Angka kecelakaan kerja di Indonesia yang tergolong tinggi, mengakibatkan adanya urgensi untuk menjaga keselamatan pekerja yang jumlahnya semakin meningkat. Sektor penyumbang kecelakaan kerja terbesar di Indonesia adalah sektor konstruksi, termasuk konstruksi gedung dan menyebabkan kerugiaan fisik maupun finansial yang besar. Kecelakaan kerja biasanya disebabkan oleh kesalahan manusia dan sangat berkaitan dengan reliabilitas manusia. Penggunaan metode Fuzzy Cognitive Reliability and Error Analysis Method (Fuzzy CREAM) ditujukan dalam penelitian untuk menganalisa pola perilaku pekerja yang mempengaruhi resiko kecelakaan kerja digabungkan dengan simulasi Bayesian Network. 176 pekerja konstruksi bangunan di wilayah Jakarta dan sekitarnya menjadi objek penelitian. Nilai Human Error Probability (HEP) berhasil didapatkan dan pemberian solusi dilakukan berdasarkan karakteristik pekerja dalam kategori Control Mode. ......Work injury rates in Indonesia are relatively high, resulting in an urgency to maintain the safety of workers whose numbers are increasing. The biggest sector contributing to work accidents in Indonesia is the construction area, including building construction and causing large physical and financial losses. Workplace accidents are usually caused by human error and are strongly related to human reliability. The use of Fuzzy Cognitive Reliability and Error Analysis Method (Fuzzy CREAM) method is aimed at research to analyze patterns of worker behavior that affect the risk of workplace accidents combined with Bayesian Network simulation. 176 construction workers in the Jakarta and surrounding areas became the object of research. The value of Human Error Probability (HEP) was successfully obtained and the solution was given based on the characteristics of workers in the Control Mode category.

Peningkatan angka kecelakaan kerja di Indonesia menuntut adanya suatu pengembangan pendekatan manajemen kesehatan dan keselamatan kerja bagi para pegawai. Nyatanya, penyebab dari suatu kecelakaan kerja masih didominasi akibat kesalahan tindakan manusia (human error). Human error sangat erat dikaitkan dengan kecelakaan industri dan dalam kasus ini human error yang terjadi pada proses fabrikasi kendaraan khusus (armoured vehicle) yang memiliki lingkungan kerja yang ekstrem. Bercermin pada hal tersebut maka dibutuhkan suatu penelitian untuk mengurangi resiko kecelakaan kerja dengan pendekatan analisis identifikasi dan pengurangan human error. Sebuah pendekatan sistematis yang mengkombinasikan teknik identifikasi serta pengukuran pada human error yang dimulai dengan mengidentifikasi mengidentifikasi setiap task-task yang berlangsung pada proses fabrikasi kendaraan khusus. Kemudian dilakukan identifikasi kegagalan kerja menggunakan metode Systematic Human Error Risk Prediction and Analysis untuk memetakan resiko serta dampak dari setiap rangkaian-rangkaian pekerjaan. Selanjutnya dilakukan pengukuran human error terhadap task-task tersebut dengan menggunakan metode Human Error Analysis and Reduction Technique (HEART) serta metode Standardized Plant Analysis Risk Human Reliability Assessment (SHERPA) yang masing-masing memiliki pendekatan yang relevan dalam menganalisis kondisi serta faktor yang mempengaruhi kegagalan dari sebuah pekerjaan. Terakhir dilakukan analisa pola kecelakaan kerja dengan metode Fault Tree Analysis (FTA). Pendekatan ini menghasilkan identifikasi kegagalan kerja secara mendetail, pengukuran human error yang relevan, serta memberikan rekomendasi dan prioritas penanggulangan kecelakaan kerja dalam manajemen kesehatan dan keselamatan kerja pada fabrikasi kendaraan khusus. ......The increasing number of work accidents in Indonesia demands a developed approach to management of occupational health and safety. In fact, the cause of an accident is still dominated by the improper work of human actions (human error). Human error is always related to industrial accidents, and in this case the human error that occurs in the fabrication process of armored vehicle that has an extreme working environment. Referring from that condition, a research to reduce the risk of workplace accidents with identification and reduction of human error analysis approaches must be needed. A systematic approaches that combine identification technique to human error start from identification of every tasks in fabrication process of armoured vehicle. Then, work failure is identified by using Systematic Human Error Risk Prediction and Analysis method to measure the risk and impact from every tasks sequences. Then, assessment of human error conducted for every tasks with Human Error Analysis and Reduction Technique (HEART) method and Standardized Plant Analysis Risk Human Reliability Assessment (SHERPA) method that have their own relevant approaches in order to analyze the conditions and factors that influence the failure of a tasks. Last step is to mapping the pattern of work accident using Fault Tree Analysis (FTA). This approaches resulting in detailed work failure identification, relevant human error measurement, and finding out the recommendation dan prioritize remedial action for reducing the work accident in occupational health and safety management for fabrication process of armoured vehicle.

Human Reliability Assessment (HRA) merupakan metode kualitatif dan kuantitatif untuk mengukur kontribusi manusia terhadap sebuah resiko. Skripsi ini membahas mengenai Human Reliability Assessment (HRA) pada proses printing kemasan plastic fleksibel pada PT. SMPI menggunakan metode Human Error Assessment and Reduction Technique (HEART) yang bertujuan untuk mengetahui nilai Human Error Probability (HEP). Penelitian ini adalah penelitian semikuantitatif dengan observasi langsung pada operator proses printing artikel MPPS pada perusahaan kemasan menggunakan mesin GR6. Nilai HEP tertinggi terdapat pada proses mesin yang sedang menjalankan proses pencetakan, kemungkinan kesalahan yang mungkin terjadi adalah operator lalai/membiarkan mesin produksi menyala. Dalam proses ini terdapat kemungkinan 65,8% human error. Dilanjutkan dengan pengukuran dan sinkronisasi jarak guide roller diatas silinder proses dimana kemungkinan kesalahan yang terjadi adalah gagal mengukur silinder, pada proses ini terdapat kemungkinan 60% human error. Dan yang ketiga, pada proses reverse check/surface print, size check (flange hole slope, raster depth, raster angle), dan Roughness check (HV-HARDNESS VICKERS) masing-masing memiliki probabilitas error yang sama yaitu sebesar 55,6 %. ......Human Reliability Assessment (HRA) is a qualitative and quantitative method to measure human contribution to a risk. This thesis discusses the Human Reliability Assessment (HRA) in the flexible plastic packaging printing process using the Human Error Assessment and Reduction Technique (HEART) method which aims to determine the value of Human Error Probability (HEP). This research is a semi- quantitative study with direct observation of operators of the MPPS article printing process at packaging companies using GR6 machines. The highest HEP value is found in the machine process that is running the printing process, the possibility of an error that might occur is the operator neglecting/letting the production machine turn on. In this process there is a 65.8% possibility of human error. Followed by measuring and synchronizing the distance of the guide roller above the process cylinder where the possible error that occurs is failure to measure the cylinder, in this process there is a possibility of 60% human error. And thirdly, in the reverse check/surface print process, size check (flange hole slope, raster depth, raster angle), and Roughness check (HV-HARDNESS VICKERS) each have the same error probability of 55.6%.

Skripsi ini membahas mengenai Human Reliability Assessment (HRA) pada proses pembangunan Jembatan Penyeberangan Orang (JPO) pada Proyek Stasiun LRT PT X menggunakan metode Human Error Assessment and Reduction Technique (HEART) dengan tujuan untuk mengetahui Human Error Probability (HEP) yang terdapat dalam proses pekerjaan pengeboran di pembangunan JPO. Penelitian ini adalah penelitian semikuantitatif dengan desain studi deskriptif yang dilakukan dengan observasi langsung proses pekerjaan pengeboran oleh 90 orang pekerja tukang bangunan di Stasiun LRT Bekasi Barat, Ciracas, dan Harjamukti. Hasil penelitian berupa jumlah HEP, dimana nilai HEP tertinggi di Stasiun LRT Bekasi Barat terdapat pada tugas Penentuan Titik Bor dengan jumlah 71,68% kemungkinan terjadinya kesalahan manusia, di Stasiun LRT Ciracas terdapat pada tugas Pengeboran dengan jumlah 49,87%, dan di Stasiun LRT Harjamukti terdapat pada tugas Pemasangan Pipa Tremi dengan jumlah 74,68% serta menyarankan adanya pengendalian dari jenis error serta kondisi yang memungkinkan error (EPCs) untuk mencegah kejadian human error yang dapat mengakibatkan kecelakaan kerja. ......This thesis discusses the Human Reliability Assessment (HRA) in the construction process of the pedestrian bridge at PT X LRT Station Project using Human Error Assessment and Reduction Technique (HEART) with the aim of knowing the Human Error Probability (HEP) contained in the drilling work process in the construction. This research is a semi-quantitative research with a descriptive study design conducted by direct observation of the drilling work process with 90 construction workers as the sample at West Bekasi, Ciracas, and Harjamukti LRT Stations. The results of the study are the number of HEP, where the highest HEP value at the West Bekasi LRT Station is in the Drill Point Determination task with a total of 71.68% the possibility of human error, at the Ciracas LRT Station there is in the Drilling task with a total of 49.87%, and at the Ciracas LRT Station the drilling task is 49.87%. The Harjamukti LRT is found in the Tremi Pipe Installation task with a total of 74.68% and suggests controlling the types of errors and conditions that allow errors (EPCs) to prevent human error events that can lead to work accidents.

Penelitian ini mengkaji aspek keandalan manusia dalam penerapan Kesehatan dan Keselamatan Kerja (K3) pada pabrik susu bayi terbesar di Jakarta dengan mengevaluasi potensihuman error pada proses produksi menggunakan metode Hierarcical Task Analysis, Fault Tree Analysis dan membandingkanmetode pengukuran keandalan manusia antara metode HEART (Human Error Assessment and Reduction Technique) dan metode SPAR-H (Standardized Plant Analysis Risk Human Reliability Assessment). Hasil penelitian ini yaitu berupaHuman Error Probabilitiesdimana resiko human error terbesar terjadi pada area Tippingdengan pengukuran metode HEART dan area Preparation dengan pengukuran metode SPAR-H.Penelitian ini juga membandingkan penggunaan metode HEART dan SPAR-H.
Abstract
This research studies human Reliability in safety application atthe biggest baby nutrition factory in Jakarta by evaluatig the risk of human error in production line. The methods in this research are Hierarcical Task Analysis (HTA), Fault Tree Analysis (FTA), and also compared Reliability assessment methods between HEART (Human Error Assessment and Reduction Technique) and SPAR-H (Standardized Plant Analysis Risk Human Reliability Assessment. The research result are Human Error Probabilities that shows the biggest human error risk are in Tipping area based on assessment by HEART and in Preparation area based on assessment by SPAR-H. HEART and SPAR-H.This result also compare the application of HEART and SPAR-H.
