"ABSTRAKApotek adalah sarana pelayanan kefarmasian tempat dilakukan praktek kefarmasian oleh apoteker. Apoteker di apotek dituntut untuk memahami dan menyadari kemungkinan terjadinya kesalahan pengobatan dalam proses pelayanan dan mengidentifikasi, mencegah, serta mengatasi masalah terkait obat, masalah farmakoekonomi, dan farmasi sosial. Oleh karena itu, untuk menghindari kesalahan tersebut, apoteker harus menjalankan praktek pelayanan kefarmasian berdasarkan standar yang telah ditetapkan pada Peraturan Menteri Kesehatan No. 73 Tahun 2016 tentang Standar Pelayanan Kefarmasian di Apotek. Salah satu bentuk pelayanan kefarmasian adalah pengkajian resep. Pengkajian resep yang dilakukan dengan benar oleh apoteker akan mencegah terjadinya kesalahan dalam pengobatan pasien. Pengkajian resep terhadap penyakit reumatik dipilih sebagai topik tugas khusus. Pengkajian resep meliputi kajian administratif, kajian kesesuaian farmasetik, dan pertinbangan klinis.

ABSTRACTPharmacy is a place where pharmaceutical practice is performed by pharmacists. Pharmacists at the pharmacy is required to comprehend and be aware of medication error possibilities in the service process and identify, prevent, and overcome drug related problems, pharmacoeconomic issues, and social pharmaceuticals problems. Therefore, to avoid medication error, pharmacists must perform the pharmaceutical practice based on the pharmaceutical practice standard that set forth in Peraturan Menteri Kesehatan No. 73 Tahun 2016 tentang Standar Pelayanan Kefarmasian di Apotek. One of pharmaceutical practice is prescription assessment. The assessment of prescriptions that performed correctly by pharmacist will prevent medication error. The assessment of rheumatic prescription was chosen as a specific task topic. Prescription studies included administrative studies, pharmaceutical conformity studies, and clinical considerations."

"Salah satu standar pelayanan kefarmasian yang dilakukan di apotek adalah pengkajian resep. Pengkajian resep bertujuan untuk menganalisis adanya masalah yang terkait dengan peresepan obat. Masalah terkait obat yang mungkin terjadi akibat kesalahan penulisan resep diantaranya obat yang tidak tepat, dosis kurang atau berlebih, alergi obat, inkompatibilitas obat, interaksi obat, duplikasi pengobatan, serta penggunaan obat yang tidak tepat. Prevalensi diabetes mellitus di Indonesia mengalami peningkatan dari 1,5% pada tahun 2013 menjadi 2,0% pada tahun 2018 berdasarkan diagnosis dokter pada penduduk umur ≥ 15 tahun. Penanganan penyakit diabetes dengan cara pendekatan pasien memerlukan kolaborasi tenaga kesehatan, dalam hal ini dokter sebagai penentu diagnosa, apoteker mendampingi khususnya dalam terapi obat merupakan salah satu tugas profesi kefarmasian. Proses pengkajian dilakukan dengan observasi dan studi literatur. Pengkajian resep secara administratif yang dilakukan terhadap resep terdapat beberapa aspek administratif yang tidak lengkap, seperti jenis kelamin, berat badan pasien, dan nomor telepon dokter tidak tercantum pada resep. Hal ini dapat diatasi dengan konfirmasi oleh petugas farmasi atau apoteker. Berdasarkan pengkajian aspek klinis terdapat adanya polifarmasi dan interaksi obat. Hal tersebut dapat diatasi dengan memberikan edukasi atau pemberian informasi obat kepada pasien.

One of the standard pharmaceutical services carried out in pharmacies is prescription review. The prescription review aims to analyze the existence of problems related to drug prescribing. Drug-related problems that may occur due to prescription errors include incorrect medication, insufficient or excessive dosage, drug allergies, drug incompatibility, drug interactions, duplication of medication, and inappropriate drug use. The prevalence of diabetes mellitus in Indonesia has increased from 1.5% in 2013 to 2.0% in 2018 based on doctor's diagnosis in residents aged ≥ 15 years. Handling diabetes using a patient approach requires collaboration between health workers, in this case the doctor as the determiner of the diagnosis, the pharmacist assisting, especially in drug therapy, is one of the duties of the pharmaceutical profession. The administrative review of prescriptions carried out on prescriptions contained several administrative aspects that were incomplete, such as the patient's gender, weight, and the doctor's telephone number not being listed on the prescription. This can be overcome by confirmation by a pharmacy officer or pharmacist. Based on the clinical aspect assessment, there is polypharmacy and drug interactions. This can be overcome by providing education or providing drug information to patient."

"Praktik kerja profesi di Apotek Kimia Farma Nomor 366 Depok Periode Bulan September Tahun 2017 bertujuan untuk memahami tugas serta tanggung jawab apoteker dalam mengelola apotek, dan melaksanakan praktik pelayanan kefarmasian sesuai dengan ketentuan perundang-undangan dan etika yang berlaku. Selain itu praktik kerja ini juga memberikan wawasan, pengetahuan, dan keterampilan serta pengalaman praktis dalam melakukan praktek kefarmasian di apotek, memberikan gambaran nyata tentang permasalahan praktek kefarmasian serta mempelajari strategi dan kegiatan-kegiatan yang dapat dilakukan dalam rangka pengembangan praktek kefarmasian. Praktik kerja profesi di Apotek Kimia Farma Nomor 366 Depok dilakukan selama empat minggu dengan tugas khusus yaitu ldquo;Analisis Resep Obat Hipertensi di Apotek Kimia Farma No. 366 Depok rdquo;. Tujuan dari tugas khusus ini adalah untuk memahami peran apoteker dalam pengkajian resep terutama obat hipertensi yang merupakan kasus terbanyak di apotek tersebut, agar tujuan terapi dapat tercapai yaitu pasien dapat sembuh dari penyakitnya dan meningkatkan kualitas hidup pasien.

This internship also render an insight, knowledge, skills and practical experience to undertake pharmaceutical practices in pharmacies, providing the real condition of pharmaceutical practice issues and learn the strategies and activities that can be undertaken in the course of pharmaceutical practice development. Internship at Kimia Farma Pharmacy Number 366 Depok was conducted for four weeks with special assignment ldquo;Analysis of Prescription for Hypertension medication at Kimia Farma Pharmacy Number 366 Depok rdquo;. The objective of this special assignment is to comprehend the role of pharmacists in prescription assessment, especially hypertension which is the most cases in this pharmacies, so that the goal of therapy can be achieved that is the patient can recover from the illness and improve the patient life quality. "Praktek kerja profesi di Apotek Kimia Farma No. 202 Depok dilaksanakan selama satu bulan mulai dari tanggal 3 Agustus 2016 sampai dengan tanggal 31 Agustus 2016. Praktek kerja profesi ini bertujuan agar mahasiswa profesi apoteker memahami tugas dan tanggung jawab apoteker dalam pengelolaan apotek serta mendapatkan pengetahuan dan pengalaman praktis untuk melakukan praktek kefarmasian di apotek. Tugas khusus yang diberikan berjudul Pengkajian Resep Kombinasi Obat Tamsulosin, Terazosin, Nifedipin, dan Candesartan pada Pasien dengan Penyakit Benign Prostatic Hyperplasia BPH dan Hipertensi. Profession internship at Apotek Kimia Farma No. 202 Depok was held for one month started from August, 3rd 2016 until August, 31st 2016. This profession internship was intended to make apothecary students understand roles and responsibilities of pharmacists in pharmacy management and also to obtain knowledge and practical experience to do pharmaceutical practice at pharmacy. The internship given a special assignment titled Prescription Assessment of Tamsulosin, Terazosin, Nifedipine and Candesartan Drug Combination in Patient with Benign Prostatic Hyperplasia BPH and Hypertension Diseases. The purpose of the special assignment was to determine the suitability of medications received to the patient 39 s diseases and to know the advantages and disadvantages of the use of the drug combination."

"Polifarmasi merupakan suatu kejadian penggunaan lima obat atau lebih oleh seorang pasien secara bersamaan. Kondisi polifarmasi umumnya ditemukan pada populasi lansia dengan penyakit kronis. Dari beberapa penyakit kronis, hipertensi merupakan penyakit kronis yang paling banyak ditemukan di seluruh fasilitas pelayanan kesehatan. Hipertensi adalah sindrom atau kumpulan gejala kardiovaskuler yang progresif dan ditandai dengan adanya peningkatan tekanan darah diatas 140/90 mmHg. Polifarmasi yang tepat membutuhkan peresepan obat secara tepat dan cermat, sehingga pasien dapat menunjukkan outcome klinis yang baik. Oleh karena itu, diperlukan sebuah proses identifikasi resep polifarmasi melalui kegiatan pengkajian resep di fasilitas pelayanan kesehatan. Pengkajian resep merupakan salah satu bentuk pelayanan kefarmasian di apotek. Kegiatan pengkajian resep meliputi kajian administratif, kesesuaian farmasetik dan pertimbangan klinis. Pengkajian atau skrining resep merupakan salah satu kegiatan yang bertujuan untuk mengidentifikasi, mencegah, dan mengatasi kesalahan pengobatan (medication error) atau Drug Related Problem (DRP). Adapun tujuan dari penelitian ini adalah untuk mengidentifikasi dan mengkaji resep polifarmasi pada pasien dengan di Apotek Roxy Poltangan September 2023. Metode yang digunakan dalam penelitian ini adalah dengan melakukan observasi dan analisa resep polifarmasi yang didalamnya tercantum obat hipertensi. Hasil penelitian ini menunjukkan bahwa secara aspek pengkajian administratif, farmasetik, dan klinis pada resep sudah cukup baik akan tetapi ditemukan DRP pada aspek keamanan terapi dikarenakan terdapat kombinasi pengobatan yang kurang tepat yakni simvastatin dan amlodipin.

Polypharmacy is the simultaneous use of five or more medications by a patient. This condition is commonly found in the elderly population with chronic diseases. Among various chronic diseases, hypertension is the most prevalent in all healthcare facilities. Hypertension is a syndrome or a progressive cardiovascular condition characterized by elevated blood pressure above 140/90 mmHg. Appropriate polypharmacy requires precise and careful medication prescription so that patients can achieve good clinical outcomes. Therefore, a process of identifying polypharmacy prescriptions through prescription assessment activities in healthcare facilities is necessary. Prescription assessment is a form of pharmaceutical service in pharmacies. The activities involved in prescription assessment include administrative review, pharmaceutical suitability, and clinical considerations. Prescription assessment or screening aims to identify, prevent, and address medication errors (medication errors) or Drug Related Problems (DRP). The purpose of this study is to identify and assess polypharmacy prescriptions in patients with hypertension at Roxy Poltangan Pharmacy in September 2023. The method used in this study involves observation and analysis of polypharmacy prescriptions that include hypertension medications. The results of this study indicate that in terms of administrative, pharmaceutical, and clinical aspects, the prescriptions were quite good; however, a DRP was found in the therapeutic safety aspect due to an inappropriate drug combination of simvastatin and amlodipine."

"Pengkajian resep termasuk dalam standar pelayanan kefarmasian di apotek. Kegiatan ini dilakukan untuk menghindari kesalahan pemberian obat (medication error). Apotek Kimia Farma 202 memiliki layanan pengkajian resep yang mencakup aspek administrasi, kesesuaian farmasetik, dan pertimbangan klinis. Diabetes Melitus (DM) adalah penyakit metabolik kronis ketika pankreas tidak mampu memproduksi cukup insulin atau ketika tubuh tidak mampu menggunakan insulin yang diproduksi secara efektif, yang menyebabkan peningkatan kadar gula darah. Terapi DMT2 didasarkan pada kondisi gula darah pasien, faktor pembiayaan, ketersediaan obat, efektivitas, efek samping, serta preferensi pasien. Pengkajian resep dilakukan pada resep yang ditebus di Apotek Kimia Farma 202 pada periode pelaksanaan Praktek Kerja Profesi Apoteker (PKPA), dan dipilih dua resep untuk laporan ini. Pada kedua resep tersebut, terdapat beberapa informasi yang belum tercantum seperti usia, berat badan pasien, dan SIP dokter dalam aspek administratif. Obat-obat yang diberikan tidak menunjukkan interaksi yang merugikan, baik pada resep 1 dan resep 2. Terdapat beberapa obat yang sebaiknya dihindari oleh pasien dengan riwayat hipersensitivitas, sehingga apoteker perlu melakukan konfirmasi terkait riwayat alergi ke pasien atau keluarga pasien. Informasi dari pengkajian aspek kesesuaian farmasetik dan pertimbangan klinis dapat sangat berguna untuk disampaikan saat penyerahan obat, sehingga dapat mengoptimalkan terapi. Pelayanan yang diberikan Apotek Kimia Farma 202 telah berjalan dengan baik. Pengkajian resep perlu terus dilakukan agar kesahalan dalam pengobatan pengobatan dapat dihindari.

The therapy for T2DM is based on the patient's blood sugar condition, financial factors, drug availability, effectiveness, side effects, and patient preferences. Prescription assessment was conducted for prescriptions redeemed at Kimia Farma 202 Pharmacy during the implementation period of the Pharmacist Professional Internship Program, and two prescriptions were selected for this report. In both prescriptions, there was some missing information in the administrative aspect, such as the patient's age, weight, and the doctor's License to Practice. The medications prescribed did not show any harmful interactions, both in prescription 1 and prescription 2. Some drugs should be avoided by patients with a history of hypersensitivity, so pharmacists need to confirm allergy history with the patient or the patient's family. Information from the assessment of pharmaceutical suitability and clinical considerations can be very useful when giving the medication to optimize therapy. The services provided by Kimia Farma 202 Pharmacy have been running smoothly. Continuous prescription assessment is necessary to avoid errors in medication administration."

"Medication error merupakan kejadian yang merugikan pasien akibat pemakaian obat selama dalam penanganan tenaga kesehatan, yang sebetulnya dapat dicegah. Pengkajian resep merupakan hal yang penting dalam mencegah terjadinya kesalahan pemberian obat (medication error). Pengkajian resep wajib dilakukan oleh apoteker pada alur pelayanan resep yang meliputi kajian administratif, kajian kesesuaian farmasetik, dan kajian pertimbangan klinis. Data yang terdapat pada tugas khusus ini diambil berdasarkan resep pada periode November 2021 di Apotek Roxy Pitara. Pengumpulan data dilakukan dengan cara mengklasifikasikan resep yang diterima oleh Apotek Roxy Pitara berdasarkan tanggal diterimanya resep masuk dan kemudian diambil secara acak sebanyak tiga resep. Selanjutnya dilakukan pengkajian terhadap tiga resep yang telah diambil secara acak, meliputi kajian administratif, kajian kesesuaian farmasetik, dan kajian pertimbangan klinis. Hasil pengkajian resep menunjukkan bahwa tidak terdapat resep yang memenuhi seluruh aspek administratif. Seluruh resep tidak mencantumkan berat badan pasien dan paraf dokter. Dari hasil kajian kesesuaian farmasetik, terdapat dua resep (resep 2 dan resep 3) yang memenuhi seluruh aspek kesesuaian farmasetik. Satu resep lainnya (resep 1) tidak memenuhi aspek kesesuaian farmasetik karena tidak mencantumkan kekuatan sediaan. Dari hasil kajian pertimbangan klinis, tidak terdapat resep yang memenuhi seluruh aspek pertimbangan klinis. Dua resep (resep 1 dan resep 3) tidak memenuhi aspek pertimbangan klinis karena tidak mencantumkan berat badan pasien anak, sehingga tidak dapat diketahui ketepatan dosis obat yang diberikan serta terdapat satu resep (resep 2) yang memiliki interaksi obat. Industri farmasi adalah badan usaha yang memiliki izin dari menteri kesehatan untuk melakukan kegiatan pembuatan obat atau bahan obat. Industri farmasi wajib memenuhi persyaratan CPOB. Salah satu area yang terdapat di industri farmasi adalah area produksi. Terdapat beberapa hal yang perlu diperhatikan pada area produksi, diantaranya adalah sistem tata udara yang memegang peran penting dalam industri farmasi untuk memberikan perlindungan terhadap lingkungan pembuatan produk, memastikan produksi obat yang bermutu serta memberikan lingkungan kerja yang nyaman bagi personil. Setiap industri farmasi perlu melakukan pemantauan terhadap sistem tata udara. Hasil dari pemantauan didokumentasikan dalam bentuk tren data. Metode yang digunakan dalam penelitian ini adalah metode retrospektif yang diperoleh dari data BAS (Building Automatic System) meliputi data suhu, kelembaban nisbi, dan perbedaan tekanan udara di Lantai 4 Area Produksi Solid (Kelas E) Non Sefalosporin PT Mahakam Beta Farma periode Januari – Juni 2021. Pengumpulan data dilakukan dengan cara mencatat data suhu, kelembaban nisbi, dan perbedaan tekanan udara. Selanjutnya dilakukan pengolahan data yang dilakukan dengan cara memplotkan data ke dalam borang statistik (Microsoft Excel) dan dilanjutkan dengan pembuatan grafik untuk mengetahui pola serta pergerakan tren dari data suhu, kelembaban nisbi, dan perbedaan tekanan udara yang telah dipantau serta posisi dari data terhadap control limit atau spesifikasinya. Kemudian dilakukan evaluasi yang berisi hasil review dan evaluasi terhadap data yang diperoleh serta dibuat kesimpulan. Hasilnya sebagian besar memenuhi persyaratan, baik suhu, kelembaban nisbi, maupun perbedaan tekanan udara dengan rincian masing-masing yaitu suhu ≤25 ESCIC, kelembaban nisbi ≤65%, dan perbedaan tekanan udara ≥5 Pa. Namun, terdapat dua data yang melewati rentang specification limit, yaitu data kelembaban nisbi AHU 4.7 dan data perbedaan tekanan udara AHU 4.4, sehingga insiden/deviasi harus dibuat.

Prescription assessment is important in preventing medication errors. Prescription assessment must be carried out by pharmacists in the flow of prescription services that include administrative studies, pharmaceutical suitability studies, and clinical consideration studies. The data contained in this particular task were taken on the basis of prescriptions in the November 2021 period at the Apotek Roxy Pitara. The data collection was done by classifying the prescriptions received by Apotek Roxy Pitara based on the date of incoming prescriptions and then randomly taken three prescriptions. Furthermore, an assessment of three prescriptions that have been taken randomly, including administrative studies, pharmaceutical suitability studies, and clinical consideration studies. The results of the prescription review show that there is no prescription that meets all administrative aspects. The entire prescription does not list the patient's weight and the doctor's signature. From the results of the study of pharmaceutical suitability, there are two prescriptions (prescription 2 and prescription 3) that meet all aspects of pharmaceutical suitability. Other prescription (prescription 1) does not meet the pharmaceutical suitability aspect because it does not include the strength of the preparation (dosage). From the results of the clinical consideration study, there is no prescription that meets all aspects of clinical consideration. Two prescriptions (prescription 1 and prescription 3) do not meet the aspect of clinical consideration because it does not include the weight of the pediatric patient, so it cannot be known the accuracy of the dose of the drug given and there is one prescription (prescription 2) that has drug interactions.

The pharmaceutical industry is obliged to meet the requirements of GMP. One of the areas in the pharmaceutical industry is the production area. There are several things that need to be considered in the production area, including air conditioning systems that play important role in the pharmaceutical industry to provide protection for the product manufacturing environment, ensure quality drug production and provide comfortable working environment for personnel. Every pharmaceutical industry needs to monitor the air system.

The results of monitoring are documented in the form of data trends. The method used in this study is a retrospective method obtained from BAS (Building Automatic System) data including temperature, relative humidity, and air pressure differences data on the 4th floor of the Non Cephalosporin Solid Production Area (Class E) of PT Mahakam Beta Farma for the January – June 2021 period. Data collection is done by recording temperature, relative humidity, and air pressure differences data. Furthermore, data processing is done by plotting the data into a statistical form (Microsoft Excel) and followed by making graphs to determine the pattern and trend movement of temperature, relative humidity, and air pressure differences data that have been monitored and the position of the data against the control limit or specifications limit. Then conducted an evaluation that contains the results of the review and evaluation of the data obtained and made conclusions. The results mostly meet the requirements, both temperature, relative humidity, and air pressure differences with the details of each temperature is ≤25 ESCIC, relative humidity ≤65%, and air pressure difference ≥5 Pa. However, there are two data that pass through the specification limit range, namely relative humidity data AHU 4.7 and air pressure differences data AHU 4.4, so the incident / deviation must be made."
