Abstrak :
Penelitian ini membahas kelengkapan resume medis rawat inap di RS Umum Hermina Depok. Tujuan penelitian ini adalah mendapatkan gambaran hasil analisis kelengkapan resume medis rawat inap. mendapatkan gambaran hasil analisis kelengkapan resume medis rawat inap. Jenis penelitian ini menggunakan kuantitatif dengan desain studi cross sectonal dan kualitatif dengan wawancara mendalam dan terstruktur menggunakan data primer dan data sekunder. Hasil penelitian ini adalah kurangnya pengetahuan, kurang sesuainya sanksi dan insentif, instrumen yang penting, format instrument kolom, huruf, perlengkapan, uji coba, revisi, sosialisasi, kelengkapan, ketidaklengkapan dan dokter penanggung jawab resume medis rawat inap.. Hasil penelitian didapatkan rata-rata resume medis rawat inap lengkap sebesar 84,6% dan yang tidak lengkap sebesar 15,4%. Hal ini menunjukkan bahwa standar kelengkapan resume medis rawat inap masih belum sesuai RS Umum Hermina Depok sebesar ≥ 95%. Kurangnya sosialisasi terkait peraturan resume medis kepada para dokter merupakan salah satu hambatan dalam pengisian resume medis rawat inap masih belum lengkap.

This research discuss about the completeness of Inpatient Medical Resume at Hermina Depok Hospital. The purpose of this research is to get the illustration of analysis the completeness of inpatient medical resume. The research uses quantitative method with cross sectional design and qualitative by in-depth interview and structural interview by using both primer and secondary data. The result of this research are less knowledge, inappropriate sanction and incentive, important items. Column instrument format, character, tools, trial test, revision, socialization, completeness, incompleteness, and physician responsible due to inpatient medical resume. Concludes that the average of completeness inpatient medical resume is about 84,6% and discompleteness medical resume is about 15,4%. It shows that the completeness of inpatient medical resume yet with the standard of Hermina Hospital Depok which is about >95%. The minimum socialization about the rule of medical resume to the physician become one of obstacle when filling inpatient medical resume still not complete.

Kanker merupakan salah satu masalah kesehatan masyarakat. Kanker membutuhkan penanganan yang lama dan berkelanjutan. Terhadap kondisi tersebut, ketersediaan data dan informasi bagi penyedia layanan dan pasien kanker membutuhkan penyelenggaraan rekam medis. Kanker memelurkan manajemen data khusus selain yang sudah terdapat dalam rekam medis. Sehingga, Rancangan Kesehatan Elektronik (RKE) Oncology menjadi salah satu upaya untuk mengelola data kanker dan alur kerjanya. Penelitian ini menggunakan metode penilaian sistem atau system assesment dengan teknik wawancara, observasi, dan telaah dokumen untuk membuat rancangan rekam kesehatan elektronik pada pasien kanker di RS Hermina Depok. Dari hasil pengumpulan data, dilakukan analisis sistem yang meliputi input, proses, output, analisis kebutuhan sistem. Penelitian juga membuat rancangan prototipe dalam bentuk antarmuka aplikasi. Hasil dari penelitian menunjukkan bahwa adanya peluang dalam rancangan kesehatan elektronik pada pasien kanker. Dengan prototipe Rancangan Rekam Kesehatan Elektronik, proses pengumpulan data menjadi lebih cepat dan menghasilkan informasi yang lebih berkualitas. Selain itu, diharapkan dapat menyediakan data yang dibutuhkan untuk layanan kanker.

ABSTRACT
Cancer is a public health problem. Cancer treatment requires a long and sustained. Againts these conditions, the availability of data and information for service providers and cancer patients in need of medical record implementation. Cancer require specialized data management beyond those already contained in the medical record. Thus, design of electronic health record become one of the efforts to manage data of cancer and workflow too. This study uses a method or system assessment by interview, observation, and document analysis to makes design of electronic health record for cancer patient in Hermina Hospital Depok. From the results of data collection, analysis system that includes input, process, output, system requirements analysis. The research also makes design of a prototype application interface. Results of the the study indicate that there are opportunities in the design of electornic health record for cancer patient. With a prototype design of electronic health record, data collection process becomes faster and produce higher quality information. In addition, is expected to provide the data required for cancer services.

Data yang digunakan adalah data primer yang dikumpulkan melalui wawancara kepada tujuh informan, melakukan observasi dan telaah dokumen. Dalam usaha untuk meningkatkan kepuasan pelanggan, rumah sakit dituntut untuk selalu melakukan perbaikan dalam pemberian pelayanan bermutu dengan meningkatkan kualitas kerja untuk memenuhi harapan pelanggan. Pelayanan yang bermutu bukan hanya dilakukan pada pelayanan medis saja, tetapi juga pada pelayanan penunjang salah satunya yaitu penanganan rekam medis di rumah sakit yang menjadi salah satu indikator mutu rumah sakit yang dapat diketahui melalui kelengkapan pengisian rekam medis. Penelitian ini bertujuan untuk mengetahui faktor-faktor yang mempengaruhi kelengkapan pengisian berkas rekam medis pasien rawat inap RS Hermina Depok Tahun 2018. Jenis penelitian ini adalah kuantitatif dan kualitatif dengan metode telaah dokumen, telaah data dan wawancara mendalam. Hasil penelitian menunjukan bahwa dari 60 sampel penelitian, ditemukan bahwa 78,3 berkas rekam medis pasien rawat inap tersebut belum terisi dengan lengkap. Selanjutnya terlihat ada hubungan antara usia dokter, pendidikan dokter dan status kepegawaian dokter dengan kelengkapan pengisian berkas rekam medis pasien rawat inap. Namun tidak ditemukan keberpengaruhan sarana dan prasarana, SPO dan biaya dengan kelengkapan pengisian berkas rekam medis pasien rawat inap di RS Hermina Depok. ......In an attempt to increase customer satisfaction, hospitals are required to always repair in provision pf service quality by improving the quality of work to meet the expectations of customers. Quality service do not only on medical section, but also on the supporting one of them is handling medical record in the hospital that was one of indicator of the quality hospital that can known through completeness filling medical record. Research aims to understand factors affecting completes charging file record medical patient in patient Hermina Hospital Depok 2018. The kind of research is quantitive and qualitative with the methods review of documentation, review of data and in depth interviews. The result of the study showed that from 60 sample, that found 78,3 file medical record inpation has yet fill with complete. There was a correlation between the ages of doctor, education of doctor and the doctors employement status completeness filling medical record inpatiens, but there was not found correlation of facilities and infrastructure, standard operating procedur and financing with completeness charging record file medical inpatient in Hermina Hospital Depok.

Abstrak :
This research discribes about time service analysis of compounding and non compounding medicines in hermina with lean sigma approach ininstallation of outpatient in hermina depok. Pharmacy time service is the important part of the hospital excellent services and lean six sigma is a methodology of decreasing the waste and variation. Thjs reaserch is a an operational research where the researcher did a direct counting in each process of compounding and non compounding prescriptions. Lean six sigma method which has been used were define, measure, analyze and improve. This reaserch aims to propose a better visual management application, 5s culture, lay out changing and electronic prescription and heijunka prescription as a long term porpuse. Using of lean six sigma method could decrease the waste of non compounding prescription service from 43 procedures to 32 procedures with time allocation from 31 minutes and 51 seconds to 16 minutes 34 seconds. For the compounding prescription could also decrease the procedures, from 32 steps to 46 steps with time allocationfrom 47 minutes and 31 seconds to 25 minutes and 7 seconds.
