"Praktik residensi keperawatan medikal bedah peminatan keperawatan endokrin merupakan kelanjutan dari pendidikan magister keperawatan yang dilaksanakan dalam rangka memberikan kesempatan dan pengalaman kepada residen perawat untuk menerapkan ilmu dan teknologi keperawatan khususnya memberi asuhan keperawatan lanjut dan kompleks dalam menyelesaikan permasalahan keperawatan endokrin pada fasilitas pelayanan kesehatan. Praktik klinik dilaksanakan di setting gawat darurat, perawatan intensif, rawat inap, dan rawat jalan di RSUPN Dr Cipto Mangunkusumo Jakarta. Penerapan teori Model Adaptasi Roy dilakukan oleh residen pada 30 kasus resume dan 1 kasus utama yaitu pada pasien SIDA on ARV, Diabetes mellitus tipe 2, Selulitis regio colli, Hepatitis C, Sirosis hepatis. Asuhan keperawatan berbasis bukti diterapkan berupa latihan Passive Stretching untuk menurunkan gula darah pada pasien yang tidak mampu melakukan latihan fisik mandiri. Inovasi pelayanan keperawatan dilakukan dengan menyusun media edukasi simulasi suntik insulin yang terdiri dari alat New Simulation Tools (alat peraga suntik insulin) video dan buku panduan edukasi suntik insulin (buklet) yang digunakan sebagai panduan dan media edukasi bagi perawat dalam memberikan edukasi suntik insulin, mengurangi rasa takut pasien terhadap penyuntikan, memberikan lebih banyak latihan, dan meningkatkan keterampilan penyuntikan insulin pasien.

The medical surgical nursing residency program specializing in endocrine nursing is a continuation of nursing master education which is carried out in order to provide opportunities and experience to nurse residents to apply nursing science and technology, especially providing advanced and complex nursing care in solving endocrine nursing problems in health care facilities. Clinical practice is carried out in emergency, intensive care, inpatient, and outpatient settings at Dr. Cipto Mangunkusumo Hospital Jakarta. The application of the Roy Adaptation Model theory was carried out by residents in 30 resume cases and 1 main case, namely in patients with SIDA on ARV, Diabetes mellitus type 2, Cellulitis regio colli, Hepatitis C, Cirrhosis hepatis. Evidence-based nursing care is applied in the form of Passive Stretching exercises to reduce blood sugar in patients who are unable to do independent physical exercise. Nursing service innovation is carried out by compiling educational media consisting of insulin injection simulation tools consisting of New Simulation Tools (insulin injection tool) videos and insulin injection education guidebooks (booklets) which are used as guidelines and educational media for nurses in providing insulin injection education, reducing patient fear of injections, providing more practice, and improving patient insulin injection skills."

Penerapan evidence based nursing Short Form Berg balance Scale telah terbukti sebagai alat skrining yang mudah, murah, hemat waktu dan memiliki reliabilitas Intraclass Correlation Coefisien yang baik untuk pengkaji keseimbangan dan resiko jatuh pada pasien DM yang sudah lanjut usia. Penerapan inovasi New Simulation Tools terbukti efektif mengurangi rasa takut pasien terhadap jarum suntik dan meningkatkan keterampilan menyuntik pada pasien yang baru pertama kali menerima terapi insulin.

The nursing profession always develops following the latest science. This is realized through a process of continuing education after the master of nursing. The residency practice program for medical surgical nursing specialists specializing in endocrine, aims to make residents have a higher level of professional competence, critical and reflective thinking and interpersonal communication when faced with complex clinical conditions. The practice was carried out in the emergency room, intensive care unit, inpatient and outpatient care at RSUPN Dr Cipto Mangunkusumo Jakarta by applying Roy's Adaptation Model theory to 30 resume cases and 1 main case in patients with type 2 diabetes melitus with complications of peripheral arterial disease and diabetic foot ulcers. . The application of evidence-based nursing Short Form Berg balance Scale has been proven as a screening tool that is easy, inexpensive, time-saving and has good Intraclass Correlation Coefficient reliability for assessing balance and risk of falling in elderly DM patients. The application of the innovative New Simulation Tools has been proven to be effective in reducing patients' fear of needles and improving injecting skills in patients who are receiving insulin therapy for the first time."

"Perawat Spesialis Medikal Bedah berperan penting dalam praktik keperawatan sebagai pemberi asuhan keperawatan kompleks, melakukan analisa ilmiah dan pembuktian ilmiah serta sebagai agen pembaharu. Melalui tulisan ini digambarkan pemberian asuhan keperawatan kepada pasien dengan diagnosis Chronic Limb Threatening Ischemia. Tujuan dari penulisan ini adalah memberikan gambaran praktek keperawatan spesialis medikal bedah menggunakan pendekatan Model Adaptasi Roy. Dari hasil pengumpulan data dan pemeriksaan fisik didapatkan 10 diagnosis keperawatan. Pemberian asuhan keperawatanpun dilakukan kepada 30 pasien sebagai kasus resume, diperoleh perilaku maladaptif pada mode fisiologis dengan diagnose terbanyak yaitu resiko infeksi dan nyeri kronik. Evidence Based Nursing dengan melakukan foot exercises kepada 8 pasien yang mengalami ulkus kaki diabetik dengan hasil yang signifikan terhadap perubahan ukuran luka. Program inovasi menerapkan Hypoglycemia-Nursing Early Warning Score System pada pasien diabetes melitus tipe 2 yang terbukti memudahkan perawat menilai resiko kejadian hipoglikemi berat selama 1 tahun. Pendekatan Model Adaptasi Roy berpengaruh besar terhadap profesi keperawatan. Model ini menjadi salah satu yang banyak digunakan dalam penelitian, pendidikan dan pelayanan keperawatan.

Surgical Medical Specialist Nurses play an important role in nursing practice as a provider of advanced nursing care, conduct scientific analysis and scientific evidence and as a reforming agent. Through of this paper we describe the provision of nursing care to patients with a diagnosis of Chronic Limb Threatening Ischemia. The purpose of this paper is to provide an overview of the nursing practice of Surgical Medical Specialists using Roy's Adaptation Model approach. From the results of data collection and physical examination obtained 10 nursing diagnoses. In nursing care was provided to 30 patients as a case of resume, obtained maladaptive behavior in physiological mode with the most diagnoses, the risk of infection and chronic pain. Evidence Based Nursing by performing foot exercises on 8 patients who suffered diabetic foot ulcers with significant results on changes in wound size. The innovative program implementated the Hypoglycemia-Nursing Early Warning Score System in patients with type 2 diabetes mellitus which was proven to make it easier for nurses to assess the risk of severe hypoglycemic events for 1 year. Roy's Adaptation Model approach greatly influencing the nursing profession. This model is one that was widely used in research, education and nursing services"

"Penyakit endokrin merupakan penyebab kematian dengan angka yang cukup tinggi baik secara nasional maupun global. Hal ini menjadi tantangan bagi seluruh tenaga kesehatan khususnya bagi perawat spesialis medikal bedah peminatan endokrin dalam ikut membantu menurunkan tingkat komplikasi yang terjadi. Praktik ilmu keperawatan yang berkualitas berdasarkan evidence base nursing terbaru menjadi landasan dalam melaksanakan peran sebagai pemberi asuhan, peneliti, pendidik dan inovator. Praktik residensi perawat spesialis peminatan endokrin berlangsung dalam dua semester telah memberikan pengalaman dan kesempatan untuk menerapkan teori keperawatan secara menyeluruh dalam mengelola asuhan kepada pasien. Penerapan model teori adaptasi Roy dilakukan oleh residen terhadap 30 kasus resume dan satu kasus kelolaan utama yaitu pada pasien Diabetes Melitus type 2 dengan riwayat KAD, akut on CKD, hipertensi, dyslipidemia dan pankreatitis akut. Peran peneliti dilakukan dengan menerapkan evidence base nursing yaitu melalui inlows sixty second sebagai alat screening risiko terjadinya luka kaki pada pasien Diabetes Melitus. Inovasi pelayanan yang dilakukan residen adalah membuat alat simulation tool yaitu alat edukasi untuk pasien Diabetes Melitus yang menerima terapi insulin pertama kali. Protokol ini mampu memberikan panduan secara sistematis, mudah dipahami, serta mudah dilaksanakan oleh perawat kepada pasien sehingga diharapkan pasien dengan terapi insulin tidak merasa ketakutan saat mendapatkan insulin pertama kali.

Endocrine disease is a cause of death with a fairly high number both nationally and globally. This is a challenge for all health workers, especially for medical surgical specialist nurses specializing in endocrine in helping to reduce the rate of complications that occur. Quality nursing science practice based on the latest nursing evidence base is the cornerstone in carrying out the role as a caregiver, researcher, educator and innovator. The endocrine specialist nurse residency practice which takes place over two semesters has provided experience and opportunity to apply nursing theory as a whole in managing care for patients. Residents applied Roy's adaptation theory model to 30 resume cases and one main managed case, namely type 2 Diabetes Melitus patients with a history of DKA, acute CKD, hypertension, dyslipidemia and acute pancreatitis. The role of the researcher is carried out by applying evidence-based nursing, namely through sixty second inlows as a screening tool for the risk of foot injuries in patients with Diabetes Melitus. The resident's service innovation is to make a simulation tool, which is an educational tool for Diabetes Melitus patients who receive insulin therapy for the first time. This protocol is able to provide guidance in a systematic, easy to understand, and easily implemented by nurses to patients so that patients on insulin therapy are expected not to feel afraid when they get insulin for the first time."

"Program residensi keperawatan medikal bedah peminatan endokrin adalah program meningkatkan kemampuan perawat spesialis yang kompeten dalam memberikan asuhan keperawatan pada pasien dengan gangguan sistem endokrin. Gangguan sistem endokrin yang paling banyak di pelayanan kesehatan adalah Diabetes melitus (DM). Kadar glukosa darah yang tidak terkontrol akan menimbulkan berbagai komplikasi baik akut maupun kronis. Dibutuhkan pendekatan dari berbagai sistem ilmu untuk memberikan penanganan dan pencegahan komplikasi pada pasien DM. Perawat Spesialis mampu memberikan asuhan keperawatan yang holistik dengan tujuan pasien DM mampu mengontrol gula darah dan beradaptasi dengan penyakitnya. Model Adaptasi Roy dapat digunakan sebagai acuan dalam memberikan asuhan keperawatan yang holistik dan komprehensif dengan mengubah perilaku yang tidak efektif menjadi efektif. "Praktik Spesialis Keperawatan Medikal Bedah Kekhususan Respirasi di RSUP Perasahabatan Jakarta merupakan aplikasi peran dan fungsi sebagai ners spesialis dengan menerapkan Model Adaptasi Roy dalam memberikan asuhan keperawatan pada pasien dengan gangguan sistem respirasi baik pada pasien kelolaan utama maupun 30 pasien resume. Selain sebagai pemberi asuhan keperawatan peran perawat spesialis respirasi adalah menerapkan tidakan berbasis bukti ilmiah pada area respirasi yaitu dengan memberikan terapi suara berbasis alam dalam mengurangi kecemasan, agitasi, dan stress pada pasien terpasang ventilator mekanik. Peran selanjutnya dari perawat spesialis respirasi adalah melakukan inovasi melalui analisi situasi dan studi literature dalam hal ini adalah penerapan The Respiratory Distress Observation Scale (RDOS) dalam menilai risiko terjadinya distress pernapasan. The Practice of Medical Nursing Specialists Specialized in Respiration in RSUP Perasahabatan Jakarta is the application of roles and functions as specialist nurses by applying Roy's Adaptation Model in providing nursing care to patients with respiratory system disorders in both the main managed patient and 30 resume patients. Aside from being a nursing care provider, the role of respiration specialist nurses is to apply scientific evidence-based nursing actions to the area of ​​respiration by providing nature-based sound terapi to reduce anxiety, agitation, and stress in patients with mechanical ventilators. The next role of respiration specialist nurses is to innovate through situation analysis and literature studies in this case is the application of The Respiratory Distress Observation Scale (RDOS) in assessing the risk of respiratory distress. Practical results show that Roy's adaptation theory in providing nursing care to patients with respiratory and natural-based sound disorders is quite effective in , agitation, and stress in patients with mechanical ventilators. As well as RDOS can be applied to patients with respiratory disorders in assessing the risk of respiratory distress."

"Penyakit kardiovaskular merupakan penyakit yang menjadi penyebab kematian tertinggi di dunia dan insidensinya semakin meningkat tiap tahun. Peningkatan angka kejadian tersebut menuntut semua tenaga kesehatan termasuk perawat untuk wajib meningkatkan kualitas pengetahuan dan keterampilan dalam penanganan kasus kardiovaskular dengan menjalankan peran spesialisnya sebagai pemberi asuhan, pendidik, peneliti dan inovator. Tujuan pelaksanaan praktik residensi ini adalah untuk mengaplikasikan peran perawat spesialis tersebut dengan menggunakan pendekatan teori keperawatan Model Teori Adaptasi Roy. Peran sebagai pemberi asuhan keperawatan dilakukan pada 30 orang pasien yang mengalami gangguan system kardiovaskular dan satu kasus kelolaan utama yakni pasien pasca operasi jantung Coronary Artery Bypass Graft. Peran sebagai pendidik dilakasanakan kepada pasien beserta keluarga. Peran peneliti dilaksanakan melalui penerapan tindakan keperawatan yang berbasis pembuktian secara ilmiah Evidence Base Practice mengenai penerapan Head Of Bed HOB Elevation untuk mengurangi nyeri punggung Back Pain dan komplikasi perdarahan dan hematom pencabutan Trans Femoral Sheath pasien pasca kateterisasi jantung. Peran sebagai innovator dilaksanakan melalui pembuatan format tindakan untuk mempersiapkan pasien dan keluarga pulang ke rumah Discharge Planning. Hasil analisis praktik residensi ini menunjukan bahwa penerapan Model Teori Adaptasi Roy efektif dalam menangani pasien dengan masalah gangguan system kardiovaskuler dan penerapan Head Of Bed HOB Elevation efektif dalam menurunkan nyeri punggung tanpa meningkatkan risiko komplikasi perdarahan dan hematom pada pasien post kateterisasi jantung melalui transfemoral sheat. Selain hal tersebut, penggunaan format Discharge Planning dapat diterapkan pada pasien yang akan dipersiapkan untuk pulang ke rumah setelah menjalani perawatan di Rumah Sakit.

Cardiovascular disease was the leading cause of death in the world and had an incidence's increased every year. Due to the increasing of cardiovascular disease incidence of cardiovascular disease, all health workers, including nurses, must improve the quality of knowledge and skills in handling of cardiovascular cases by performing their nurse specialist roles as care giver, educator, researcher and innovator.

The purpose of this residency practice is to apply the role of nurse specialist with Roy Adaptation Model Theory approach. Nursing care was performed on 30 patients with cardiovascular system disorders and one major case of coronary artery bypass graft. Role as educator has done to patients and families.

The role of the researcher is in the application of evidence based nursing practice Head Of Bed HOB Elevation on post cardiac catheterization's patients to reduce back pain and complications haemorrhage and hematoma post retraction of Trans Femoral Sheath. The role as innovator implemented on creating discharge planning form to prepare the patient and the family returns to home.

The results of the analysis of the residency practice showed that the application of Roy Adaptation Model was effective for patients with cardiovascular system disorders, Head Of The Bed HOB Elevation effective in reducing back pain without increasing the risk of bleeding and hematoma's complications in patients post trans femoral sheath's retraction after cardiac catheterization. In addition, discharge planning form can be applied in patients who needs to be prepared to return home after hospital treatment. "

"Perawat yang mempunyai keahlian khusus dalam asuhan keperawatan yang telah lulus dari program spesialis keperawatan disebut Ners spesialis. Kompetensi keperawatan inti dan keperawatan berbagai masalah kardiovaskular harus dimiliki oleh perawat spesialis keperawatan medikal bedah peminatan kardiovaskular. Tujuan utama residensi spesialis selama pendidikan 2 semester adalah melakukan asuhan keperawatan medikal bedah khususnya sistem kardiovaskular dengan menggunakan pendekatan teori model Adaptasi Roy. Asuhan keperawatan pada 1 kasus utama pada pasien ADHF dan 30 kasus resume yang terdiri dari 8 kasus besar gangguan sistem kardovaskular meliputi kasus sindrom koroner akut, kelainan katup jantung, kelainan jantung bawaan dewasa, kelainan pembuluh darah, gagal jantung, infeksi, kasus bedah (CABG, Katup) dan disritmia. Penerapan teori model adaptasi Roy pada pasien gangguan sistem kardiovaskular untuk dapat beradaptasi pada stimulus internal dan eksternal yang dimiliki. Manajemen nyeri pasca bedah jantung dengan terapi komplementer dan alternatif berupa foot massage. Proyek inovasi penerapan Augmentative and Alternative Communication pada pasien sadar yang terpasang ventilasi mekanik dapat mengurangi kecemasan dan peningkatan kepuasan terhadap pelayanan dalam komunikasi pasien-perawat. Pengabdian masyarakat dilakukan untuk memberikan pendidikan atau pengetahuan kepada pasien, keluarga maupun masyarakat dalam melakukan latihan aktifitas fisik setelah serangan jantung sehingga dapat meningkatkan kualitas hidup

Nursing care for 1 main case in ADHF patients and 30 resume cases consisting of 8 major cases of cardiovascular system disorders including cases of acute coronary syndrome, heart valve disorders, adult congenital heart defects, vascular disorders, heart failure, infections, surgical cases (CABG, valves) and dysrhythmias. Application of Roy's adaptation model theory to patients with cardiovascular system disorders to be able to adapt to their internal and external stimuli. Pain management after cardiac surgery with complementary and alternative therapies in the form of foot massage. The innovation project of implementing Augmentative and Alternative Communication in conscious patients who are attached to mechanical ventilation can reduce anxiety and increase satisfaction with services in patient-nurse communication. "Latarbelakang: Praktik Spesialis Keperawatan Medikal Bedah Kekhususan Endokrin di RSUPN dr. Cipto Mangunkusumo Jakarta merupakan kegiatan pembelajaran dalam mengaplikasikan peran dan fungsi Ners Spesialis. Belum ada Karya Ilmiah Akhir Spesialis (KIAS) yang menelaah pasien diabetes melitus dengan komplikasi selulitis dan Chronic Kidney Disease (CKD) dengan penerapan teori adaptasi Roy. Tujuan: KIAS ini bertujuan untuk menganalisis asuhan keperawatan pada pasien diabetes melitus yang mengalami selulitis dan CKD maupun resume 30 kasus. Metode: KIAS ini terdiri dari analisis kasus utama, resume 30 kasus, penerapan evidence based nursing dan proyek inovasi. Teori model keperawatan yang dipakai adalah teori adaptasi Roy. Hasil: Ners spesialis berperan tidak hanya dalam memberikan asuhan keperawatan langsung tetapi menerapkan intervensi berbasis bukti ilmiah pada area endokrin yaitu memberikan aplikasi pelembab mengandung urea, gliserin dan petrolatum untuk mengatasi xerosis dan fisura tumit kaki pasien diabetes melitus tipe 2. Selain itu, Ners spesialis juga melakukan inovasi melalui analisis situasi dan kajian literatur untuk mencegah kejadian hipoglikemia berat pada pasien diabetes melitus tipe 2 melalui penerapan Hy-Newss bundle. Kesimpulan: Teori model adaptasi Roy sesuai untuk diaplikasikan pada pasien dengan gangguan endokrin. Pemberian pelembab mengandung urea, gliserin dan petrolatum efektif untuk menurunkan skor derajat xerosis pada pasien diabetes melitus tipe 2. Hy- Newss dapat diterapkan sebagai alat skrining risiko terjadi hipoglikemia berat pada pasien diabetes melitus tipe 2.

Background: Medical surgical nursing specialist practice in endocrine specific at RSUPN dr. Cipto Mangunkusumo Jakarta is a learning activity in applying the role and function of nurse specialist. There is no KIAS that examines diabetes mellitus patients with complications of cellulitis and Chronic Kidney Disease (CKD). Aim: This KIAS aims to analyze nursing care in patients with diabetes mellitus who have cellulitis and CKD or resume 30 cases. Method: This KIAS consists of main case analysis, 30 case resumes, application of evidence based nursing and innovation projects. The nursing model theory used is Roy’s adaptation theory. Results: Nurse specialist play a role not only in providing direct care but applying scientific evidence-based practice in the endocrine area, which is providing moisturizing applications containing urea, glycerin and petrolatum to treat xerosis and heel fissures in patient with type 2 diabetes mellitus. In addition, nurse specialist innovate through situation analysis and literature review to prevent the incidence of severe hypoglycemia in patients with type 2 diabetes mellitus through the application of Hy-Newss bundle. Conclusion: Roy’s adaptation model theory is suitable for application in patients with endocrine disorders. Moisturizers containing urea, glycerin and petrolatum are effective in reducing the degree of xerosis score in patient with type 2 diabetes mellitus. Hy-Newss bundle can be applied as a screening tool for the risk of severe hypoglycemia in patients with type 2 diabetes mellitus."

"Praktek residensi keperawatan pada sistem neurologi dituntut mampu memberikan asuhan keperawatan secara komperhensif, menerapkan Evidence Based Nursing (EBN) serta mampu berperan sebagai innovator di ruang perawatan. Pemberi asuhan keperawatan dilakukan dengan menggunakan pendekatan model adaptasi Roy pada pasien stroke hemoragik dan 30 pasien lainnya dengan berbagai gangguan system neurologi. Perilaku maladaptif paling banyak adalah mode adaptasi fisiologi. Penerapan EBN Pelvis Stability Training (PST) pada lima pasien stroke menunjukkan keefektifan dalam  meningkatkan kontrol pergerakan, kekuatan otot panggul, kecepatan berjalan, dan ADL melalui kestabilan keseimbangan panggul pada pasien stroke. Program inovasi keperawatan berupa penerapan program pencegahan kejadian jatuh pada pasien neurologi di rawat inap.

Nursing residency practice in the neurology system is demanded to be able to provide comprehensive nursing care, implementation of Evidence Based Nursing (EBN) and be able to act as an innovator in the treatment room. Nursing care was performed using Roy's adaptation model approach in hemorrhagic stroke patients and 30 other patients with various neurological system disorders. Maladaptive behavior is the most common mode of physiological adaptation. The implementation of EBN Pelvis Stability Training (PST) in five stroke patients showed effectiveness in increasing movement control, pelvic muscle strength, walking speed, and ADL through stability in pelvic balance in stroke patients. The nursing innovation program is in the form of a fall prevention program in neurology patients hospitalized."
