"[Merumuskan Learning Organization Melalui Analisis Budaya Keselamatan Pasien Dan Budaya Organisasi Di RS.Masmitra Langkah awal membangun keselamatan pasien adalah melakukan penilaian terhadap budaya keselamatan pasien yang mana diperlukan pengkajian budaya organisasi sebagai panduan dalam menerapkan keselamatan pasien. Penelitian deskriptif dengan metode kuantitatif potong lintang dilanjutkan metode kualitatif ini bertujuan mengukur budaya keselamatan pasien, mengidentifikasi profil budaya organisasi dan merumuskan learning organization untuk membangun keselamatan pasien di RS.Masmitra. Budaya Hierarchy didapati sebagai budaya organisasi yang dominan saat ini di RS.Masmitra yang membutuhkan manajemen pengetahuan dalam upaya transformasi budaya keselamatan pasien. Hal ini sejalan dengan hasil penelitian budaya keselamatan pasien yang menyatakan dimensi pembelajaran organisasi dan perbaikan berkelanjutan merupakan budaya terlemah di RS.Masmitra. Oleh karena itu, perumusan learning organization sangatlah tepat untuk membangun keselamatan pasien di RS.Masmitra.

"Membangun budaya keselamatan pasien merupakan langkah awal dalam pengembangan keselamatan pasien. Budaya keselamatan pasien di rumah sakit merupakan bagian dari budaya organisasi, sehingga pengkajian tentang budaya organisasi diperlukan untuk menjadi panduan dalam mengembangkan keselamatan pasien. Penelitian ini bertujuan mengetahui budaya keselamatan pasien di kalangan pemberi pelayanan di RSUD dr. H. Abdul Moeloek Provinsi Lampung dan mengidentifikasi profil budaya organisasi pada jajaran pimpinannya.
Penelitian deskriptif dengan interpretasi dan analisis kualitatif ini mengambil subyek penelitian para pemberi pelayanan dan jajaran manajemen rumah sakit, dengan cara menyebarkan kuesioner dan melaksanakan Consensus Decision Making Group (CDMG). Instrumen penelitian menggunakan kuesioner AHRQ (Agency for Heath Research and Quality) membagi budaya keselamatan pasien menjadi 1.) Budaya Keterbukaan, 2.) Budaya Keadilan, 3.) Budaya Pelaporan, 4.) Budaya Belajar, 5.) Budaya Informasi. Sedangkan kuesioner Organization Culture Assesment Instrument (OCAI) menilai 6 kriteria budaya yaitu 1.) Karakter Dominan, 2.) Kepemimpinan Organisasi, 3.) Manajemen Karyawan, 4.) Perekat Organisasi, 5.) Penekanan Strategis, 6.) Kriteria Keberhasilan. Budaya organisasi terbagi menjadi Tipe Clan, Adhocrazy, Market dan Hierarki.
Hasil penelitian mendapatkan bahwa budaya keterbukaan, terutama kerjasama dalam unit merupakan dimensi budaya keselamatan pasien yang terkuat dan dominan. Sedangkan respons non punitive dan pencatatan merupakan dimensi yang terlemah. Tipe budaya Hierarki didapatkan sebagai tipe budaya organisasi yang dominan sekaligus kuat untuk saat ini dan yang diharapkan. Hal ini menjadi panduan untuk memilih strategi peningkatan mutu melalui Competing Value Framework dalam rangka pengembangan dan peningkatan keselamatan pasien. Rencana tindak lanjut dibuat dan disepakati dalam Consensus Decision Making Group (CDMG) untuk membumikan unsur keselamatan pasien dalam visi dan misi organisasi serta penguatan budaya keselamatan melalui pelatihan keselamatan pasien bagi seluruh staf. Blamming culture harus secara perlahan dan signifikan segera dihilangkan dalam semua bentuk pelayanan di rumah sakit.

Building a culture of patient safety is the first step in the development of patient safety. Culture of patient safety in hospitals is part of the culture of the organization, so that the assessment of organizational culture needed to be a guide in developing patient safety. This study aims to determine the patient safety culture among providers in dr. H. Abdul Moeloek Lampung and identify organizational culture profile in the ranks of leadership.
Descriptive study with qualitative interpretation and analysis of the study subjects took care providers and hospital management board, by distributing questionnaires and implement the Consensus Decision Making Group (CDMG). The research instrument used questionnaires AHRQ (Agency for Heath Research and Quality) dividing the patient safety culture to 1.) Cultural Openness, 2.) Cultural Justice, 3) Cultural Reporting, 4.) Cultural Learning, 5.) Cultural Information. While questionnaires Organization Culture Assessment Instrument (OCAI) assesses six cultural criteria, namely 1.) Dominant character, 2) Organizational Leadership, 3) Employee Management, 4.) Adhesive Organization, 5.) Strategic Emphasis, 6.) Success Criteria. Organizational culture is divided into Type Clan, Adhocrazy, Market and Hierarchy.
Results of the study found that a culture of openness, especially cooperation in the unit are the dimensions of patient safety culture is strongest and dominant. While the non-punitive responses and recording the weakest dimension. Type Hierarki culture obtained as the dominant type of organizational culture as well strong for the current and expected. It serves as a guide to select a quality improvement strategy through Competing Value Framework in order to develop and increase patient safety. Follow-up plan prepared and agreed in the Consensus Decision Making Group (CDMG) to ground elements of patient safety in the vision and mission of the organization and strengthening a culture of safety through patient safety training for all staff. Blamming culture must gradually and significantly soon be eliminated in all forms of service in hospitals.

"Penelitian tesis ini dilakukan di Organisasi Nirlaba XYZ, yang bekerja untuk memperbaiki kualitas pendidikan di Indonesia. Tantangan yang dihadapi organisasi ini adalah untuk membangun budaya pembelajaran organisasi agar bisa mencapai tujuan-tujuan strategisnya. Penelitian ini adalah penelitian korelasional dengan desain non-eksperimental. Hasil penelitian menunjukkan bahwa faktor yang mempengaruhi budaya pembelajaran organisasi di XYZ adalah tools dan teknologi untuk berbagi pengetahuan dan perceived behavioral control. Untuk meningkatkan budaya pembelajaran organisasi di Organisasi Nirlaba XYZ, peneliti merekomendasikan intervensi berupa workshop tentang solusi tools dan teknologi untuk berbagi pengetahuan serta pelatihan manajemen waktu untuk meningkatkan faktor perceived behavioral control.

This research is conducted in the Organization XYZ; it is a not-for-profit organization working to improve the quality of education in Indonesia. The challenge faced by the organization is to build an organizational learning culture in order to achieve its strategic objectives. The research is a correlational research with a non-experimental design. The result shows that the factors affecting organizational learning culture in XYZ are tools and technology for knowledge sharing and perceived behavioral control. As interventions to improve organizational learning culture in XYZ, the researcher recommends a workshop on tools and technology for knowledge sharing in XYZ and time management training to increase perceived behavioral control among the staff."

Dimensi terlemah terutama pada staffing, respon non punitive terhadap kesalahan, serah terima dan transisi. The concept used was the concept of patient safety culture from AHRQ 2004 which is adopted from Puspitasari M. research 2009 , then for improvement used the concept of system reliability form Marx D. 2010 . The research design was sequential explanatory, used questionnaire from AHRQ which has been translated to Indonesia language, followed by FGD to formulate the weak dimension improvement effort.Patient safety culture status categorized into medium culture, average of positive perception 70,82 . The greatest strengths are in organizational learning and continuous improvement, feedback and communication about patient safety, communication openness. Weaknesses are primarily in staffing, non punitive responses to errors, handover and transitions must be fixed immediately. "Budaya keselamatan pasien menjadi isu penting dalam peningkatan mutu pelayanan dan kepuasan pasien, serta pengurangan beban cost rumah sakit. Tujuan penelitian ini untuk mendapatkan gambaran budaya keselamatan pasien di kalangan perawat rawat inap RS Trimitra. Penelitian ini menggunakan studi cross sectional dengan metode penelitian deskriptif kuantitatif dan kualitatif. Penelitian kuantitatif menggunakan instrumen rumah sakit milik AHRQ dan penelitian kualitatif menggunakan instumen observasi.
Hasil penelitian menunjukkan terdapat 4 budaya kategori kuat (supervisor, kerjasama, komunikasi, handsoff dan transisi), 4 budaya kategori sedang (organizational learning, respon non-punitive terhadap kesalahan, staffing, persepsi perawat terkait keselamatan pasien) dan 1 budaya lemah (frekuensi pelaporan insiden). Perilaku perawat yang diamati (ketepatan identifikasi pasien, ketepatan prosedur pemberian obat, dan pencegahan infeksi) menunjukkan sebagian besar perilaku tidak sesuai SPO/standar lain yang berlaku. Berdasarkan teori swiss cheese model, hal ini diakibatkan masih ada celah pada setiap layer pertahanan keselamatan pasien, yang pada satu waktu semua pertahanan dalam kondisi lemah mengakibatkan insiden/perilaku lalai terjadi. Saran perbaikan diperlukan pada setiap dimensi budaya keselamatan pasien.

Patient safety culture is an important issue in improving quality of care and patient satisfaction, as well as a reduction in the cost burden of the hospital. The purpose of this study to get an overview of patient safety culture among inpatient-nurse in Trimitra hospital. This study use a cross-sectional with quantitative descriptive methods and qualitative research. Quantitative research using hospital's instruments by AHRQ and qualitative research using observation instrument.
The results showed there are four strong culture (supervisor, teamwork, communication, handsoff and transitions), four medium culture (organizational learning, response of non-punitive to errors, staffing, nurses' perception related to patient safety), and one weak culture (reporting frequency incident). Nurse behavior observed (the accuracy of patient identification, precision drug delivery procedures, and the prevention of infection) showed that most of the nurse behavior is not appropriate SPO/other applicable standards. Based on the theoretical swiss cheese model, this incident caused by holes on each layer defenses, which at one time, all the defenses in weak condition. Suggested improvements needed in every dimension of patient safety culture.

"Keselamatan dan kepuasan pasien merupakan indikator kualitas pelayanan belum terpenuhi secara optimal. Model budaya belajar perawat membentuk perawat kompeten dalam penerapan sasaran keselamatan pasien. Penelitian bertujuan mengidentifikasi budaya belajar tentang keselamatan pasien, mengembangkan model budaya belajar perawat dan mengidentifikasi pengaruhnya terhadap kompetensi perawat, keselamatan dan kepuasan pasien. Desain operasional riset, tahap pertama kualitatif fenomenologi dengan sembilan partisipan, tahap kedua pengembangan model, dan tahap ketiga desain quasi eksperiment pre-post test design with control group dengan 139 responden perawat dan 240 responden pasien.
Hasil penelitian tahap pertama didapat sembilan tema yaitu arti keselamatan, persiapan penerapan, fokus keselamatan, implemetasi, masalah, tahapan budaya belajar, aktivitas pembelajaran dan dukungan pengembangan budaya belajar. Tahap kedua diperoleh model budaya belajar "SAYA perawat" tentang sasaran keselamatan pasien dengan komponen, perawat; Sistem dukungan, aktivitas pembelajaran dan tahapan budaya belajar. Tahap ketiga model budaya belajar "SAYA Perawat" tentang sasaran keselamatan pasien berpengaruh secara bermakna terhadap kompetensi perawat dalam penerapan sasaran keselamatan pasien (p 0,0001), keselamatan (p 0,0001) dan kepuasan pasien (p 0,0001). Terdapat hubungan bermakna kompetensi perawat dalam penerapan keselamatan pasien dengan keselamatan (p 0,0001) dan kepuasan pasien (p 0,0001). Model budaya belajar ini agar dapat diimplementasikan di ruang perawatan untuk memenuhi keselamatan dan kepuasan pasien.

Safety and patient satisfaction which are indicators of quality cannot be optimaly fulfilled. Learning culture models about patient safety form nurses competent in the application of patient safety. This study aims to identify nurses learning culture about patient safety, develop a model of nurse learning culture and identify its affect on nurses competency, patient safety and satisfaction. The research design used operational research, the first phase used qualitative phenomenology with nine participants, the second stage is the development of the model, and the third stage is the quasi experimental design pre-post test design with control group with 139 nurses and 240 patient respondents.
The results of the first phase of research obtained nine themes, namely the meaning of safety, preparation for application, focus on safety, implementation, problems, stages of learning culture, learning activities and support for the development of a learning culture. The second stage was obtained learning culture models "SAYA Nurses" about patient safety goals is fit and valid there have components, nurses as the subject of development; support systems, learning activities and the learning culture stage . The third stage of the learning culture model "SAYA Nurses about patient safety goals has a significant effect on nurses' competence (p 0,0001), patient safety (p 0,0001) and satisfaction (p 0,0001). There is a significant relationship between nurses' competence with patient safety (p 0,0001) and satisfaction (p 0,0001). The learning culture models to be implemented in the ward to safety and satisfaction.

"Budaya keselamatan pasien (BKP) adalah penerapan sistem asuhan pasien dalam organisasi yang tercermin dalam sikap, perilaku, keterampilan, komunikasi, kepemimpinan, pengetahuan, tanggung jawab, dan nilai yang ada dalam diri petugas kesehatan. Penelitian ini bertujuan untuk mengetahui perilaku organisasi berdasarkan karakteristik individu, kelompok dan organisasi terhadap budaya keselamatan pasien oleh perawat rawat inap di RS Hermina Daan Mogot RSHDM).
Desain penelitian ini menggunakan metode cross sectional dengan responden seluruh (111) perawat pelaksana pada unit rawat inap RSHDM. Data kuesioner dianalisis menggunakan metode univariat, bivariat dan multivariat.
Hasil penelitian menunjukkan karakteristik individu, karakteristik kelompok dan karakteristik organisasi terhadap BKP di RSHDM adalah baik. Hasil BKP perawat pelaksana rawat inap RSHDM menunjukkan nilai baik. Tanggung jawab menjadi satu-satunya variabel yang tidak ada hubungan dengan BKP sedangkan kepemimpinan paling berhubungan dengan BKP.

Patient safety culture (PSC) is the application of patient care systems in the organization which are reflected in the attitudes, behaviors, skills, communication, leadership, knowledge, responsibility, and values that exist in health care workers. This study aims to determine the organizational behavior based on the characteristics of individuals, groups and organizations on patient safety culture by nurses on inpatient units in Hermina Hospital Daan Mogot (HHDM).
The design of this study using cross-sectional method with respondents from all (111) nurses on inpatient units in HHDM. Questionnaire data were analyzed using univariate, bivariate and multivariate analyzes.
The results showed the characteristics of an individual, group characteristics and organizational characteristics of the PSC in HHDM is good. Results PSC inpatient nurses HHDM shows good value. Responsibility to be the only variable that did not match while the leadership were most associated with PSC.

"Tesis ini membahas analisis pengaruh budaya organisasi dan organisasi pembelajaran terhadap strategi bisnis serta persepsi terhadap kinerja perusahaan dalam konteks PT Indofarma (Persero) Tbk. Penelitian ini adalah penelitian kuantitatif dan dilakukan melalui survei terhadap karyawan PT Indofarma (Persero) Tbk. Hasil penelitian menunjukkan bahwa budaya organisasi dan organisasi pembelajaran berpengaruh signifikan positif terhadap cost leadership, differentiation dan focus strategy. Lebih lanjut ditemukan bahwa cost leadership strategy tidak berpengaruh positif terhadap kinerja perusahaan, sedangkan differentiation dan focus strategy berpengaruh signifikan positif terhadap kinerja perusahaan.

This thesis describes the influence analysis of organizational culture and learning organization to business strategies and its perception to the corporate performance in the context of PT Indofarma (Persero) Tbk. This research is a quantitative study and conducted through a survey of employees of PT Indofarma (Persero) Tbk. The results showed that organizational culture and learning organization have positive significant effect on cost leadership, differentiation and focus strategies. Further found that the cost leadership strategy does not have positive effect on company performance, while differentiation and focus strategies have positive and significant effect on the performance of the company."

"Tesis ini membahas tentang pengaruh perilaku kepemimpinan dan budaya organisasi pembelajar terhadap komitmen organisasi di organisasi pelajar. Permasalahan yang terjadi adalah minimnya kehadiran pengurus IKOSI Zona 2 dalam setiap agenda. Penelitian ini dilakukan melalui pendekatan kuantitatif dengan mengadopsi instrumen Organizational Commitment Questionnaire (OCQ) dari Mowday, Steers dan Porter (1979), instrumen Perceived Leadership Behavior Scales (PLBS) dari House dan Dessler (1974), dan instrumen Dimensions of Learning Organization Questionnaire dari Watkins dan Marsick (1997), ditambah 3 pertanyaan demografis berupa jenis kelamin, jabatan dan masa kepengurusan.
Setelah data dianalisis dengan menggunakan perangkat lunak SPSS ver 21.0, hasil penelitian menyimpulkan tidak terdapat pengaruh jenis kelamin dan jabatan terhadap komitmen organisasi, perilaku kepemimpinan, dan budaya organisasi pembelajar, terdapat pengaruh yang positif dan signifikan dari perilaku kepemimpinan dan budaya organisasi pembelajar secara masing-masing terhadap komitmen organisasi di IKOSI Zona 2. Namun terdapat pengaruh yang positif dan tidak signifikan secara simultan perilaku kepemimpinan dan budaya organisasi pembelajar terhadap komitmen organisasi di IKOSI Zona 2. Untuk itu, perlu adanya eksplorasi lebih jauh tentang alasan minimnya kehadiran pengurus dalam agenda IKOSI Zona 2.

This thesis discusses the influence of leadership behavior and learning organization culture on organizational commitment in student organizations. The problem that occurs is the lack of board presence IKOSI Zone 2 in each agenda. This research was conducted through a quantitative approach by adopting instrument Organizational Commitment Questionnaire (OCQ) of Mowday, Steers and Porter (1979), the instrument Perceived Leadership Behavior Scales (PLBS) of the House and Dessler (1974), and the instrument of Dimensions of Learning Organization Questionnaire from Watkins and Marsick (1997) plus three demographic questions such as gender, position and time management.
After the data were analyzed using SPSS software ver 21.0, the results of the study concluded that there was no influence of gender and position to organizational commitment, leadership behavior, and learning organization culture, there is a positive and significant effect of leadership behavior and learning organization culture respectively on organizational commitment in IKOSI Zone 2. Yet there is a positive influence and simultaneously insignificant behavior leadership and learning organization culture on organizational commitment in IKOSI Zone 2. Therefore, it's need for further exploration of the reasons for the lack of presence in the board agenda IKOSI Zone 2.

"ABSTRAK
Rumah sakit sebagai organisasi kesehatan harus mengembangkan budaya
keselamatan pasien dalam upaya meningkatkan kualitas pelayanannya untuk
mencegah terjadinya kejadian yang tidak diharapkan. Untuk mengetahui tingkat
budaya keselamatan pasien di Rumah Sakit Santa Elisabeth Batam dilakukan
pengukuran dengan tool Manchester Patient Safety Framework ( MaPSaF) yang
dipublikasilkan oleh National Patient Safety Agency (NPSA). Penelitian ini
merupakan jenis studi observasional dengan menggunakan desain semikuantitatif.
Dari hasil penelitian diperoleh Dimensi Budaya Keselamatan Pasien di Rumah
Sakit St. Elisabeth Batam menurut tool MaPSaF berada pada tingkat antara
Birokratik dan Proaktif. Rumah sakit sudah memiliki sistem untuk keselamatan
pasien dengan memiliki kekuatan pada 5 dimensi yaitu komitmen, prioritas,
kesalahan sistem, evaluasi insiden serta pendidikan dan pelatihan. Sebaliknya
ditemukan kelemahan sistem pada beberapa dimensi yang menunjukkan bahwa
keselamatan pasien belum dilaksanakan secara komprehensif. Dengan adanya
kelemahan sistem budaya keselamatan di rumah sakit maka perlu dilakukan
pengembangan pada dimensi-dimensi yang menjadi tulang punggung keselamatan
pasien yaitu pada dimensi perekaman insiden, pembelajaran dan perubahan
perilaku, komunikasi, manajemen kepegawaian serta kerjasama tim keselamatan
pasien.

ABSTRACT
Hospital as a healthcare organization must develop a culture of patient
safety in an effort to improve the quality of service in order to prevent the
occurrence of adverse event. Measurement tool to determine the level of patient
safety culture in Santa Elisabeth Batam Hospital is the Manchester Patient
Safety Framework ( MaPSaF) which is published by the National Patient safety
Agency (NPSA).This research is an observational study using a design
semiquantitatively. The result showed Dimensional Patient Safety Culture in St.
Elisabeth Batam Hospital according to MaPSaF tool is at level between
Bureaucratic and Proactive. Hospital already has a system for patient safety by
having strenght on the dimension of commitment to continuos improvement,
priority given to patient safety, system errors and individual responsibility as
well as staf education and training of personal about patient safety. Instead
discovered weakness in the system on several dimensions indicate that patient
safety has not been comprehensively implemented. With the weakness of the
system of safety culture in hospital it is necessary to develop the dimensions of the
backbone, namely the dimension of recording incidents and best practice,
learning and effecting change, communication about safety issues, personnel
management and safety issues as well as teamwork patient safety."
