"Latar belakang: Diagnosis tuberkulosis (TB), khususnya pada pasien HIV masih merupakan masalah tersendiri, terutama pada daerah dengan sumber daya terbatas. Pemeriksaan mikroskopis hapusan bakteri tahan asam (BTA) merupakan metode yang sederhana dan cepat tetapi hanya mendeteksi 30% -40% kasus Tb sedangkan kultur (baku emas) membutuhkan waktu pemeriksaan yang berminggu-minggu. Metode genotipe (PCR dan isothermal amplification) memiliki sensitivitas yang tinggi dan kerjanya cepat tetapi metode ini masih sangat kompleks dan membutuhkan peralatan khusus. Cross-priming amplification (CPA) merupakan metode amplifikasi DNA secara isothermal dengan menngunakan multiprimer dan enzim polymerase dengan tehnik pembacaan hasil amplifikasi yang sederhana.
Tujuan: Penelitian ini bertujuan untuk mengetahui perbedaanCPA dan PCR TB LMK FKUI dalam mendeteksi M. tuberculosis pada sputum pasien tersangka TB tanpa/dengan HIV.
Metode: 20 sputum pasien non-HIV tersangka TB dan 37 sputum pasien HIV tersangka TB diperiksa dengan CPA dan PCR TB LMK FKUI.
Hasil: Semua yang terdeteksi positif dengan CPA juga dideteksi positif oleh PCR tetapi 20% hasil yang terdeteksi negatif oleh CPA terdeteksi sebagai positif di PCR dan semua yang terdeteksi negatif oleh PCR terdeteksi negatif juga di CPA sedangkan hasil negatif di CPA (20%) masih terdeteksi positif oleh PCR. Dalam mendeteksi M. tuberculosis pada sputum pasien HIV/AIDS tersangka TB paru Terdapat hubungan bermakna antara CPA dengan PCR TB LMK FKUI, hampir semua hasil positif oleh CPA (94.1%) juga dideteksi positif oleh PCR tetapi 30% hasil negatif oleh CPA terdekteksi sebagai positif oleh PCR dan hampir semua hasil negatif oleh PCR (93.3%) terdeteksi negatif juga oleh CPA.
Kesimpulan: Terdapat hubungan bermakna antara CPA dan PCR TB LMK FKUI dalam mendeteksi M. tuberculosis pada sputum pasien tersangka TB paru. PCR TB LMK FKUI lebih sensitif dibanding CPA dalam mendeteksi M.tuberculosis.

Background: The diagnosis tuberculosis (TB) especialy in HIV patients remains a major obstacle to global control of TB, especially in resource limited settings. Light microscopy in sputum smears as common method in detection acid-fast bacilli (AFB) is specific but it only detects 30% to 40% of TB patients, while culture methods as gold standart in TB diagnostic require several weeks of incubation time. Genotypic method (polymerase chain reaction (PCR) and isothermal amplification) is known very sensitive and works fast but it requires special equipment and complex protocols in amplifying and detection amplified products. Cross-priming amplification (CPA) principle is isothermal amplification using multiple cross-linked primers (six to eight primers) and detection of amplified products is performed on special design plastic which is easy to performed and identified.
Objective: The study aimed to determine difference of PCR and CPA to detect M. tuberculosis in sputum specimens from suspected pulmonary tuberculosis without/with HIV patients.
Methods: 20 sputum samples suspected pulmonary TB of non-HIV patients and 37 sputum samples suspected pulmonary TB of HIV patients were subjected to CPA and PCR TB LMK FKUI.
Results:. All samples which were positive by CPA were also PCR positive but 20% result that were CPA negative were still positive by PCR and all samples with PCR negative were negative detected by CPA while some samples that were negative by CPA were still positive detected by PCR (20%). In HIV/AIDS population, there were significant correlation between CPA and PCR TB LMK FKUI which all positive result of CPA (94.1%) were also PCR positive but 30% of CPA negative were still CPA positive and almost all of PCR negative (93.3%) were CPA negative.
Conclusion: There were significant correlation between CPA and PCR TB LMK FKUI in ditected of M.tuberculosis in sputum of suspected pulmonary TB in populations of non HIV/AIDS patients and HIV/AIDS patients.

"HIV/AIDS merupakan penyakit menular yang menyerang sistem immun. Penelitian ini bertujuan untuk menggali dan memahami berbagai pengalaman pasien HIV/AIDS dengan TB dalam mengkonsumsi obat antiretroviral dan anti tuberculosis. Metode penelitian kualitatif dengan pendekatan fenomenologi digunakan dalam penelitian ini. Wawancara mendalam dilakukan kepada enam orang partisipan. Analisis data dengan menggunakan metode Colaizzi. Penelitian ini menghasilkan tujuh tema, yaitu pengalaman minum obat, perubahan fisiologi, perubahan psikologi, perubahan sosiologi, permasalahan ekonomi, pengalaman di pelayanan kesehatan serta spirit dan spiritualitas. Berdasarkan tema yang dihasilkan, disarankan agar perawat dapat memberikan asuhan keperawatan yang komprehensif dan holistik mencakup biopsikososial spiritual pada pasien karena mereka mengalami perubahan-perubahan besar dengan HIV/AIDS.

HIV / AIDS is an infectious disease that attacks the immune system. This study aims to explore and understand the various experiences of HIV / AIDS with TB patients in consuming antiretroviral and anti-tuberculosis drugs. Qualitative research method with phenomenological approach used in this study. In-depth interviews carried out to six participants. Analysis of the data by using Colaizzi method. This research identify seven themes, namely experience taking medication, changes in physiology, psychology changes, changes in sociology, economic problems, experience in health car and spirit & spiritualit. Based on the theme generated, it is recommended that nurses can provide a comprehensive and holistic biopsychosocial spiritual nursing care cover to patients as they undergo major changes with HIV / AIDS.

"ABSTRAK
Kasus HIV/AIDS dan tuberkulosis banyak diderita masyarakat perkotaan.
Penyakit pada masyarakat perkotaan berkaitan dengan karakteristik lingkungan
dan gaya hidup masyarakat perkotaan. Kasus HIV biasanya disertai dengan
berbagai infeksi oportunistik. Infeksi opportunistik yang paling banyak ditemukan
adalah tuberkulosis, diikuti dengan kandidiasis oral, dan diare. Masalah
keperawatan utama pada pasien dengan tuberkulosis adalah ketidakefektifan
bersihan jalan napas berhubungan dengan retensi sputum. Penulisan karya ilmiah
akhir ini bertujuan untuk menganalisis intervensi mandiri keperawatan berupa
kombinasi antara anjuran minum air putih hangat, postural drainase, dan batuk
efektif untuk mengatasi masalah bersihan jalan napas tidak efektif. Intervensi
dilakukan pada pasien HIV/AIDS dengan tuberculosis di ruang rawat penyakit
dalam RSCM. Hasil dan evaluasi dari intervensi yang dilakukan yaitu
berkurangnya penumpukan sputum pada jalan napas pasien, sehingga pasien tidak
merasakan sesak seperti sebelumnya dan bisa bernapas lebih lega. Perawat
diharapkan dapat melakukan intervensi mandiri keperawatan berupa kombinasi
antara anjuran minum air putih hangat, postural drainase, dan batuk efektif

"Tuberkulosis (TB) Paru merupakan infeksi oportunistik (IO) yang paling banyak ditemukan dan penyebab kematian terbanyak pada HIV/AIDS. Infeksi bakteri TB yang menyerang paru menyebabkan gangguan pernafasan. Salah satu masalah keperawatan yang muncul adalah bersihan nafas tidak efektif akibat hipersekresi sputum dari proses inflamasi yang berulang. Karya ilmiah akhir keperawatan ini bertujuan menganalisis intervensi batuk efektif dan terapi nebulizer yang dilakukan secara bersamaan dalam menangani masalah ketidakefektifan jalan nafas pada pasien HIV/AIDS koinfeksi TB Paru. Intervensi batuk efektif dan terapi nebulizer diintegrasikan dalam asuhan keperawatan. Evaluasi didapat berupa berkurangnya frekuensi batuk, dan sesak nafas yang pasien keluhkan, serta pasien mampu melakukan batuk efektif disertai pengeluaran sputum yang sebelumnya sulit dikeluarkan.

Tuberculosis (TB) Lung is the most common opportunistic infection (IO) and the leading cause of death in HIV / AIDS. TB bacterial infections that attack the lungs cause respiratory problems. One of the nursing problems that arises is ineffective breathing due to sputum hypersecretion of the inflammatory repeated process. This final nursing paper aims to analyze effective cough interactions and nebulizer therapies performed simultaneously in dealing with airway ineffectiveness in HIV / AIDS patients coinfected with pulmonary tuberculosis. These interventions are integrated into nursing care. Evaluation was obtained in the form of reduced frequency of cough, and shortness of breath that patient complained, and patient able to perform effective cough with sputum expenditure which previously difficult to expend.

"Praktek Spesialis Keperawatan merupakan sebuah proses penting dari pendidikan profesi dalam rangka mengaplikasikan peran perawat spesialis yang berdampak pada upaya peningkatan kualitas layanan keperawatan. Praktek ini dilakukan dan dikembangkan berdasarkan ilmu pengetahuan dan teknologi melalui berbagai penelitian menggunakan model Pendekatan Kebutuhan Dasar Virginia Henderson dan Teori Sosial Kognitif Bandura di RS Fatmawati dan RS Persahabatan Jakarta. Tiga kompetensi yang harus dicapai dalam praktek residensi ini adalah memberikan asuhan keperawatan menggunakan pendekatan teori keperawatan, menerapkan implementasi keperawatan yang berbasis bukti ilmiah (evidencebased nursing practice), dan melakukan proyek inovasi keperawatan yang berguna bagi lahan yang menjadi tempat praktek residensi. Sebagai pemberi asuhan keperawatan, mahasiswa residensi telah memberikan asuhan keperawatan menggunakan pendekatan teori Virginia Henderson pada satu kasus kelolaan utama dan 30 kasus resume pasien HIV/AIDS dengan berbagai infeksi oportunistik. Sebagai peneliti, mahasiswa telah mengaplikasi penerapan terapi spiritual terhadap penurunan tingkat stres pada orang dengan HIV/AIDS. Pengaruh terapi tersebut selaras dengan evidencebase nursing terkait menunjukkan hasil bahwa terdapat pengaruh yang signifikan antara pemberian terapi spiritual terhadap penurunan Tingkat stress pada orang dengan HIV/AIDS ODHA). Sebagai inovator, mahasiswa telah melakukan inovasi edukasi berbasis web untuk meningkatkan pengetahuan pasien Sistemic Lupus Eritematosus (SLE) di RS Persahabatan Jakarta. Hasil inovasi menunjukkan bahwa penggunaan media edukasi melalui webApp yang diakses menggunakan barcode menunjukkan peningkatan pengetahuan yang cukup signifikan.

Nursing Specialist Practice is an important process of nurse’s professional education to apply the role of specialist nurses which has an impact on efforts to improve the quality of nursing services. This practice is carried out and developed based on irginia Henderson's Model of Basic Needs Approach and Bandura's Cognitive Social Theory science and technology through various research at Fatmawati Hospital and Persahabatan Hospital. The three competencies that must be achieved in this residency practice are providing nursing care using a theoretical nursing approach, implementing evidencebased nursing practice, and conducting nursing innovation projects that are useful for the land where the residency practice is located. As a nursing care provider, the resident students have provided nursing care using the Virginia Henderson theory approach on a primary management case and 30 resume cases of HIV/AIDS patients with various opportunistic infections. As researchers, students have applied the application of Spiritual Therapy to Reduce Stress Levels in People with HIV/AIDS. The effect of the therapy is in line with related evidence-based nursing, showing that there was a signifact effect on decreasing level of stress in PLWH. Meanwhile, as an innovator, student has carried out web-based educational innovations to increase knowledge of Systemic Lupus Erythematosus (SLE) patients at Persahabatan Hospital Jakarta. The results of the innovation show that the use of educational media neglected webApp accessed using barcodes showed significant results in increasing knowledge of SLE patients."

"Human Immunodeficiency Virus (HIV) menjadi masalah utama kesehatan global yang sedang dihadapi oleh berbagai negara, termasuk Indonesia. Penderita HIV lebih rentan untuk terkena infeksi oportunistik, salah satunya tuberkulosis. Penelitian ini bertujuan untuk mengidentifikasi faktor-faktor yang berhubungan dengan infeksi oportunistik tuberkulosis pada pasien HIV di RSPI Prof. Dr. Sulianti Saroso tahun 2015-2019. Studi case control dilakukan dengan menggunakan data register pra-ART dan rekam medis. Jumlah sampel sebanyak 465 responden, yang terdiri dari 155 kasus dan 310 kontrol. Pengambilan sampel kasus menggunakan total sampling, sedangkan kontrol menggunakan simple random sampling. Analisis yang dilakukan adalah analisis univariat, bivariat menggunakan chi-square, dan multivariat menggunakan regresi logistik. Hasil uji regresi logistik menunjukkan ada hubungan yang bermakna secara statistik antara stadium HIV dengan infeksi oportunistik tuberkulosis (OR=33,03; 95% CI : 14,96 – 72,89) dengan nilai p <0,001, tetapi tidak ada hubungan yang bermakna secara statistik antara usia, jenis kelamin, jumlah CD4, jumlah viral load, pendidikan, status bekerja, perilaku seks berisiko, transfusi darah, dan penggunaan napza suntik dengan infeksi oportunistik tuberkulosis. Dibutuhkan perhatian khusus terhadap pasien HIV stadium lanjut (III-IV) untuk segera melakukan pemeriksaan tuberkulosis (TB).

Human Immunodeficiency Virus (HIV) is a major global health problem currently being faced by countries, including Indonesia. People with HIV are more susceptible to opportunistic infections, including tuberculosis. This study aims to identify the factors associated with opportunistic tuberculosis infection in HIV patients in RSPI Prof. Dr. Sulianti Saroso in 2015-2019. Case control study was carried out using pre-ART register data and medical records. The total sample of 465 respondents consists of 155 cases and 310 controls. The case samples were collected using a total sampling technique, while the control ones were collected using a simple random sampling technique. The analysis conducted was univariate analysis, bivariate analysis using chi-square, and multivariate analysis using logistic regression. The logistic regression test results showed that there was a statistically significant relationship between the stage of HIV and opportunistic tuberculosis infection (OR=33,03; 95% CI: 14,96 – 72,89) with p value <0,001, but there was no significant relationship between age, sex, CD4 cell count, viral load, education, work status, sexual behavior, blood transfusion, and injection drug use with opportunistic tuberculosis infection. Special attention is needed for patients with advanced HIV stage (III-IV) to immediately conduct a tuberculosis (TB) examination.

"ABSTRAK
Toksoplasmosis merupakan penyakit yang disebabkan oleh Toxoplasma gondii. Penyakit infeksi ini dapat menyebabkan kondisi fatal bila terjadi pada pasien imunokompromis, misalnya adalah toksoplasma ensefalitis (TE) yang menyerang sistem saraf pusat. Untuk menegakkan diagnosis Toxoplasma sebagai penyebab kelainan SSP (sistem saraf pusat) pada pasien HIV sangat sulit, sehingga diperlukan metode pemeriksaan lain sebagai alternatif, salah satunya adalah pemeriksaan PCR mendeteksi gen B1 dari Toxoplasma gondii. Penelitian ini bertujuan untuk mengembangkan metode PCR pada sampel CSS (cairan serebrospinal) yang sesuai untuk menegakkan diagnosis TE dan mengetahui keunggulan dan kelemahan pemeriksaan PCR dibandingkan pemeriksaan IgG anti-Toxoplasma pada cairan serebrospinal. Penelitian dilakukan pada cairan serebrospinal pasien HIV/AIDS dengan gangguan serebral. Hasil pemeriksaan PCR dari 88 sampel CSS pasien HIV yang datang ke Laboratorium Parasitologi FK UI, adalah 23 (26,1%) positif dan 65 (73,9%) negative T. gondii. Ada hubungan postif bermakna antara pemeriksaan PCR dengan pemeriksaan IgG anti-Toxoplasma dari CSS.

ABSTRACT
Toksoplasmosis is a disease caused by infection of Toxoplasma gondii. This infection can caused a life threatening condition in immunocompromised patients, for example toxoplasma ensefalitis (TE) which attack central nervous system. It is very difficult to diagnose Toxoplasma as a cause of CNS infection in HIV patient, so we need another methods as alternative, one of which is one of which is a PCR detection of Toxoplasma gondii B1 gene. This research aims to develop a PCR method on samples Cerebrospinal Fluid (CSF) that suitable for TE diagnosis and determine the advantages and disadvantages PCR methods compared to detection of anti-Toxoplasma IgG from CSF. The study was conducted in the cerebrospinal fluid of patients with HIV / AIDS with cerebral disorders. PCR examination results of 88 samples CSS HIV patients who came to the Laboratory of Parasitology FK UI, was 23 (26.1%) positive and 65 (73.9%) negative T. gondii. There is a significant positive relationship between PCR and detection anti-Toxoplasma IgG from CSF.

"Infeksi HIV/AIDS akan menimbulkan infeksi berkepanjangan dan gangguan pada semua sistem tubuh serta masalah psikologis seperti depresi dan akhirnya menimbulkan fatigue. Intervensi untuk mengatasi fatigue salah satunya dengan self care practice. Tujuan penelitian adalah mengidentifikasi hubungan depresi dan self care practice dengan tingkat fatigue pada penderita HIV/AIDS. Metode penelitian menggunakan analitik korelasi dengan cross sectional, dan jumlah sempel 98 responden. Pengambilan sampel dengan tehnik purposive sampling. Hasil penelitian menunjukan rata-rata usia 33,2 tahun, berjenis kelamin laki-laki 85,7%, berpendidikan tinggi 91,8%, berpenghasilan > UMR 73,5%, Telah mengkonsumsi ARV selama 36,96 bulan, rata-rata kadar CD4 310 cell/mm3, ratarata Hb 13 gr%.
Analisis hubungan menunjukan ada hubungan yang bermakna antara depresi, self care practice dengan tingkat fatigue (p<0,05). Analisis selanjutnya menunjukan responden yang mengalami depresi dan mempunyai self care practice yang kurang beresiko terjadi fatigue berat setelah dikontrol oleh kadar haemoglobin. Diketahui bahwa depresi merupakan faktor yang dominan yang berhubungan dengan fatigue. Rekomendasi peneliti adalah peningkatan peran perawat sebagai konselor terhadap gejala depresi dan fatigue pada pasien HIV dan dikembangkan strategi self care practice.

HIV/ AIDS infection will cause prolonged infection and disturbance to all body system and also psychological such as depression and eventually fatigueness. One of the interventions to deal with fatigue is by self care practice. The research?s goal is to identify the relation between depression and self care practice on fatigue level of HIV/AIDS patient. The method of the research applied correlation analysis with cross sectional. There were 98 respondents. Sample was taken by purposive sampling technique. The research showed that 85,7 % male respondents with 33,2 years of age in average, 91,8 % highly educated, 73,5 % earns higher than Regional Minimum Wage, has consumed ARV for 36 months, CD4 rate average of 310 cell/mm3, Hb rate 13 gr% in average.
The analysis showed that there was a significant relation between depression, self care practice with fatigue level of (p0,05). Further analysis showed that respondents that underwent depression and had lower self care practice will risk heavy fatigue after controlled by hemoglobin rate. It was found that depression is the dominant factor related to fatigue. The researcher recommends that there should be an increase of nurse?s role as counselor to depression symptom and fatigue of HIV patient. There should also efforts to develop self care practice.

Sedangkan, Human Immunodeficiency Virus (HIV) merupakan enveloped retrovirus yang menyerang sel CD4 pada sistem imun tubuh manusia. Terinfeksi virus HIV dapat menyebabkan sindrom immunodefisiensi (acquired immunodeficiency syndrome/AIDS) yang merupakan tahap akhir dari penyakit HIV. Terjadinya koinfeksi tuberkulosis disertai infeksi HIV/AIDS dapat mempercepat perburukan fungsi sistem imun pada tubuh pasien. Pemantauan terapi obat ialah kegiatan memastikan terapi obat bagi pasien berjalan dengan aman, efektif, dan rasional dengan tujuan untuk meningkatkan efektivitas terapi, meminimalkan reaksi obat tidak dikehendaki (ROTD), efisiensi biaya pengobatan, serta menghormati pilihan pasien terkait terapi. Kegiatan pemantauan terapi obat dilakukan pada salah satu rumah sakit umum daerah dan pengambilan data dilakukan secara prospektif pada pasien dengan diagnosis tuberkulosis di ruang perawatan ataupun melalui catatan rekam medik pasien. Masalah mengenai obat diidentifikasi dengan metode SOAP (Subjective, Objective, Assessment, Plan). Kesimpulan yang didapat dari studi pemantauan terapi obat pasien tuberkulosis ini ialah terapi telah diberikan sesuai dengan kondisi klinis yang dialami pasien.

Tuberculosis (TB) is a chronic infectious disease caused by infection of the bacteria Mycobacterium tuberculosis. Meanwhile, Human Immunodeficiency Virus (HIV) is an enveloped retrovirus that attacks the CD4 cells of human immune system. Infection with the HIV virus can cause acquired immunodeficiency syndrome (AIDS), which is the final stage of HIV disease. The occurrence of co-infection between tuberculosis and HIV/AIDS infection can accelerate the deterioration of the immune system function in the patient's body. Therapeutic drug monitoring is an activity to ensure that drug therapy for patients is running safely, effectively, and rationally with the aim of increasing the effectiveness of therapy, minimizing adverse drug reactions (ADRs), efficiency of treatment costs, and respecting patient choices regarding therapy. Drug therapy monitoring activities were carried out at one of the regional general hospitals and data collection was carried out prospectively on patients with a diagnosis of tuberculosis in the treatment room or through patient medical records. Problems regarding drugs were identified using the SOAP (Subjective, Objective, Assessment, Plan) method. The conclusion obtained from this study of therapeutic drug monitoring for tuberculosis patients is that therapy has been given in accordance with the clinical conditions experienced by the patient.

"Bunuh diri merupakan kegawatdaruratan psikiatri yang lazim ditemukan pada pasien HIV/AIDS dan dihubungkan dengan berbagai faktor. Namun, studi ini belum pernah dilakukan di Indonesia, padahal studi ini penting untuk diteliti untuk menentukan faktor-faktor yang harus diintervensi sehingga terbentuk pelayanan yang optimal terhadap pasien HIV. Penelitian ini bertujuan untuk mengetahui hubungan antara ide bunuh diri dengan faktor-faktor yang memengaruhinya pada pasien HIV/AIDS. Metode penelitian adalah dengan cross-sectional. Subjek dikumpulkan sebagai sampel menggunakan teknik consecutive sampling. Kriteria inklusi adalah orang dewasa berusia 18-65 tahun dengan diagnosis HIV/AIDS yang sedang menjalani pengobatan ARV di Poliklinik Khusus HIV/AIDS RSCM. Analisis data bivariat menggunakan uji Chi-square dan Fisher exact test, serta analisis multivariat menggunakan uji regresi logistik. Dari total 86 subjek, 20 23,3 diantaranya memiliki ide bunuh diri. Melalui uji Chi-square, hubungan terbukti bermakna pada 5 variabel, yaitu depresi p=0,000, ansietas p=0,001, tidak menikah p=0,007, jumlah CD4.
Suicide is a psychiatric emergency commonly found in HIV AIDS patients and is associated with various factors. However, this study has not been conducted in Indonesia. Besides, this study is important to determine the factors which must be intervened to establish optimum service for HIV AIDS patients. The aim of this study is to find the association between suicidal ideation and its determinant factors in HIV AIDS patients. Observational study with cross sectional method was conducted. Samples were collected using consecutive sampling technique. The inclusion criteria were adults aged 18 65 with HIV AIDS diagnosis and currently undergoing ARV treatment at Poliklinik Khusus HIV AIDS RSCM. Univariate analysis was performed using Chi square and Fisher exact test, while multivariate analysis was performed using logistic regression test. Of the total 86 subjects, 20 23.3 had suicidal ideation. Chi square test proved significant association on 5 variables depression p 0,000, anxiety p 0,001, being unmarried p 0,007, CD4 count."
