"Chevron Geothermal & Power Operation Indonesia, merupakan suatu perusahaan terbesar yang bergerak dalam bidang minyak dan gas bumi. Dalam proses produksi, perusahaan menyadari bahwa frekuensi resiko kemungkinan terjadi kecelakaan kerja jauh lebih tinggi. Pada umumnya kecelakaan kerja disebabkan oleh dua faktor yaitu manusia dan lingkungan.
Tetapi frekuensi terjadinya kecelakaan kerja lebih banyak terjadi karena faktor manusia, karen isa manusia yang paling banyak berperan dalam menggunakan peralatan diperusahaan. Oleh karena itu, Chevron GPO-I melaksanakan program pelatihan Behavior Based Safety, yang diharapkan dapat menurun tingkat kecelakaan kerja.
Dalam penelitian ini, penulis menggunakan teori Donald Kirkpatrick yang mempunyai 4 tingkatan untuk mengukur evaluasi pelaksanaan pelatihan, yaitu Reaksi, Pembelajaran, Perilaku dan Hasil. Metode penelitian yang digunakan adalah dengan pendekatan kuantitatif dan jenis penelitiannya yaitu deskriptif, pengumpulan data dengan menggunakan kuesioner dan wawancara. Sampel yang diambil sejumlah 30 responden dan teknik analisis data dengan menggunakan skala likert.
Dari hasil analisis penelitian bahwa pelaksanaan pelatihan behavior Based Safety sangat baik. Karena peserta yang mengikuti pelatihan tersebut menyadari akan pentingnya keselamatan dalam bekerja. Dari hasil analisis dapat ditarik kesimpulan bahwa Chevron GPO-I telah melaksanakan pelatihan sesuai dengan kebutuhan perusahaan. Oleh karena itu pelatihan Behavior Based Safety perlu dilaksanakan agar kecelakaan kerja dapat diminimalisir.

Chevron Geothermal & Power Operation Indonesia is a big Company moveable that in oil and gas. In the Production Process, the company to realize that risk frequency a possibility consist for more a high of work accident. In general of work accident caused by two factors are human & area.
But to consist of frequency work accident caused by human factor, because the human is more a part use tools in company. Therefore Chevron Geothermal & Power Operation Indonesia to accomplish behavior based safety training program, and the hope is can of down the work accident.
In the research this, the writer use Donald L Kirkpatrick theory his have four step for to stroll performance training program are reaction, learning, behavior and result. The use research method is with quantitative approach and the kind research is descriptive, the gathering material with quesionare and interview use. The sample to take is 30 respondent and material analysis technic with of likert scale use.
And the result of examination analysis that the performance behavior based safety training is very good. Because of training follower is to become conscious will the important safety in work. And the analysis result can to draw a conclusion that Chevron Geothermal & Power Operation Indonesia performance with the need company. Therefore behavior based safety training to obliged performed, so that work accident can the minimize."

Chevron is the world's largest private producer of geothermal energy, generating more than 630 megawatts of clean, reliable and affordable energy in Indonesia now. Chevron Geothermal manages two geothermal projects in Indonesia ? Darajat and Salak, both on the island of Java. The Darajat project supplies geothermal steam, which generates 259 megawatts of electricity. Chevron Geothermal Indonesia realizes to be more productive, Chevron needs strong culture organizations. Chevron has The Chevron Way that explains who we are, what we do, what we believe and what we plan to accomplish. It establishes a common understanding not only for those of us who work here, but for all who interact with us. The objective of organizational culture is to aligning employee?s perception and behavior to achieve the company mission. Organizational culture and organizational character are make organization different from other. Based on the purpose of this research, the theory that used is corporate culture that the researcher from Stephen P. Robbins. Research question is about how employee?s perception Corporate Culture at Chevron Geothermal Indonesia, Ltd. This research using quantitative approaches that has 1 variable, descriptive characteristic and catagorizad as cross sectional research. Collecting data technique was done by survey and bibliography research. Technique in taking sample is total sampling; those overall employees of Division Facilities Engineering are 30 respondents. Analysis in this research was done by using frequency distribution table, which a part of unvariate analysis and it made based on data that come within questioner. The result show that employee?s have positive perception toward corporate culture on Chevron Geothermal Indonesia, Ltd Division Facilities Engineering Jakarta. The corporate culture is quite good with strong culture. The management should be improved so that the employee can work optimally."

On this paper, the research is how perception of operational officer Customer Service Division about evaluation training of achievement motivation training at PT 'X' and what is the problems that PT 'X' faced of operational Officer Customer Service Division employees and company in evaluation of Achievement Motivation Training implementation?.
The kinds of this research is descriptive research with quantitative approaches. Data in this research gathered from field research method by distributing questionnaire to the respondent and interview. The samples of this research were 63 contract employees, taken from population using total sampling technique. The data of the univariate were collected using Likert scale which delivered frequency distribution. Company especially on Customer Service Division, employee that have been follow Achievement Motivation Training hope that this training can be implementing every day whether in the office or around the office, so far from the mistake that cause from the behavior that not follow the procedure.
For all perception of officer about Achievement Motivation Training that had been done by the company in PT.'X' in regional office Jabotabek & Jabar reach good result, according to reaction dimention, learning, and behavior by the officer which has follow the training feel satisfied and feel useful to the job, so the mistake that cause by working not detailed can be reduced."

Tesis ini bertujuan untuk mendapatkan hasil evaluasi atas implementasi Behavior Based Safety (BBS) program dilihat dari aspek input ? proses ? output pada pekerja PT.Chevron Indonesia Company Kalimantan Timur tahun 2014.
Dari hasil penelitian didapatkan, Aspek input berjalan baik sesuai dengan panduan, bahkan pemahaman akan pelatihan mendapatkan hasil sangat baik. Aspek proses, terdapat 30% anggota tim pengarah yang pasif, 10% hasil observasi tidak memenuhi kriteria observasi yang berkualitas, ada peningkatan jumlah bimbingan 66%. Aspek output; partisipasi yang hampir 100% karena observasi diwajibkan oleh manajemen, peningkatan perilaku aman 0.3% - 0.4% dari tahun sebelumnya, TRI rate menunjukan trending penurunan overall 40% sejak penerapan program di Kalimantan dan hasil review berkelanjutan yang mendapat hasil kenaikan kelulusan untuk kriteria tim pelaksana, lembar observasi, training, komunikasi, serta analisa data sedangkan yang tidak lulus adalah kriteria dukungan manajemen, tindak lanjut komentar yang membutuhkan tindak lanjut dan penghargaan kepada observer dan tim pelaksana.
Program BBS yang sudah dijalankan lebih 6 tahun ini menunjukan tanda-tanda penuaan atau membosankan diperlukan dukungan manajemen untuk memperkuat peran BBS dalam mencapai tujuan yang diharapkan.

Behavior Based Safety (BBS) is one of proactive process/program that is used widely in industry as efforts to increase workplace safety and the environment through the increasing of safe behavior and reducing risk behaviors in the workplace. The program started with the establishment of implementation team, develop observation sheets, and observation training for workers as input aspect. Then aspects of the process included activities of observation, effectiveness of the implementation team, result of observation and coaching. The output aspect are compliance to submit observation, the participation rate, the percentage (%) safe behavior, Total Recordable Incident (TRI) rate and sustainability review.
Aims of this thesis is to obtain an evaluation of the implementation of Behavior Based Safety (BBS) program reviewed from the aspect input - process - output on workers PT.Chevron East Kalimantan Indonesia Company in 2014.
Results of this research, in all aspect input items were well done accordance with the reference, understanding the training get very good. Aspects of the process, there was 30% passive steering team members, 10% of the observations not meet the criteria of quality observation, increasing of 66% coaching activities. Aspects output; the participation of nearly 100% since observations is mandatory, increasing in safe behavior 0.3% - 0.4% from the previous year, the TRI rate shows decline trending of 40% overall since the implementation of the program in Kalimantan and the results of sustainability review that gets the results of the increase in pass to the criteria of the implementation team, observation sheets, training, communication, and data analysis while not pass criteria are management support, follow-up comments that require follow-up steering team and regocnize appreciation to the observer and the implementation team.
BBS programs that have been run over 6 years is showing signs of aging or boring which is management support needed to strengthen the role of BBS in achieving the desired objectives.

"ABSTRACT
Energi listrik menjadi kebutuhan utama dari kehidupan saat ini. Namun, energi listrik berkaitan erat dengan bahan bakar. Saat kebutuhan listrik yang terus meningkat di lain hal ketersediaan bahan bakar terus berkurang. Oleh karena itu, dibutuhkan energi alternatif pengganti bahan bakar fossil tersebut yaitu, Panas Bumi. Namun, pada kenyataannya produksi Panas Bumi akan terus mengalami penurunan performa dari reservoirnya. Ada beberapa alternatif yang dapat dilakukan untuk meningkatkan performa tersebut. Penelitian ini dilakukan untuk mencari peluang debottlenecking dengan simulasi aliran pada sumur produksi menggunakan simulator Geoflow, dan mengevaluasi sistem surface facilities. Simulasi dilakukan dalam 3 variasi sumur yaitu As-Is, 5 Years Cycle, dan 10 Years Cycle dan variasi surface facilities constraint dan unconstraint. Hasil evaluasi menunjukkan bahwa bottleneck terjadi pada Pad 14 Header dan 14/18 Corridor yang teridentifikasi kecepatan fluida melebihi 40 m/s dan perbedaan tekanan yang signifikan. Sehingga dapat dilakukan debottlenecking opportunity melalui metode twinning pipe yang memberikan penurunan pada kecepatan aliran dan perbedaan tekanan dengan nilai WHP yang baru untuk masing-masing sumur. Hasil perhitungan ekonomi menunjukkan bahwa debottlenecking opportunity memberikan hasil yang lebih optimis pada variasi sumur As-Is dengan nilai NPV 3,04 $MM, IRR 37% dan payback period 6,87 tahun pada Pad 14 Header dan nilai NPV 1,66 $MM, IRR 19%, Payback period 7,93 pada 14/18 Corridor.

ABSTRACT
Electricity energy nowdays becoming priority needs and utility for our daily activity. But, to produce electricity needs fuels. When electricity demand increasing while the availability of fuels decreasing everyday. So, we need alternative energy to subtitute fuels, One of them are geothermal. Meanwhile, the production of geothermal will be decreasing each day. There are alternative to do in term to improving the performance of reservoir geothermal. This research have goal to find debottlenecking opportunity with simulation in wellbore using Geoflow and evaluate the system of surface facilities. Simulation have 3 variation of wells they are As-Is, 5 years cycle, 10 years cycle and also the variation of surface facilitie constraint and unconstraint. The result of evaluation shown the bottleneck occurs in Pad 14 Header and 14/18 Corridor that have velocity more than 40 m/s and significant pressure different. So, debottlenecking opportunity can be perform by doing twinning pipe that give decreasing in velocity and pressure different with new value of WHP for each wells. The result of evaluation economy give solution that debottlenecking opportunity have more optimistic result in variation wells As-Is with score NPV 3,04 $MM, IRR 37%, Payback Period 6,87 years in Pad 14 Header and NPV 1,66 $MM, IRR 19%, Payback Period 7,93 years in 14/18 Corridor.

"Kecelakaan kerja seringkali disebabkan oleh perilaku tidak aman. Program Behavior Based Safety memiliki peranan penting dalam menciptakan zero accidents. Implementasi dari program BBS memfokuskan pada observasi dimana dari observasi tersebut akan membentuk suatu kewaspadaan terhadap perilaku tidak aman dan meningkatkan perilaku aman. PT X sudah menerapkan program BBS dari tahun 2008-2016 dan didapatkan hasil evaluasi internal yang cukup baik, akan tetapi setelah ditanyakan secara langsung terhadap observer (karyawan), ditemukan praktik observasi yang tidak sesuai dengan kaidah BBS yang sudah disepakati yang mana hal ini dapat mempengaruhi hasil evaluasi internal PT X, dimana hal ini ditunjukkan ketika reinforcement ditiadakan, jumlah observer mengalami penurunan sebesar 68,2%. Tujuan penelitian ini mengevaluasi program BBS yang ada di PT X. Penelitian ini merupakan penelitian deskriptif dengan pendekatan kuantitatif. Evaluasi yang dilakukan menggunakan 3 parameter besar, dengan total 8 sub parameter dengan penyebaran kuesioner terhadap divisi operasional, perawatan, tim pendukung ( Field Engineering Support, Laboratory, Medical dan Facility Management). Hasil analisis data menyebutkan bahwa evaluasi internal yang dilakukan oleh PT X masih kurang dimana terdapat unsur-unsur yang belum dimasukkan dalam evaluasi tersebut, antara lain keterlibatan pekerja dalam penentuan baseline, goal setting dan komitmen pekerja dan manajemen dalam pemberian feedback. Dan ditemukan bahwa reinforcement merupakan faktor penting dalam menjalankan program BBS ini.

Work accidents are mostly caused by unsafe behavior. Behavior Based Safety (BBS) program plays an important role to create zero accidents. The implementation of behavior-based safety program is focused to observe the behavior which is in observation will create unsafe behavior awareness and improve safe behavior. PT X has implemented BBS program from 2008-2016 and got good internal evaluation results, but after crosscheck directly to observer (employee), found observation practice which is not in accordance with BBS rules which it can influence the results of internal evaluation PT X, indicated when reinforcement is eliminated, the number of observers decreased by 68.2%. The purpose of this study is to evaluate the BBS program in PT X. This research is a descriptive research with quantitative approach. The evaluation was conducted using 3 large parameters, with a total of 8 sub-parameters with questionnaires distributed to the operational, maintenance, support gorup (Field Engineering Support, Laboratory, Medical and Facility Management) divisions. The results of the data analysis indicate that the internal evaluation conducted by PT X is still lacking where there are elements that have not been included in the evaluation, including the involvement of workers in determining the baseline, goal setting and commitment of workers and management in giving feedback. And found that reinforcement is an important factor in implementing BBS program."

"ABSTRAK
Penelitian ini bertujuan untuk mengukur pelaksanaan program keselamatan dan kesehatan kerja melalui persepsi karyawan pada departemen maintenance and operation pada PT. Truba Jaya Engineering. Pendekatan yang digunakan adalah pendekatan kuantitatif. Peneliti menguji sebanyak 41 responden menggunakan teknik total sampling. Data dikumpulkan menggunakan kuesioner dengan menilai ukuran persepsi karyawan melalui skala likert. Selanjutnya data dianalisis dengan metode rentang skala dan pengkategorian persepsi karyawan menjadi sangat rendah, rendah, sedang, tinggi dan sangat tinggi.
Hasil penelitian menunjukan bahwa secara keseluruhan tingkat persepsi karyawan terhadap pelaksanaan program K3 tergolong dalam kategori persepsi tinggi. Saran penelitian agar perusahaan terus meningkatkan pelaksanaan program K3.

ABSTRACT
This study aims to measure the implementation of occupational health and safety program through the perceptions of employees in the department of maintenance and operation at PT. Truba Jaya Engineering. The approach used is a quantitative method. Researchers tested by 41 respondents used a total sampling technique. Data were collected using a questionnaire to assess the size of the perceptions of employees through a Likert scale. Furthermore, the data were analyzed with the range of scales and categorical perception of employees to be very low, low, medium, high and very high.
The results showed that overall levels of employee perceptions of the implementation of the program are in high category. Research suggestions that the company continues to enhance the implementation of the OHS program.

"Data ILO 2 juta pekerja meninggal per tahun. 2021 ada 234 ribu kecelakaan kerja di Indonesia. Industri kimia 12% dari2 019-2021 kecelakaan kerja 80.607 kasus.Program Behavior-based safety (BBS) yaitu pendekatan keselamatan berdasarkan perilaku manusia. Pendekatan untuk meningkatkan kesadaran, keterlibatan, tanggung jawab karyawan menciptakan lingkungan kerja aman. program BBS di perusahaan apakah mampu meningkatkan tingkat kepatuhan karyawan, serta merubah perilaku keselamatan kerja. Penelitian dilakukan dengan metode penelitian kuantitatif jenis studi cross sectional. Data yang dikumpulkan memberikan gambaran hubungan antara variabel. Desain penelitian cross-sectional digunakan untuk mengumpulkan data mengenai program Behavior Based Safety (BBS), tingkat kepatuhan terhadap perilaku keselamatan kerja, dan perilaku keselamatan kerja di PT. X pada satu titik waktu tertentu. Data dikumpulkan melalui kuesioner yang diisi oleh respondenVariasi nilai tingkat kepatuhan mampu dijelaskan oleh variasi nilai program BBS sebesar 14,3%,sedangkan sisanya 85.7% dijelaskan oleh variasi variabel lain di luar model penelitian. Variasi nilai Perilaku keselamatan kerja karyawan mampu dijelaskan oleh variasi nilai Program BBS dan Tingkat kepatuhan sebesar 49.6%, sedangkan sisanya 50.4% dijelaskan oleh variasi variabel lain di luar model penelitian terdapat hubungan positif dan signifikan Program BBS terhadap Tingkat Kepatuhan. Ada hubungan positif dan signifikan antara Program BBS terhadap Perilaku Keselamatan Kerja. Ada hubungan positif dan signifikan antara Tingkat Kepatuhan terhadap Perilaku Keselamatan. Hubungan Program BBS terhadap Perilaku keselamatan kerja bersifat pengaruh langsung. Oleh karena itu, variabel intervening Tingkat Kepatuhan tidak memediasi hubungan Program BBS terhadap Perilaku Keselamatan Kerja.

Behavior-based safety (BBS) program, namely a safety approach based on human behavior. An approach to increasing employee awareness, involvement, and responsibility creates a safe work environment. Whether the BBS program in the company is able to increase employee compliance levels and change work safety behavior. The research was carried out using quantitative research methods, a type of cross sectional study. The data collected provides an overview of the relationship between variables. A cross-sectional research design was used to collect data regarding the Behavior Based Safety (BBS) program, the level of compliance with work safety behavior, and work safety behavior at PT. X at a certain point in time. Data were collected through questionnaires filled out by respondents. Variations in compliance level values were able to be explained by variations in BBS program values of 14.3%, while the remaining 85.7% were explained by variations in other variables outside the research model. Variations in employee safety behavior values can be explained by variations in BBS Program values and compliance levels of 49.6%, while the remaining 50.4% are explained by variations in other variables outside the research model. There is a positive and significant relationship between the BBS Program and Compliance Levels. There is a positive and significant relationship between the BBS Program and Work Safety Behavior. There is a positive and significant relationship between the level of compliance and safety behavior. The relationship between the BBS Program and work safety behavior has a direct influence. Therefore, the intervening variable Compliance Level does not mediate the relationship between the BBS Program and Work Safety Behavior"

"Penelitian ini difokuskan untuk melihat hubungan antara pelaksanaan program K3 dengan produktivitas kerja karyawan pada divisi operasional PT Surveyor Indonesia, Jakarta. Penelitian ini menggunkan metode kuantitatif. Data dikumpulkan dengan cara survey, yang menggunakan kuesioner. Terdapat 5 dimensi untuk program K3 yang merupakan teori dari Mathis & Jackson dan 2 dimensi untuk produktivitas kerja karyawan dari teori Stephen P. Robbin. Adapun sampel yang digunakan adalah karyawan PT Surveyor Indonesia pada divisi operasional yang berjumlah 50 orang.
Peneliti menggunakan factor analisis untuk mengukur validitas dan Cronbach?s alpha untuk realibilitas. Untuk mengetahui ada atau tidaknya hubungan antara kedua variabel tersebut, maka digunakan teknik analisis spearman. Hasil dari pengukuran tersebut ditemukan bahwa terdapat hubungan yang kuat antara pelaksanaan program K3 dengan produktivitas kerja karyawan. Saran yang diusulkan dalam penelitian ini agar perusahaan lebih meningkatkan komitmen dan kebijakan K3, untuk dapat meningkatkan produktivitas kerja karyawannya.

This study focused on the relationship between occupational health and safety programme with employee productivity at the operational division of PT Surveyor Indonesia, Jakarta. This study use the quantitative method. Data were collected by survey, using questionnaires. There are five dimensions to occupational health and safety program that is the theory of Mathis & Jackson and two dimensions to the work productivity of employees of the theory of Stephen P. Robbin. Samples used the employees of PT Surveyor Indonesia on the operational divisions, amounted to 50 people.
Researchers used factor analysis to measure the validity and Cronbach's alpha for reliability. To find out whether there is any relationship between two variables, the analytical technique used spearman. The results of these measurements found that there is a strong enough relationship between occupational health and safety program with employee productivity. Suggestions proposed in this study in order to further enhance the company's commitments and policies occupational health and safety, to be able to increase employee productivity.
