"Pelaksanaan keselamatan pasien oleh mahasiswa masih belum optimal sehingga diperlukan peningkatan peran pembimbing klinik melalui penerapan quality circle. Tujuan penelitian ini untuk mengidentifikasi pengaruh penerapan quality circle dalam meningkatkan pelaksanaan keselamatan pasien mahasiswa (pengetahuan, sikap, dan tindakan). Desain penelitian menggunakan quasi experiment pre test and post test nonequivalent control group design dengan sampel 34 mahasiswa keperawatan kelompok intervensi dan 34 mahasiswa kelompok kontrol di dua rumah sakit yang berbeda. Intervensi dilakukan dengan diterapkannya quality circle oleh pembimbing klinik yang berfokus pada permasalahan keselamatan pasien. Teknik sampling menggunakan consecutive sampling. Hasil penelitian menunjukkan terjadi peningkatan yang bermakna pada pengetahuan dan tindakan keselamatan pasien mahasiswa (p<0,001; α=0,05). Kesimpulan penelitian ini menunjukkan terdapat pengaruh penerapan quality circle terhadap pelaksanaan keselamatan pasien. Rekomendasi penelitian ini adalah perlunya pemanfaatan dan pengembangan teknik problem solving quality circle untuk mengoptimalkan pelaksanaan keselamatan pasien.

Implementation of patient safety by nursing students is still not optimal so that the necessary increase in clinical instructor role through the application of quality circles. The purpose of this study to identify the effect of the application of quality circles in enhancing the implementation of patient safety of students (knowledge, attitudes, and skills). Quasi-experimental research design using pretest and posttest nonequivalent control group design with a sample of 34 nursing students of intervention group and 34 students of control group in two different hospitals. The intervention is done with the implementation of quality circle by clinical instructor that focuses on patient safety issues. The results showed an increase was significant in nursing student?s knowledge and actions of patient safety (p <0.001; α = 0.05). Conclusions of this study shows there are significant effect of the quality circle on the implementation of patient safety. Recommendations of this study is the need to utilize and develop quality circle as problem solving techniques to optimize the implementation of the patient safety."

"Masalah keselamatan pasien yang ditimbulkan oleh mahasiswa dapat menimbulkan berbagai dampak yang merugikan bagi pasien. Pembimbing klinik berperan penting dalam membimbing mahasiswa dalam pelaksanaan program keselamatan pasien. Komunitas praktisi pembimbing klinik ditengarai mampu meningkatkan peran pembimbing klinik dalam program keselamatan pasien oleh mahasiswa. Penelitian ini bertujuan untuk mengukur pengaruh komunitas praktisi pembimbing klinik terhadap peran pembimbing klinik dalam program keselamatan pasien oleh mahasiswa. Metode penelitian ini menggunakan desain pre experimental dengan sampel sebanyak 36 mahasiswa profesi Ners yang sedang praktik di ruang rawat inap RSISA yang mendapatkan bimbingan dari pembimbing klinik yang mengikuti komunitas praktisi pembimbing klinik. Data dianalisis dengan menggunakan uji Wilcoxon. Hasil penelitian ini menunjukkan terdapat pengaruh komunitas praktisi pembimbing klinik terhadap peran pembimbing klinik dalam pelaksanaan program keselamatan pasien oleh mahasiswa p < 0,001. Penelitian ini merekomendasikan perlunya keterlibatan pembimbing klinik dari institusi pendidikan untuk menjadi bagian dari komunitas praktisi pembimbing klinik.

The problem of patient safety which is caused by nursing students can effect disadvantage to patient. The clinical instructor have important role to guide nurse student in implementation patient safety program. Community of practice clinical instructor was predicted can increase the role of clinical instructor in patient safety program by student.

The purpose of this study is to explore the the influence community of practice clinical instructor to the role of clinical instructor in patient safety program by student. This research is pre experimental with the number of samples are 36 Ners student in ward RSISA who get guidance from clinical instructor. The data analysis that use of this research is wilcoxon test.

The result show that there is significant influence community of practice clinical instructor to the role of clinical instructor in patient safety program by student p < 0,001. The researcher suggests that the clinical instructor from education institution should be part in community of practice clinical instructor."

"Pelaksanaan supervisi klinik yang adekuat meningkatkan pelayanan keperawatan optimal, identik dengan proses penyelesaian masalah. Desain penelitian quasi eksperimen pre-post with control group. Penelitian ini bertujuan mengidentifikasi pengaruh penerapan quality circle oleh perawat clinical care manager terhadap kemampuan melakukan supervisi klinik. Sampel penelitian diperoleh secara purposive sampling terdiri dari 105 perawat pelaksana dan 14 perawat clinical care manager. Instrumen penelitian berjumlah 42 pernyataan yang dikembangkan dari Manchester Clinical Supervision Scale. Penerapan quality circle berpengaruh secara signifikan terhadap peningkatan kemampuan supervisi klinik pada aspek pengembangan profesional, aspek peningkatan keterampilan klinik, aspek alokasi waktu untuk refleksi, dan aspek kualitas hubungan interpersonal supervisorsupervisee (p=0,000, p=0,004, p=0,007, p=0,017; α=0,05). Pelaksanaan quality circle secara konsisten berpeluang meningkatkan kemampuan supervisi klinik dan kualitas pelayanan keperawatan. Keberlangsungan quality circle memerlukan dukungan dari institusi pelayanan keperawatan dan perlunya standar pelaksanaan supervisi klinik sesuai kompetensi dan tingkat usia perawat pelaksana.

Implementation of adequate clinical supervision will enhancing the nurse services, is identical to the problem solving process. The research design was quasi experimental pre-post with control group, the aim of this study was to identify the effect of quality circle implementation by clinical care manager nurse to enhance the ability of clinical supervision. Samples obtained by purposive sampling consist of 105 nurses and 14 clinical care manager nurses. Instrument amounted 42 statements were developed from Manchester Clinical Supervision Scale. The application of quality circle increased significantly the ability of clinical supervision on professional development aspect, clinical skill improvement aspect, time allocation for reflection aspect, and interpersonal relationship quality aspect (p=0,000, p=0,004, p=0,007, p=0,017, α=0,05). Consistent implementation of quality circle potentially increasing the ability of clinical supervision and the quality of nursing services. Sustainability of clinical supervision requires the support of nursing care institution and standards procedure of clinical supervision is needed appropriate to the level of competency and age of the nurse."

"Kualitas merupakan elemen kunci yang tidak dapat diabaikan dalam persaingan dan merupakan salah satu masalah penting bagi keberhasilan perusahaan, termasuk dalam perusahaan konstruksi. Di sisi lain, jika kebijakan manajemen mutu tidak dilaksanakan atau partisipasi terbatas akan berdampak negatif terhadap daya saing perusahaan. Pencapaian kinerja dapat lebih baik jika penerapan manajemen mutu terpadu didukung oleh perusahaan. Gugus Kendali Mutu GKM telah ditemukan menjadi teknik sederhana dan produktif dari Total Quality Management TQM dan diusulkan dapat diterapkan dalam perusahaan konstruksi. Penelitian ini bertujuan untuk mengetahui pengaruh keefektifan gugus kendali mutu terhadap kinerja perusahaan konstruksi di Indonesia. Regresi digunakan untuk menganalisis efek hubungan antara tujuan gugus kendali mutu terhadap kinerja perusahaan konstruksi dengan aplikasi SPSS 22. Berdasarkan analisis data yang diperoleh dari survei, disimpulkan bahwa tujuan gugus kendali mutu berpengaruh terhadap variabel kinerja perusahaan konstruksi di Indonesia sebesar 61.

Quality is a key element that can not be ignored in the competition and is one of the critical issues for the success of the company, including in the construction industry. On the other hand, if the quality management policy is not implemented or limited participation will both negatively affect the management of the project and competitiveness of the firms. This will also decrease the survival potential of construction firms within the industry. Achievement of performance can be better if the implementation of integrated quality management is supported by the company. Quality Control Circle QCC has been found to be a simple and productive technique of Total Quality Management TQM and is proposed to be applicable in construction companies. This study aims to determine the effect of quality control group objectives on the performance of construction companies in Indonesia. Regression was used to analyze the effect of the relationship between the objectives of the quality control group on the performance of the construction company with software SPSS 22. Based on the data analysis obtained from the survey, it was concluded that the objective of the quality control circle influenced the performance variable of construction companies in Indonesia by 61."

"Budaya keselamatan pasien merupakan pondasi utama dalam pelaksanaan keselamatan pasien. Penelitian ini bertujuan untuk mengetahui pengaruh pemberdayaan champion keselamatan pasien terhadap penerapan budaya keselamatan pasien di Rumah Sakit Haji Jakarta. Metode penelitian ini menggunakan pre-experiment design: pretest-posttest without control group design, sampel yang digunakan 81 perawat. Data analisis dengan menggunakan Mc Nemar. Hasil menunjukkan terdapat peningkatan persentasi penerapan budaya keselamatan pasien setelah program pemberdayaan champion keselamatan pasien dengan pengaruh yang tidak bermakna secara statistik (p= 0,451; CI= 0.084-0.928). Penelitian ini merekomendasikan perlunya pengembangan champion keselamatan pasien dengan memperhatikan kriteria kelayakan sebagai champion keselamatan pasien serta membangun program pemberdayaan champion keselamatan pasien berkelanjutan sesuai kebutuhan rumah sakit.

The culture of patient safety is the main foundation in the implementation of patient safety. This study aimed to determine the influence of the patient safety champion empowerment on the application of patient safety culture in Jakarta Hajj Hospital.

This research method used pre-experimental design: pretest-posttest without control group design, the sample was 81 nurses. Data were analyzed using Mc Nemar test.

The results showed there was an increase in the percentage of the implementation of a patient safety culture after the program of patient safety champions empowerment had been implemented, which was not statistically significant (p = 0.451; CI = 0084-0928).

It is recommended to develop the patient safety champions by taking into account the eligibility criteria as a patient safety champion and to build a sustainable program of patient safety champion empowerment that suitable to the needs of the hospital."

"Budaya keselamatan memiliki peran penting dalam mewujudkan pelayanan keperawatan yang aman bagi pasien. Masih ditemui masalah terkait budaya keselamatan pasien dan sikap pelaporan insiden keselamatan pasien. Budaya keselamatan pasien dapat berhubungan dengan sikap perawat dalam pelaporan insiden. Penelitian ini bertujuan mengetahui hubungan budaya keselamatan pasien dengan sikap perawat dalam pelaporan insiden keselamatan pasien. Penelitian Cross Sectional menggunakan cluster sampling ini dilakukan dengan pengisian kuesioner yang melibatkan 400 perawat di tiga rumah sakit umum daerah di tiga kabupaten Derah Istimewa Yogyakarta. Hasil didapatkan adanya hubungan yang signifikan antara budaya keselamatan pasien dengan sikap perawat dalam pelaporan insiden keselamatan pasien p=0,005 . Hasil regresi linear menunjukkan variabel yang paling mempengaruhi sikap pelaporan secara berurutan yaitu jabatan, budaya keselamatan pasien, level kompetensi, masa kerja, dan usia perawat R2=0,892. Kesimpulan adalah bahwa budaya keselamatan pasien memiliki peran penting terkait sikap perawat dalam pelaporan insiden keselamatan pasien, upaya untuk memperkuat budaya keselamatan pasien dapat memperbaiki sikap perawat dalam pelaporan insiden keselamatan pasien. Rekomendasi yang diberikan yaitu perbaikan pengaturan staf di rumah sakit, penyelenggaraan pelatihan atau diskusi rutin sebagai tindak lanjut dari pelaporan insiden, menghilangkan budaya menyalahkan terkait pelaporan insiden, memberikan apresiasi kepada perawat yang bersedia melaporkan insiden, menumbuhkan budaya saling mendukung antar perawat dalam pelaporan insiden keselamatan pasien.

Safety culture has important role in realizing a safe nursing service for patients. Problems related to patient safety culture and patient safety incident reporting are still encountered. The safety culture of the patient may relate to the nurse 39 s attitude in incident reporting. This study aims to determine the relationship of patient safety culture with the attitude of the nurses in reporting patient 39 s safety incidents. Cross sectional study using cluster sampling was conducted by filling a questionnaire involving 400 nurses at three regional public hospitals in three districts in the province of Yogyakarta special region.

The result shows that there is a significant correlation between patient safety culture and nurse attitude in reporting patient 39 s safety incident p 0,005 . Linier regression result shows consecutively that their position, patient safety culture, level of competence, year of service and age affect their attitude in reporting an accident R2 0,892.

The conclusion is that the patient safety culture has an important role in the nurse 39 s attitude in reporting the patient 39 s safety incident, efforts to strengthen the patient 39 s safety culture could improve the nurse 39 s attitude in reporting the patient 39 s safety incident.

Recommendations include improvements in hospital staffing, regular training or regular discussions as a follow up to incident reporting, eliminating a culture of incident reporting error, giving appreciation to nurses willing to report incidents, fostering a mutually supportive culture among nurses in reporting patient safety incidents. "

"Keselamatan pasien merupakan indikator kualitas pelayanan yang berdampak pada kepuasan pasien. Ketidakpatuhan terhadap penerapan keselamatan pasien dapat menimbulkan insiden keselamatan pasien. Pelaksanaan penerapan keselamatan pasien yang belum optimal berkaitan dengan human error (beban kerja, pemanfaatan waktu dan sumber daya manusia, dan pembagian beban kerja yang belum sesuai). Dibutuhkan kemampuan perawat primer/ketua tim sebagai lini terdepan dalam pemberian asuhan melalui pelaksanaan pendelegasian yang efektif. Penelitian ini bertujuan untuk mengidentifikasi hubungan antara pelaksanaan pendelegasian dengan penerapan keselamatan pasien. Rancangan penelitian adalah penelitian kuantitatif dengan pendekatan cross sectional pada 160 perawat pelaksana yang diambil secara total sampling pada salah satu rumah sakit di Jakarta. Analisis dilakukan dengan uji chi square dan regresi logistik ganda.  Hasil penelitian menunjukkan ada hubungan antara pelaksanaan pendelegasian dengan penerapan keselamatan pasien (p 0,001; OR 4,302 2,215-8,356). Bidang Keperawatan membuat perencanaan capaian indikator mutu melalui pelaksanaan SKP yang belum optimal.  Kepala ruangan melakukan monitoring dan evaluasi terhadap  pelaksanaan pendelegasian yang dilakukan perawat primer/ketua tim.

Patient safety is an indicator of service quality that has an impact on patient satisfaction. Non-compliance with the application of patient safety could lead to incidents of patient safety. The implementation of patient safety that is not optimal is related to human error (workload, utilization of time and human resources, and the division of workload that is not appropriate). It takes the ability of the primary nurse / team leader as the frontline in providing care through the implementation of effective delegation. This study aims to identify the relationship between the implementation of delegation and the application of patient safety. The study design was a quantitative study with a cross sectional approach. The samples were taken on 160 nurses with a total sampling method at one hospital in Jakarta. The analysis was done by chi square test and multiple logistic regression. The results showed that there was a relationship between the implementation of delegation and the application of patient safety (p 0.001; OR 4.302; CI 2.215-8.356) Director of Nursing makes planning the achievement of quality indicators through the implementation of patient safety goals that is not optimal The head manager monitoring and evaluation implementation of the delegations carried out by the primary nurse / team leader."

"No blaming culture merupakan bagian dari budaya keselamatan pasien. Penelitian ini bertujuan untuk mengidentifikasi hubungan no blaming culture dengan pelaksanaan keselamatan pasien oleh perawat di instalasi rawat inap, RSUP Fatmawati Jakarta. Metode penelitian ini menggunakan design deskriptif korelasi dengan pendekatan cross sectional. Sampel penelitian berjumlah 152 perawat, pemilihan sampel dilakukan dengan consecutive sampling. Instrumen penelitian terdiri dari 3 bagian kuisioner yaitu kuisoner data demografi, kuisioner no blaming culture dan kuisioner pelaksanaan keselamatan pasien. Analisis data menggunakan uji chi-square, yang menunjukkan ada hubungan yang bermakna antara no blaming culture dengan pelaksanaan keselamatan pasien oleh perawat di rumah sakit (p=0,003, CI: 1,376-5, 118). Penelitian ini merekomendasikan penerapan no blaming culture di RS dan diharapkan penelitian selanjutnya akan mengidentifikasi hal-hal yang dapat meningkatkan pelaporan kesalahan oleh di RS.

This study aims to determine the relationship of no blaming culture with patient safety implementation by nurses at inpatient installation in Fatmawati Hospital Jakarta. This study uses descriptive correlation design withcross-sectional. Sample was 152 nurses, which is selected by consecutive sampling.

This study uses 3 questionnaire namely demographic questionnaire, patient safety implementation questionnaire and no blaming culture questionaire. This study showed there was significant correlation between no blaming culture with patient safety implementation by nurses in hospitals (p = 0.003, CI 1,376-5,118).

This study recommended the implementation of ~uu blaming culture in hospital to increase patient safety and for further research it is expected to identify the way to improve error reporting by nurse."
