"[ABSTRAKNyeri merupakan salah satu masalah utama pada pasien kanker anak. Karya Ilmiah Akhir ini bertujuan memberikan gambaran penerapan, analisis Comfort Theory Kolcaba pada pasien kanker anak dengan nyeri, dan pencapaian kompetensi ners spesialis keperawatan anak. Penerapan teori dilakukan pada lima pasien, menggunakan pendekatan proses keperawatan. Pengkajian dan penetapan masalah keperawatan menggunakan struktur taksonomi Kolcaba. Intervensi dan implementasi menggunakan intervensi kenyamanan standar, coaching, dan comfort food for the soul. Evaluasi menggunakan comfort behavior scale menunjukkan hasil diakhir perawatan, nyeri berkurang, terkontrol, dan kenyamanan bertambah. Kompetensi tercapai sesuai target. Comfort Theory bisa menjadi pilihan dalam merawat pasien kanker anak dengan nyeri.

"Praktik residensi keperawatan medikal bedah merupakan salah satu bentuk pendidikan keperawatan berkelanjutan yang ditempuh setelah tercapainya gelar magister keperawatan. Tujuannya adalah mencetak ners spesialis keperawatan medikal bedah yang mampu berperan sebagai clinical leader bagi tim pelayanan keperawatan, dengan fokus pada pasien dewasa yang mengalami gangguan pememenuhan kebutuhan dasar manusia akibat gangguan struktur dan fungsi pada sistem organ tubuh, berikut dengan respon pasien yang ditimbulkan. Praktik klinik dilaksanakan di setting gawat darurat, perawatan intensif, rawat inap, dan rawat jalan di RSUPN Dr Cipto Mangunkusumo Jakarta, dan RSUP Fatmawati Jakarta. Dengan fokus pada pasien dengan gangguan sistem endokrin, asuhan keperawatan diberikan dengan menggunakan pendekatan teori model adaptasi Roy pada pasien dengan masalah utama pada gangguan kelenjar tiroid dan pankreas. Contoh kasus yang telah dikelola antara lain pasien dengan komplikasi akut dan kronis dari diabetes, stroma tiroid, keganasan tiroid, serta gangguan multi sistem yang melibatkan diabetes. Asuhan keperawatan berbasis bukti diterapkan berupa latihan kaki Buerger Allen Exercise bagi pasien dengan PAD untuk meningkatkan perfusi kaki. Inovasi pelayanan keperawatan dilakukan dengan fokus pada pencegahan hipoglikemia berat pada pasien diabetes di rawat inap melalui bundle intervensi Hy-NEWSS.

The medical surgical nursing residency program is a continuous nursing education process following the master of nursing degree completion. The graduates are medical surgical nurse specialist, who will take role as clinical nursing leaders in providing care for adult patients with compromised human basic need fulfillment due to structural and functional dysfunctions of body systems along with the responses. The clinical education was conducted in RSUPN Dr Cipto Mangunkusumo Jakarta and RSUP Fatmawati Jakarta covering emergency unit, intensive care unit, inpatient, and outpatient care unit.  Using the Roy Adaptation Model as the framework, patient care was aimed to patients with pancreas and thyroid problems. The examples of the cases were, but not limited to, acute and chronic complications of diabetes, thyroid stroma and malignancy, and multisystem cases related to diabetes. The Buerger Allen Exercises was implemented as evidence based nursing, and the nursing innovation project was aimed towards severe hypoglycemia prevention for patients with diabetes in inpatient setting through the Hy-NEWSS intervention bundle.

"Praktik residensi keperawatan medikal bedah adalah rangkaian dari program pendidikan magister keperawatan yang dilaksanakan dalam tatanan pelayanan keperawatan. Keperawatan medikal bedah berfokus pada konsep dan prinsip dasar medikal dan bedah dalam penerapan ilmu dan tehnologi keperawatan untuk memenuhi kebutuhan pasien dewasa yang mengalami perubahan fisik dengan atau tanpa gangguan struktural. Praktik residensi keperawatan dilaksanakan di RSPAD Gatot Soebroto dan RSUP Fatmawati. Kegiatan Residensi terdiri dari pemberian asuhan keperawatan pada pasien dengan gangguan muskuloskeletal menggunakan pendekatan teori Virginia Henderson. Asuhan keperawatan telah diberikan pada pasien dengan fraktur, osteoarthritis, SCI, osteosarcoma, spondylitis TB dan beberapa kasus lainnya. Pendekatan teori Henderson membantu pasien untuk memenuhi kebutuhannya selama mengalami gangguan muskuloskeletal. Penerapan evidence based nursing (EBN) melalui intervensi edukasi pada pasien yang akan menjalani operasi tulang belakang membantu pasien dalam menurunkan kecemasan dan nyeri. Intervensi edukasi membantu pemahaman pasien dalam menghadapi operasi sehingga mengurangi kecemasan dan dapat mengelola nyeri dengan lebih baik. Proyek inovasi melalui aplikasi peran perawat dalam program Enhace Recovery for Orthopedic Surgery (EROS) pada pasien operasi Total Hip Replacement (THR). Aplikasi peran perawat membantu pasien dalam persiapan operasi sampai dengan rehablitasi paska operasi.

The residency program in medical medical nursing is a series of nursing master education programs carried out in the order of nursing services. Medical surgical nursing focuses on concepts and basic medical and surgical principles in the application of nursing science and technology to meet the needs of adult patients who experience physical changes with or without structural disorders. The practice of nursing residencies was carried out at Gatot Soebroto Army Hospital and Fatmawati General Hospital. Residency activities consist of giving nursing care to patients with musculoskeletal disorders using the Virginia Henderson theory approach. Nursing care has been given to patients with fractures, osteoarthritis, SCI, osteosarcoma, TB spondylitis and several other cases. Henderson's theoretical approach helps patients to meet their needs while experiencing musculoskeletal disorders. The application of evidence based nursing (EBN) through educational interventions in patients who will undergo spinal surgery helps patients reduce anxiety and pain. Educational interventions help understanding patients in the face of surgery so as to reduce anxiety and manage pain better. The innovation project through the application of the role of nurses in the Enhace Recovery for Orthopedic Surgery (EROS) program in patients operating Total Hip Replacement (THR). The application of the role of nurses helps patients in preparation for surgery until postoperative rehabilitation."

"Praktik residensi keperawatan medikal bedah sistem muskuloskeletal merupakan program lanjutan pendidikan magister keperawatan yang berfokus pada konsep dan prinsip dasar medikal bedah dengan menerapkan ilmu dan tehnologi keperawatan untuk memenuhi kebutuhan pasien dewasa yang mengalami perubahan fisik dengan atau tanpa adanya gangguan struktural pada sistem muskuloskeletal. Salah satu model keperawatan yang dapat diterapkan untuk memenuhi kebutuhan pasien gangguan muskuloskeletal adalah pendekatan Teori Keperawatan adaptasi Roy. Pendekatan ini merupakan model asuhan keperawatan yang dapat membantu pasien beradaptasi dengan berbagai permasalahan yang ditimbulkan akibat gangguan sistem muskuloskeletal. The practice of residency in medical surgical nursing of the musculosceletal system is an advanced program of nursing master education that focuses on the concepts and basic principles of medical surgery by applying nursing science and technology to meet the needs of adult patients who experience physical changes with or without structural disorders of the musculosceletal system. One of nursing model that can be applied to meet the need of patients with musculosceletal disorder is Roys Adaptation Theory approach. This approach is a model of nursing care that can help patient to adapt various problems caused by disorder of the musculosceletal system. Another activity of residency practice is implementing evidence based nursing (EBN) through ambulation program intervention in postoperative patients lower limb their mobility. In addition, activities through the application of the role of nurses in implementing of "enhanced recovery after surgery (ERAS)" program in hip surgery patients are innovative projects that can be implemented in the surgical ward to speed up the recovery process, reduce length of stay (LOS), prevent complication, also reduce post morbidity and mortality operation."

"Praktik residensi keperawatan medikal bedah peminatan keperawatan endokrin merupakan kelanjutan dari pendidikan magister keperawatan yang dilaksanakan dalam rangka memberikan kesempatan dan pengalaman kepada residen perawat untuk menerapkan ilmu dan teknologi keperawatan khususnya memberi asuhan keperawatan lanjut dan kompleks dalam menyelesaikan permasalahan keperawatan endokrin pada fasilitas pelayanan kesehatan. Praktik klinik dilaksanakan di setting gawat darurat, perawatan intensif, rawat inap, dan rawat jalan di RSUPN Dr Cipto Mangunkusumo Jakarta. Penerapan teori Model Adaptasi Roy dilakukan oleh residen pada 30 kasus resume dan 1 kasus utama yaitu pada pasien SIDA on ARV, Diabetes mellitus tipe 2, Selulitis regio colli, Hepatitis C, Sirosis hepatis. Asuhan keperawatan berbasis bukti diterapkan berupa latihan Passive Stretching untuk menurunkan gula darah pada pasien yang tidak mampu melakukan latihan fisik mandiri. Inovasi pelayanan keperawatan dilakukan dengan menyusun media edukasi simulasi suntik insulin yang terdiri dari alat New Simulation Tools (alat peraga suntik insulin) video dan buku panduan edukasi suntik insulin (buklet) yang digunakan sebagai panduan dan media edukasi bagi perawat dalam memberikan edukasi suntik insulin, mengurangi rasa takut pasien terhadap penyuntikan, memberikan lebih banyak latihan, dan meningkatkan keterampilan penyuntikan insulin pasien.

The medical surgical nursing residency program specializing in endocrine nursing is a continuation of nursing master education which is carried out in order to provide opportunities and experience to nurse residents to apply nursing science and technology, especially providing advanced and complex nursing care in solving endocrine nursing problems in health care facilities. Clinical practice is carried out in emergency, intensive care, inpatient, and outpatient settings at Dr. Cipto Mangunkusumo Hospital Jakarta. The application of the Roy Adaptation Model theory was carried out by residents in 30 resume cases and 1 main case, namely in patients with SIDA on ARV, Diabetes mellitus type 2, Cellulitis regio colli, Hepatitis C, Cirrhosis hepatis. Evidence-based nursing care is applied in the form of Passive Stretching exercises to reduce blood sugar in patients who are unable to do independent physical exercise. Nursing service innovation is carried out by compiling educational media consisting of insulin injection simulation tools consisting of New Simulation Tools (insulin injection tool) videos and insulin injection education guidebooks (booklets) which are used as guidelines and educational media for nurses in providing insulin injection education, reducing patient fear of injections, providing more practice, and improving patient insulin injection skills."

"Program residensi keperawatan medikal bedah peminatan endokrin adalah program meningkatkan kemampuan perawat spesialis yang kompeten dalam memberikan asuhan keperawatan pada pasien dengan gangguan sistem endokrin. Gangguan sistem endokrin yang paling banyak di pelayanan kesehatan adalah Diabetes melitus (DM). Kadar glukosa darah yang tidak terkontrol akan menimbulkan berbagai komplikasi baik akut maupun kronis. Dibutuhkan pendekatan dari berbagai sistem ilmu untuk memberikan penanganan dan pencegahan komplikasi pada pasien DM. "Penyakit kardiovaskular merupakan sekumpulan penyakit yang mempengaruhi sistem peredaran darah termasuk jantung dan pembuluh darah, disebabkan oleh banyak faktor yang mencakup penyakit bawaan dan didapat. Penyakit kardiovaskular yang disebabkan oleh aterosklerosis meliputi penyakit arteri koroner, penyakit arteri serebral, dan penyakit arteri perifer dengan dua komplikasi utama infark miokard dan stroke iskemik. Perawat spesialis memiliki peran penting sebagai pemberi asuhan keperawatan langsung, peneliti, dan inovator. Peran sebagai pemberi asuhan keperawatan langsung dijalankan dengan mengelola satu kasus utama tentang asuhan keperawatan pada pasien pascaoperasi CABG dengan Low Ejection Fraction, PMI, dan topangan IABP serta 30 kasus resume keperawatan. Peran sebagai peneliti dijalankan melalui penerapan EBN tentang penggunaan gel ice pack untuk mengurangi nyeri pada pasien pascaoperasi jantung yang menjalani chest tube removal. Cardiovascular diseases are a group of diseases that affect the circulatory system, including the heart and blood vessels, caused by many factors that include congenital and acquired diseases. Cardiovascular diseases caused by atherosclerosis include coronary artery disease, cerebral artery disease, and peripheral artery disease, with two major complications: myocardial infarction and ischemic stroke. Specialist nurses play an important role as direct nursing care providers, researchers, and innovators. The role of direct nursing care providers is carried out by managing a major case of nursing care for post-CABG patients with Low Ejection Fraction, PMI, and IABP support, as well as 30 nursing resume cases. The role of researchers is carried out through the application of EBN on the use of ice pack gel to reduce pain in post-cardiac surgery patients undergoing chest tube removal. The role of innovators is carried out in the preparation of innovation projects on the application of the FICA Spiritual Assessment Tool to meet the spiritual needs of patients with cardiovascular disorders. The results of the residency practice analysis show that Levine's conservation model theory approach can be applied in providing nursing care to patients with cardiovascular disorders, the application of ice pack gel can reduce pain during chest tube removal in post-cardiac surgery patients, and the FICA Spiritual Assessment Tool is easy to apply as an effort to meet the spiritual needs of patients with cardiovascular disorders."

"Prakktik residensi merupakan bagiam dari pendidikan profesi yang bertujuan untuk memebentuk perawat spesialis. Beberapa kompensasi yang harus dicapai dalam praktik residensi keperawatan medikal ebdah adalah manajemen kasus   menggunakan teori model keperawatan, penerapan model keperawatan berbasis bukti, dan proyek inovasi. Model adaptasi Roy digunakan dalam asuhan keperawatan dengan pasien dengan gangguan system pernapasan. Pendekatan model adaptasi Roy bertujuan untuk mempertahankan integritas adaptasi manusia. Model adaptasi Roy berperan dalam membantu mengembangkan mekanisme koping yang adaptif terhadap stimulusdari lingkungan. Pada target kompetensi penerapan asuhan berbasis bukti dengan penerapan Aroma Hand Massage untuk mengurangi nyeri, tingkat kecemasan dan depresi pada pasien dengan kanker paru terminal.Hasil penerapan menunjukkan bahwa terapi aroma hand massage menurunkan skor nyeri dan depresi pada pasien dengan kanker paru terminal. Proyek inovasi adalah penerapan mobile phone application untuk pengkajian dan manajemen keperawatan mandiri pada pasien dengan kanker paru. Hasil penerapan aplikasi tersebut  menunjukkan bahwa kemanfaatan, mudah dalam penggunaan dan kepuasan responden dalam mengaplikasikannya.

The residency practice is a part of professional education to build specialist nurses. Some of the competencies must be achieved in residency practice of medical surgical are case management using nursing model theory, the application of  evidence-based nursing practice, and innovation projects. Roys adaptation mode is used in nursing care in patients with impaired respiratory function. Roys adaptation model aimed to develop adaptive coping mechanism from environment stimuli.

The implementation of evidence based nursing used Aroma Hand Massage was effective and useful significantly to reduce score pain and depression on patient with terminal lung cancer. Innovation project was the use of mobile phone application for assessment and management nursing intervention  on patient with lung cancer it was  useful, easy and fun to be applied."

"Peran perawat spesialis diperlukan untuk peningkatan kualitas pelayanan asuhan keperawatan yang kompleks dan akurat sebagai pemberi asuhan keperawatan tingkat lanjut kasus neurosain, pendidik, advokat, serta agen pembaharu melalui penerapan evidence based nursing (EBN) dan proyek inovasi. Asuhan keperawatan tingkat lanjut menggunakan teori Adaptasi Roy yaitu pengelolaan pasien meningitis tuberkulosis sebagai kasus utama dan 30 resume gangguan sistem neurologi. Teori Roy banyak bertujuan meningkatkan perilaku adaptif dan mengubah perilaku inefektif. Diagnosis keperawatan yang paling banyak ditemukan pada pasien gangguan sistem neurologi adalah ketidakefektifan perfusi jaringan serebral dan hambatan mobilitas fisik. Penerapan EBN dilakukan pada pasien stroke yang mengalami disfagia. Pasien diberikan latihan menelan shaker exercise hasilnya menunjukkan peningkatan kemampuan menelan dan tidak terjadi aspirasi. Proyek inovasi kelompok menerapkan Pengembangan Media Edukasi Perawatan Pasien Brain Tumor Craniotomy. Penerapan proyek inovasi meningkatkan pengetahuan pasien, keterampilan pasien latihan napas dalam dan mobilisasi setelah operasi, dan menambah kepercayaan diri perawat saat memberikan edukasi. Pengalaman praktik residensi diharapkan menambah kompetensi dan peran perawat spesialis di lahan klinik.

The role of nurse specialists is needed to improve the quality of complex and accurate nursing care services as providers of advanced nursing care in cases of neuroscience, educators, advocates, and agents of reform through the application of evidence based nursing (EBN) and innovation projects. Advanced nursing care uses Roy's Adaptation theory, which is the management of meningitis tuberculosis patients as the main case and 30 resumes of neurological system disorders. Roys theory aims to improve adaptive behavior and change ineffective behavior. The most common nursing diagnoses found in patients with neurological system disorders are ineffective perfusion of cerebral tissue and barriers to physical mobility.

EBN application is performed on stroke patients who have dysphagia. The patient is given training to swallow the exercise shaker, which results in increased swallowing ability and no aspiration. The group innovation project applies Development of Educational Media for Nursing Brain Tumor Craniotomy Patients. The application of innovation projects increases patient knowledge, the skills of patients in deep breathing exercises and mobilization after surgery, and increases nurse confidence when providing education. The residency practice experience is expected to increase the competency and role of specialist nurses on the clinic grounds."

"Praktek Spesialis Keperawatan merupakan bagian penting dari pendidikan profesi dalam rangka mengaplikasikan peran perawat spesialis untuk meningkatkan kualitas pelayanan. Praktek spesialis keperawatan dilakukan dan dikembangkan berdasarkan ilmu dan teknologi. Kompetensi yang harus dicapai selama praktek residensi keperawatan medikal bedah adalah memberikan asuhan keperawatan dengan menggunakan teori model keperawatan, penerapan tindakan keperawatan berbasis bukti ilmiah (evidence based nursing practice) dan melakukan proyek inovasi. Peran sebagai pemberi asuhan keperawatan, residen melakukan asuhan keperawatan dengan menggunakan pendekatan teori Model Adaptasi Roy pada  30 pasien dengan gangguan sistem respirasi. Target kompetensi dalam hal penerapan tindakan keperawatan berbasis bukti ilmiah adalah penerapan terapi akupresur untuk mengurangi mual muntah pada pasien kanker paru yang menjalani kemoterapi.Hasil penerapan menunjukkan bahwa akupresur efektif mengurangi mual muntah pada pasien kanker paru yang menjalani kemoterapi. Peran selanjutnya sebagai innovator, proyek inovasi yang dilakukan berupa penggunaan mobile phone application untuk pengkajian dan manajemen keperawatan mandiri pada pasien kanker paru. Hasil penerapan menunjukkan bahwa mobile phone application bermanfaat bagi pasien dalam manajemen gejala secara mandiri.

Nursing Specialist Practice is an important part of professional education in order to apply the role of specialist nurses to improve service quality. Nursing specialist practice is carried out and developed based on science and technology. Competency that must be achieved during the residency practice of surgical medical nursing is to provide nursing care using the theory of nursing models, application of scientific evidence-based nursing practice, and do innovation projects. Role as a provider of nursing care, resident carries out nursing care using the Roy Adaptation Model theory approach in 30 patients with respiratory system disorders. The competency target in the application of scientific evidence-based nursing actions is the application of acupressure therapy to reduce nausea and vomiting in lung cancer patients undergoing chemotherapy.

The results of the application showed that acupressure effectively reduced nausea and vomiting on patients with lung cancer undergoing chemotherapy. The next role as an innovator, innovation projects carried out in the form of the use of mobile phone applications for the assessment and management of independent nursing on patients with lung cancer. The results of the application show that mobile phone applications are beneficial for patients in symptom management independently."
