"ABSTRAK
Penelitian ini bertujuan untuk mengevaluasi pelaksanaan Program Pelayanan Jarak Jauh PPJJ Tahun 2017 sebagai program home care bagi penyandang disabilitas intelektual di Kabupaten Bandung Barat serta mengidentifikasi faktor-faktor pendukung dan penghambatnya. Penelitian ini menggunakan pendekatan kualitatif dengan jenis penelitian evaluasi formatif yang membandingkan temuan dengan Pedoman PPJJ sebagai prosedur dan kebijakan pengelola program dan Pedoman Home Care Tuna Grahita Kementerian Sosial sebagai best practice standard . Hasil penelitian menunjukkan bahwa secara umum 10 tahapan PPJJ Tahun 2017 dilaksanakan dengan berpedoman pada kebijakan pengelola program. Di sisi lain, terdapat tahapan yang tidak sesuai dengan pedoman home care, serta tahapan yang tidak didasari prosedur apapun. Pelaksanaan PPJJ Tahun 2017 didukung oleh beberapa faktor, antara lain pengalaman pendamping dalam kegiatan sosial, kepedulian pendamping terhadap anak berkebutuhan khusus, adanya Rehabilitasi Berbasis Masyarakat RBM , solidaritas pendamping, dan dukungan dari berbagai pihak. Adapun faktor penghambat program mencakup kesulitan pendamping dalam menyusun laporan bantuan stimulan, sikap orang tua penerima pelayanan yang tidak proaktif terhadap program, kondisi psikologis penerima pelayanan yang labil, materi pelatihan pendamping yang kurang lengkap, jarak antar kecamatan yang jauh, dan kecemburuan sosial dari aparat, kader, dan masyarakat di salah satu kecamatan.

ABSTRACT
This study aims at evaluating the implementation of The Distance Service Program DSP in 2017 as a home care program for persons with intellectual disabilities in West Bandung regency and also identifying the supporting and inhibiting factors. This study used a qualitative approach with the type of formative evaluation research that compares the findings with the DSP Guidelines and Home Care for Intellectual Disability Guidelines by Ministry of Social Affairs of Indonesia. The results showed that generally 10 stages of DSP in 2017 were implemented by referring to program management policy. Some stages are inconsistent with home care guidelines and that are not based on any procedure. The implementation of DSP in 2017 is supported by several factors, such as community worker rsquo s experience in social activities, their caring concerns to children with special needs, the existence of Community Based Rehabilitation CBR institution, solidarity of community workers, and support from various parties. The inhibiting factors of the program include the community worker 39 s difficulties in preparing stimulant assistance reports, parental attitudes of non proactive service to the program, psychological condition of beneficiaries, incomplete training materials, distances between remote sub districts, and social jealousy from the apparatus, cadres, and communities in one of the districts."

"ABSTRAK
Jepang adalah salah satu negara maju yang mengalami masalah penurunan tingkat kelahiran dan peningkatan populasi lansia. Upaya pemerintah dalam menangani masalah ini antara lain dengan mengeluarkan kebijakan kesejahteraan sosial untuk lansia sebagaimana diatur dalam Undang-Undang Kesejahteraan Sosial Roujin Fukushi Hou , yang pertama kali disahkan pada tahun 1963. Saat ini di Jepang ada banyak perusahaan kesejahteraan sosial yang tumbuh dan NPO Organisasi Nirlaba yang membantu pemerintah dalam layanan perawatan jangka panjang untuk lansia. LTCI Asuransi Perawatan Jangka Panjang adalah skema asuransi yang membantu para lansia dalam perawatan. LTCI diterapkan secara luas di berbagai perusahaan kesejahteraan dan NPO tersebar di seluruh Jepang. Fokus penelitian ini adalah pelayanan lansia oleh NPO yang berdasarkan skema LTCI dengan menggunakan teori New Institutionalism oleh Victor Nee. Lembaga berinteraksi dengan jejaring sosial dan norma sosial dalam mengarahkan tindakan ekonomi. Lembaga adalah struktur sosial yang memberikan pedoman untuk tindakan bersama dengan mengatur kepentingan mereka sendiri. Hipotesis dalam penelitian ini adalah perlunya keberadaan NPO yang memiliki skema LTCI sebagai wakil pemerintah dalam pelayanan kesejahteraan sosial untuk lansia. Penelitian ini akan difokuskan di kota Tokyo, Jepang. Penelitian ini menggunakan metode kualitatif melalui studi pustaka dan dokumen. Data bersumber dari pemerintah dan pernyataan yang diterbitkan dan jurnal, buku dan situs internet. .Kata Kunci: Peran NPO, LTCI, bantuan pemerintah, layanan perawatan, lansia

ABSTRACT
Japan is one of the developed countries that experienced problems of declining birth rates and an increase in the elderly population. The government 39 s efforts in dealing with this issue are, among others, by issuing a social welfare policy for the elderly as stipulated in the Social Welfare Act Roujin Fukushi Hou , which was first enacted in 1963. Currently in Japan there are a lot of growing social welfare companies and NPOs Non Profit Organization that help government in long term care service for elderly. LTCI Long Term Care Insurance is an insurance scheme that helps the elderly in care. LTCI is widely applied in various welfare companies and NPOs are scattered throughout Japan. The focus of this research is the elderly service by NPO based on the LTCI scheme by using New Institutionalism theory by Victor Nee.. Institutions interact with social networks and social norms in directing economic actions. Institutions are social structures that provide guidelines for joint action by regulating their own interests. The hypothesis in this research is the need of existence of NPO which have LTCI scheme as representative of government in social welfare service for elderly. This research will be focused in the city of Tokyo, Japan. This research uses qualitative method through literature study and documents. "The Malaysia Ministry of Health reported adolescents’ low utilization of healthcare services, although they need this service as a consequence of their involvement in risky behavior. This cross-sectional study aimed to determine adolescents’ perception on risk-taking behavior and their utilization of health care services. A modified self-administered questionnaire was used to collect data from 250 secondary school students aged 13 and 14 years in one of the selected schools in Malacca, Malaysia. Data were analyzed using SPSS 20. Fast-food consumption, loitering after school, physical fighting, smoking, and non-use of helmets were the most reported risky behaviors among adolescents in Malaysia. More than half of the adolescents who knew about health care services had a positive perception on their utilization of such services. Thus, the promotion of adolescents’ health services helps increase their use of these services and consequently achieve a healthy lifestyle."

"Ketatnya kompetisi yang ditimbuikan oleh, di antaranya, kebijakan pemerintah yang memungkinkan Perseroan Terbatas dan modal asing mendirikan rumah sakit serta berbagai variabel yang menyebabkan cepatnya perubahan pasar mengisyaratkan analisis dan perencanaan matang bagi pendirian sebuah rumah sakit. Dari segi pemasaran, misalnya, perlu iebih dulu dilakukan identifikasi dan segmentasi pasar, pemilihan kelompok pelanggan serta penetapan pasar sasaran. Di tengah situasi kompetitif seperti inilah Yayasan Jakarta Medika melakukan persiapan untuk mengembangkan Poliklinik Jakarta Medical Center (JMC) menjadi rumah sakit. Basis bagi rencana ini adalah terns meningkatnyajumlah pengunjung poliklinik dan sisa hasil usaha selama tiga tahun terakhir, relatif tingginya loyalitas pasien serta tingginya minat pada fasilitas rawat inap sehari dan tersebarnya 25 klinik 24 jam milik yayasan.
Pada penelitian ini dilakukan segmentasi geografi, demografi dan psikografi terhadap para pengguna jasa, masyarakat, dokter praktek swasta, dan perusahaan pelanggan untuk menganalisis karakteristik para pengguna jasa dan pasar potensial sebagai bahan penetapan pasar sasaran (targeting) dan pola pasar sasaran, untuk selanjutnya dilakukan positioning terhadap 2 RS kompetitor ( RS Tria Dipa 1 RS Siaga Raya ) serta pernyataan positioning sebagai bentuk positioning terhadap pelanggan.
Penelitian yang bersifat deskriptif ini merupakan studi multi-responden yang menggunakan data primer berdasarkan survei terhadap 116 pasien, 30 dokter, 30 orang masyarakat sekitar poliklinik, 9 perusahaan pelanggan dan 2 RS kompetitor ( RS Tria Dipa dan RS Siaga Raya ). Analisis didukung pula oleh data sekunder dari berbagai sumber termasuk Biro Pusat Statistik dan Lembaga Demografi Fakultas Ekonomi Universitas Indonesia.
Hasil segmentasi geografis dan demografis menunjukkan bahwa segmen pasar terbesar bagi poliklinik JMC adalah pasien yang datang dari lima kotamadya di DKI Jakarta dan beberapa wilayah sekitar seperti Bogor dan Depok. Mayoritas pengunjung klinik adalah mereka yang berada pada kelompok usia produktif 20 - 39 tahun, dengan tingkat pendidikan D3 dan universitas, bekerja sebagai karyawan swasta dengan penghasilan per bulan Iebih dari Rp 1 juta. Dari segmentasi psikografis didapat pula data bahwa kebanyakan dari pengunjung adalah mereka yang berasal dari status sosial ekonomi menengah ke atas yang memiliki kesadaran relatif tinggi tentang nilai kesehatan, sebagaimana tampak dari kesediaan menganggarkan belanja kesehatan yang juga relatif tinggi. Ini juga tampak dari pilihan kelas rawat inap sampai WIP pada pasien.
Penelitian pasar potensial pada masyarakat sekitar menunjukkan perbedaan karakter demografis serta pengeluaran kesehatan yang boleh jadi menyebabkan aspek psikografis pilihan kelas perawatan dan tingkat demand lebih rendah terhadap rumah sakit JMC yang direncanakan didirikan. Oleh karena itu gambaran akan pasar potensial dilengkapi dengan perbandingan antara supply (jumlah fasilitas sarana tenaga kesehatan) dengan demand (jumlah populasi di Jakarta Selatan) serta penghitungan proyeksi kunjungan rawat jalan, current demand maximum market potensial dan MDI yang menunjukkan hasil perhitungan 22,51 %, artinya pasar pelayanan jasa kesehatan masih dapat bertambah (potensial).
Gambaran segmen pasar poliklinik JMC dan pasar potensial menjadi dasar penetapan pasar sasaran berikut: pasien kelas menengah keatas dengan pendidikan, penghasilan dan kesadaran kesehatan yang relatif tinggi, serta memiliki tuntutan mutu layanan kesehatan yang tinggi. Penelitian ini juga menemukan bahwa pola pemilihan pasar sasaran adalah spesialisasi terpilih ( multi coverage segment) sementara positioning dalam struktur pasar terhadap kompetitor adalah market follower atau market challenger. Pelayanan kesehatan yang tinggi dengan sentuhan kemanusian (high touch) adalah positioning yang ditujukan kepada pelanggan.

Market Segment, Target and Positioning Analysis for the Development of Jakarta Medical Center (JMC) Policlinic into Hospital in Year 2000.The staff competition resulting from, among other things, government policies which enable private enterprises and foreign capitals to venture into the hospital industry as well the presence of variables that lead to rapid market change stipulate well-laid plans and analysis before establishing a hospital. A process to identify market segments, for instance, is needed for marketing purposes, in addition to the selection of consumer groups and market targets. The growing competition provides the backdrop for the Yayasan Jakarta Medika's preparations to develop its Jakarta Medical Center (JMC) Policlinic into a hospital. The basis for this plan includes the steady increase in the number of patients, the healthy profit margin over the past three years, patients' loyalty, and expressed interests in one-day-care facilities provided by a total of 25 24-hour clinics owned by the Yayasan's in the areas surrounding the policlinic.
Geographic, demographic and psycho-graphic segmentation processes were carried out in this study, involving patients, residents in the surrounding area, private doctor practices and companies that use the policlinic's facilities for their employees' health care. The purpose of the segmentation is to analyze the characteristics of the facility users and the potential market in order to establish target market and its pattern. The process is also needed to determine the positioning against two competitor hospitals, namely the Tria Dipa Hospital and Siaga Raya Hospital, as well as positioning toward customers.
The study is descriptive and multi-respondents, using primary data from a survey that involved 116 patients, 30 doctors, 30 residents in the surrounding areas, nine user companies, and the two competitor hospitals. A set of secondary data obtained from agencies such as the Central Bureau of Statistics and the Institute of Demography at the University of Indonesia School of Economics, back up the analysis.
The geographic and demographic segmentation shows that the largest market segment for the policlinic is patients coming from five Jakarta mayoralties and surrounding areas such as Bogor and Depok. The majority of patients are those of productive ages of 20 to 39, university-educated, employees of private companies who make more than Rp 1 million per month. The psyche-graphic segmentation reveals that most patients are those of upper middle social economic class with relatively high-health awareness as indicated in willingness to allocate relatively high health-care budget.and demand for the better an-patient care facilities.
The group of residents studied showed different characteristics demographically than those of the patients, including lower income, demands for lower level of in-patient care facilities and lower demands for the planned JMC hospital. The author therefore supplements the description of potential market with a calculation of supply and demand, as well as projection of out-patient care demands, current demand maximum market potential and MDI which yields the figure of 22,51%-indicating potential growth for health-care market.
Descriptions of the market segment and potential market of the JMC policlinic form the basis for the establishment of the target market as follows: upper middle class patients with high educational background, income and health awareness as well as demand for quality health care. This study also establishes multi-coverage segment as the pattern for the target market, while the JMC positioning against competitor hospitals is that of market follower/challenger. The positioning vis-a-vis customer is one that promises health care services with high human touch.
