas. Serta melakukan pengkajian ulang untuk penetapan Tunjangan Penghasilan Pegawai Negeri Sipil, agar tidak ada disparitas antar instansi.

This thesis discusses the analysis of fairness distribution of capitation services in puskesmas, between puskesmas and the opinion of the health office about the administrative costs of Bogor district capitation funds in 2019. This research is a qualitative study with a case study design. The results of the study show that justice is internally fair. Whereas external justice has not been fair and justice between agencies has not been fair. So that there needs to be a salary survey to determine the amount of points of professional workforce and management workforce. In addition, there needs to be further research related to risk adjustments based on age, sex and geography to establish capitation rates between puskesmas. As well as conducting a review for the establishment of Civil Servants Income Allowances, so that there is no disparity between agencies"

"Latar Belakang : Tujuan menganalisis kebijakan pemanfaatan dana kapitasi JKN pada FKTPPuskesmas di Kabupaten Bogor mengacu Permenkes 21 tahun 2016.
Metode : Kualitatif denganRapid Assesment Procedure, wawancara mendalam pada 12 informan, sampel purposive, terkaittujuan penelitian.
Hasil : Terdapat disparitas tinggi dana kapitasi puskesmas meliputi peserta,norma kapitasi, jumlah dokter dan rasio dokter antara berbagai puskesmas. Wawancaramendalam didapatkan sulitnya pemenuhan rasio dokter ideal, norma kapitasi rendahmenunjukkan kuantitas kualitas puskesmas rendah, belum semua puskesmas melakukan prosesperencanaan dengan benar, puskesmas kapitasi kecil sulit dalam operasional dan yang besarberlebih operasional dan berpotensi menumpuk, penentuan poin cukup jauh berbeda antar tenaga,pemenuhan obat-obatan terkendala oleh pengadaan, potensi overlapping kapitasi dengan BOKdan kualitas pelayanan dokter menurun pada rasio dokter per peserta besar.
Kesimpulan dan saran : Rasio dokter dengan peserta masih dibawah standar 1:5000 peserta perlu upayapemerataan, porsi kapitasi 60 untuk Jasa dan 40 opersional lain, ketercukupannya berbedaperlu ada backup dana operasional untuk yang kurang, adanya disinsentif jasa pelayanan perludikaji ulang, kapitasi porsi 40 dapat komplementer dengan BOK, sisa anggaran menguntungkanbila alternatif kegiatan mampu efektif efisien sesuai kebutuhan masyarakat, perlu perbaikanmekanisme pengadaan obat, dalam fleksibilitas anggaran perlu didorong PPK-BLUD padapuskesmas.
Background The purpose of analyzing the policy of utilization of JKN capitation fund at FKTPPuskesmas in Bogor Regency refers to Permenkes 21 year 2016.
Method Qualitative with RapidAssessment Procedure, in depth interview on 12 informant, purposive sample, related to researchobjectives.
Results There is a high disparity of capitation funds for puskesmas coveringparticipants, capitation norms, number of doctors and the ratio of doctors between variouspuskesmas. In depth interviews found the difficulty of fulfilling the ideal physician ratio, lowcapitation norms indicated the low quality of the puskesmas, not all the puskesmas did the properplanning process, the small capitation clinics were difficult in the operational and theoverwhelming operational and potentially piled up, Drug fulfillment is constrained byprocurement, the potential for overlapping capitation with BOK and the quality of physicianservices decreases in the ratio of physicians per large participant.
Conclusions and suggestions The ratio of physicians to participants is still below the standard of 1 5000 participants needequalization effort, 60 capitation portion for services and 40 other opersional, differentsufficiency there should be operational fund backups for the less, the disincentives of service needto be reviewed, Capitation of 40 portion can be complementary with BOK, the rest of the budgetis advantageous if the activity alternative can be effectively efficient according to societyrequirement, need improvement of drug procurement mechanism, budget flexibility need to bepushed PPK BLUD at puskesmas.

"Puskesmas merupakan ujung tombak pelaksana pelayanan kesehatan yang sangat strategis dalam melaksanakan berbagai kebijakan dan program kesehatan, seperti SPM, PISPK, dan KBK-BPJS. Kebijakan-kebijakan yang dilaksanakan secara bersama-sama menimbulkan situasi koeksistensi. Penelitian ini bertujuan untuk mengetahui apakah pelaksanaan kebijakan SPM, PISPK, dan KBK-BPJS di Puskesmas terjadi koeksistensi secara mutually exclusive (saling berdiri sendiri), competitive (berkompetisi), complementary (saling mendukung) dan integrated (terintegrasi) dalam hal tenaga, waktu, sarana, dana, dan pelaporan di Puskesmas di Kabupaten Bogor. Penelitian ini menggunakan pendekatan kualitatif melalui wawancara mendalam terhadap 15 orang informan yang berada di Puskesmas Bojonggede, Puskesmas Cibinong, Puskesmas Cirimekar, Puskesmas Kemuning dan Dinas Kesehatan Kabupaten Bogor. Hasil penelitian menunjukkan bahwa koeksistensi secara mutually exclusive terjadi pada aspek pelaporan, sistem pelaporan program mempunyai aplikasi masing-masing seperti SIKDA, SIMPUS, dan Laporan Suplemen pada program SPM, Web Keluarga Sehat pada program PISPK, dan P-Care untuk pelaporan KBK-BPJS. Koeksistensi secara competitive terjadi pada aspek tenaga dan waktu kerja. Pelaksanaan program yang dinilai paling berat adalah PISPK, sementara SPM dinilai program rutin yang biasa dilakukan di puskesmas. KBK BPJS dinilai lebih mudah dilaksanakan daripada PISPK dalam hal pencapaian angka kontak. Complementary terjadi pada aspek sarana dan dana. Pelaksanaan ketiga kebijakan SPM, PISPK, dan KBK-BPJS sistemnya belum terintegrasi sempurna.<

Centre or in Indonesia called Puskesmas plays a crucial and strategical role as a health care provider in implementing various policies and health program such as Minimum Service Standards (SPM), Healthy Indonesia Program with family approach (PIPSK), and Capitation-Based on Service Commitment (KPK-BPJS). Implementing the policies and programs simultaneously creates a condition called coexistence. This study aims to investigate whether implementation of the policies in Puskesmas works in a coexistence manner that is mutually exclusive, competitive, complementary and integrated in terms of human resources, work time, health facilities, funds and reporting. This study used a qualitative approach through in-depth interviews with 15 informants who were met at the community health centre in Bojonggede, Cibinong, Cirimekar, Kemuning and at the department of health of Bogor. The results of this study showed that the coexistence of mutually exclusive occurs in reporting. Specifically, program reporting systems have their own applications including SIKDA, SIMPUS, supplement report for SPM program; Health Family Web for PISPK and P-Care for KBK-BPJS. This study also found that the coexistence of competitive occurs in human resources and work time. PISPK is claimed as the most difficult program to carry out at the health centre in Bogor in comparison to KPK-BPJS in terms of achieving contact rates. Also, the program that routinely is done at the primary health centre in Bogor is SPM. The current study further indicates that the coexistence of complementary occurs in health facilities and funds. Finally, the coexistence of integrated policies such as implementations of SPM, PIPSK and KPK-BJS has not been fully worked at the community health centre in Bogor."
