"Praktik residensi keperawatan medikal bedah adalah rangkaian dari program pendidikan magister keperawatan yang dilaksanakan dalam tatanan pelayanan keperawatan. Keperawatan medikal bedah berfokus pada konsep dan prinsip dasar medikal dan bedah dalam penerapan ilmu dan tehnologi keperawatan untuk memenuhi kebutuhan pasien dewasa yang mengalami perubahan fisik dengan atau tanpa gangguan struktural. Praktik residensi keperawatan dilaksanakan di RSPAD Gatot Soebroto dan RSUP Fatmawati. Kegiatan Residensi terdiri dari pemberian asuhan keperawatan pada pasien dengan gangguan muskuloskeletal menggunakan pendekatan teori Virginia Henderson. Asuhan keperawatan telah diberikan pada pasien dengan fraktur, osteoarthritis, SCI, osteosarcoma, spondylitis TB dan beberapa kasus lainnya. Pendekatan teori Henderson membantu pasien untuk memenuhi kebutuhannya selama mengalami gangguan muskuloskeletal. Penerapan evidence based nursing (EBN) melalui intervensi edukasi pada pasien yang akan menjalani operasi tulang belakang membantu pasien dalam menurunkan kecemasan dan nyeri. Intervensi edukasi membantu pemahaman pasien dalam menghadapi operasi sehingga mengurangi kecemasan dan dapat mengelola nyeri dengan lebih baik. Proyek inovasi melalui aplikasi peran perawat dalam program Enhace Recovery for Orthopedic Surgery (EROS) pada pasien operasi Total Hip Replacement (THR). Aplikasi peran perawat membantu pasien dalam persiapan operasi sampai dengan rehablitasi paska operasi.

The residency program in medical medical nursing is a series of nursing master education programs carried out in the order of nursing services. Medical surgical nursing focuses on concepts and basic medical and surgical principles in the application of nursing science and technology to meet the needs of adult patients who experience physical changes with or without structural disorders. The practice of nursing residencies was carried out at Gatot Soebroto Army Hospital and Fatmawati General Hospital. Residency activities consist of giving nursing care to patients with musculoskeletal disorders using the Virginia Henderson theory approach. Nursing care has been given to patients with fractures, osteoarthritis, SCI, osteosarcoma, TB spondylitis and several other cases. Henderson's theoretical approach helps patients to meet their needs while experiencing musculoskeletal disorders. The application of evidence based nursing (EBN) through educational interventions in patients who will undergo spinal surgery helps patients reduce anxiety and pain. Educational interventions help understanding patients in the face of surgery so as to reduce anxiety and manage pain better. The innovation project through the application of the role of nurses in the Enhace Recovery for Orthopedic Surgery (EROS) program in patients operating Total Hip Replacement (THR). The application of the role of nurses helps patients in preparation for surgery until postoperative rehabilitation."

"Asuhan keperawatan yang diberikan oleh perawat sangat mempengaruhi kualitas pelayanan kesehatan yang akan diterima oleh pasien. Upaya yang dilakukan untuk meningkatkan kualitas asuhan keperawatan tersebut salah satunya dengan menerapkan peran ners spesialis. Tujuan penulisan Karya Ilmiah Akhir (KIA) ini adalah mampu menampilkan peran ners spesialis sebagai pemberi asuhan keperawatan lanjut, pembaharu dan peneliti. Metodologi dan pendekatan yang digunakan adalah dengan menggunakan studi literature. Karya Ilmiah Akhir berisi tentang : penerapan teori Virginia Henderson pada pasien dengan Neglected Fracture Left Shaft Femur, penerapan Progressive Muscle Relaxation (PMR) sebagai Evidance Based Nursing dan proyek inovasi yaitu Aplikasi Peran Perawat dalam Program Enhanced Recovery Orthopedic Surgery (EROS) pada Pasien Operasi Total Hip Replacement (THR). Hasil : Penerapan teori Virginia Henderson efektif diterapkan pada kasus muskuloskeletal. Intervensi Progressive Muscle Relaxation (PMR) juga efektif dalam menurunkan Anxiety pasien. Kesimpulan : bahwa teori Virginia Henderson dapat digunakan pada pasien dengan Neglected Fracture Left Shaft Femur untuk meningkatkan kemandirian akibat perubahan fisik dan psikologis. Intervensi Progressive Muscle Relaxation (PMR) dapat digunakan sebagai salah satu alternatif intervensi keperawatan mandiri dalam membantu mengurangi Anxiety pasien fraktur ekstremitas yang akan di lakukan tindakan operasi elektif.

Conclusion: that Virginia Hendersons theory is appropriately used in patients with Neglected Fracture Left Shaft Femur to improve independence due to physical and psychological changes. The intervention of Progressive Muscle Relaxation (PMR) can be used as one of the alternative independent nursing interventions to help reduce Anxiety patients in limb fracture elective surgery."

Penerapan Evidence Based Nursing (EBN) melalui intervensi latihan isometrik pada pasien pasca operasi fraktur ektremitas bawah, efektif menurunkan nyeri dan meningkatkan kekuatan otot. Proyek inovasi berupa pengembangan Enhanced Recovery after Surgery (ERAS) for orthopaedic untuk mempercepat proses pemulihan pasien operasi hip. Pemberian asuhan keperawatan pada pasien dengan gangguan muskuloskeletal menggunakan pendekatan teori Model Adaptasi Roy dapat membantu pasien untuk beradaptasi terhadap penyakitnya. Intervensi latihan isometrik jika dikombinasikan dengan terapi non farmakologis yang lain seperti nafas dalam berpotensi bisa memberikan efek yang lebih baik terhadap penurunan nyeri.

The purpose of writing this final scientific paper is to analyzethe practical activities of nurse specialist medical surgical nursing based on the competency standards of medical surgical nursing specialists, specifically the musculoskeletal system. Nurse specialists in conducting nursing care in patients withmusculoskeletal disorders use the Roy Adaptation Model theory approach both in thecase of main and resume managed. The application of Evidence Based Nursing (EBN)through isometric exercise intervention in patients with lower extremity fracture surgery, effectively reduces pain and increases muscle strength. The innovation project was in the form of developing Enhanced Recovery after Surgery (ERAS) for orthopedics to accelerate the recovery process for hip surgery patients. Giving nursing care to patients with musculoskeletal disorders using the Roy Adaptation Model theory approach can help patients adapt to the disease. An isometric exercise intervention if combined with other non-pharmacological therapies such as deep breathing has the potential to have a better effect on reducing pain."

As a researcher, the nurse applied Evidence- Based Nursing Practice in implementing nursing interventions. The application of strength and mobilization training was safe for the patient and improved ROM (Range of Motion) and gait. In addition, as an agent of change, the nurse implemented audio-visual (web)-based education (EduHart), and the readiness to go back home increased significantly. This education method was not limited by time and place to be accessed as needed and help improve the personal status, knowledge, coping abilities, and support for hip arthroplasty patients. A series of activities carried out in residency practice aims to improve the quality of holistic nursing care services."

"Fraktur merupakan masalah sistem muskuloskeletal yang paling sering terjadi. Penanganan fraktur yang tidak tepat dapat mengakibatkan osteomielitis. Osteomielitis tibia merupakan osteomielitis terbanyak kedua setelah femur. Perawat berperan sebagai pemberi asuhan keperawatan, peneliti dan innovator. Peran sebagai pemberi asuhan keperawatan dilakukan dengan pendekatan self care deficit theory pada klien osteomielitis. Masalah yang paling sering terjadi pada klien dengan gangguan sistem muskuloskeletal adalah konstipasi, peran sebagai peneliti dilakukan dengan praktik berbasis bukti (evidence based nursing) yaitu bowel management untuk mencegah konstipasi pada klien pasca Total Hip Replacement (THR) dan Total Knee Replacement (TKR). Inovasi pada tempat praktik dilakukan dengan menyediakan panduan komunikasi dengan metode SBAR pada kasus dengan gangguan sistem muskuloskeletal untuk mendukung komunikasi efektif inter-intra disiplin saat handover maupun komunikasi melalui telepon.

"Gangguan pada muskuloskeletal menimbulkan dampak fisiologis, psikologi, sosial dan ekonomi bagi individu yang mengalaminya. Peran dan kontribusi tenaga kesehatan khususnya perawat spesialis medikal bedah peminatan muskuloskeletal sangat diperlukan dalam menangani permasalahan kesehatan ini. Peran perawat peneliti dilakukan pada penerapan evidence base nursing berupa pengaturan posisi lateral pasca pembedahan tulang belakang untuk menurunkan intensitas nyeri dan meningkatkan mobilitas. Inovasi keperawatan yang dilakukan oleh residen berupa penerapan pengkajian risiko deep vein thrombosis pada klien dengan masalah muskuloskeletal ekstremitas bawah. Pengkajian ini bertujuan sebagai skrining pada klien yang berisiko terjadi deep vein thrombosis sehingga dapat diberikan tindakan pencegahan sebelumnya.

The role and contribution of health workers, especially medical surgical specialist nurses specializing in musculoskeletal care, is needed to deal with this health problem. The provision of evidence-based nursing care supports quality health care. This became the basis for the role of specialist nurses as caregivers, educators, researchers, and innovators. The specialist nurse residency, which was held for two semesters, provided experience and opportunity to apply Orem's self-care nursing theory model to 30 resume cases and one main case, namely a client with multiple traumatic closed fractures of the femur, open fractures of the olecranon, and distal ulna. The role of the research nurse was to apply evidence-based nursing in the form of setting the lateral position after spinal surgery to reduce pain intensity and increase mobility. Residents of the nursing facility implemented an innovative approach of assessing the risk of deep vein thrombosis in clients with lower extremity musculoskeletal issues. This study aimed to screen clients who are at risk for deep vein thrombosis so that preventive measures can be provided beforehand."

"Praktek Spesialis Keperawatan merupakan sebuah proses penting dari pendidikan profesi dalam rangka mengaplikasikan peran perawat spesialis yang berdampak pada upaya peningkatan kualitas layanan keperawatan. Praktek ini dilakukan dan dikembangkan berdasarkan ilmu pengetahuan dan teknologi melalui berbagai penelitian. Teori Virginia Henderson dan Teori Sosial Kognitif Bandura di RSUP FatmawatiJakarta. Sebagai peneliti, mahasiswa residensi telah mengaplikasi penerapan skrining Mual dan Muntah pasien HIV/AIDS menggunakan Index of Nausea, Vomiting and Retcing dan hasil dari evidence based nursing ini menunjukkan bahwa kuisioner INVR akurat dan reliable untuk menilai status mual dan muntah pada pasien HIV. Sedangkan sebagai innovator, telah dilakukan inovasi optimalisasi teknologi digital dalam meningkatkan kepatuhan minum obat ARV pada pasien dengan HIV / AIDS yang mana media teknologi digital berupa laman web yang dengan berbagai topik edukasi dengan web yang dapat di akses menggunakan barcode. Hasil penerapan proyek inovasi ini menunjukkan manfaat yang sangat signifikan bagi peningkatan pengetahuan dan kemudahan pasien dalam mengakases informasi penting tentang penyakit HIV sehingga dapat meningkatkan kepatuhan dalam minum obat ARV.

As researchers, residency students have applied the application of Nausea and Vomiting screening for HIV/AIDS patients using the Index of Nausea, Vomiting and Retcing and the results of this evidence-based nursing show that the INVR questionnaire is accurate and reliable to assess the status of nausea and vomiting in HIV patients. Meanwhile, as an innovator, digital technology optimization innovations have been carried out in improving adherence to taking ARV drugs in patients with HIV / AIDS where digital technology media is in the form of web pages with various educational topics with the web that can be accessed using barcodes. The results of the implementation of this innovation project show very significant benefits for increasing knowledge and ease of patients in accessing important information about HIV disease so as to improve adherence in taking ARV drugs."
