"Drug Related Problem (DRPs) adalah kesalahan pengobatan yang mempengaruhi pada outcome klinis dari pasien. Jika tidak dikelola dengan baik, mengakibatkan dampak yang lebih luas dan serius bahkan sampai terjadi kejadian sentinel. Di Puskesmas Kecamatan Kalideres terdapat poli PTM yang melayani pasien dengan penyakit kronis seperti hipertensi, diabetes melitus tipe 2, hiperglikemia dan sebagainya. Pasien poli PTM ini biasa mendapat resep yang berisikan obat polifarmasi yang berisiko terjadi Drug Related Problems (DRPs). Oleh karena itu pada penulisan tugas khusus ini dilakukan pengidentifikasian jenis DRP yang terjadi pada pasien poli PTM di Puskesmas Kecamatan Kalideres pada periode April 2023. Metode yang digunakan adalah pengklasifikasi DRP berdasarkan Hepler-Strand dan kemudian direkomendasikan penyelesaian permasalahan berdasarkan literatur. DRPs menurut Hepler-Strand diklasifikasikan sebagai berikut: 1) Pemilihan obat yang tidak tepat; 2) Dosis Subterapeutik; 3) Kegagalan menerima obat; 4) Overdosis; 5) Reaksi obat yang merugikan; 6) Interaksi obat ; 7) Penggunaan obat tanpa indikasi dan; 8) Indikasi yang tidak diobati. Berdasarkan hasil validasi yang dilakukan dapat disimpulkan bahwa dari 5 resep pasien Poli PTM di Puskesmas Kecamatan Kalideres yang diindentifikasi DRPs, terdapat 1 resep yang tidak terdapat DRP dan 4 resep lainnya terdapat DRP. DRP yang didapat adalah terjadinya interaksi obat dan penggunaan obat tanpa indikasi.

Drug Related Problems (DRPs) are medication errors that affect the clinical outcomes of patients. If not managed properly, it can have a wider and more serious impact and even lead to sentinel events. At the Kalideres Sub-district Health Center, there is a NCD clinic that serves patients with chronic diseases such as hypertension, type 2 diabetes mellitus, hyperglycemia and so on. These NCD poly patients usually receive prescriptions containing polypharmacy drugs that are at risk of Drug Related Problems (DRPs). Therefore, in this special assignment writing, the types of DRPs that occurred in Poly PTM patients at the Kalideres District Health Center in the April 2023 period were identified. The method used is DRP classification based on Hepler-Strand and then recommended problem solving based on the literature. DRPs according to Hepler-Strand are classified as follows: 1) Inappropriate drug selection; 2) Subtherapeutic dose; 3) Failure to receive the drug; 4) Overdose; 5) Adverse drug reactions; 6) Drug interactions; 7) Use of drugs without indications and; 8) Untreated indications. Based on the results of the validation carried out, it can be concluded that of the 5 prescriptions of PTM Poly patients at the Kalideres Subdistrict Health Center that DRPs were identified, there was 1 prescription that did not have DRPs and 4 other prescriptions had DRPs. DRPs obtained are the occurrence of drug interactions and the use of drugs without indications.

"Tuberkulosis (TBC) merupakan penyakit dengan risiko penularan yang tinggi. Ketidakpatuhan terhadap pengobatan menyebabkan respon pengobatan suboptimal sehingga muncul resistensi dan penularan penyakit terus menerus. Indonesia menempati urutan kedua sebagai negara yang memiliki jumlah kasus TBC terbesar setelah India (WHO, 2015). Reaksi obat tidak dikehendaki (ROTD) akibat penggunaan obat antituberkulosis (OAT) dapat mempengaruhi kepatuhan pasien. Tujuan penelitian ini adalah menilai pengaruh ROTD terhadap peningkatan risiko ketidakpatuhan berobat pada penderita TBC paru dewasa kategori I di RS Urip Sumoharjo Lampung. Desain penelitian yang digunakan adalah kasus kontrol dengan penggunaan data rekam medik. Kelompok kasus adalah pasien TBC paru dewasa kategori I yang tidak patuh dan ditandai dengan kejadian putus berobat, yaitu pasien TBC paru yang tidak memulai pengobatan atau menghentikan pengobatan selama dua bulan berturut-turut atau lebih. Kelompok kontrol adalah pasien dengan pengobatan lengkap dan dinyatakan sembuh. Populasi penelitian ini adalah seluruh rekam medik pasien TBC paru dewasa dengan umur ≥ 15 tahun kategori I yang berobat jalan di RS Urip Sumoharjo Lampung pada bulan Agustus 2013 s.d Juli 2015. Analisis data menggunakan uji chi square dan uji regresi logistik multivariat. Hasil penelitian menunjukkan bahwa pasien yang mengalami ROTD mempunyai risiko 2,143 kali untuk tidak patuh dibandingkan dengan pasien yang tidak mengalami ROTD setelah dikontrol oleh variabel pendidikan, penyakit penyerta, dan jenis OAT. Jenis OAT KDT merupakan faktor risiko terbesar bagi ketidakpatuhan pasien TBC paru dewasa kategori I. Pasien yang mendapatkan OAT jenis KDT berisiko sebesar 11,996 kali untuk tidak patuh dibandingkan dengan pasien TBC paru yang mendapatkan jenis OAT satuan setelah dikontrol oleh variabel ROTD, pendidikan, dan penyakit penyerta.

Tuberculosis is a disease with a high risk of transmission. Poor adherence to treatment led to suboptimal response so it appears resistance and continuous transmission of the disease. In 2015 Indonesia ranks second of countries with the highest number of estimated incident tuberculosis cases after India. Adverse drug reactions of tuberculosis agents increase the risk of poor adherence. The objective of this study was to assess the risk of adult pulmonary TB patients non adherence caused by adverse drug reactions of antituberculosis agents at Urip Sumoharjo Hospital Lampung. This research is an observational study with design case control using data from medical record. Cases were defined as the pulmonary tuberculosis patients who didn?t begin treatment or stop their antituberculosis for 2 month or more. Controls were defined as the pulmonary tuberculosis patients with full treatment and were declared cured. The study population was all medical record of adult pulmonary tuberculosis patient age ≥ 15 years old with category I treatment at Urip Sumoharjo Hospital Lampung period august 2013 ? july 2015.. Data was analyzed by using chi square and multivariate logistic regression method. Result of this study showed that the patient who has adverse drug reactions of antituberculosis agent risk nonadherence 2,143 times higher than patient who has not adverse drug reactions of antituberculosis agent after controlled by educational variables, co-morbidities, and the type of OAT. Fixed dose combination of antituberculosis drug was the highest risk factor for patient non adherence. The patient who received fixed dose combination of antituberculosis drug risk nonadherence 11,996 times higher than patient who received separate antituberculosis drug after controlled by educational variables, co-morbidities, and adverse drug reactions."

"Tuberkulosis (TB) dan Human Immunodeficiency Virus (HIV) menjadi beban utama penyakit menular terutama di negara-negara berpenghasilan rendah. Pada pasien penderita Tuberkulosis dengan adanya infeksi penyerta seperti koinfeksi HIV, diestimasikan akan meningkatkan risiko terjadinya drug-induced liver injury (DILI) akibat OAT sebanyak 4 kali lipat. Dilakukan pemantauan terapi obat ini untuk menganalisis masalah terkait obat (MTO)/ Drug’s Related Problem (DRP) yang terjadi pada pengobatan pasien dan memberikan rekomendasi tindak lanjut menggunakan metode SOAP. Diperoleh hasil bahwa pasien memiliki diagnosis ikterus obstruktif, anemia normositik normokrom, hiponatremia hipoosmolar euvolemik, Dili et causa OAT, HIV on ARV, TB on OAT dan terdapat interaksi obat kategori antara sukralfat dengan dolutegravir yang menyebabkan Sukralfat menurunkan efek dari dolutegravir dengan pengikatan kation dlm saluran GI. Hasil analisis DRP disajikan dalam SOAP sebagai komunikasi tertulis untuk menyampaikan rekomendasi kepada dokter penanggung jawab (DPJP).

Tuberculosis (TB) and Human Immunodeficiency Virus (HIV) are the main burden of infectious diseases, especially in low-income countries. In patients suffering from Tuberculosis with accompanying infections such as HIV co-infection, it is estimated that the risk of drug-induced liver injury (DILI) due to OAT will increase by 4 times. This drug therapy monitoring is carried out to analyze drug-related problems (MTO)/Drug-Related Problems (DRP) that occur during patient treatment and provide follow-up recommendations using the SOAP method. The results showed that the patient had a diagnosis of obstructive jaundice, normochromic normocytic anemia, euvolemic hypoosmolar hyponatremia, Dili et causa OAT, HIV on ARV, TB on OAT and there was a major category of drug interaction between sucralfate and dolutegravir which caused Sucralfate to reduce the effect of dolutegravir by binding cations in GI tract. The results of the DRP analysis are presented in SOAP as written communication to convey recommendations to the doctor in charge (DPJP).

"Pemantauan terapi obat (PTO) merupakan kegiatan farmasi klinik yang dilakukan oleh seorang apoteker yang mencakup kegiatan untuk memastikan terapi Obat yang aman, efektif dan rasional bagi pasien. Kegiatan dalam PTO meliputi pengkajian pemilihan Obat, dosis, cara pemberian Obat, respons terapi, Reaksi Obat yang Tidak Dikehendaki (ROTD), pemberian rekomendasi penyelesaian masalah terkait Obat; dan pemantauan efektivitas dan efek samping terapi Obat pada pasien untuk menurunkan risiko terjadinya Drug Related Problem (DRP). Pemantauan terapi obat harus dilakukan pada pasien yang dirawat di RSUD Tarakan Jakarta dengan komplikasi penyakit salah satunya adalah Hypertensive Heart Disease yang disertai penyakit penyerta seperti AKI, Non-insulindependent diabetes mellitus with multiple complications, Hiponatremia serta Pneumonia unspecified. Pemantauan terapi obat dilakukan selama 10 hari. Berdasarkan pengobatan yang diterima pasien untuk mengobati penyakitnya, ditemukan adanya 4 jenis DRP yaitu terapi obat yang tidak perlu, mengalami efek samping, interaksi obat dan overdose.

Medication Therapy Monitoring (MTM) is a clinical pharmacy activity performed by a pharmacist to ensure safe, effective, and rational drug therapy for patients. MTM involves assessing drug selection, dosing, administration route, therapeutic response, Adverse Drug Reactions (ADRs), providing recommendations to resolve drug-related problems (DRPs), and monitoring the effectiveness and side effects of drug therapy to reduce the risk of DRPs. MTM needs to be conducted for patients admitted to RSUD Tarakan Jakarta with complications from various diseases, including Hypertensive Heart Disease accompanied by conditions such as Acute Kidney Injury (AKI), Non-insulin-dependent diabetes mellitus with multiple complications, Hyponatremia, and unspecified Pneumonia. The drug therapy monitoring lasts for 10 days. Based on the treatment received by the patients for their respective conditions, four types of DRPs were identified: unnecessary drug therapy, experiencing side effects, drug interactions, and overdose.

"Rumah sakit ialah institusi pelayanan kesehatan yang menyelenggarakan pelayanan kesehatan perorangan secara paripurna yang menyediakan pelayanan rawat inap, rawat jalan, dan gawat darurat. Dimana diperlukan pelayanan kefarmasian yang berkontribusi dalam institusi pelayanan di rumah sakit. Salah satu cara untuk meningkatkan mutu pasien yaitu dengan pelaksanaan Pematauan Terapi Obat (PTO) pasien. Pemantauan Terapi Obat (PTO) adalah suatu proses yang bertujuan memastikan bahwa terapi obat yang diberikan kepada pasien adalah aman, efektif, dan sesuai dengan kebutuhan mereka. PTO berperan dalam meningkatkan efektivitas terapi dan mengurangi risiko efek samping obat yang tidak diinginkan. Kegiatan PTO mencakup evaluasi terhadap pemilihan obat, dosis yang diberikan, cara pemberian obat, respons pasien terhadap terapi yang diberikan, serta deteksi reaksi obat yang tidak diharapkan (ROTD). Hasil dari PTO dapat berupa rekomendasi perubahan dalam terapi obat yang sedang berjalan atau alternatif terapi yang lebih sesuai. Peran seorang apoteker dalam Pemantauan Terapi Obat (PTO) sangat penting karena memiliki peran memastikan bahwa pasien menerima obat sesuai dengan rekomendasi dokter, memonitor respons pasien terhadap terapi, memberikan informasi yang relevan tentang obat, dan mengidentifikasi serta mencegah potensi interaksi obat yang berbahaya. Sehingga pasien menerima manfaat maksimal dari pengobatan yang diterima. Hasil Pematauan Terapi Obat (PTO) Ny. LA, pasien menerima terapi yang sesuai dengan diagnosa dan keluhan pasien, namun terjadi beberapa Drug Related Problem seperti interaksi obat, dosis terlalu rendah, dan reaksi obat merugikan seperti efek samping obat dan obat kontraindikasi. Dilakukan Intervensi atas masalah pengobatan Ny. LA dan intervensi diterima kemudian dilakukan penyesuaian terkait terapi pasien.

A hospital is a health care institution that organizes comprehensive individual health services that provide inpatient, outpatient, and emergency services. Where pharmaceutical services are needed that contribute to service institutions in hospitals. One way to improve patient quality is through the implementation of patient Drug Therapy Monitoring (PTO). Drug Therapy Monitoring (PTO) is a process aimed at ensuring that the drug therapy given to patients is safe, effective, and appropriate to their needs. PTO plays a role in improving the effectiveness of therapy and reducing the risk of unwanted drug side effects. PTO activities include evaluation of drug selection, dosage, mode of administration, patient response to therapy, and detection of adverse drug reactions (ROTDs). The outcome of PTO may be recommendations for changes in current drug therapy or alternative, more appropriate therapies. The role of a pharmacist in Drug Therapy Monitoring (PTO) is very important because it has the role of ensuring that patients receive drugs in accordance with the doctor's recommendations, monitoring the patient's response to therapy, providing relevant information about drugs, and identifying and preventing potential dangerous drug interactions. So that patients receive the maximum benefit from the treatment received. The results of Mrs. LA's Drug Therapy Monitoring (PTO), the patient received therapy in accordance with the diagnosis and patient complaints, but there were several Drug Related Problems such as drug interactions, too low doses, and adverse drug reactions such as drug side effects and contraindicated drugs. Intervention was carried out on Mrs. LA's treatment problems and the intervention was accepted and adjustments were made regarding the patient's therapy.

"Congestive Heart Failure (CHF) merupakan ketidakmampuan jantung untuk memompa darah untuk memenuhi kebutuhan oksigen dan nutrisi ke jaringan tubuh. Hipertensive Heart Disease (HHD) adalah suatu penyakit yang berkaitan dengan dampak sekunder pada jantung karena hipertensi sistemik yang berkepanjangan. CHF dan HHD sering terjadi dikarenakan hipertensi dan merupakan penyebab utama kematian. Ateroskeloris dapat terjadi karena adanya peningkatan dari kadar kolesterol yang tidak normal sehingga mengakibatkan adanya akumulasi kolesterol di dalam dinding pembuluh darah. Diabetes Melitus merupakan gangguan metabolik yang ditandai dengan kadar gula darah yang melebihi batas normal. Pneumonia adalah infeksi radang paru yang disebabkan oleh mikroorganisme. Pemantauan terapi obat pada pasien dilakukan dengan mengambil data pasien dengan cara observasi dengan melakukan visite. Selanjutnya dilakukan studi literatur melalui data rekam medis pasien, e-book, peraturan pemerintah, maupun sumber lain. Diperoleh hasil bahwa pasien dengan diagnosis congestive heart failure ec hypertensive heart disease, atherosclerotic heart disease, diabetes mellitus dan pneumonia adanya ketidaksesuaian dosis penggunaan insulin berdasarkan dosis harian PERKENI. Pada pengobatan pasien adanya potensi interaksi obat pada sucralfate dan furosemide, sehingga perlu dijeda dalam pemberian obat. Interaksi obat pada humalog dan lantus, serta furosemide dan humalog yang diberikan secara bersamaan perlu dilakukan monitoring gula darah pasien. Analisis diinterpretasikan dalam bentuk SOAP sebagai komunikasi tertulis untuk menyampaikan rekomendasi kepada Dokter Penanggung Jawab Pelayanan (DPJP).
Congestive Heart Failure (CHF) is the inability of the heart to pump blood to fulfill the oxygen and nutrients to body tissues. Hypertensive Heart Disease (HHD) is a disease associated with secondary impacts on the heart due to prolonged systemic hypertension. CHF and HHD often occur due to hypertension and are the main causes of death. Atherosclerosis can occur due to an increase in abnormal cholesterol levels, resulting in the accumulation of cholesterol in the walls of blood vessels. Diabetes Mellitus is a metabolic disorder characterized by blood sugar levels that exceed normal limits. Pneumonia is an inflammatory lung infection caused by microorganisms. Monitoring drug therapy in patients is carried out by collecting patient data by observing by conducting visits. Next, a literature study was carried out using patient medical record data, e-books, government regulations, and other sources. The results showed that patients with a diagnosis of congestive heart failure ec hypertensive heart disease, atherosclerotic heart disease, diabetes mellitus and pneumonia had a mismatch in insulin dosage based on the PERKENI daily dose. In patient treatment, there is a potential for drug interactions with sucralfate and furosemide, so it is necessary to pause the administration of the drug. Drug interactions with humalog and lantus, as well as furosemide and humalog given simultaneously require monitoring of the patient's blood sugar. The analysis is interpreted in SOAP form as written communication to convey recommendations to the Doctor in Charge of Services (DPJP)."
