"Dalam menjalankan upaya pelayanan kesehatan, Puskesmas wajib menyelenggarakan pelayanan kefarmasian diantaranya penyimpanan obat LASA, pengkajian resep dan pelayanan resep. Pelayanan kefarmasian ini wajib mengikuti standar pelayanan kefarmasian di Puskesmas yang di atur dalam Peraturan Menteri Kesehatan nomor 74 tahun 2016. Pada laporan ini dilakukan pembuatan daftar obat LASA,dan alur pelayanan resep di Puskesmas Kecamatan Kramat Jati. Laporan ini dibuat berdasarkan hasil observasi, penelusuran literatur dari peraturan perundang-undangan yang berlaku serta wawancara dengan Apoteker Penanggung Jawab di Puskesmas Kecamatan Kramat Jati. Hasil observasi menunjukan bahwa dalam daftar obat LASA terdapat 18 obat yang tampak sama (look alike) dan 12 obat dengan pengucapakan sama (sound alike). Hasil observasi terhadap alur pelayanan resep menunjukan bahwa terdapat dua alur pelayanan resep yaitu alur pelayanan resep dalam gedung dan poli ISPA. Kedua hasil observasi tersebut menunjukan bahwa Penyimpanan obat LASA dan alur pelayanan resep di Puskesmas Kecamatan Kramat Jati telah sesuai dengan standar pelayanan kefarmasian di Puskesmas yang diatur dalam Peraturan Menteri Kesehatan nomor 74 tahun 2016.

In carrying out health service efforts, Community Health Centers are required to provide pharmaceutical services including storing LASA drugs, reviewing prescriptions and providing prescription services. Pharmaceutical services are required to comply with pharmaceutical service standards at the Puskesmas which are regulated in Minister of Health Regulation number 74 of 2016. This report contain the lists of LASA drugs, and the flow of prescription services at the Kramat Jati District Health Center. This report was prepared based on observations, literature searches from applicable laws and regulations and interviews with the pharmacist in charge at the Puskesmas Kramat Jati District. The observation results show that in the LASA drug list there are 18 drugs that look alike and 12 drugs that sound alike. The results of observations on the prescription service flow indicated that there were two prescription service flows, namely the prescription service flow in the building and URTI poly. Both of these observations show that LASA drug storage and prescription service flow at the Kramat Jati District Health Center are in accordance with the pharmaceutical service standards at the Health Center stipulated in Minister of Health Regulation number 74 of 2016."

"Berdasarkan Peraturan Kementerian Kesehatan No. 74 Tahun 2016, obat merupakan bahan atau paduan bahan, termasuk produk biologi yang digunakan untuk mempengaruhi atau menyelidiki sistem fisiologi atau keadaan patologi dalam rangka penetapan diagnosis, pencegahan, penyembuhan, pemulihan, peningkatan kesehatan dan kontrasepsi, untuk manusia. Maka dari itu, perlu diketahui cara mendapatkan, menggunakan, menyimpan, dan membuang obat dengan benar Hal ini diperlukan agar didapatkan manfaat obat yang maksimal dan efektif. Sehubungan dengan hal tersebut, perlu dilakukan edukasi dan sosialisasi mengenai hal tersebut. Dalam rangka edukasi dan sosialisasi DAGUSIBU (Dapatkan, Gunakan, Simpan, dan Buang) obat yang baik dan benar di Puskesmas Kecamatan Kramat Jati, tugas khusus berupa pembuatan spanduk mengenai DAGUSIBU dibuat dan diletakkan di Puskesmas Kecamatan Kramat Jati, Jakarta Timur.

"Standar pelayanan kefarmasian yang diatur dalam PMK No. 73 Tahun 2016 merupakan pedoman yang harus diimplementasikan di apotek untuk mengoptimalkan kesehatan di komunitas. Laporan ini mendeskripsikan implementasi pelayanan kefarmasian dan pengkajian resep pasien diabetes melitus tipe 2 dan hipertensi di apotek kimia farma 002 periode november 2022. Berdasarkan hasil observasi dan wawancara kepada petugas apotek dapat disimpulkan pelaksanaan kegiatan pengelolaan sediaan farmasi, alat kesehatan, dan BMHP di Apotek Kimia Farma 002 sudah cukup baik dan sesuai dengan PMK No. 73 Tahun 2016. Pengkajian yang dilakukan pada satu resep belum memenuhi aspek kelengkapan resep, yaitu aspek administratif dan pertimbangan klinis. Petugas apotek perlu menghubungi pihak dokter atau rumah sakit untuk mengkonfirmasi resep. Berdasarkan hasil observasi dan wawancara kepada petugas apotek dapat disimpulkan pelaksanaan kegiatan pelayanan swamedikasi di Apotek Kimia Farma 002 sudah cukup baik. Penggalian informasi pasien yang dilakukan sudah sesuai dengan literatur. Berdasarkan hasil observasi dan wawancara kepada petugas apotek dapat disimpulkan pelaksanaan kegiatan PIO sudah cukup baik dan sesuai dengan PMK No. 73 Tahun 2016, namun tidak didokumentasikan dengan baik.

The standard of pharmaceutical services regulated in Ministerial Decree No. 73 of 2016 is a guideline that must be implemented in pharmacies to optimize community health. This report describes the implementation of pharmaceutical services and the assessment of prescriptions for patients with type 2 diabetes mellitus and hypertension at Chemist Pharmacy 002 during the period of November 2022. Based on the results of observations and interviews with pharmacy staff, it can be concluded that the implementation of the management of pharmaceutical preparations, medical devices, and health supplies at Chemist Pharmacy 002 is already quite good and in accordance with Ministerial Decree No. 73 of 2016. "Pelayanan dan pengkajian resep merupakan salah satu pelayanan farmasi klinik di apotek. Resep harus selalu ditulis dengan benar karena resep memiliki peran yang besar dalam terapi pengobatan dan kesehatan pasien. Sebagai penyakit yang termasuk ke dalam lima besar penyebab kematian di dunia dengan prevalensi 17,4%, obat asma banyak diresepkan kepada pasien sehingga dalam penulisan resepnya perlu diperhatikan untuk menghindari pemberian obat yang tidak rasional akibat dari kesalahan penulisan resep. Oleh karena itu, tugas khusus ini bertujuan untuk melakukan pengkajian resep obat pada pasien asma agar dapat mengetahui kesesuaian dan kelengkapan resep serta mendeteksi adanya kesalahan pada peresepan seperti adanya interaksi obat. Tugas khusus ini disusun melalui pengkajian resep untuk pasien asma yang menggunakan layanan BPJS. Setiap resep yang diterima akan dilakukan skrining resep terkait dengan kelengkapan administrasinya. Setelahnya resep diserahkan kepada tim penyiapan resep untuk pengambilan obatnya dari rak, kemudian penulisan etiket, dan terakhir penulisan kelengkapan lainnya seperti copy resep (jika ada) dan Kartu Kendali untuk pasien BPJS. Setelahnya akan dilakukan pengkajian bagi resep yang mengandung obat-obatan asma sesuai persyaratan farmasetik dan klinisnya. Dua buah resep yang dikaji pada tugas khusus ini belum memenuhi seluruh aspek administrasi, farmasetik, dan klinis.

Prescription services and reviews are one of the clinical pharmacy services at the pharmacy. Recipes must always be written correctly because recipes play a big role in medical therapy and patient health. As a disease that is included in the top five causes of death in the world with a prevalence of 17.4%, asthma medication is widely prescribed to patients, so care needs to be taken when writing prescriptions to avoid irrational medication administration due to prescription errors. Therefore, this special task aims to assess drug prescriptions for asthma patients to determine the suitability and completeness of the prescription and detect errors in prescribing such as drug interactions. This special task is prepared by reviewing prescriptions for asthma patients who use BPJS services. Every prescription received will be screened regarding the completeness of its administration. After that, the prescription is handed over to the prescription preparation team to take the medicine from the shelf, then write the label, and finally write other equipment such as a copy of the prescription (if any) and a Control Card for BPJS patients. After that, a review will be carried out for prescriptions containing asthma medicines according to pharmaceutical and clinical requirements. The two prescriptions studied in this special assignment did not meet all administrative, pharmaceutical, and clinical aspects."

"Salah satu faktor penunjang kelancaran pelayanan kesehatan di puskesmas adalah tersedianya obat yang cukup untuk masyarakat yang melakukan pengobatan di puskesmas. Ketersediaan obat yang optimal dapat direncanakan dari proses perencanaan termasuk proses pencatatan dan pelaporan. Proses perencanaan kebutuhan obat tiap tahunnya dilakukan dengan sistem berjenjang (bottom-up) dari puskesmas yang berada di kelurahan hingga ke kecamatan. Tiap puskesmas akan membuat dan melaporkan Laporan Pemakaian dan Lembar Permintaan Obat (LPLPO). Formulir ini berfungsi untuk memonitoring pemakaian dan permintaan obat setiap bulan dan dapat diketahui profil penggunaan obat yang paling banyak hingga paling sedikit digunakan di puskesmas sehingga dapat disesuaikan dengan pola penyakit yang tersebar di masyarakat wilayah kerja puskesmas tersebut. Penelitian ini bertujuan untuk mengetahui kesesuaian kartu stok obat dengan LPLPO dan mengetahui profil 10 (sepuluh) obat terbanyak di Puskesmas Kecamatan Kramat Jati Periode Oktober-Desember 2022 sehingga dapat membantu dalam proses perencanaan dan permintaan untuk periode selanjutnya. Pengambilan data menggunakan metode retrospektif dengan melihat LPLPO Puskesmas Kecamatan Kramat Jati Periode Oktober-Desember 2022. Hasil dari penelitian ini berupa 10 (sepuluh) profil penggunaan obat terbanyak dari 228 obat di Puskesmas Kecamatan Kramat Jati Periode Oktober-Desember 2022 dengan urutan sebagai berikut: Metformin 500 mg; Parasetamol 500 mg; Calcium lactate 500 mg; Guaifenesin 100 mg; Chlorpheniramine maleate (CTM) 4 mg; Amlodipin 5 mg; Deksametason 0,5 mg; Vitamin B Kompleks; Glimepirid 2 mg; dan Vitamin B6 10 mg.

One of the factors supporting the smooth operation of healthcare services at health centers is the availability of sufficient medications for the community seeking treatment. Optimal medication availability can be planned through processes including recording and reporting. The annual medication planning process is conducted in a hierarchical (bottom-up) system from health centers in villages to those in districts. Each health center will prepare and report the Medication Usage Report and Request Form (LPLPO). This form functions to monitor monthly medication usage and requests, allowing the identification of the most to least used medications at the health center, which can then be adjusted according to the disease patterns prevalent in the community served by the health center. This study aims to determine the conformity of medication stock cards with LPLPO and to identify the profile of the ten most used medications at the Kramat Jati District Health Center for the period of October-December 2022, thereby assisting in the planning and requesting process for the next period. Data collection was conducted retrospectively by reviewing the LPLPO of the Kramat Jati District Health Center for the period of October-December 2022. The results of this study include the profile of the ten most used medications out of 228 at the Kramat Jati District Health Center for the period of October-December 2022, in the following order: Metformin 500 mg; Paracetamol 500 mg; Calcium lactate 500 mg; Guaifenesin 100 mg; Chlorpheniramine maleate (CTM) 4 mg; Amlodipine 5 mg; Dexamethasone 0.5 mg; Vitamin B Complex; Glimepiride 2 mg; and Vitamin B6 10 mg."

"Praktik Kerja Profesi Apoteker (PKPA) merupakan salah satu upaya untuk memberikan bekal keterampilan pada mahasiswa apoteker agar memiliki keterampilan kefarmasian sehingga  nantinya calon apoteker diharapkan mampu melakukan praktik kefarmasian secara profesional, legal, dan etik di Puskesmas. Pelaksanaan praktik kerja profesi ini berlangsung selama dua minggu dengan tugas khusus, yakni Evaluasi Rasionalitas Penggunaan Antipsikotik Pasien Skizofrenia di Puskesmas Kecamatan Kramat Jati Periode Oktober 2022. Tujuan dilakukannya tugas khusus ini untuk mengevaluasi rasionalitas peresepan antipsikotik pada pasien skizofrenia di Puskesmas Kecamatan Kramat Jati tahun 2022. Metode yang digunakan pada tugas khusus ini adalah menggunakan desain observasional deskriptif dengan teknik pengambilan sampel secara retrospektif pada pasien skizofrenia periode bulan Oktober 2022. Subjek pada penelitian, yakni data resep pasien skizofrenia Puskesmas Kecamatan Kramat Jati bulan Oktober 2022. Instrumen penelitian yang digunakan, yakni data excel yang diperoleh melalui website ePuskesmas. Berdasarkan hasil penelitian maka dapat disimpulkan bahwa penggunaan obat antipsikotik pada pasien skizofrenia di Puskesmas Kecamatan Kramat Jati Periode Agustus 2022 dinilai belum memenuhi ketepatan penggunaan antipsikotik. Hasil  evaluasi menunjukkan ketepatan indikasi sebanyak 25 pasien [100%], tepat pemilihan obat sebanyak 18 pasien [72%] , tepat pasien sebanyak sebanyak 25 pasien [100%], tepat dosis sebanyak 24 pasien [96 %], serta tepat frekuensi sebanyak 24 pasien [96%].

Pharmacy intenrship is one of the efforts to provide skills provision for pharmacist students so that they have pharmaceutical skills so that later pharmacist candidates are expected to be able to practice pharmacy in a professional, legal and ethical manner at the district health center. The duration internship is for two weeks with a special assignment, namely  Evaluation of Rational Antipsychotics Schizophrenic Patients at the Kramat Jati District Health Center. The purpose of this special assignment was to evaluate the rationality of prescribing antipsychotics in schizophrenic patients at the Kramat Jati District Health Center in 2022. The method used in this special assignment was to use a descriptive observational design with a retrospective sampling technique in schizophrenia patients The research subjects were prescription data of schizophrenic patients at the Kramat Jati District Health Center in October 2022. The research instrument used was data excel obtained through the ePuskesmas website. Based on the results of the study, it can be concluded that the use of antipsychotic drugs in schizophrenic patients at the Kramat Jati District Health Center for the August 2022 period is considered not to meet the accuracy of antipsychotic use. The results of the evaluation showed the accuracy of the indications for 25 patients [100%], the correct drug selection for 18 patients [72%] , the correct dosage for 25 patients [100%], the correct dosage for 24 patients (96 %), and the correct frequency for 24 patients. patients [96%]."

"Migrain merupakan gangguan nyeri kepala primer dengan prevalensi tinggi dan cukup berdampak pada masalah sosial ekonomi. Studi yang dilakukan yaitu pengkajian kelengkapan resep serta analisis terhadap resep obat migrain yang diterima oleh Apotek Roxy Galaxy selama periode Januari 2023.Pengkajian dilakukan dengan mengumpulkan resep yang diterima selama bulan Januari tahun 2023 kemudian memilih dua resep resep yang memuat obat migran untuk dilakukan pengkajian resep.Pengkajian resep obat migrain yang diterima di Apotek Roxy selama periode bulan Januari 2023 ditemukan bahwa resep yang diterima sudah rasional namun kelengkapan administratif dan farmasetik tidak tercantum dengan lengkap. Resep yang diterima sudah rasional namun perlu dilakukan pengkajian resep dengan data yang lebih banyak dan periode pengkajian lebih panjang sehingga data yang diperoleh lebih banyak dan beragam.

Migraine is a primary headache disorder with a high prevalence and quite an impact on socio-economic problems. The study carried out was a review of the completeness of prescriptions and an analysis of migraine drug prescriptions received by Apotek Roxy Galaxy during the January 2023 period. A review of migraine drug prescriptions received at the Apotek Roxy during the January 2023 period found that the prescriptions received were rational but the administrative and pharmaceutical aspects were not listed completely. The prescriptions received are rational, but it is necessary to review the prescriptions with more data and a longer review period so that the data obtained is more numerous and varied."

"Kegiatan penyimpanan merupakan salah satu kegiatan pengeloalan sediaan farmasi dan BMHP yang bertujuan untuk memelihara mutu sediaan farmasi, menghindari penggunaan yang tidak bertanggungjawab, menjaga ketersediaan, serta memudahkan pencarian dan pengawasan. Dalam proses penyimpanan, rentan sekali terjadi kesalahan dalam pengambilan obat terutama pada obat-obatan yang memiliki rupa-mirip (Look Alike), dan ucapan-mirip (Sound Alike) (Kemenkes RI, 2019). Puskesmas Kecamatan Ciracas (PKC Ciracas) belum menerapkan metode Tallman Lettering dalam penyusunan produk LASA dan melakukan beberapa pembelian produk obat baru yang belum tersusun sesuai dengan abjad, sehingga perlu dilakukan evaluasi penyusunan kembali produk farmasi yang terdapat di gudang dan menerapkan metode Tallman Lettering sesuai dengan ketentuan penyimpanan produk LASA. Menurut ISMP, Institute for Safe Medication Practices (2008), beberapa penelitian menunjukkan bahwa penggunaan Tallman Lettering dapat membuat nama obat dengan ucapan mirip lebih mudah dibedakan (Filk, 2006), dan lebih sedikit menimbulkan kesalahan (Filk. R, 2006). Dari 451 tanggapan, survey yang diterima ISMP, hampir semua (87%) merasa bahwa Tallman Lettering membantu mengurangi kesalahan pemilihan obat, dan dua pertiga melaporkan bahwa Tallman Letter mampu mencegah pengeluaran atau pemberian obat yang salah. Penyimpanan obat-obatan dan alat kesehatan dilakukan mulai dari pendataan produk, penyusunan ulang tempat penyimpanan berdasarkan kategorinya (bentuk sediaan dan alat kesehatan), pemilihan produk yang termasuk LASA dan menggunakan Tallman Lettering, pembuatan tag nama masing-masing obat dan alat kesehatan, hingga penyusunan obat dan alat kesehatan sesuai dengan namanya di tempat penyimpanan yang telah dibuat.

In the storage process, errors are very prone to occur in taking drugs, especially for drugs that have a Look-Alike and Sound-Alike appearance (Ministry of Health RI, 2019). The Ciracas District Health Center (PKC Ciracas) has not implemented the Tallman Lettering method in preparing LASA products and has purchased several new drug products that have not been arranged alphabetically, so it is necessary to evaluate the rearrangement of pharmaceutical products in warehouses and apply the Tallman Lettering method in accordance with the provisions storage of LASA products. According to ISMP, Institute for Safe Medication Practices (2008), several studies have shown that the use of Tallman Lettering can make drug names with similar pronunciations easier to distinguish (Filk, 2006), and cause fewer errors (Filk. R, 2006). Of the 451 survey responses received by ISMP, almost all (87%) felt that Tallman Lettering helped reduce medication selection errors, and two-thirds reported that Tallman Lettering was able to prevent dispensing or administering the wrong drug. Medicines and medical devices storage is carried out starting from product data collection, rearranging storage places based on category (dosage forms and medical devices), selecting products that are included in LASA and using Tallman Lettering, making name tags for each drug and medical device, to compiling drugs and medical devices according to their names in the storage area that has been made.
